      • Correlation between Nasal Obstruction Symptoms and Objective Parameters of Peak Nasal Inspiratory Flow Metry and Acoustic Rhinometry

        Background and Objectives:Measurement of the peak nasal inspiratory flow rate (PNIFR) is a useful technique forobtaining a quick measure of nasal obstruction and changes in PNIFR, reflecting changes in symptoms. The aim of thepresent study was to correlate changes in nasal obstruction symptoms with changes in several parameters of acousticrhinometry (AR) and peak nasal inspiratory flow metry (PNIFM) before and after decongestion and to examine whetherchanges in PNIFR correlate with changes in nasal cross-sectional areas and volume. Materials and Methods:The subjects ofthe current study were 30 patients with nasal obstruction symptoms and 20 normal subjects. Subjective nasal patency wasassessed by visual analogue scale (VAS). We measured PNIFR and minimal cross-sectional area (MCA), cross-sectionalarea at distances of 3.3 (CA3.3), 4.0 (CA4.0), and 6.4 (CA6.4) cm from the nostril and volume from the nostril 6.4cm (V6.4) towards the choanae, in each nasal cavity before and after decongestion. Results:The VAS had no significantcorrelation with PNIFR, each cross-sectional area and volume in bilateral nasal cavities before decongestion. There was asignificant correlation between the changes in VAS and PNIFR and MCA before and after decongestion. There was asignificant correlation between changes in PNIFR and MCA and CA3.3 in one side and both sides of nasal cavity before andafter decongestion. Conclusion:These results suggest that PNIFM and AR may have no sensitive diagnostic values inestimating the severity of nasal obstruction symptoms in the nondecongested state of the bilateral nasal cavities, but PNIFMand AR can be recommended especially in provocation studies because PNIFR and MCA reflect changes in subjectivesymptoms by mucosal changes.

        비강에 발생한 신경내분비성 소세포암 1예

        Small cell neuroendocrine carcinomas of the sinonasal tract are extremely uncomon tumors, and they have been recognized as a highly agresive neoplasm with poor prognosis. The authors experienced a case of small cell neuroendocrine carcinoma of e round or oval and hyperchromatic. Imunohistochemical study showed small cell neuroendocrine carcinoma. The chest X-ray and CT scan of chest showed no evidence of primary lung lesion. The patient received chemotherapy, but died from cerebral infarction and its complication. We report our case with a brief review of the literatures.(Korean J Otolaryngol 2003 ;46 :164-7)

        알레르기 비염 마우스 모델에서 Toll-like Receptor 2의 역할

        Background and Objectives:Mammalian Toll-like receptors (TLR) participate innate immune responses to microbial path-imune responses. The objective of this study is to determine whether TLR 2 stimulation in vivo would modulate subsequent allergic responses in an Aspergillus fumigatus (Af) murine model of allergic rhinitis (AR). Materials and Method:Mice were sensitized via intraperitoneal injection with Af antigen and alum, and received a series of three daily intranasal Af antigen chal-lenge. A TLR 2 agonist, PamCys was administrated intranasally one day before sensitization or one day before the first intranasal Results:PamCys decreased the al-lergen induced nasal recruitment of eosinophils and interleukin (IL)-5 in nasal lavage fluids compared with mice treated with no PamCys. Serologic data revealed that PamCys downregulated Af-specific IgE in the sera of PamCys-treated mice. In addition, spleen cells from the PamCys-treated mice displayed attenuated Af-specific IL-4 and IL-5, but increased interferon (IFN)- and imunosuppressive IL-10. Conclusion:model by shifting T helper 2 (Th2) biased imune parameters towards Th1 dominance. (Korean J Otorhinolaryngol-Head Neck Surg 2007 ;50 :989-95)

        비용종을 동반한 만성 비부비동염 환자에서 혈청 Eosinophil Cationic Protein(ECP)의 임상적 의의

        Eosinophils play a major role in the pathophysiology of chronic rhinosinusitis with nasal polyposis (CRS/NP). The secreted form of eosinophil cationic protein (ECP) is considered to be the marker of eosinophil activity. The aim of this study is to investigate the relationship between serum ECP and CRS/NP. Subjects and Method:A retrospective review of medical records was carried out on 198 patients who had undergone endoscopic sinus surgery from March, 2001 through July, 2006 at our institution. We reviewed blood eosinophil counts, tissue eosinophil, serum ECP, symptom scores, CT scan scores according to the Lund-Mackay system, recurrence of polyp and revision surgery during follow-up. Results:The serum ECP concentration was significantly higher in the CRS/NP patients than in the normal control subjects. The serum ECP concentration was significantly correlated with the blood eosinophil count, CT score and tissue eosinophil count. The serum ECP concentration was significantly higher in patients with recurrence or revision surgery than in patients without recurrence or revision surgery. Conclusion:The results suggest that the level of serum ECP is correlated with the disease severity, and is one of prognostic factors after endoscopic sinus surgery.         일측 전정기관을 제거한 흰쥐의 전정보상에 대한 전기자극의 효과

        Background and Objectives:The effect of electrical stimulation (ES) on vestibular compensation was investigated for 28 days after unilateral labyrinthectomy (Lx) in 24 rats. Materials and Method:The rats were divided into 4 groups and were treated as follows:A) Lx only, B) Lx with ES (0.6 mA) for 2 days, C) Lx with ES (0.6 mA) for 7 days, D) Lx with ES (1.2 mA) for 2 days. ES with pulse wave (10 Hz) was applied to temporal portion bilaterally (8 hr/day). Cathodal currents were transmitted to the electrode on the destructive side, anodal to that on the intact side. Postoperatively, spontaneous nystagmus (SN) was recorded with the video camera and yaw and roll head tilt (YHT, RHT) were measured using photo images. Horizontal vestibulo-ocular reflex (VOR) was evaluated at various frequencies (0.05, 0.1, 0.5, 1, 2, 3 Hz with peak angular velocity of 40°/sec) with the magnetic search coil system. Locomotor movement was monitored with a video camera. Results:The ES groups showed faster compensation in SN, YHT, and RHT than the Lx group. At 3 days after Lx, the ES groups showed significant improvement in the gain over the whole frequency and asymmetry at low frequency compared to that of the Lx group. But the ES group had no effect on compensation of gain and asymmetry on and after 7 days of post-labyrinthectomy. There was no significant difference in vestibular compensation with respect to duration and intensity of ES. ES had no significant effect on the mean velocity and mean deviation of locomotor movements. Conclusion:These results suggest that ES has a favorable effect on the suppression of early static symptoms but has no effect on the compensation of dynamic symptoms after the recovery of resting discharge of the vestibular nuclei neurons.

      • 알레르기성 비염 환자에서의 만성 부비동염 빈도에 대한 임상적 고찰

        Nasal allergy is frequently associated with sinus disease, and it is believed that allergic rhinitis predisposes to sinus disease. The role of allergy in sinusitis is a pertinent yet unresolved issue. Radiological and allergological evaluation were performed on 179 patients, who complained of allergic symptoms, and who didnt have nasal polyp, mechanical and structural nasal obstruction, recent URI symptoms, during the period from August 1990 to August 1992 in Otolaryngologic Department of Kang Nam St. Marys Hospital. The results were as follows 1) Sinusitis was detected in 67 cases among 135 cases(49.6'%) in allergic group and 10 cases among 44 cases(22.7%) in non-allergic group. 2) There was a high prevalence of abnormal sinus X-ray in a population of allergic children. 3) In PNS X-ray findings, bilateral involvement of maxillary sinus was revealed as 47 cases(70.1'%) in allergic group. 4) Total blood eosinophil count was not related to incidence of sinusitis. 5) Wheal size for HD and DF, specific IgE level for DP and DF, eosinophil count in nasal secretion, serum IgE and symptom grading were related to incidence of sinusitis. 6) Specific IgG and IgG for DP and DF were not related to incidence of sinusitis. 7) In the allergic group, sinusitis was much more revealed in the patients who showed the positive skin test to house dust, house dust mite(perennial allergen) than to pollens(seasonal allergen),

        Aspergillus 알레르기 비염 마우스 모델에서의 비만세포의 역할

        Background and Objectives:Mast cells are the main effector cells in the immediate phase of the allergic responses. To evaluatethe role of mast cells on the development of allergen-induced eosinophilic inflammation in a murine model of allergic rhinitis(AR), we compared the responses of mast cell deficient (W/Wv) mice with congenial normal (W/W+) mice. Materials and Method:Mice were sensitized intranasally with an extract of Aspergillus fumigatue (Af) for 3 weeks, and challenged intranasallywith a single dose (acute model) or single dose every day for 5 consecutive days (chronic model). Interleukin (IL)-5 in nasallavage fluids (NLF), histologic examination and Af-specific immunoglobulins (Ig) were examined in W/Wv mice and W/W+mice. Results:In the sensitized W/Wv mice, the number of eosinophils in the nasal mucosa and IL-5 in NLF after repeatedallergen challenge were not significantly different from those of W/W+ mice. In contrast, the number of eosinophils in the nasalmucosa and IL-5 in NLF after single allergen challenge were partially attenuated in W/Wv mice. The levels of Af-specific IgEand IgG1 in both acute and chronic models were not different between W/Wv mice and W/W+ mice. Conclusion:These findingssuggest that eosinophilic inflammation caused by repeated allergen challenge are independent of mast cells in this AR model.(Korean J Otolaryngol 2006;49:912-7)

        이하선에 발생한 타액선관내 유두종 1예

        Intraductal papilloma is a rare benign salivary gland tumor that occurs most commonly in the minor salivary gland. We recently experienced a case of intraductal papilloma arising from the parotid gland in a 24-year-old woman. Radiologic imaging shows a well-circumscribed unicystic mass in the right parotid gland. Superficial parotidectomy was performed. Micoscopically, the mass has papillary epithelial proliferation within the dilated lumen of the salivary duct.
