        간경변증과 원발성 간암 환자의 혈중 동과 아연에 관한 연구

        N/A The copper and zinc concentrations were measured in sera of 28 cases of liver cirrhosis, 10 cases of liver cirrhosis with hepatocellular carcinoma, 17 cases of hepatocellular carcinoma and 48 normal control cases by atomic absorption spectrometer in Chosun University Hospital from July 1985 to July 1986. The result were as follows; 1) The mean concentrations of copper in serum of normal control and liver cirrhosis group were 96.03±5. 02, 86.61±7.98 ㎍/dl respectively, but in serum of hepatocellular carcinom a with and without liver cirrhosis the concentrations were each 123.74±13.04 and 127.93± 24.24 ㎍/dl, which were significantly higher than that of normal control group (p<0.001). 2) The mean concentration of zinc in serum of normal control group was 104.81±3.40 ㎍/dl, but in serum of liver cirrhosis, hepatocellular carcinoma with and without liver cirrhosis the concentrations were each 69.03±10.55, 76.01±11.12 and 58.72±12.65 ㎍/dl, which were significantly lower than the former (p<0.001). 3) The ratio of copper to zinc level in serum of normal control group was 0.940.06, but in serum of liver cirrhosis, hepatocellular carcinoma with and without liver cirrhosis the concentration-ratios were each 1.36±0.05, 1.65±0.03 and 2.34±0.59, which were higher than the former. But in serum of hepatocellular carcinoma with and without liver cirrhosis, the ratios of copper to zinc level were significantly higher than that of liver cirrhosis (p < 0,001). Consequently, in hepatocellular carcinoma with and without liver cirrhosis group, the serum copper levels were significantly higher in comparison with that of normal control group. In hepatocellular carcinoma with and without liver cirrhosis group, the serum zinc levels were significantly lower in comparison with that of normal control group. In hepatocellular carcinoma with and without liver cirrhosis, the ratios of copper to zinc level were significantly highter than that of normal control group, These findings demonstrate the usefulness of these parameters as the diagnostic aid in patients with hepatocellular carcinoma.

        정상인에서 도플러 초음파술을 이용하여 측정한 니트로글리세린 투여후 두개내 혈류속도의 변화

        N/A Objectives: With advances in transcranial ultrasound technology, transcranial color-code Doppler ultrasonography provides the anatomical location of basal transcranial vessel and their blood flow. To study of changes of cerebral blood flow, we measured continuously the blood flow velocity of middle cerebral artery by transcranial Doppler(2.5MHz) before and after sublingual administration of 0.6mg. nitroglycerin in twenty healthy young persons. Methods: Pulse rate and systolic and diastolic blood pressure and velocity of percussion(P), tidal (T) and dicrotic(Dc) wave, and diastole(Ds), velocity time integral(VTI), mean velocity(V), pulsatility index(PI) were measured. Results: Heart rate was increased, and systolic blood pressure was decreased significantly two minutes after administration of nitroglycerin. Diameter of outer margin of color Doppler spectrum was increased, and P, T, Ds, VTI and V were decreased significantly after two minutes. PI was increased significantly after two minutes, but returned to baseline level soon. Conclusion: We concluded that dilatation and decreased blood flow velocity of middle cerebral artery were resulted by nitroglycerin.

      • 골수검사를 시행한 철결핍성빈혈의 임상적 고찰

        Iron deficiency is a common one of nutritional deficiency, which denotes a deficit in total body iron. resulting from iron requirements that exceed iron supply. It remains the most common cause of anemia, both in the Korea and world wide. The development of iron deficiency progresses in stage. storage iron depletion occurs first. at this stage, a bone marrow aspirate staine with Prussan blue shows marked reduced or absent deposits of iron in macrophages and accompained by a decreased in the level of serum ferritin. in the next stage, the iron binding capacity of the serum rises, followed by a drop in serum iron. the final stage is the development of microcytic and hypochromic in circulating red cells. Bone marrow examinations is not essential diagnostic modality of iron deficiency anemia (IDA), but is necessary for early definitive diagnosis of IDA. So, We studied 65 cases of IDA patients who were performed bone marrow aspiration and biopsy. and 60 cases of normal healthy control from January 1990 to necember 1993 in Cho-sun University. The results of the analysis were as follows : 1) The total number of 65 patients were 26 males and 39 females. The peak incidence was at ages of 21-30 years and mean ages was 34.8 years. 2) Gastrointestinal bleeding was the most common cause of IDA (gastric ulcer 23.1%, duodenal ulcer 9.2%, gastric cancer 9.2% and hemorrhagic gastritis 4.2 %). 3) Most patients showed medical attention because of general (61.5%), gastrointestinal (58.5%) or cardio-pulmonary (36.9%)symptoms. 4) The mean hemoglobin level was 7.71± 0.83 g/dl, Mean corpuscular volume(MCV) 71.63± 4.56 fL, Red cell volume distribution width(RDW) 17.23±0.72 %, corrected reticulocyte 1.49±0.45%, serum iron 35.07±25.45 ug/dl, TIBC 352.44±43.99 ug/dl and serum ferritin 8.93±8.42 ng/ml. The peripheral red cell morphology was mainly microcyhz and hypochromic ficture(89.2% and 87.7%). Bone marrow iron stians showed that the suitable iron was absent, grade 0 in 92.3% and grade 1 in 7.7% 5) All cases were treated with iron preparation and 26.2% with blood transfusion(from 1 unit to 15 units). 69.2% showed complete recovery with therapeutic maneuvers and 30.1% relapse or no reponse which was probably stemmed from uncorretability(9.2%), uncooperability (9.2%), side effect(3.1%) and replapse of underlying diseases(6.2%). IDA is not definitive diagnosis, but one of the clinical sign of etiologic diseases. So, for the definintive treatment. it is important that the etiologic diseases were corrected.

      • 인슐린 비의존형 당뇨병 환자에서 24시간 혈압 변동 및 요중 알부민 배설량에 관한 연구

        Recently, blood pressure(BP) starts to increase in close relationship to development of persistent microalbuminuria in diabetes. Thus, author studied 24-hour BP changes in diabetics compared to the those in nondiabetics and the changes of urinary albumin excretion (UAE) according to 24-hour BP changes in diabetics with microalbuminuria. Twenty-four-hour ambulatory blood pressure monitoring(ABPPM) was performed in 38 NIDDM patients(18 normotensives and 20 hypertensives) and in 37 control subjects without diabetes(18 normotensives and 19 hypertensives) . Also 24-hour ABPM was performed in 38 diabetics (20 microalbuminurit and 18 normoalbuminuric). The analysis of individual recordings allowed us to detect an impaired circadian pattern(the disappearance of the nocturnal BP decrease or a paradoxical BP increase) in 30% of the normotensive and 31% of the hypertensive diabetics. Twenty-four-hour ABPM(night systolic/diastolic ±SD mmHg) was significantly higher in microabluminuric(124.2 ±11.4/75.1±8.2 mmHg) than in normoalbuminuric subject, (114.0±7.6/64.6±6.6 mmHg, p<0.05). This study suggests that not only awaking BP but also especially day-night minimum SBP decrease are highly responsible for the renal impairment in NIDDM patients, unrelation to hypertension. Thus. ABPM reflects the association between UAE and blood pressure in diabetics more precisely than clinical measurements. and a significant correlation exists between rate of increase in Urinary albumin excretion and blood pressure in diabetics.

      • 결핵치료제인 시판 리팜피신제제의 생체내 이용율

        This study was attempted to investigate the dissolution rate and the bioavailability of commercially available rifampicin preparations after oral administration in nine normal volunteers. Cross-over design was used for the analysis. The dissolution test was conducted in artificial gastric juice and artificial intestinal juice using basket method with four rifampicin preparations (A, B, C, and D) which were pharmaceutically equivalent. The results were as follows; The dissolution rate was higher in the order of A>C>B>D in artificial gastric juice, and A>B>C>D in artificial intestinal juice. Area under the blood concentration time curve (AUC) was larger in order of A>C>B>D. Relative bioavailability(%) of rifampicin preparations were 93.6% for B, 97% for C, 84.4% for D preparation compared with standard A preparation. Peak blood concentration (Cmax) was higer in order of A>B>C>D. Peak time (Tmax) was larger in order of D>C>A>B. Absorption rate constant (Ka) was larger in order of B>A>D>C. there was little difference in elimination rate constant and biological half-life. The correlation of the dissolution rate in artificial gastric juice and area under the blood concentration time curve as well as correlation of the dissolution rate in artifical intestinal juice and peak time showed significant linear relationship. From the result of the this experiments, it can be concluded that for the choice of preparations, bioavailability must be performed in man because the bioavailability of rifampicin preparations showed big differences among commercial preparations which were abvertised as pharmaceutically equivalent, and that the bioavailability of rifampicin preparations in man partially could be predicted from the results of dissolution rate.

      • 대동맥 이첨판과 승모판 이탈증을 동반한 대동맥 판하부 협착 1예

        Discrete subaortic stenosis is a rare cardiac disorder especially in Orientals which occupies eight to ten percent of aortic stenosis, It occurs mainly before age of eighteen and accompanies with aortic valve abnormalities such as aortic stenosis, supravalvrlar stenosis, bicuspid valve and muscular hypertrophic subaortic stenosis, and other cardiac abnormalities such as ventricular septal defect, pulmonary stenosis and coarctation of aorta. There is no report of discrete subaortic stenosis accompanied with mitral valve prolapse. The major complications of discrete subaortic stenosis are aortic insufficiency, and subacute bacterial endocarditis. We report a case of asymptomatic nineteen-year-old male of discrete subaotic stenosis with bicuspid aortic valve and mitral valve prolapse, which is comfirmed by two-dimensional and Doppler echocardiography including transesophaigeal echocardiography.

        Histiocytic medullary reticulosis (HMR) is a malignant noeplastic process, with tumor cells belonging to the histiomonocytic line, and originally described by Scott and Robb-Smith in 1939 characterized by its acute onset, progression to death within a few months and its clinical features are fever, lymphadenopathy, hepatosplenomegaly, jaundice, and purpura common laboratory findings are characterized by severe anemia, leukopenia, thrombocytopenia and histologically by systemic progressive invasion of morphologically atypical histiocytes or of their precursor. 61 and 37-year-old males were admitted with camplaints of high fever and chilling sensation and were treated by cyclophosphamide, vincristine, prednison and antiobiotics, but patients all died 17 and 14 days after admission due to probably sepsis and respiratory failure.

      • 고혈압 허혈성 심질환에서 혈청 Apolipoprotein의 변화에 관한 연구

        The overwhelming evidence that low density lipoprotein (LDL) plays a major role and high density lipoprotein (HDL) plays a preventive role in cardiovascular disease is beyond refutation. Since the protein moiety of HDL and LDL is the most important determinant of its compositional and structural stability, attensions has turned to the estimation of HDL and LDL protein rather than cholesterol as an index of plasma HDL and LDL concentration. We measured and compared the levels of serum apolipoprotein A (apo-A), and apolipoprotein 13 (apo-B), HDL--cholesterol (HDL-C), total cholesterol, and triglyceride in control group and patients with hypertension and ischemic heart disease. The results arc summerized as follows. 1. In hvpetensive patients, the level of apo-A is significantly lower (p < 0. 00l) than control group, other levels shows no significant difference. 2. In patients with ischemic heart disease, the levels of apo-A and HDL-C are significantly lower (p < 0.001, p < 0.005, respectively) and apo-B and apo-B/apo-A are higher (p < 0.005, p < 0.001, respectively) than control group. Other lipid levels shows no significant difference. It seems that apo-A and apo-B/apo-A are more useful than apo-B and HDL-C for identifying ischemic heart disease.
