        한국 여성의 자궁경부암 검진에 있어서 자궁경부 확대촬영검사의 역할

        We did cervicographic and cytologic examiantion to confirm the possibility of national wide screening system. 1,308 women were prospectively screened in university hospital and private clinics between jan. 1995 and Sep. 1995. Cervigram were evaluated by at least 3 cervicography evaluators. 1,075(82.1%) women had negative cervigram, 87(6.6%) had atypical cervigram, 97(9.8%) had positive cervigrams. 19(1.5%) was technically defective. 5 invasive cancer were detected in this population. Cervical punch biopsy were token 84 womens because they had atypical or positive cervigram of abnormal cytology result in primal screening smears. Patients(82.1%) had negative cervigrams. 87(6.6%) were technically defective. 5 invasive cancer were detected in this population. Among all positive cervigrams 1 of 8 invasive cancers had negative cytology and 27 of 48 cervical intraepithelial neoplasia lesion had negative cytology. The sensitivity of cervicography was 96.6%, the specificity of cervicography was 98.1%, positive predictive value of cervicography was 71.2%, negative predictive value was 99.3%, The false negative rate of cervicography was 3.4%, the false positive rate was 1.9%. The sensitivity of cytology was 52.5%, the specificity of cytology was 99.8%, positive predictive value of cytology was 91.2%, negative predictive value was 97.7%, The false negative rate of cytology was 47.5%, the false positive rate was 3%. Combined cytology and cervicography is more sensitive, specific and efficient for screening cervical cancer. We concluded cervicography had a potentiality as a adjunctive method for the national wide cervical cancer scrrening.

        난소암 세포주들의 전이능력과 Type Ⅳ collagenase 효소 활성화의 비교

        비활성 형태인 recombinant 72-kDa MMP-2를 분비하는 MCF-7MMP-2 세포주와 난소암 세포주인 CAOV- 4, SW626, NIH: OVCAR-3, SK-OV-3를 이용하여 MMP-2의 분비 능력과 활성화 양상을 in vitro에서의 난소암 세포의 침윤 능력과 비교 연구하여 다음과 같은 결과를 얻었다. 1. 난소암 세포주들은 모두 비활성형 MMP-2를 분 비하였으나 NIH: OVCAR-3, SK-OV-3, SW626 세포 주에서는 활성형 MMP-2를 보였다. 2. Boyden chamber를 이용한 세포주들의 침윤 능 력 검사에서 SK-OV-3, NIH: OVCAR-3, SW626, 그리 고 CAOV-4 세포주의 순으로 침윤 능력을 보였고 이는 활성형 62kDa의 MMP-2 소견과 일치하였다. 3. Matrigel을 이용한 세포주들의 성장검사에서 NIH: OVCAR-3, SK-OV-3 세포주들은 SW626, CAOV- 4 세포주들보다 수지상 돌기를 만드는 정도가 심하였 으며 세포들의 분포 양상은 집단화를 보였다. 4. MMP-2의 활성물질인 Concanavalin A를 CAOV- 4 세포주 배양액에 투여하여 활성형 MMP-2가 생산 됨이 확인되었다. 5. Cycloheximide를 SK-OV-3, NIH: OVCAR-3 및 SW626 세포주에 투여하여 MMP-2의 생산 및 분비가 억제되는 것을 확인하였다. 이상의 결과에서 난소암 세포주들의 침윤과 전이 에는 MMP-2가 직접적으로 관여함이 밝혀졌다. 특히 MMP-2의 활성화 정도와 in vitro 침윤 능력은 밀접 한 관계에 있으며 특히 난소암 세포주들에 의해 분 비되는 MMP-2는 효소 조절물질에 의해서 활성 및 억제될 수 있음을 확인함으로서 향후 이러한 효소 조절물질을 이용하여 난소암의 전이 예방과 치료에 응용할 수 있게 될 것이다. Our purpose in this study was to detect the secretory pattern and the degree of enzymatic activity of MMP-2 and to evaluate the invasive activty in vitro in each cancer cell line, using four kinds of human ovarian cancer cell lines (CAOV-4, SW626, NIH: OVCAR-3, and SK-OV-3) as well as MCF-7MMP-2 cell line which secretes recombinant 72-kDa MMP-2 as an inactive form. The results were as follows; 1. An inactive form of MMP-2 was secreted by all ovarian cancer cell lines, but the activated form of MMP-2 was also secreted by NIH: OVCAR-3, SK-OV-3, and SW626. 2. In the Boyden chamber chemoinvasion assay, the invasive activity was shown in SK-OV-3, NIH: OVCAR-3, SW626, and CAOV-4 cell lines in order. This finding corresponds with the secretory pattern of 62 kDa MMP-2, activated form. 3. In the outgrowth morphology on Matrigel, the dendritic processes of NIH: OVCAR-3 and SK-OV-3 cell lines were seen to emerge more actively than those of SW626 and CAOV-4 cell lines. NIH: OVCAR-3 and SK-OV-3 cell lines also had a clustered formation. 4. An inactive form of MMP-2 secreted by CAOV-4 was activated by the treatment with Concanavalin A, MMP-2 activator. 5. The productions and secretions of MMP-2 in SK-OV-3, NIH: OVCAR-3, and SW626 cell lines were inhibited by the treatment with cycloheximide, MMP-2 inhibitor. From the above results, we showed that the invasion and metastasis of human ovarian cancr cell lines have directly relations with MMP-2. It was identified that the secretory patterns of activated MMP-2 were closely associated with the invasiul activity of human ovarian cancer cell lines in vitro and that the MMP-2 secreted by human ovarian cancer cell lines might been activated or inhibited by enzyme regulators. In the future, we wish this study applied to the metastatic prevention and treatment of human ovarian cancer by using enzyme regulators.

        All-trans Retinoic Acid에 의한 JEG-3 세포주의 m-RNA 표현차이

        Effects of all-trans retinoic acid(ATRA) on differentiation and gene expression have been investigated in human choriocarcinoma cell line JEG-3. RA, an active metabolite of vitamine A, has antitumor effects through induction of differentiation, inhibition of angiogenesis, inhibition of cell proliferation, and suppression of viral oncogenes. ATRA is one of important mediators of cellular differentitation and has been shown to stimulate human chorionic gonadtropin(hCG) secretion by JEG-3 choriocarcinoma cells in vitro. In contrast to proliferation of JEG-3 cell lines. b-hCG secretion by JEG-3 cells was simulated at high concentration of ATRA 10^-6M. The differential display technique was applied to compare mRNAs from JEG-3 cells and retinoic acid stimulated JEG-3 cells. Complementary DNA fragments corresponding to several apparently diffentially expressed nRNAs were recovered and sequenced. In conclusion, ARTA physiologic concentration stimulates the differantiation of JEG-3 cells without increasing the cell number and induces and suppreses the expression of some genes.         완전포상기태 예후인자로서 접합체형의 임상적 의의

        To evaluate the possible correlation between the origin of complete hydatidiform mole(CHM) and subsequent persistent gestational trophoblastic tumor(GTT) after molar evacuation, we have studied genetic origin patterns against conventional clinical parameters -patient`s age, gestational age, uterine size for gestational age, serum β-hCG levels before molar evacuation- in 69 patients with CHM. In our study, each of large uterine size for gesta-tional age, serum β-hCG levels before molar evacuation, and genetic origin of CHM had a prognostic significance of subsequent persistent GTT. However, each of gestational age and patient`s age is not a good prognostic indicator for subsequent persistent GTT. Among the patients with persistent GTT, there are no differences in clinical parameters- patient`s age, gestational age, tumor age(the interval between evacuation of CHM and initiation of chemotherapy), serum β-hCG levels before molar evacuation and before initiation of chemotherapy- according to the origin of CHM. There are no differences in the analysis of sex-chromosome and variable number tandem repeat sequence YNZ22 and APOB gene in the extracted DNA from frozen tissues and paraffin blocks and from EDTA treated peripheral blood and dried blood specimen on Wartman paper. It is suggested that analysis of sex-chromosome and polymorphism of YNZ22 and APOB gene from the extracted DNA of paraffin block and dried blood specimen on Wartman paper is the valauble experiment to evaluate the origin and the classification of hydatidiform mole and seems to be the sensitive molecular genetic method in predicting subsequent persistent GTT.

        원발성 난소 평활근육종 1 예

        저자들은 53세된 환자에서 난소에 발생된 원발성 평활근육종 1예를 경험하였기에 간단한 문헌고찰과 함께 보고하는 바이다. Primary leiomysarcomas of the ovary are extremely rare. Fewer than 20 cases have been described in the literature. This cse is occurred in a 53 years old multiparous woman. We experienced a case of pure ovarian leiomysarcoma which may originate from smmoth muscle cells of the ovarian blood vessel. We report this case with a brief view of the literature.
