        Simultaneous Translabyrinthine Tumor Removal and Cochlear Implantation in Vestibular Schwannoma Patients

        Refinement of surgical techniques has allowed hearing preservation after tumor resection to be prioritized. Moreover, restoration of hearing after tumor removal can be attempted in patients with bilateral vestibular schwannomas or those with a schwannoma in the only-hearing ear. Cochlear implantation (CI) has emerged as a proper method of acoustic rehabilitation, provided that the cochlear nerve remains intact. Studies of electrical promontory stimulation in patients after vestibular schwannoma resection have demonstrated favorable results. We describe herein two cases of hearing rehabilitation via CI implemented at the time of vestibularschwannoma resection. Tumors were totally removed, and cochlear implant electrodes were successfully inserted in both cases. Also, post operative CI-aided hearing showed improved results.

        소총 소음에 의한 음향외상의 임상양상

        One of the major occupational hazards of working in the military service is being subjected to intense impulse noise, hence acoustic trauma. Acute acoustic trauma is a preventable disease, but the effective treatment modality has not been established yet. Acute acoustic traumas caused by exposure to rifle gunshot noise are common in young Korean males in the military service because of conscription policy. Through the analysis of the clinical presentation of acoustic traumas caused by rifle gunshot noise, we suggest effective preventive modalities. Subjects and Method:165 soldiers who had otologic symptoms after performing regular rifle gunshot exercise without any hearing protective measures were analyzed. They all had been tested with K2 rifle. History taking, physical examination, pure-tone audiometry, and impedence audiometry, temporal MRI, and Newmann’s tinnitus inventory questionnaire survey were performed. Results:Most common and annoying symptom was tinnitus. The average outcome of postexposure air conduction thresholds were 22.3dB HL in the affected ear and 11.8dB HL in the unaffected ear. There was no correlation between severity of tinnitus and severity of hearing loss. Asymmetry of hearing loss related to head position during gunshot was not found. This may be due to the fact that during the mass gunshot training, each patient can be influenced by the gunshot noise of the adjacent gunner’s shooting. Conclusion:Widening the distance between firing lanes and providing bilateral hearing protective devices against acoustic trauma must be taken into serious consideration.         초음파 센서 기반 장애물 인지 이동 로봇 설계

        Intelligent robots are widely needed in various areas of industry from extremely dangerous environments to service tasks. For autonomous mobile robots, it is significant to move itself safely to a destination point, recognizing its surroundings. Advances in sensor technology and its applications are achieved in order to develop an intelligent robot. In this paper, a mobile robot with a path-finding algorithm is presented. The path-finding algorithm is the one that does not only find a path to designated destination and also recognizes obstacles on the way, calculating its distance. 10 ultrasonic sensor are mounted on the front and rear of the mobile robot to figure out its position. Specular reflection and wide viewing angle, which are inherent characteristics of ultrasonic waves, cause errors in measuring distance.

        Polycel?瑛? 사용한 이소골 재건술의 청력 개선효과

        Background and Objectives:During the last decade, the surgical use of alloplasts has become more and more widespread among otologists. In this study, we evaluated the hearing results of ossiculoplasty using Polycel?? prosthesis. Subjects and Method: One hundred eighty eight cases who underwent ossicular chain reconstruction using Polycel?? prosthesis and had been followed up postoperatively for more than 12 months at Severance Eye-ENT Hospital from 1998 to 2002 were reviewed retrospectively. Postoperative hearing results were assessed by measuring the postoperative air-bone gap (ABG) and closure in air-bone gap. Successful postoperative ABG criteria were defined as the following three groups;ABG of ≤10 dB, ABG of ≤20 dB, and ABG of ≤30 dB. Several prognostic factors such as the condition of middle ear mucosa and ossicles, presence or absence of cholesteatoma, surgical method, staging and revision surgery were analyzed. Results:Of the total of 188 cases, 22 cases (11.7%) were ≤10 dB ABG, 96 (51.1%) cases were ≤20 dB ABG, and 158 (84.0%) cases were ≤30 dB ABG. The hearing results were good for healthy middle ear mucosa and cases which contained stapes superstructure, the hearing results were good. Conclusion:Polycel?? is a good material to be used in ossiculoplasty as a hydroxyapatite and autologous bone. In ossiculoplasty, good prognostic factors of the middle ear condition were healthy middle ear mucosa and the presence of stapes superstructure.

        장병들의 사격에 의한 소음성 난청

        Background and Objectives:Noise induced hearing loss (NIHL) is a preventive disease and yet the effective treatment modalityhas not been established. Acute acoustic traumas caused by an exposure to gunshot noise are common in young Koreanmales in the military service. Considering the significant lack of awareness on this serious issue as well as the absence of properprotection gears, an in-depth analysis is desperately needed. Subjects and Method:3,650 soldiers performed a regular gunshotexercise without any hearing protective measures. Out of 3,650 soldiers, 8 patients with hearing impairment after the regular gunfirevisited aeromedical squadron. They were all right-handed males:7 had been tested with K-2 rifle and 1 had been tested withK-5 revolver. History taking, physical examination, pure-tone audiometry, and impedence audiometry were performed. In thenext round of gunshot practice, all soldiers performed gunfire with unilateral hearing protection. Results:The average outcomeof postexposure air conduction thresholds were 6.5 dB in the right ear and 33.1 dB in left ear. After medical treatment, hearingimpairment was much improved;however, tinnitus was not diminished. In the next round of gunshot practice, the supplementof unilateral earplug protection proved its effectiveness on acoustic trauma caused by gunfire noise. Conclusion:Asymmetryof hearing loss was related to head position during gunfire. Unilateral hearing protection device was enough to protect hearingfrom acoustic trauma caused by gunshot. At the same time, it can effectively prevent a potential firearm accident that can becaused by trainees mishearing the instruction of firearm instructor if both earplugs are worn. Thus, providing unilateral earplugprotection against acoustic trauma must be taken into serious consideration. (Korean J Otolaryngol 2006;49:887-91)

      • 무선 통신과 영상인식을 활용한 지능형 자율주행 자동차 개발에 관한 연구

        논문은 지능형 자동차 개발을 위한 마이크로마우스를 활용한 차량-도로 간 통신, 모터제어, 영상처리와 물체감지 센서들을 활용한 도로 상황정보 인식 시스템 등을 활용하여 지능형 자동차의 운행을 구현하고 테스트 할 수 있다. 이를 활용하여 검증된 기술들을 실제 차량에 적용하기 위해서 다양한 실험과 테스트를 실시한다. 또한 누구나 쉽게 간단한 마이크로 컨트롤러를 통한 지능형 자동차를 위한 다양한 아이디어를 접목할 수 있도록 시스템을 설계한다. 이를 활용하여 영상처리를 통한 차선인식과, 직전 좌회전 및 도로의 교통표시마크를 읽어 들여 차체를 컨트롤하고 차량-도로간 통신을 이용해서 신호등의 정보를 입력받아 교차로를 안전하게 통과하게 된다. 또한 앞 차량을 인식하여 자동으로 속도조절과 차량 간 거리를 유지하는 시스템을 구현한다.

Until now, there have been no reports of schwannomas extending simultaneously in the inner ear, middle ear, and jugular foramen. We report the first experience of a vestibular schwannoma simultaneously involving the IAC, cochlea, vestibule, middle ear, external ear, and jugular foramen which removed via modified surgical technique called as‘ Rope-skipping’
