A single phase Ba2(Ti8,472Zr0.528)O20 was prepared by the oxalate method from aqueous solution of BaCl2 TiCl4 and ZrOCl2. The structure ananlysis has been carried out by Rietveld analysis method at room temperature. Powder X-ray data of Ba2(Ti8,472Zr0.528)O20 was indexed with the triclinic lattice(a=7.4587 $\AA$ b=14.0672 $\AA$, c=13.3327 $\AA$, $\alpha$=89.87, $\beta$=79.45 ${\gamma}$=84.46) The R (Residual) values of structure refinement were in a range between 10.00 and 8.00 This analysis proved that the added Zr occupied Ti sites in th structure of Ba2Ti9O20. Ba2(Ti8,472Zr0.528)O20 has excellent dielectric properties(dielectric constant K=40.49 at 5.42 GHz Q=4621) so that it can be used as good microwave dielectric materials.

N/A Background : Aneurysms of sinus Valsalva are rare anomalies thought to be primarily congenital in origin. To evaluate the clinical features and surgical results associated with ruptured aneurysm of sinus of Valsalva, we reviewed the clinical characteristics and follow up results of the patients. Methods : From January, 1991, through June 2001, 33 patients with ruptured aneurysm of sinus of Valsalva underwent surgical correction from the above five university hospitals. The patients included 24 men and 9 women, with a mean age of 32±13 years. We reviewed symptoms, physical examination findings, ECG findings, the coexistent cardiac anomalies, echocardiographic findings, surgical results in 33 patients.Results : Patterns of fistula tract were right coronary sinus-right ventricle in 24 patients (73%), noncoronary sinus- right atrium in 3 patients (9%), noncoronary sinus - right ventricle in 3 patients (9%), right coronary sinus - right atrium in 2 patients (6%) and right coronary sinus ?right ventricle and right atrium in 1 patient (3%). Combined cardiac anomalies were VSD (20 cases; 61%), ASD (1 case; 3%) and PDA (1 case; 3%). But, only 12 cases of VSD (60%) were diagnosed with preoperative transthoracic echocardiography. The aneurysmal sacs were excised in all patients and the defects were closed with a patch in 21 patients (64%) and without patch in 12 patients (36%). No recurrence was observed during follow-up periods (68±38 months) for all survivors and there was no late complications or symptoms. Conclusion : In sinus of Valsalva aneurysm patients, combined cardiac anomalies, especially VSD, are common. Careful preoperative echocardiography should be performed to detect combined anomalies. 급성 심근경색 환자에서 예후 예측인자로 혈청 감마-글루타밀 트랜스퍼라제가 유용한가?

목적: 혈청 감마-글루타밀 트랜스퍼라제(GGT)는 관상동맥 죽상경화반 내의 저밀도 지단백(LDL)의 산화과정을 촉매 하여 관상동맥 질환의 진행에 관여하며, 관상동맥 질환의 과거력이 있는 환자에서 심장사와 재경색의 독립적인 예후 예측인자로 알려져 있다. 저자는 관상동맥 질환의 과거력이 없는 급성 관상동맥 증후군 환자에서 예후 예측인자로서의 혈청 GGT의 효용성을 연구하였다. 방법: 흉통을 주소로 응급실을 방문하여 급성 심근경색으로 진단받은 환자의 혈청 GGT 값을 측정하여 응급실 방문당시 혈청 GGT 값이 정상범위(남자: 8-61 U/L; 여자: 5-31 U/L)에 있었던 192명(남/여=143/49, 평균 연령: 60.8±11.8세)의 환자를 대상으로 하여, 추적기간(16.5±10.8개월) 내 심장사건이 재발한 환자에서의 혈청 GGT 값을 심장사건이 없었던 환자의 혈청 GGT 값과 비교 하였다. 결과: 급성 심근경색 환자 192명중 추적 기간 내 17명의 환자에서 심장사와 재경색이 있었으며, 23명의 환자에서 불안정협심증이 있었다. 이 환자들의 혈청 GGT 값을 심장사건이 없었던 환자의 혈청 GGT 값과 비교하였을 때 통계적으로 유의한 차이를 보였다(29.5±10.0 U/L 대 25.0±11.2 U/L p=0.024). 그러나 다변량 분석에서 혈청 GGT에 영향을 줄 수 있는 혼란변수들과 알려진 심혈관계 질환의 위험인자로 보정하였을 경우 독립적인 예후 예측인자가 되지 못했다. 결론: 심질환의 과거력이 없는 심근경색 환자의 예후 예측인자로 심근경색의 급성기에 측정한 혈청 GGT 값은 통계적으로 유의한 차이는 있으나, 독립적인 예후 예측인자가 되지 못했다. Background: Serum gamma-glutamyl transferase activity (GGT) is able to catalyse low-density lipoprotein oxidation in coronary atherosclerotic plaques and has a role in the pathogenesis of atherosclerosis. GGT has been shown to be an independent risk factor for cardiac mortality in patients with a previous myocardial infarction. The purpose of this study is to determine the prognostic value of GGT within its normal range at an acute stage in patients with acute myocardial infarction. Methods: In a retrospective study, GGT and other cardiac risk factors were evaluated in 192 patients (M/F=143/49; mean age: 60.8±11.8 years) who were diagnosed with an acute myocardial infarction at the emergency room. We compared the serum GGT values for each patient with or without a cardiac event, including cardiac death, non-fetal myocardial infarction and unstable angina, after an acute myocardial infarction for a mean follow-up of 16.5±10.8 months. Results: During the follow-up period, 17 patients underwent cardiac death and experienced an acute myocardial infarction and 23 patients had unstable angina. Although the mean GGT values were significantly different from patients with cardiac events (29.5±10.0 U/L vs 25.0±11.2 U/L, p=0.024), serum GGT was not an independent cardiac risk factor for a cardiac event based on multivariate analysis adjusted for age, sex, alcohol and known cardiovascular risk factors. Conclusions: Serum GGT within its normal range at an acute stage in patients that experienced an acute myocardial infarction is not an independent prognostic marker. (Korean J Med 72:281-289, 2007)

N/A Objectives: Tetrofosmin is a new Tc-99m labeled myocardial perfusion imaging agent that shows slow clearance from the myocardium and rapid clearance from background organs. The purpose of this study was to assess the ability of exercise Tc-99m tetrofosmin single photon emission computed tomography(SPECT) to identify and localize coronary artery disease. Methods : The study population consists of 49 patients with suspected coronary artery disease who underwent both exercise Tc-99m tetrofosmin SPECT and coronary angiography within 2 weeks. Using the one day imaging protocol, 7-10mCi of Tc-99m tetrofosmin was injected at rest and 4 hours later, 25 mCi of Tc-99m tetrofosmin was injected at peak exercise. Results: The sensitivity and specificity of exercise Tc-99m tetrofosmin SPECT was 94.9% and 60% for detection of coronary artery disease with luminal narrowing more than 50%, and 96.4% and 33.3% for detection of coronary artery disease with luminal narrowing more than 70%. The sensitivity of exercise Tc-99m tetrofosmin SPECT for detection of individual stenosed vessels(≥50% and ≥70% coronary stenosis) was 79.2%, 100% for LAD, 55.6%, 55.6% for LCx and 85.0%, 92.9% for RCA and the specificity was 88.0%, 75.0% for LAD, 100%, 87.5% for LCx, 86.2%, 77.1% for RCA respectively. Conclusion: Exercise Tc-99m tetrofosmin SPECT appears to be an feasible and useful noninvasive test for the diagnosis of coronary artery disease
Flecainide 독성과 관련된 치명적인 심실상빈맥의 1예

평소 말기 신부전으로 혈액투석을 받고 있는 72세 여자 환자가 폐렴에 의한 심부전의 악화로 입원하였으며 발작성 심방세동의 치료를 위해 flecainide 50 mg 하루 세 번이 투여 되었다. 폐렴 및 심부전의 치료 후 회복되어 퇴원을 고려하던중 flecainide 독성으로 인해 wide QRS complex 형태의 빈맥이 발생하였으며 불응성 심인성 쇼크로 진행하였다. 승압제 등 일반적인 치료로 심인성 쇼크 회복되지 않아 bicarbonate 정주하며 체외막 산소공급 및 혈액 관류 실시하였으며 환자는 합병증 없이 성공적으로 회복되어 퇴원하였다. 저자들은 말기신부전 환자에서 flecainide 독성으로 발생한 불응성 심인성 쇼크를 체외막 산소공급 및 혈액관류로 성공적으로 치료한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Flecainide acetate is a potent class Ic anti-arrhythmic drug with a major sodium channel-blocking effect. Flecainide toxicity can cause myocardial impairment and precipitate circulatory collapse, particularly in patients with renal failure. Electrical and hemodynamic deterioration during flecainide toxicity may not respond to conventional treatments. We report the successful management of flecainide toxicity using extracorporeal membrane oxygenation (ECMO), hemoperfusion, and bicarbonate administration maintaining alkalinity. 목적 : 정량적관상동맥조영술이 최근 널리 보급되고 있지만 국내외를 막론하고 이의 정확도 및 재현도에 관한 연구가 불충분하여 본 연구를 하였다. 대상 및 방법 : 110명의 환자에서 미국 General Electric사의 Advantx L/C/LP 디지탈혈관찰영기와 미국 USCI사의 좌관상동맥용 Judkins형의 Pro-Flo XT^(TM) 6F 도자를 이용하여 좌관상동맥조영술을 같은 각도에서 3회 실시하고 도자의 외경을 일본 Mitutoyo사의 정밀도 0.01㎜의 micrometer로 측정한 후 측정된 도자의 실제 직경과 정량적관상동맥조영술로 측정된 관상동맥의 직경을 비교하였다. 결 과 : USCI® Pro-Flo XT^(TM) 6F 좌관상동맥용 도자의 실제 외경은 1.91±0.03㎜이었고 정량적관상동맥조영술로 측정 한 도자의 외경은 1.94±0.11㎜이었다. 도자를 5.7F로 calibration했을 매 도자의 외경은 1.87±0.12㎜이었다. 세번 다 다른 화상에서 측정한 경우 근위부관상동맥 직경의 차이는 0.02±0.12㎜이었고 같은 화상에서 3회 측정하였던 경우에는 0.11±0.01㎜이었다. 결론 : 정량적관상동맥조영술의 정확도 및 재현도는 사용에 적합한 정도이나 적지 않은 문제점에 대한 보완이 필요하다고 생각된다. During quantitaive coronary angiography(QCA), contrast filled catheter is usually used as a scaling device. However the real diamerer of catheter is almost always smaller than the listed diameter. We measured the real diameter of catheters with precision micrometer(Mitutoyo® Japan). After caliberating the catheter as listed French size and diameter, we did QCA at the same tube position of the diameters of catheter and proximal left coronary artery. All measurements were done 3 times consecutively. The mean real diameter of USCI® Pro-Flo XT^(TM) 6 F left Judkin catheter was 1.91±0.03 ㎜. The mean QCA diamter of the same catheter was 1.94±0.11 ㎜. When we calibrated catheter as 1.91 ㎜. the mean QCA diameter was 1.87±0.12 ㎜. The mean differences of the diameters of the proximal left coronary artery were 0.20±0.12 ㎜ when the measurements were done in three different frames. When the measurements were done in the same frmae, the mean difference decreased to 0.11±0.01 ㎜. We conclude that the precision and reproducibility of QCA are within reasonable range, but more refimement is needed.
