        Establishment and Selection of Indicator Materials for Cervi Parvum Cornu Pharmacopuncture

        Background: Recently, Cervi Parvum Cornu pharmacopuncture has been widely used. But no studies on the indicator materials for Cervi Parvum Cornu pharmacopuncture have been conducted. The aim of this study was to select indicator materials that would aid in the uniform preparation of standardized Cervi Parvum Cornu pharmacopuncture. Methods: Three lots of Cervi Parvum Cornu pharmacopuncture were analysed. Each lot was prepared using the same methods and materials. Chondroitin sulfate, alanine, and leucine were selected as the indicator materials for Cervi Parvum Cornu. For standardization, chondroitin sulfate analysis was performed using the colorimetric method, while alanine and leucine were analyzed using liquid chromatography–mass spectrometry (LC–MS). Results: Analysis of the three lots of Cervi Parvum Cornu pharmacopuncture found chondroitin sulfate levels of 108.9 ± 17.3 ug/ml, 118.8 ± 5.0 ug/ml and 112.3 ± 11.9 ug/ml. Alanine levels were 44.9 ± 2.8 ug/ml, 44.6 ± 0.3 ug/ml, and 43.9 ± 0.2 ug/ml. Leucine levels were 29.6 ± 0.7 ug/ml, 29.0 ± 0.1 ug/ml, and 29.4 ± 0.1 ug/ml. Conclusion: These results suggest that chondroitin sulfate, alanine, and leucine may be useful for the standardization of Cervi Parvum Cornu pharmacopuncture.

        진교의 파골세포 분화 및 골 흡수 유전자 억제기전 연구

        . Objectives : Osteoporosis is the most common bone disease and osteoporosis fracture is the leading cause of decreased life. Bisphosphonate and selective estrogen receptor modulators are the best choice of treatment for osteoporosis. However, when used for a long time, they increase the probability of side effect such as osteonecrosis of the jaw. Thus, it is crucial to develop alternative medicine to treat osteoporosis. Gentianae Macrophyllae Radix, a herbal medicine, is mainly to treat rheumatoid arthritis. However, the effect of the water extract of Gentianae Macrophyllae Radix (w-GM) on osteoporosis has not been investigated. Thus, we examine whether w-GM can inhibit osteoclast differentiation and bone resorption on receptor activator of nuclear factor kappa-B (NF-κB) ligand (RANKL)-treated RAW 264.7 cells. In this study, RAW 264.7 cells were used as an osteoclast differentiation model by treating them with RANKL. Methods : RAW 264.7 cells were used to determine the effect of w-GM on osteoclast differentiation and bone resorption. The number of tartrate-resistant acid phosphatase (TRAP)-positive cells, TRAP activity and pit formation assay were examined. In addition, protein expressions were measured by western blot and mRNA expressions were analyzed by reverse transcription polymerase chain reaction. Results : Treatment with w-GM inhibited the number of TRAP-positive cells, TRAP activity and pit area. In addition, w-GM decreased protein expression such as mitogenactivated protein kinase, NF-κB, c-Fos and nuclear factor of activated T-cells cytoplasmic 1 (NFATc1). It also inhibited the mRNA levels such as c-Fos, NFATc1, TRAP, NF-κB, calcitonin receptor and cathepsin K in RANKL-treated RAW 264.7 cells. Conclusions : These results suggest that w-GM has inhibitory effects via osteoclast differentiation, thus it could be a new medication for osteoporosis.

        Combination Therapy of Conventional and Korean Medicine for Multiple Vertebral Compression Fractures Associated with Postpartum Osteoporosis: A Case Report

        Multiple vertebral compression fractures are a rare condition and are a serious consequence of postpartum osteoporosis. This report describes the case of a 35-year-old woman who had given birth to her first child 2 months before the onset of pain. Magnetic resonance imaging showed compression fractures of 7 vertebrae. The patient was treated with both conventional, and Korean medicine methods including acupuncture and herbal prescriptions. The patient’s progress was assessed using self-reported symptoms, scale scores and laboratory test results. Her pain was gradually alleviated and biochemical inflammation marker levels improved, but her functional status remained severely impaired. Clinical practitioners who treat women in the postpartum period must be aware of osteoporosis and potential vertebral fractures and need to consider Korean medicine as an alternative therapy to help such patients.

        The Implications of X-ray Use in Chuna Manual Therapy from the Viewpoint of Korean Medicine Doctors

        Background: The purpose of this survey study was to understand how utilization of X-rays as an adjunct to Chuna manual therapy (CT) supports treatment, as assessed by Korean medicine doctors (KMDs). Methods: A survey was emailed to all 18,289 members of the Association of Korean Oriental Medicine(AKOM)to determine the implications of X-ray use in CT. Surveys were collected from September 22, 2017 to October 15, 2017. Results: Of the 18,289 KMDs 562 completed the survey. The implications of a radiological diagnosis (X-ray) with CT was assessed using 5 items in a questionnaire: time to diagnosis, accuracy of treatment, patient comprehension and satisfaction, CT effect, and safety of CT. Survey participants identified improvement in patient comprehension and satisfaction as the most important factor for X-ray use with CT, followed by increased safety of CT. From the determinant factors for selection of CT intensity and specific techniques, severity of clinical symptoms was shown to be the most influential factor. Degenerative changes of the spine and degree of spinal malposition were also reported to be highly influential. Conclusion: The KMDs’ that participated in this study indicated that utilization of X-rays in conjunction with CT administration improved patient comprehension and satisfaction, and CT safety. Installation of radiological equipment in Korean medicine clinics where CT is provided may increase safety and patients’ satisfaction.

        복합부위통증증후군의 침구치료 효과에 대한 연구동향 고찰

        A Review of Research on the Effects of Acupuncture and Moxibustion Treatment to Complex Regional Pain Syndromes Ho Sun Kim1,2,*, Young Hyun Bae1, Hae Sol Kim1, Chang Yong Suh1, No Hyeon Kim1, Kyu Jin Yang1 and Gi Bum Lee1 1Department of Acupuncture and Moxibustion Medicine, Gang-Nam Jaseng Hospital 2Department of Science in Korean Medicine, Graduate School, Kyung Hee University Objectives : This study was performed to review articles concerning acupuncture or moxibustion treatment for Complex Regional Pain Syndromes. Methods : On-line databases including PubMed, Cochrane Library, Ovid-EMBASE, NDSL and OASIS were searched to find articles concerning acupuncture or moxibustion treatment for Complex Regional Pain Syndromes. Several overlapping articles and those not relevant to the topic were excluded, as well as review articles and commentaries. Results : 16 case reports were selected through the procedure, all of which presented successful treatment cases of Complex Regional Pain Syndromes with acupuncture and moxibustion. Most of them presented one single patient case and were published in Korea. However, almost half of them were printed in English, and had been continuously published since 2005. It was remarkable that pharmacopuncture was predominantly used to treat Complex Regional Pain Syndromes. Also, we noted that Chinese scalp acupuncture needs to be studied further as two successful cases were presented from the U.S army. Conclusion : The results show that several case reports have been published regarding Complex Regional Pain Syndromes treated with acupuncture and moxibustion. However, all of them were case reports which are given low status in Evidence-Based Medicine. We expect that a more diverse range of articles, including case control studies, cohort studies and randomized controlled trials will be performed in the near future, and that a unified outcome measure will be developed for Complex Regional Pain Syndromes.

        경항통 및 경추 추간판 탈출증의 한의진료 임상현황조사를 위한 웹기반 설문조사

The aim of this study was to assess current Korean medical treatment practice patterns for neck pain. Methods : A preliminary questionnaire was developed to investigate current practice patterns of neck pain and cervical intervertebral disc displacement (IDD) treatment, and underwent further revision through external review. The final questionnaire was distributed as a webbased survey to 18,289 potential respondents by email. Results : The response rate was 3.34 %. Most participants replied that they received multiple Korean medicine interventions for neck pain treatment consisting of such methods as acupuncture, wet cupping, interferential current therapy and electroacupuncture. A total of 378 respondents acknowledged that diagnostic testing was needed for neck pain treatment. The most commonly used Korean Standard Classification of Diseases (KCD) principal diagnosis code for neck pain was M542, and for cervical IDD was M501. Conclusion : This survey study helps determine current practice patterns of neck pain, and recognizes the need for use of diagnostic devices in neck pain treatment. These results are further anticipated to provide basic data for clinical practice guidelines (CPGs) and future studies using Korean National Health Insurance and Health Insurance Review & Assessment Service data.

        청소년기 요추간판탈출증에 대한 한방 보존적 치료 12례의후향적 분석

        Objectives : This study was performed to assess the effectiveness of conservative Korean traditional medical treatment on lumbar intervertebral disc herniation in 12 adolescents. Methods : Data were collected from adolescent patients diagnosed with lumbar intervertebral herniation by MRI, hospitalized at Jaseng Korean Medicine Hospital from January 1, 2014 to December 31, 2015. A total of 12 patients were included in the study. Patients were treated by acupuncture, pharmacopuncture, electroacupuncture, herbal medicine, chuna manual therapy, physical therapy during hospitalization period. To measure treatment outcomes, a verbal numerical rating scale (VNRS), Oswestry disability index (ODI), range of motion (ROM), Straight leg raising test (SLR), and EQ-5D were used. Results : The average age of the participants was 16.05±1.62. The average of duration of symptom was 19.25±26.83 (weeks) and the average length of hospitalization was 23.8±16.4 (days). 2 patients (17 %) suffered from only lumbar pain, while 10 patients (83 %) suffered from lumbar and leg pain. No participant suffered from only leg pain. 8 patients (67 %) were diagnosed with disc herniation in one segment, and 4 patients (33 %) were diagnosed with disc herniation in multiple segments. A total of 17 disc segments (28 %) were herniated. The most herniated lumbar level was L4-5. 3 segments were diagnosed with bulging (17 %), 6 with protrusion (35 %), and 8 with extrusion (47 %). Extrusion was the most frequent herniated type. After treatment, the average VNRS of lumbar pain significantly decreased from 5.58±1.62 to 2.91±1.56 (p<0.001) and the average VNRS of leg pain significantly decreased from 5.16±2.51 to 3.08±1.8 (p<0.001). ODI significantly decreased from 48.87±18.72 to 28.57±15.05 (p<0.05), and EQ-5D significantly increased 0.58±0.31 to 0.80±0.12 (p<0.05). Range of flexion significantly improved from 61.25±32.62 to 68.33±26.22 (p<0.05). Conclusion : Conservative Korean Traditional Medical Treatment may be effective in the treatment of lumbar intervertebral disc herniation in adolescence. However, more case reports and clinical research are needed.

        경항통에 대한 추나요법의 효과: 체계적 문헌고찰과 메타분석

        Objectives : To determine the effectiveness of Chuna manual therapy for neck pain Methods : We searched 7 electronic databases(OASIS, NDSL, Ovid-MEDLINE, Ovid-EMBASE, Cochrane library, AMED, CNKI) to find all Randomized controlled trials that used Chuna manual therapy as a treatment for neck pain. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results : 7 RCTs met our inclusion criteria. The meta-analysis of 7 studies showed favorable results for the use of Chuna manual therapy. High risk of bias were observed for performance bias and detection bias. Conclusions : Our systematic review found favorable results using Chuna manual therapy for neck pain. But there are several limitations in our study due to lack of well-designed RCT. To obtain stronger evidence, further clinical trials would be needed.

        파킨슨병에 대한 국내 한의학 관련 실험 연구 동향

        Objectives: The purpose of this study is to review experimental studies of Parkinson`s Disease in Korean medical journals, which have been published since 1990. Methods: Experimental studies related to Parkinson`s disease published in Korean medical journals, registered with Korea research foundation, were searched. Articles were grouped into journal of publication, subject, and publication year. Experimental study subjects and models were analysed. Results and Conclusions: Twenty-two articles about experimental studies of Parkinson`s Disease in Korean medical journals were included in our review. There were 13 articles in which herbs were used for treatment, 4 articles in which bee venom was used for treatment, 4 articles in which acupuncture was used for treatment, and 1 article in which pharmacopuncture was used for treatment. Most of the studies used experimental models of MPTP-induced Parkinson`s disease and MTT assay was used to analyse cell toxicity and cell viability. Pole test, rotarod test, Morris water maze test, and forced swimming test were commonly used to analyse MPTP-induced behavioral deficits in animals. Analysis on experimental studies of Parkinson`s disease was categorized into the subjects of dopaminergic cell protective effect, cell-apoptosis protectiveness, anti-inflammatory effect, and anti-oxidative effect.

        비아환(肥兒丸)이 난소적출로 유발된 흰쥐의 골다공증 모델에 미치는 영향

        Objectives Osteoporosis is a common disease in adults with prevalence rate of more than 40%, and occurs more to female than to male. The purpose of this study is to identify the effect of Bia-hwan (Feier-wan) on the osteoporosis of ovariectomized rats. Methods 24 female rats were randomly assigned to a SHAM group, a control group, and a BAH group. Ovaries of the control group and the BAH group were extracted. After than, general animal feed were given to the SHAM group and the control group, animal feed contained BAH were given to the BAH group through mouths of them. After 8 weeks, rats were sacrificed and their weight, calcium, phosphorus, estradiol, total-cholesterol, triglyceride, ALP, albumin, AST, ALT, the weight of femur and tibia ash per body ratio, both area and thickness of trabecular bone, the area of osteoblast and the number of osteoclast were measured. Results The level of calcium, phosphorus, AST in the serum of the BAH group increased significantly compared to the control group. There was no significant difference between the control group and the BAH group in the level of estradiol, total-cholesterol, triglyceride, ALP, albumin, and ALT in the serum. There was no significant difference between the control group and the BAH group in the weight of femur and tibia ash per body ratio. The thickness of trabecular of the BAH group increased significantly compared to the control group, but there was no significant difference in the trabecular area. The area of osteoblast and the number of osteoclast of the BAH group decreased significantly compared to the control group. Conclusions From the results of the above study, oral intake of BAH can inhibit the decrease of calcium, phosphorus in blood and prevent the thinning process of the trabecular by suppressing activation of osteoclast. Thus, BAH may have effects on the treatment and prevention of osteoporosis. (J Korean Med Rehabil 2017;27(2):19-27)
