        Deep neck infection is an infection in the potential spaces and fascial planes of the neck, either with abscess formation or cellulitis. In the preantibiotics era most cases of deep neck infection were secondary to an oropharyngeal infection. Moreover, today manupulation of intubation tube and gastroenteroscopy may cause deep neck infection by iatrogenic trauma. We experience 1 case of deep neck infection which originate from pharyngeal penetrating injury following gastroenteroscopy.

        Foreign bodies of the parapharyngeal space can cause severe complications such as descending suppurative mediastinitis, jugular thrombophlebitis, cavernous sinus thrombosis and carotid erosion. Therefore, early diagnosis and surgical intervention are needed to reduce morbidity and mortality. We present a case of a toothbrush as foreign body in the parapharyngeal space in 28-year-old male patient. The tooth brush was broken and the remnant of that was left in the patient's mouth. Under general anesthesia, intraoral approach was undertaken and successfully the toothbrush was removed, and then incision and drainage was done by transcervical approach. We report this case with review of literature.

        The incidence of intracranial complications of chronic otitis media with cholesteatoma, including lateral sinus thrombophlebitis, has been significantly reduced since the advent of antibiotics. But, it is still a serious disease accompanied by a high morbidity and mortality rate. Therefore, a high index of suspicion is essential for early diagnosis and appropriate management. The signs and clinical courses of lateral sinus thrombophlebitis are non-specific and the final diagnosis rests on radiological investigations including CT and MRI. Prompt antibiotics and surgery are definite treatment modalities. Early surgical treatment may reduce the possibility of the intracranial complications and shortens the duration of illness. Recently we experienced a case of lateral sinus thrombophlebitis complicated with chronic otitis media with cholesteatoma, and reported this case along with literature survey.

        Metastatic carcinomas to the thyroid gland are rare, and thyroid involvement by secondary carcinomas commonly results from direct the extension of malignant cells from adjacent organs such as the larynx or the trachea. The common primary sites of thyroid metastasis are kidney, breast, lung, and lymphoid tissue. Among head and neck cancers, nasopharyngeal carcinoma has a relatively high incidence of distant metastases to other sites and commonly involving sites are bone, lung, and liver. Recently, we experienced a case of a 43-year-old male who had been presented with neck mass for 3 months. He was diagnosed non-keratinizing nasopharyngeal carcinoma in 1993. And, thyroid metastasis of nasopharyngeal carcinoma was confirmed by total thyroidectomy. So we report this rare case with the review of literatures.

        건설재해 예방을 위한 혹서기 건설작업온도지수 프로토타입 개발

        The recent climate of Korea is changing gradually to subtropical one. This phenomenon causes that the number of days and casualties of extreme heat season is increased in Korea to date. This study focuses on the development of the prototype of the temperature index for construction work to curb the construction work disasters in the season. Firstly, we analyze the cutting edge technology about the index for heat or temperature. Secondly, we review the increasement in casualty in the construction site by the extreme heat in Korea through the Korea Occupational Safety and Health Agency(KOSHA). Thirdly, we analyze the characteristics of construction work factor for considering the CWTI (Construction Work Temperature Index). Lastly, we suggest to develop the basic prototype of the temperature index against construction work disaster in the extreme heat season, as the CWTI. We suggest the CWTI as TI+WI+HI which presents temperature index combined by Weather Index and Human Index. We validate the CWTI with the case study in specific construction site in the Daegu which is the hottest city in Korea. This study can provide the CWTI as a basic framework and tool to reduce the terrible accidents in construction site.

      • 모드스펙트럼해석법에 의한 교량의 내진설계에 관한 연구

        As an importance of seismic design have been increasing, it is necessary to establish an adequate guidelines of which designers can make use for the effective design according to the types of bridges. When applying the codes, seismic design forces are determined by single mode spectral analysis method for the general bridges whose geometric shape is relatively simple, where seismic design forces are obtained only using the fundamental frequency. On the contrary, when the geometeric irregularities such as span length and pier hight are developed, one can expect a substantial errors which could be taken into account in multi-mode spectral analysis. For the determination and comparison of the design forces by mode spectral analysis, the program based on design codes which performs a 3-dimensional dynamic analysis and make it easier for the designers to analyze the reponse displacements of mode shapes by means of the post processor is developed and applied to several example bridges.

      • 단일 및 다중모드 스펙트럼 해석법에 의한 교량의 내진설계 연구

        이 연구에서는 현행 우리나라 도로교 시방서의 내진규정에 부합하는 단일모드 스펙트럼 해석법과 다중모드 스펙트럼 해석법을 연속교에 각각 적용하여 해석하고 그 결과를 비교하였으며, 적절한 적용방법을 제시하였다. 이를 위하여 교량을 공간뼈대 구조물로 모델링 하였고, 교량의 지간과 기둥의 높이를 변화적 규모를 조절함으로써 얻은 다양한 고유주기에 대해서 두 가지 해석방법을 각각 적용하여 해석한 결과를 분석, 비교하였다. 고유주기가 4초 이내의 교량에 있어서 단일모드 스펙트럼해석 결과가 다중모드 스펙트럼해석 결과보다 비교적 타당한 안전측의 결과를 보였으며 고유주기가 4초 이상이 되면 단일모드 스펙트럼해석에 큰 오차의 발생이 예상된다. 또한 교량의 기둥 높이에 대한 지간의 비가 클수록 교축 직각방향보다 진동이 지배적인 것을 알 수 있었다. It can be noted from the results that for the bridges whose natural periods are below 4 seconds, seismic forces by the single mode spectral analysis method are a little higher than that by the multi--mode spectral analysis method, while for the bridges whose natural periods are above 4 seconds it is recommanded to apply multi-mode spectral analysis method.

      • Buckling Analysis of Orthotropic Thin Plates Using The Energy Method

        오늘날 건설현장에서 사용되는 재료들 중에는 균질한 재료와 함께, 각방향에 따라 재료성질이 변화하는 다양한 이방성재료의 사용이 일반적이다. 롤링방향에 따라 상당한 이방성을 나타내는 강판이나 크리스탈 및 콘크리트 등도 이러한 이방성재료에 포함된다. 실제 구조물의 건설시에는 이러한 구조재료의 이방성을 고려한 설계 및 시공이 요구되며, 특히 두께가 얇은 판부재의 경우에는 발생할 수 있는 좌굴문제에 대한 고려가 반드시 필요하다고 할 수 있다. 본 연구에서는 이러한 직교이방성 거형박판에 수직응력, 휨응력, 전단응력 및 수직응력과 휨응력의 조합응력이 작용할 때의 좌굴거동을 해석하기 위하여 대표적인 에너지법인 Galerkin방법을 사용한다. 이 때 수치예제의 거형박판은 다양한 종횡비 a/b를 가지며, 지지조건은 구속-자유, 힌지-힌지, 구속-힌지, 그리고 구속-구속 등의 지지조건을 대상으로 하였다. 외력에 의해 평형상태에 있는 박판요소에 대한 외력일과 변형에너지로 구성되는 총포텐셜에너지에 대한 미분방정식을 유도하고, Galerkin방법에 따라 지지조건을 만족하는 처짐함수를 대입하여 각각의 처짐파라메타에 대한 변분을 취하면 해 즉, 좌굴계수를 얻는다. 이렇게 얻어진 좌굴계수를 Timoshenko의 결과와 비교하였다. 본 연구에서 얻어진 결과는 설계 또는 시공에 있어서의 박판의 좌굴문제에 대하여 다양한 조건들을 적용할 수 있으므로 매우 유용하게 사용될 수 있다고 판단된다.
