        선천비진행야맹증의 분류와 다국소망막전위도 소견

        Purpose: To report on the classification of congenital stationary night blindness (CSNB) and multifocal ERG findings. Methods: We studied three patients (case 1-3) with night blindness but normal fundus and one patient (case 4) with decreased vision after trauma with respect to the patients` medical history, familial history, refraction, slit lamp examination, fundus examination, FAG, ERG, and multifocal ERG. Results: The ERG in case 1 showed a decreased rod response, and this patient was diagnosed with Riggs-type CSNB. The typical “negative ERG” was found in all three cases of ERG (except case 1), and these cases were diagnosed with Schubert-Bornschein-type CSNB. Based on Miyake`s classification, case 2 was incomplete, and cases 3 and 4 were complete. The mfERG of the cases with complete Schubert-Bornschein type had delayed implicit times of the first-order kernel and decreased amplitudes of the second-order kernel. The delayed implicit times and the decreased amplitudes of the first- and second-order kernels were found in the cases of Riggs- type and incomplete Schubert-Bornschein-type.

        안와가성종양의 임상양상과 스테로이드 반응성에 대한 분석

        Purpose: To evaluate the clinical features and treatment outcomes of steroid therapy for orbital inflammatory pseudotumor. Methods: Sixty-four patients diagnosed with orbital inflammatory pseudotumor were reviewed retrospectively. Patients with a follow-up period of less than 6 months were excluded from the study. The pseudotumor was classified into myositic, lacrimal, anterior, diffuse, or apical type according to orbital computed tomography findings. All patients were initially treated with systemic corticosteroids and evaluated for response to the treatment. Treatment outcome was considered a “success” if the patient had complete relief of symptoms with no recurrence, and a “failure” if the patient had no or only partial relief of symptoms or showed relapse. Factors affecting the treatment outcome were analyzed. Results: The most frequent lesion subtype was myositis. Periorbital edema was the most common symptom and was evident in 53.1% of the patients. Thirty-eight patients (59.4%) showed treatment success. Age, sex, bilaterality, and mean follow- up length did not correlate with the treatment outcome. A short interval from symptom onset to treatment time and apical subtype were significantly associated with good steroid response (p < 0.05). Conclusions: In orbital inflammatory pseudotumor, myositis was the most common subtype. A short interval from symptom onset to treatment time and apical subtype were associated with good steroid response.

        증식당뇨망막병증에서 수정체초음파유화술 및 인공수정체삽입술을 병행한 유리체절제술의 임상분석

        Purpose: To describe the results and complications of combined pars plana vitrectomy and phacoemulsification with intraocular lens implantation in patients with proliferative diabetic retinopathy, and to analysis the factors to be associated with visual outcome. Methods: Retrospective study was performed on 70 eyes of 58 patients who had undergone combined pars plana vitrecotmy and phacoemulsification with intraocular lens implantation because of proliferative diabetic retinopathy. Preoperative, intraoperative, and postoperative complicated factors to be associated with visual outcome were analyzed. Results: The changes of visual acuity were classified into “improved” 57eyes (81%), “stationary” 7eyes (10%), and “decreased” 6eyes (9%). Posterior capsule rupture during operation was occurred in 2 eyes (2.9%). After cataract, macular edema and neovascular glaucoma were developed in order of incidence of postoperative complications. The factors to be associated with visual outcome were diabetic nephropathy, posterior capsule rupture, macular edema, neovascular glaucoma, and retinal detachment. (P<0.05) Conclusions: Combined pars plana vitrectomy and phacoemulsification with intraocular lens implantation may be relatively safe for the patients with proliferative diabetic retinopathy.

        당뇨황반부종에서 망막두께분석기로 측정한 망막두께와 다국소망막전위도의 반응의 연관성

        Purpose: To assess the correlation between retinal thickness determined with a retinal thickness analyzer (RTA) and the responses of multifocal electroretinogram (mfERG) in patients with diabetic macular edema. Methods: RTA and mfERG were performed in patients with diabetic macular edema (20eyes, 10 patients) and normal subjects (10eyes, 10 patients). The measured retina of RTA and mfERG overlapped based on the anatomical landmarks of the fundus. The center of macula was defined as “Area 1”, and the central 5 degree retina including the center of macula was defined as “Area 2”. The correlations between retinal thickness and the local responses in each area were assessed. Results: The mean retinal thickness of patients with diabetic macular edema was significantly greater than that of normal subject, and the results of mfERG typically represented decreased amplitudes and delayed latencies. In the matching of retinal thickness and local responses in Area 2, amplitudes of N1 & P1 decreased and latency of P1 delayed as the retinal thickness increased. The correlation was prominent in patients with diabetic macular edema. Conclusions: In the matching of RTA and mfERG of patients with diabetic macular edema, retinal thickness and local responses were significantly correlated.

        저장 고춧가루의 색 관련 이화학적 특성과 관능적 특성과의 관계

        본 연구는 장기간 저장하는 동안 고춧가루의 품질변화에 대한 물리적 측정치와 시각적으로 인지하는 관능치와의 상관관계를 규명하고자 하였다. 이를 위하여 빛의 투과 여부가 다른 2종류의 포장재 linear low density polyethylene(LLDPE)과 aluminum/linear low density polyethylene(Al/LLDPE)를 선정하여 저장온도와 기간에 따른 수분함량, 색도, ASTA color value, capsanthin 함량 변화를 측정하고고춧가루의 시각으로 인지하는 색과 기호도에 대한 관능검사를 실시하여 비교분석하였다. 고춧가루의 초기 수분 함량은 저장고의 상대 습도에 의해 영향을 받으며 저장고의 온도나 포장재의 재질에 따른 차이는 나타나지 않았다. 색차계로 측정한 L, a, b값 중 적색도를 나타내는 a값은 저장기간이 늘어날수록 감소하였고 황색도를 나타내는 b값은 증가하였다. 두 측정값 모두 저온보다는 고온에서 저장 시 더 많은 영향을 받았으며 포장재의 재질별로 유의적인 차이는 나타나지 않았다. 또한 ASTA color value와 capsanthin 함량은 저장기간이 길어질수록 감소하였고 고온에서 저장 시 더 유의적인 차이가 있었으며 ASTA color value는 빛이 투과하는 LLDPE 재질의 포장재에 저장하였을 경우 더 많이 감소였다. 관능검사 결과 저장 기간이 길어질수록 황색의 강도는 증가하고 매운맛 색과 적색은 감소하여 점차 어두운 적색을 띄며 고온에서 저장 시 더 어두운 적색을 띄는 것으로 평가되었다. 포장재의 재질에 따른 차이는 매운맛 색에 대한 평가에서만 나타났으며 다른 항목에서는 나타나지 않았다. 전체적인 기호도는 적색과 매운맛 색이 감소하고 황색의 강도가 증가할수록 감소하는 것으로 평가되었다. 색소 성분의 물리적 측정치와 관능적 기호도의 상관관계에 대하여 조사한 결과 a값, ASTA color value, capsanthin 함량은 전체적인 외관기호도와 정의 상관관계가 있으며 그중 ASTA color value가 관능적 기호도와 가장 높은 상관관계가 있었다. 따라서 본 연구 결과 고춧가루 저장 시 빛이 통과하지 않는 Al/LLDPE 재질의 포장재에 저장하였을 경우에 품질 변화가 적었고 색소 성분의 물리적 측정치와 관능적 기호도 간에 높은 상관관계가 있는 것으로 나타났으며 고춧가루의 소비자 선호도는 색도계를 이용한 적색도나 ASTA color value, capsanthin 함량에 의해 결정될 수 있다. The present study investigated the relationship between physical conditions and sensory properties of red pepper powder stored at different temperatures (-20℃, 0℃, 20℃) and in different wrap materials (linear low density polyethylene, LLDPE; aluminum/linear low density polyethylene, Al/LLDPE). Red pepper powders with initial moisture contents of 17.35±0.32% were not affected by the packaging material or storage temperature. Hunter a values of red pepper powder decreased while Hunter b values increased as storage temperature increased. There were no significant differences according to packaging materials. The American Spice Trade Association (ASTA) color value (60.85±0.13) significantly decreased with increasing temperature from -20℃ to 20℃ (21.70±0.06 to 56.03±0.24). The ASTA color value of samples packed with LLDPE (21.70±0.06) decreased more compared to samples packed with other materials. Further, capsanthin contents (13.74±0.02 mg/100 g) significantly decreased with increasing temperature from -20°C (0.25±0.01 to 0.28±0.01 mg/100 g) to 20℃ (0.13±0.01 to 0.25±0.01 mg/100 g). In the sensory evaluation of red pepper powder, overall acceptability was influenced by redness, yellowness, and pungency color. All physicochemical and sensory properties of red pepper powder were compared. Hunter a values, ASTA color values, and capsanthin contents were significantly associated with sensory preferences. In particular, ASTA color value showed a close relationship with color preference for red pepper (R2=0.922). Thus, we suggest that the preference for red pepper powder can be determined based on instrumental measurements of Hunter a values, ASTA color values, and capsanthin contents.

        가시오가피를 이용한 기능성 건강음료의 제조

        본 연구에서는 가시오가피 줄기를 이용하여 농축액을 제조한 후 운동수행능력 증진 효과를 확인하고, 관능검사를 통해 가시오가피의 유용한 기능적인 특성을 살린 기호성 있는 건강 음료를 제조한 후 그 품질 특성을 조사하여 가공식품 및 식품소재로서 가시오가피의 활용도를 높이기 위한 기초 연구 자료로 제시하고자 하였다. 본 실험에서 사용한 건가시오가피 줄기의 일반성분 분석 결과 수분함량 6.50±0.12%, 조단백질 5.89±0.16%, 조지방 1.18±0.11%, 조회분 3.03±0.40%로 나타났다. 건가시오가피 줄기 추출물을 이용하여 제조한 가시오가피 농축액의 DPPH 라디칼 소거능은 증류수로 희석한 10% 농축액에서 87.42±1.63%로 나타나 positive control인 butylated hydroxy anisole (BHA) 1%의 DPPH 라디칼 소거활성 88.65±2.42%와 비슷한 수준으로 높은 활성을 보였으며, 최대운동수행능력을 측정한 결과 가시오가피 농축액을 섭취한 생쥐에서 대조군 생쥐에 비해 농도 의존적으로 운동수행 거리와 시간이 증가하는 것으로 확인되었다. 그리고 관능검사를 통해 선호도가 가장 높은 가시오가피 음료를 제조하여 그 특성을 확인한 결과 당도는 14.9 °Brix, pH는 4.51, 색도는 L값(명도) -62.70, a값(적색도) 3.67, b값(황색도) -6.12로 확인되었다. 따라서 본 연구의 결과는 가시오가피 농축액을 이용한 기호도가 증진된 기능성 음료로의 개발이 가능할 것으로 생각된다. This study was carried out to develop a functional healthy drink using 60% ethanol of dried Acanthopanax senticosus stem extract (ASE). The preparation, physical activity, anti-oxidant activity, and sensory properties of ASE were investigated. The moisture, crude protein, crude lipid, and ash contents of dried ASE were 6.50±0.12%, 5.89±0.16, 1.18±0.11, and 3.03±0.40, respectively. The 1,1-Diphenyl-2- picrylhydrazyl (DPPH) radical scavenging activity was 87.42±1.63% at 1/10 folds diluted ASE. In total, 40 male ICR mice were divided into five groups including the control (PBS), positive control (Red ginseng 200 mg/kg/day), and ASE-treated groups at doses of 35, 70, and 140 mg/kg/day for five weeks, respectively. ASE was administrated orally one time per day for five weeks before treadmill exercises, and normal and positive controls were fed PBS and red ginseng extract. In the treadmill test, ASE-treated mice (140 mg/kg/day) could run 1.4 times longer than the control mice. Healthy drinks were prepared with the addition of ASE at levels of 0.97% or 0.49% (A, B, and C type). Among the healthy drinks, the B type (ASE, 0.97%) was revealed to have the highest level of taste and overall acceptability through a sensory evaluation. The brix and pH of the ASE health drink (B type) were 14.9 and 4.51, respectively. These results indicated that the dried stem of Acanthopanax senticosus could be used as a functional material in the health drink industry.

        RAW264.7 대식세포에서 Heme Oxygenase-1의 유도에 의한 개망초 (Erigeron annuus L.) 꽃 Methanol 추출물의 항염증 효과

        본 연구에서는 LPS에 의해 활성화된 RAW264.7 대식세포에서의 NO 생성 및 iNOS와 HO-1 단백질 발현의 변화를 측정하여 개망초(Erigeron annuus L.) 꽃(EAF) methanol 추출물의 항염증 효과를 확인하였다. RAW264.7 대식세포 에 LPS를 처리한 결과 NO의 함량이 11.48 μM 수준으로 증가하였다. 그러나 EAF methanol 추출물(25, 50, 100, 200 μg/mL)을 처리하였을 때 NO의 함량이 12.82, 9.61, 6.83, 2.52 μM로 농도 의존적으로 감소하였다. 또한 EAF methanol 추출물은 NO의 생성에 관여하는 iNOS 단백질의 발현을 농도 의존적으로 저해하였으며 HO-1 단백질의 발현을 유도하였 다. EAF methanol 추출물에 의한 HO-1 발현이 NO 생성에 미치는 영향에 대해 확인하기 위해 HO-1의 inhibitor인 ZnPP를 사용하였다. 실험결과 ZnPP를 처리하여 HO-1의 발현을 저해하였을 때 추출물에 의해 감소된 NO의 함량이 다시 증가되었다. 본 연구 결과, EAF methanol 추출물은 염증을 유발하는 중요 인자인 NO를 저해하였고, iNOS의 발현을 억제시켰으며 산화적 손상으로부터 세포 보호 방어기작에 관여하는 HO-1의 발현 등 항염증에 우수한 효과를 보였으며 항염증 연구의 기초 자료로 활용될 것으로 예상된다. This study investigated the anti-inflammatory effect of Erigeron annuus L. flower (EAF) methanol extract. We examined the involvement of heme oxygenase-1 (HO-1) in the inhibitory activities of EAF methanol extract on nitric oxide (NO) production in lipopolysaccharide (LPS)-stimulated RAW264.7 macrophages. Cell viability and NO assays were performed. In addition, inducible nitric oxide synthase (iNOS) and HO-1 expressions were detected by Western blotting and blocking HO-1 activity on NO production. EAF methanol extract (25, 50, 100, 200 μg/mL) significantly inhibited LPS-stimulated NO production (p<0.05; 12.82, 9.61, 6.83, 2.52 μM) in a concentration- dependent manner. EAF methanol extract also reduced the expression of iNOS protein. The EAF methanol extract induced the expression of HO-1 in a dose-dependent manner. Blockage of HO-1 activity by zinc protoporphyrin suppressed EAF methanol extract-induced reductions in the production of NO. The present results suggest that EAF methanol extract has a potent anti-inflammatory effect in RAW264.7 macrophages through the induction of HO-1.

        황반부종의 유리체강내 트리암시놀론 주사 후 빛간섭단층촬영에서 나타난 국소 악화

        Purpose: To evaluate the relationship between focal aggravation of the macular edema on retinal thickness change analysis (RTCA) of optical coherence tomography (OCT) and fluorescein angiographic findings in patients after an intravitreal triamcinolone injection (IVTA). Methods: From May 2004 to April 2005, RTCA in a fast macular thickness map (FMTM) of OCT was performed before and 4 weeks after IVTA in patients who had macular edemas caused by diabetic retinopathy and retinal vein occlusion. Patients underwent IVTA, and focal aggravation of the edema was detected. After overlapping the OCT findings onto angiographic pictures, the relationship between the focal aggravation on OCT and the leaking point on angiography was investigated. Result: In the patient group, focal aggravation was found in 5 of 27 eyes (18.5%); in the control group, only 2 of 30 eyes (6.7%) showed focal aggravation. As compared with angiographic pictures, 4 eyes with focal aggravation showed no relation to the leakage on angiography. Conclusions: Focal aggravation of macular edemas after IVTA on RTCA are not related to the leakage on angiography and could be an error from several artifacts.
