Long-term Outcomes of Protocol-Based Treatment for Newly Diagnosed Medulloblastoma

Purpose The Korean Society of Pediatric Neuro-Oncology (KSPNO) conducted treatment strategies for children with medulloblastoma (MB) by using alkylating agents for maintenance chemotherapy or tandem high-dose chemotherapy (HDC) with autologous stem cell rescue (ASCR) according to the risk stratification. The purpose of the study was to assess treatment outcomes and complications based on risk-adapted treatment and HDC.Materials and Methods Fifty-nine patients diagnosed with MB were enrolled in this study. Patients in the standard-risk (SR) group received radiotherapy (RT) after surgery and chemotherapy using the KSPNO M051 regimen. Patients in the high-risk (HR) group received two and four chemotherapy cycles according to the KSPNO S081 protocol before and after reduced RT for age following surgery and two cycles of tandem HDC with ASCR consolidation treatment.Results In the SR group, 24 patients showed 5-year event-free survival (EFS) and overall survival (OS) estimates of 86.7% (95% confidence interval [CI], 73.6 to 100) and 95.8% (95% CI, 88.2 to 100), respectively. In the HR group, more infectious complications and mortality occurred during the second HDC than during the first. In the HR group, the 5-year EFS and OS estimates were 65.5% (95% CI, 51.4 to 83.4) and 72.3% (95% CI, 58.4 to 89.6), respectively.Conclusion High intensity of alkylating agents for SR resulted in similar outcomes but with a high incidence of hematologic toxicity. Tandem HDC with ASCR for HR induced favorable EFS and OS estimates compared to those reported previously. However, infectious complications and treatment-related mortalities suggest that a reduced chemotherapy dose is necessary, especially for the second HDC.
Protein-Losing Enteropathy as a Complication of the Ketogenic Diet

The ketogenic diet is an effective treatment for the patients with intractable epilepsy, however, the diet therapy can sometimes be discontinued by complications. Protein–losing enteropathy is a rarely reported serious complication of the ketogenic diet. We present a 16-month-old Down syndrome baby with protein-losing enteropathy during the ketogenic diet as a treatment for West syndrome. He suffered from diarrhea, general edema and hypoalbuminemia which were not controlled by conservative care for over 1 month. Esophagogastroduodenoscopy and stool alpha-1 antitrypsin indicated protein-losing enteropathy. Related symptomswere relieved after cessation of the ketogenic diet. Unexplained hypoalbuminemia combined with edema and diarrhea during ketogenic suggests the possibility of protein-losing enteropathy, and proper evaluation is recommended in order to expeditiouslydetect it and to act accordingly.
BaTiO3에서 Dy2O3 첨가가 결정구조 , 입자성장 및 유전특성에 미치는 영향

The crystal structure, grain growth behavior, and dielectric properties of BaTiO3 have been studied with the addition of Dy2O3. The powders were synthesized at ratios of (100-x)BaTiO3-xDy2O3 (mol%, x = 0, 0.5, 1.0, 2.0) by a conventional solid-state synthesis, and the powder compacts were sintered at 1250°C for 2 hours in air. As the amount of added Dy2O3 was increased, the crystal structure of the sintered samples changed from a tetragonal to a pseudo-cubic structure, and the tetragonality decreased. In addition, a secondary phase of Ba12Dy4.67Ti8O35 appeared when Dy2O3 was added. The average grain size after sintering decreased and abnormal grains appeared as the amount of Dy2O3 increased. It can be explained that the grain growth behavior of the Dy2O3 added-BaTiO3 occurs due to the two-dimensional nucleation and growth, and is governed by the interface reaction. Further, the correlation between crystal structure, microstructure, and dielectric properties was discussed. Dy2O3 첨가량에 따른 BaTiO3의 결정구조, 입자성장 거동 및 유전특성에 대해 연구하였다. 고상합성법으로 (100-x) BaTiO3-xDy2O3(mol%, x=0, 0.5, 1.0, 2.0) 비율로 합성하고, 공기 중 1250°C에서 2시간 동안 소결하였다. Dy2O3가 첨가되면서 소결체의 결정구조는 정방정계 구조에서 입방정계 구조로 전이되어 tetragonality(c/a)가 감소하였다. 또한, Dy2O3가 첨가 시 Ba12Dy4.67Ti8O35은 이차상이 확인되었다. Dy2O3의 첨가량이 증가할수록 소결 후 평균입자의 크기가 감소하고 비정상 입자성장 거동을 보였다. 이를 통해 Dy2O3가 첨가된 BaTiO3의 입자성장은 이차원 핵생성 및 성장에 의해 입자성장이 일어나고 계면 반응이 지배적인 것으로 판단할 수 있다. 또한, 결정구조 및 미세구조와 유전특성과의 상관관계에 대해서 고찰하였다.

The purpose of this study is to investigate how much the purposes of history education emphasized in the national curriculum or history education are realized in the regular examinations for high school students. The author in this study classified the items of the history subject exams that are regularly conducted for all high school students into types of the purposes of history education, identified their characteristics and problems, and presented how to improve the items in schools. The subjects of this study were the items in the midterm exam of the first term of 2008 for the six high schools located in Taegu. The definition and characteristics of educational estimation were established as a estimation method, and the items of six schools were classified by their types based on the estimation categories of history subject provided by the Korea Institute for Curriculum and Evaluation and the sub-categories of historical reasoning that is recently focused on. Meanwhile, the items were classified by the use of subject materials in their composition, and the characteristics of the items in schools were identified when compared to the recent History of Korea Exam with the level of high school students. According to the results of this study, items for asking knowledge of facts and conceptions were excessively shown but items for asking the estimation and attitudes on historical facts were rarely shown. Also, the analysis of the items based on historical reasoning revealed that items for asking reasoning were at sufficiently low rate. As for the review of the items by subject materials, the items by using of materials were of small number. In order to improve those problems, some suggestions may be presented as follows. First, educational purposes and estimation standards in which historical interpretation and reasoning are reestablished and items in which the several reestablished purposes are well-balanced should be provided. Second, items for measuring historical imagination that reveals the characteristics of history most, as subjective questions, and of utilizing various materials should be prepared.

The sensitivity of slot coating process with different sloped upstream die configurations has been investigated using the viscocapillary model for a coating liquid with constant viscosity (Newtonian) through frequency response technique. Amplitude ratios of coating thickness with respect to a sinusoidal disturbance at flow rate are compared under different input frequency and upstream die lip angle conditions. Amplitude ratio values decrease as the upstream die lip angle increases, implying that the coating system is less sensitive to the given disturbance due to the larger space condition in the upstream die region. Amplitude ratio curves obtained from various conditions can be usefully unified in a single one with the help of new dimensionless time between fluid time scale in upstream region and perturbed time scale by input frequency.

The characteristics of corrosion fatigue crack propagation of ALBC3 in 3% NaCl solution have been evaluated for 150rpm, loading frequency of 2.5Hz. The fatigue crack propagation rate in 3% NaCl solution simulated that in air. And it was found that anti-corrosion of fatigue crack propagation of ALBC3 material was good.
