        조기 대장암의 내시경 절제술 후 림프절 전이의 위험 인자

        Background/Aims: Although endoscopic resection is widely used for the treatment of early colorectal cancer, the risk factors for lymph node metastasis are not clear. This study was designed to determine the risk factors for lymph node metastasis in patients with colorectal cancer who are treated by endoscopic resection. Methods: The medical records of patients with histologically-proven early colorectal cancers who were treated by endoscopic resection between January 2002 and September 2008 were retrospectively reviewed. Information regarding the demographic data of patients and the clinicopathologic characteristics were recorded and analyzed. Results: Twenty-nine patients who underwent subsequent surgical treatment after endoscopic resection for early colorectal cancer were enrolled in this study. Six patients (20.7%) had lymph node metastases on surgical pathologic examination. The predictive factors for lymph node metastasis were tumor morphology (non-polypoid flat tumors [p=0.019]), absence of background adenomas (p=0.033), and deep submucosal invasion ≥2,000 um (p=0.012). Unexpectedly, the presence of vascular invasion was not associated with lymph node metastasis. Conclusions: The presence of vascular invasion might not be an absolute indication for additional surgical treatment of early colorectal cancer; however, deep submucosal invasion, accompanied by a gross tumor with a non-polypoid flat morphology, and the absence of background adenomas are potential risk factors for lymph node metastasis.         외상성 출혈성 쇼크 환자에서 발생한 비 폐쇄성 장간막 허혈

        Non-occlusive mesenteric ischemia (NOMI) encompasses all forms of mesenteric ischemia with patent mesenteric arteries. NOMI is commonly caused by decreased cardiac output resulting in hypoperfusion of peripheral mesenteric arteries. We report a case of NOMI secondary to hemorrhagic shock and rhabdomyolysis due to trauma. A 42-year-old man presented to our trauma center following a pedestrian trauma. On arrival, he was drowsy and in a state of hemorrhagic shock. He was found to have multiple fractures, both lung contusion and urethral rupture. An initial physical examination and abdominal computed tomography (CT) scan revealed no evidence of intra-abdominal injury. High doses of catecholamine were administered for initial 3 days due to unstable vital sign. On day 25 of hospitalization, follow- up abdominal CT scan demonstrated that short segment of small bowel loop was dilated and bowel wall was not enhanced. During exploratory laparotomy, necrosis of the terminal ileum with intact mesentery was detected and ileocecectomy was performed. His postoperative course was uneventful and is under rehabilitation.         The objective of this work was to investigate the lateral deformation behavior at impact surface in sleeved polymers during Taylor impact. This test setup has been generally used to determine the dynamic yield stress of cylinder projectile. However, the impact behavior of sleeved projectile is very complex as compared with the one of cylinder projectile. Therefore, this study investigated experimentally the influence of sleeve material and thickness on them. Taylor impact tests of sleeved projectiles were carried out in the velocity range between 100~300 m/s velocity. High-speed photography provided a series of images representing the deformation behavior of projectiles during impact. Experimental data were compared with the data obtained by using the AUTODYN 3D-simulation. The numerical simulation results showed similar behaviors to those obtained by experiments. Through the numerical simulation it to predict the behavior at higher impact velocity. Also, it was possible to understand the dynamic behavior of inner core and outer jacket materials induced during the impact through the comparison of experimental and numerical data.

        다발성 외상환자에서 혈관계 접근을 통해 치료한 쇄골하동맥 손상 2례

        Introduction: Surgical treatment of subclavian artery (SA) injury is challenging because approaching the lesion directly and clamping the proximal artery is difficult. This can be overcome by using an endovascular technique. Case 1: A 37-year-old male was drawn into the concrete mixer truck. He had a right SA injury with multiple traumatic injuries: an open fracture of the right leg with posterior tibial artery (PTA) injury, a right hemothorax, and fractures of the clavicle, scapula, ribs, cervical spine and nasal bone. The injury severity score (ISS) was 27. Computed tomography (CT) showed a 30-mm-length thrombotic occlusion in the right SA, which was 15 mm distal to the vertebral artery (VA). A self-expandable stent(8 mm×40 mm in size) was deployed through the right femoral artery while preserving VA flow, and the radial pulse was palpable after deployment. Other operations were performed sequentially. He had a viable right arm during a 13-month follow-up period. Case 2: A 25-year-old male was admitted to our hospital due to a motorcycle accident. The ISS was 34 because of a hemothorax and open fractures of the mandible and the left hand. Intraoperative angiography was done through a right femoral artery puncture. Contrast extravasation of the SA was detected just outside the left rib cage. After balloon catheter had been inflated just proximal to the bleeding site, direct surgical exploration was performed through infraclavicular skin incision. The transected SA was identified, and an interposition graft was performed using a saphenous vein graft. Other operations were performed sequentially. He had a viable left arm during a 15-month follow-up period. Conclusion: The challenge of repairing an SA injury can be overcome by using an endovascular approach.

        외상 후 지연성으로 발생한 장간막 동정맥루: 증례보고

        Introduction: A post-traumatic mesenteric arteriovenous fistula (AVF) is extremely rare. Case Report: A previously healthy 26-year-old male was injured with an abdominal stab wound. Computed tomography (CT) showed liver injury, pancreas injury and a retropancreatic hematoma. We performed the hemostasis of the bleeding due to the liver injury, a distal pancreatectomy with splenectomy and evacuation of the retropancreatic hematoma. On the 5th postoperative day, an abdominal bruit and thrill was detected. CT and angiography showed an AVF between the superior mesenteric artery (SMA) and the inferior mesenteric vein with early enhancement of the portal vein (PV). The point of the AVF was about 4 cm from the SMA`s orifice. After an emergent laparotomy and inframesocolic approach, the isolation of the SMA was performed by dissection and ligation of adjacent mesenteric tissues which was about 6 cm length from the nearby SMA orifice, preserving the major side branches of the SMA, because the exact point of the AVF could not be identified despite the shunt flow in the PV being audible during an intraoperative hand-held Doppler-shift measurement. After that, the shunt flow could not be detected by using an intraoperative hand-held Doppler-shift measuring device. CT two and a half months later showed no AVF. There were no major complications during a 19-month followup period. Conclusion: Early management of a post-traumatic mesenteric AVF is essential to avoid complications such as hemorrhage, congestive heart failure and portal hypertension.

        860 MHz 대역 고출력 Cartesian 피드백 송신기 설계

        This paper presents the design of 860 MHz band transmitter for improving power amplifier's linearity using Cartesian feedback method. For eliminating the effects of gain, phase mis-match, and DC offset, we estimate the property variations using ADS software. The implemented Cartesian feedback transmitter exhibits IMD3 of -54 dBc at an output power of 43 dBm and this result shows that the linearity is improved for 22.4 dB, compared with the test of the power amplifier without Cartesian feedback system. Thus, we verify that the proposed Cartesian feedback transmitter can be applied to narrow-band transmitter systems. 본 논문은 Cartesian 피드백 방법을 이용하여 전력 증폭기의 선형성을 개선한 860 MHz 대역의 송신기의 설계에 관한 것이다. 피드백 루프를 통하여 발생하는 이득 및 위상의 불일치와 DC offset에 의한 영향을 제거하기 위하여 ADS simulation을 이용하여 특성 변화를 예측하였다. 제작된 Cartesian 피드백 송신기는 출력 전력 43 dBm에서 -54 dBc의 IMD3 특성을 나타내었으며, 이는 선형화 전과 비교하여 22.4 dB의 선형성 개선 효과가 있음을 의미한다. 따라서 본 논문에서 제작한 Cartesian 피드백 송신기를 이용하여 협-대역 전송시스템에 적용할 수 있음을 확인하였다.

        著者들은 內臧山의 昆蟲相을 調査하기 위하여 1990년 9월까지 이 地域의 昆蟲들을 採集하였다. 著者들의 文獻調査에 의하면 이 地域에 대한 傳門的인 昆蟲相은 지금까지 두번 수행된 적이 있다. 또한 이 地域은 觀光地 임으로 여러 文獻에서 採集地 記錄되고 있다. 採集된 모든 漂本들을 同定한 結果 12目 115科 831種이 確認, 同定되었다. 그 중에서 稀貴種으로 保護가 要求되는 種은 Parnassius bremeri(붉은 점모시나비)와 Melitaea phobe(암어리표범나비), Aporia crataegi(상제나비), Spindasis takanonis(쌍꼬리부전나비), Bibasis striata(큰수리팔랑나비), Chlorophorus diadema(범하늘소), Chrysochroa fulgidissima(비단벌레)이다. In order to study the insect fauna from Mt. Maejang authors collected insects from April, 1990 to September, 1990. Up to now the insect fauna of this area has been professionaly surveyed twice according to our references studies. After we examined all the specimens, 831 species of 115 families under 12 orders were identified in this region. Among them 7 species; parnassius bremeri, Melitaea phobe, Apocrina crataegi, Spindasis takanosis, Bibasis striata, Chlorophorus diadema and Chrysochroa fulgidissima are rare species to be protected.

        In order to study the insect fauna from Mt. Unjang, authors collected all the insects in this region from April, 1990 to September, 1990. Up to now the insect fauna of this area has not been professionally surveyed yet according to my references studies. Examining all the specimens, 180 species of 32 families under 5 orders were indentified in this region. Among them 4 species; Parnassius bremeri, Melotaea phobe, Bibasis striata and Aporia crataegi are rare species to be protected.

Till now the insect fauna of this area has not been professionally surveyed yet according to my references studies. After we examined all the specimens, 179 species of 30 families under 6 orders were identified in this region. Among them 4 species; Parnassius bremeri, Melitaea phoebe, Aporia critaegi and Bibasis striata are rare species to be protected.
