        효과적인 BAHAⓇ Attract의 이식술식

        An implantable bone conduction hearing aid is a device that assists hearing by transmittingsound vibration through the bone. It is suitable not only for patients with conductive hearingloss and congenital external aural atresia but also for those with unilateral hearing loss, chron-ic otitis media, or patients whose normal structure has been destroyed due to previous surgery. In this paper, we introduce a novel surgical technique for an implantable bone conductionhearing aid (BAHA® Attract) that can be performed through minimal incision in a short timeunder local anesthesia based on accurate anatomical characteristics. We believe this noveltechnique could provide convenience for patients and surgeons. Additionally, through thistechnique, we can expect that various types of implantable bone conduction hearing aids thatfunction effectively and safely can be provided for patients.

      • Neuromodulation for the Treatment of Tinnitus

        Tinnitus is a perception of sounds in the absence of external noise. Tinnitus can affect an individual’s life, prevent productive work or impair the quality of life. There are 2 types of tinnitus, objective and subjective, the latter being the most challenging of hearing disorders. Tinnitus has various forms and it can be difficult to relate a specific event with the appearance of tinnitus. Moreover, detection of tinnitus and evaluation of its severity is impossible. Therefore, treatment is usually based on the patient’s own assessment. To date, various forms of treatment have been administered with minimal success. Many different treatments have been attempted and then discontinued. The treatment goal of eliminating symptoms for severe tinnitus is rarely achieved. However, some symptoms of tinnitus can often be reduced to improve the patient’s quality of life allowing him or her to work despite residual effects of the disorder. In the present study we evaluated electrical stimulation, including transcranial direct current stimulation, transcranial magnetic stimulation for the treatment of tinnitus.

        CliP-Coupler를 이용하여 등골에 시행한 인공중이이식술 1예

        Middle ear implantation is an effective treatment for sensorineural, conductive and mixedhearing loss. The active middle ear implant’s floating mass transducer (FMT) is placed on themiddle ear structure or coupled to the inner ear fluid. Since the device is not directly placed onthe external auditory canal, patients are free from feedback phenomenon and occlusion effectresulting from hearing aids. A 66-year-old patient suffered from bilateral hearing loss. The patienthad difficulties in adapting to a conventional hearing aid because of feedback phenomenonin the right ear. The patient underwent a middle ear implantation using a CliP-Coupler to connectstapes head and FMT in the left ear. To our knowledge, this is the first case reported in Koreaon middle ear implantation with CliP-Coupler at the stapes, for which hearing results weresuccessful at postoperative 7 months.         중두개와 접근법으로 제거한 재발성 진주종 1예

        Cholesteatoma is a benign disease but it has an aggressive feature that may lead to serious conditions. Many strategies have been introduced for the treatment cholesteatoma, yet no definite single method has been established: it should be treated respectively regarding the completion of cholesteatoma, conservation of tympanic mucosa, proper ventilation of middle ear and preservation of hearing. A 25-year old patient visited our clinic for right-sided facial paralysis of 5 days. The patient underwent canal wall down mastoidectomy 10 years ago due to cholesteatoma and a few revision surgeries under local anesthesia for recurrence. Pre-operative temporal bone CT showed suspected recurred cholesteatoma in internal auditory canal and labyrinthine segment of facial nerve nearby. The patient underwent a removal of cholesteatoma via middle cranial fossa approach. This case report is a successful completion of recurred cholesteatoma, for which no recurrence is shown. Facial nerve function is improved at postoperative 1 year.

        Saccular Pathology Is Most Commonly Found in Patients With General Vestibular Disorders

        Objectives. The first purpose of this study was to investigate the difference in the frequency of involvement of the superior vestibular nerve (SVN) and inferior vestibular nerve (IVN) territories in general vestibular disorders, and to identify which IVN territory was more commonly involved in patients with IVN lesions. The second purpose was to investigate the correlation of the degree of each saccular and posterior semicircular canal (PSCC) dysfunction, as represented by the parameters of cervical vestibular evoked myogenic potential (cVEMP) and video head impulse test (vHIT), in patients with pathology of the IVN territory. Methods. In total, 346 patients with dizziness who underwent the caloric test, cVEMP, and vHIT were enrolled. Canal weakness in the caloric test, interaural amplitude difference (IAD) of cVEMP, and vestibulo-ocular reflex gain of the vestibulo-ocular reflex gain of the posterior semicircular canal (p-VOR) in vHIT were analyzed. Results. Among the enrolled patients, 15.6% had total vestibular nerve dysfunction, 14.5% had solely SVN dysfunction, and 29.5% had solely IVN dysfunction. Isolated saccular pathology was most common in patients with IVN pathology, followed by those with total IVN dysfunction and PSCC dysfunction. IAD and p-VOR were statistically well correlated, and the correlation was strongest in patients with both pathologic IAD and pathologic p-VOR (n=23, r=0.944), followed by patients with normal IAD and pathologic p-VOR (n=27, r=0.762) and patients with pathologic IAD and normal p-VOR (n=106, r=0.339). Conclusion. Abnormal results were more common in vestibular tests investigating the IVN than in vestibular tests investigating the SVN in patients with general vestibular disorders. Isolated saccular pathology was more frequent than PSCC or combined pathology in patients with IVN dysfunction. Patients with abnormal p-VOR in vHIT had a higher probability of having both saccular and PSCC pathologies than patients with an abnormal IAD. This study describes the characteristics of vestibular-system subregions and provides guidance for clinically interpreting the combination of cVEMP and vHIT results.

        Hearing Rehabilitation with Combined Electroacoustic Stimulation and Ossiculoplasty

        We report a case of hearing rehabilitation following combined cochlear implantation and ossiculoplasty. A 71-year-old patient visited the clinic for right-sided mixed hearing loss. We targeted neural and conductive components, performing two different operations simultaneously. At two months post-operative, the patient showed satisfactory results with respect to hearing threshold and speech comprehension. Our experience suggests that careful evaluation of patients and consideration of the diverse array of available treatment strategies can be used to provide personalized rehabilitation with maximal effectiveness. To the best of our knowledge, this is the first report to take such an integrated approach to treat hearing impairment and is thus likely to have clinical importance for otologists.

        Implantable devices for aural rehabilitation are widely developed worldwide. In the manuscript, we review the implantable middle ear hearing devices currently in use, such as the Vibrant Soundbridge system (Med-El Corporation), the MAXUM system (Ototronix LLC), the fourth-generation of Carina prosthesis (Otologics LLC), and the Esteem device. Implantable hearing aids will unquestionably be the major drivers of advancement in otologic practice in early future, improving the quality of life of an increasingly aged population, which will consequently require increased levels of hearing support.         Eustachian tube dysfunction (ETD) is a common condition in clinical otolaryngology practice. The eustachian tube (ET) is the only pathway connecting the middle ear cavity and thenasopharynx, and has a ventilation function to equalize the pressure of the middle ear and theatmosphere, a discharge function of middle ear secretions into the nasopharynx, and a defensefunction to prevent reflux from the nasopharynx. The most important function of the ET is tomaintain a consistent middle ear air pressure for efficient sound transmission. Various methodshave been used to measure the physiological function of ET. Traditional methods includethe Valsalva, Toynbee, and Politzer test. Physiological methods include the 9-step Bluestoneinflation-deflation test and sonotubometry. Evaluation of ETD using questionnaires is also beingconducted, and currently, imaging tests are also conducted to evaluate the structure of ET. However, no standard protocols have been established until now. In this article, the types,methods, and advantages and disadvantages of the currently available ET function tests areintroduced

      • 사용성을 고려한 가정용 가습기 디자인 개발에 관한 연구

        현대사회에 가전제품들은 사람들에게 편리성을 제공하기 위해 만들어졌다. 그 중 가습기는 과거부터 공간의 습도를 조절해 사람들이 바이러스나 세균에 면역력이 좋게 만들어주고 수분이 필요한 모든 생물들에게 도움이 되는 제품이었다. 이런 가습기 제품은 다양한 종류로 시장에 출시되고 있는데 가열식, 초음파식, 복합식, 자연식 네 가지 방식으로 습도를 조절한다. 그러나 가습기 살균제가 가습기가 사람의 건강을 도우는 제품이라는 인식에서 건강을 해치는 제품으로 변한 요인이 되고 있다. 가습기 살균제는 먼지 및 세균 번식이 잘 일어나는 초음파 가습기 내부의 청소용으로 사용되어 왔었는데 사용하게 되면 원인 미상의 폐 손상이 발생할 확률이 높아지고 사망자까지 나타나 큰 화두로 대두되자 사람들이 가습기에 대한 이미지가 나빠져 더 이상 구입하지 않는 모습을 보였다. 이에 따라 가습기 살균제를 사용할 필요가 없고 청소하기 편하게 디자인을 한다면 사람들이 가습기에 대한 인식도 달라질 것이라고 생각해 자연식 가습기를 중심으로 가습기 조형성 및 은유적 표현 방법을 살펴보고 발전방향을 모색하고자 한다. In modern society, the consumer products were made in order to provide convenience to people. Humidifier from the past to adjust the humidity of the space so people"s natural immunity to the virus or bacteria makes good. Also to all species of water needed was a product that will help. Humidifier to the various types of products on the market. Humidity control in four ways: heating, ultrasonic, combination, air washers. However, humidifier disinfectant change was recognized as unhealthy products. Humidifier disinfectant has been used in the cleaning of the humidifier inside because there are dust and bacteria breed well. If you used a humidifier disinfectant, you would increase the likelihood of lung injury of unknown cause. Deaths represented emerged as a big issue so people who have a bad image for the humidifier appearance do not have to buy anymore. Therefore, I want to design easier to clean the humidifier and you do not need to use a humidifier disinfectant. So, I find direction of development that air washers humidifiers considering formative and usability.
