      • 신체형 장애에서의 Testosterone과 통각 역치의 상관관계

        DSM-IV 진단 기준에 의하여 신체화 장애, 미분화 신체형 장애, 그리고 달리 분류되지 않은 신체형 장애여성 환자 13명과 정상 여성 20명을 대상으로 압통에 대한 통각 역치와 혈청 testosterone을 측정하여 다음과 같은 결과를 얻었다. 1) 가변 하중 압통 예민도 측정기 (Variable Weight Pressure Algometer)로 측정한 통각 역치는 환자군이 153.8${\pm}$39.5gm/$0.05mm^2$로서 대조군의 197.5${\pm}$66.7gm/$0.05mm^2$보다 유의하게 낮았다(p<0.05). 2) 혈청 testosterone치는 환자군에서는 0.175${\pm}$0.081ng/ml였고, 대조군에서는 0.174${\pm}$0.108ng/ml로서 양군간에 유의한 차이가 없었다. 3) Testosterone과 통각 역치와의 상관관계를 보면, 환자군에서는 혈청 testosterone치와 통각 역치 사이에 유의한 정적 상관관계가 있었으나(r=0.632, p<0.05, two tailed, Pearson's correlation test), 대조군에서는 유의하지 않았다(r=-0.405). 이상의 결과로 보아 신체화 장애 환자와 정상인 사이에 통각 예민도 조절 기전이 서로 다를 수 있다고 사료 된다. Thirteen female patients of somatization disorder, undifferentiated somatoform disorder and somatoform disorder, NOS, diagnosed by DSM-IV were studied for their pain threshold and serum testosterone and the results were compared with the respective data of 20 control females. The results are as follows : 1) The pain threshold as measured by Variable Weight Pressure Algometer was significantly lower in the patient group(153.8${\pm}$39.5 gm/$0.05mm^2$) as compared to the control group(197.5${\pm}$66.7 gm/$0.05mm^2$)(p<0.05). 2) There was no significant difference of serum testosterone between the patient(0.175${\pm}$0.081 ng/ml) and the control(0.174${\pm}$0.108 ng/ml) groups. 3) A significant positive correlation was noted between the pain threshold and serum testosterone in the patient group(r=0.632, p<0.05, two tailed, Pearson's correlation test), but not in the normal control group(r= -0.405). From these results, it was suggested that the role of testosterone in endogenous pain control system might be more important in somatoform disorders than normals.

        허혈-재관류 : 미세혈관 기능 장애의 기전

        미세혈관의 모든 분절(세동맥, 모세혈관, 세정맥)에 있는 내피세포의 기능은 허혈-재관류(ischemia-reperfusion)에 노출시 장애가 발생하게 되는데, 세동맥에서는 혈관확장 능력이 떨어지고, 모세혈관에서는 체액 여과 및 백혈구에 의한 혈관의 막힘(leukocyte plugging)이 증가되며, 세정맥에서는 백혈구-내피세포 접착(leukocyte-endothelial cell adhesion)과 단백의 혈관 외 누출이 증가한다. 활성 산소(reactive oxygen species)와 nitric oxide의 생산의 불균형은 이들 반응을 촉진시키며, 심혈관 질환의 위험 인자인 hypercholesterolemia, 당뇨병, 고혈압 등은 I/R에 대한 미세혈관 반응을 더욱 악화시킨다.

        펨토초레이저 백내장 수술과 고식적 백내장 수술에서 수정체낭원형절개의 비교

        Purpose: To compare parameters of femtosecond laser and manual continuous curvilinear capsulorhexis (CCC) using anterior segment optical coherence tomography (AS-OCT). Methods: Femtosecond laser cataract surgery was performed in 30 eyes of 30 patients, and conventional cataract surgery with a manual CCC in 30 eyes of 30 patients. One month after surgery, CCC diameters, circularity of capsulotomy, and distance between the CCC and pupil center were analyzed using the AS-OCT. Results: Mean maximal CCC diameter was 4.96 ± 0.23 mm in the femtosecond laser group, and 4.70 ± 0.82 mm in the manual CCC group. Mean minimal CCC diameter was 4.91 ± 0.23 mm in the femtosecond group, and 4.48 ± 0.64 mm in the manual CCC group showing significant difference between the two groups (<EM>p</EM> = 0.000). Circularity of capsulotomy was 0.99 ± 0.01 in the femtosecond group and 0.91 ± 0.13 in the manual CCC group and was statistically different between the 2 groups (<EM>p </EM>= 0.000). The distance from the CCC center to the pupil center was 0.18 ± 0.09 mm in the femtosecond group and 0.26 ± 0.17 mm in the manual CCC group. The CCC center was closer to the pupil center in the femtosecond than in the manual CCC group (<EM>p</EM> = 0.038). Conclusions: Properly sized and shaped capsulotomy can be expected in femtosecond laser CCC compared with manual CCC. J Korean Ophthalmol Soc 2014;55(12):1800-1807

        ME와 CP가 다른 유기사료 급여가 토종닭의 생산능력 계육품질 및 혈액성상에 미치는 영향

        본 연구는 토종닭(한협 3호)에 사육 기간별 대사에너지 및 단백질 수준이 다른 유기사료의 급여가 생산성, 계육품질, 혈액성상에 미치는 영향을 구명하기 위하여 수행하였다. 실험설계는 전기, 중기, 후기에 각각 $2{\times}3$(ME 3,000, 3,100kcal/kg; CP 21, 22, 23%), $3{\times}3$(ME 3,100, 3,150, 3,200kcal/kg; CP 18, 19, 20%), $3{\times}3$(ME 3,100, 3,150, 3,200kcal/kg; CP 15, 16, 17%) 요인실험이며 처리구당 총 공시수 및 반복은 전기에 720수, 4반복, 중기에 702수, 6반복, 후기에 468수, 4반복으로 하였다. 사육 전기 4주간에 사료에너지 수준은 생산성에 영향을 미치지 않았지만 CP는 23% 처리구에서 증체량, 사료섭취량이 21, 22% 처리구보다 매우 증가하였다(P<0.05). 사료요구율은 사료내 단백질 수준이 높을수록 개선되었다(P<0.05). 사육 중기에 증체량은 사료내 에너지가 낮을수록, 단백질은 높을수록 증가하는 경향을 나타내었다. 사료내 에너지 및 단백질 수준은 사료섭취량에 영향을 미치지 않았다. 사료에너지 수준에 따른 사료요구율은 CP 19, 20% 처리구에서 18% 처리구보다 개선되었다(P<0.05). 사육 후기 증체량은 사료에너지와 단백질 수준에 따른 차이가 없었다. 사료섭취량은 ME 3,150, 3,200kcal/kg 처리구에서 ME 3,100 kcal/kg보다 감소하였지만 사료요구율은 개선되었다(P<0.05). 혈중 총단백질, 알부민, 글루코오스, 중성지방은 에너지 및 단백질수준에 따른 차이가 없었다. 혈중 총콜레스테롤은 ME 3,100kcal/kg 처리구에서 ME 3,200kcal/kg 처리구보다 감소하였고 CP 처리구간 차이는 없었다(P<0.05). 가열감량은 ME 3,150kcal/kg 처리구에서 ME 3,200kcal/kg 처리구보다 감소하였다(P<0.05). 수분함량, 전단력, pH은 ME, CP 처리구간 차이를 나타내지 않았다. 단백질용해성은 에너지 및 단백질수준이 높을수록 증가하였다. 계육내 콜라겐(collagen) 함량은 에너지 수준이 높을수록 증가하는 경향이 있다. 계육의 백색도는 사료내 에너지수준이 감소할수록, 단백질수준은 증가할수록 개선되는 경향이 있었다. 적색도는 ME 3,150kcal/kg 처리구에서 ME 3,100kcal/kg 보다 증가하였다(P<0.05). 황색도는 ME 3,100, 3,200kcal/kg 처리구에서 낮아졌으며, CP 15, 17% 처리구에서 CP 16% 처리구보다 매우 낮아졌다(P<0.05). 따라서, 사육기간별 적정 유기사료내 에너지 및 단백질수준은 사육전기 4주간에 ME 3,000kcal/kg, CP 23%, 중기 4주간에 ME 3,100kcal/kg, CP 19%, 후기 2주간에 ME 3,150kcal/kg, CP 16% 급여가 생산성 개선에 적합하였다. This study was conducted to investigate the effect of organic dietary metabolizable energy (ME) and crude protein (CP) on productivity, meat quality and blood composition of crossbred chicks (hanhyub 3 ho) for different growring periods. Experiments were factorially designed with ME 3,000, 3,100kcal/kg and CP 21, 22, 23% for starter (0-4wks); ME 3,100, 3,150, 3,200kcal/kg and CP 18, 19, 20% for grower (5-8wks); ME 3,100, 3,150, 3,200kcal/kg and CP 15, 16, 17% for finisher (9-10wks). The total number of chicks and replicate of each treatment for starter, grower, finisher were 720, 4; 702, 6; 468, 4, respectively. The diets was mixed with more 90% organic feed ingredients. The productivity were not influence by dietary ME for starting period. Weight gain and feed intake were significantly increased in CP 23% treatment than CP 21, 22% treatment (P<0.05). FCR was improved as dietary CP increased (P<0.05). Weight gain was tended to be increased by decreaing ME content and increasing CP contents for growing period. Feed intake showed no difference among the ME and CP treatment groups. FCR improved significantly in CP 19, 20% compared with CP 18% (P<0.05). Weight gain was not significantly different between ME and CP treatments for finishing period. Feed intake was lower in ME 3,150, 3,200kcal/kg than the ME 3,100 kcal/kg treatment (P<0.05). FCR was higher in ME 3,100kcal/kg than ME 3,150, 3,200kcal/kg treatment (P<0.05). Interaction on productivity was not exited between ME and CP treatment groups for different feeding periods. There was no difference in the total protein, albumin, glucose and triglyceride amount in blood depending on dietary ME and CP contents. Total cholesterol was greatly decreased in ME 3,100kcal/kg than the ME 3,200kcal/kg (P<0.05), but not different between CP treatments. Cooking loss of breast meat was lower in ME 3,150kcal/kg than the ME 3,200kcal/kg (P<0.05). Moisture, shear force and pH were not statistically different among treatments. Protein solubility was increased by increasing ME and CP in diets. The collagen was tended to increase as dietary ME increased. Redness was remarkably higher in ME 3,150kcal/kg than the ME 3,100kcal/kg (P<0.05). Yellowness of meat fed ME 3,150kcal/kg showed significantly lower than other treatments (P<0.05). Therefore, the optium ME and CP to improve the productivity for each period were 3,000kcal/kg, 23%; 3,100kcal/kg, 19%; 3,150kcal/kg, CP 16%, repectively.

        수종 지르코니아 세라믹의 굴곡강도에 관한 연구

        Statement of problem: Increasing demand of esthetic restorations made lots of kinds of ceramic materials. Among them, zirconia has been being focused by many dentists. But, mechanical properties of zirconia were still unclear. Purpose : The purposes of this study were to analyze the flexural strength of various zirconia ceramics which had been currently used for clinic i.e., In-Ceram Zirconia(Vita Zahnfabrik, Bad $S\"{a}ckingen$, Germany), Celay Zirconia(Vita Zahnfabrik, Bad $S\"{a}ckingen$, Germany) and CAD/CAM Zirconia (Adens Zi-Ceram. Seoul , Korea). Material and methods: The four point bending test(ASTM Cl161) was used to measure the flexural strength of a specimen before and after circular heat treatment and fatigue loading. Results : 1. The average value of flexural strengths of CAD/CAM Zirconia, Celay Zirconia, In-Ceram Zirconia in dry condition were 806.5 MPa, 669.9 MPa, 605.6 MPa, respectively. There was a statistically significant difference in strength among the types (P<0.05). 2. After thermocycling, the average flexural strengths of CAD/CAM Zirconia, Celay Zirconia, In-Ceram Zirconia were 791.2 MPa, 604.2 MPa, 605.4 MPa, respectively. CAD/CAM Zirconia showed statistically significant higher strength(P<0.05). The others showed no significant difference after thermocycling(P>0.05). 3. After fatigue loading in wet condition. the average flexural strengths of CAD/CAM Zirconia, Celay Zirconia, In-Ceram Zirconia were 806.0 MPa, 674.9 MPa, 601.7 MPa, respectively. There was a significant difference in strength among the types(P<0.05). 4. There was no statistically significant difference in strength of the specimens according to experimental methods except for before and after thermocycling in Celay Zirconia(P>0.05). Conclusion: Besides high esthetic quality, zirconia had sufficiently high mechanical strength.

        AgNWs/Ga-doped ZnO 복합전극 적용 CdSe양자점 기반 투명발광소자

        The silver nanowires (AgNWs) were synthesized by the conventional polyol process, which revealed 25 ㎛ and 30 nm of average length and diameter, respectively. The synthesized AgNWs were applied to the CdSe/CdZnS quantum dot (QD) based transparent light-emitting device (LED). The device using a randomly networked AgNWs electrode had some problems such as the high threshold voltage (for operating the device) due to the random pores from the networked AgNWs. As a method of improvement, a composite electrode was formed by overlaying the ZnO:Ga on the AgNWs network. The device used the composite electrode revealed a low threshold voltage (4.4 Vth) and high current density compared to the AgNWs only electrode device. The brightness and current density of the device using composite electrode were 55.57 cd/㎡ and 41.54 mA/㎠ at the operating voltage of 12.8 V, respectively, while the brightness and current density of the device using (single) AgNWs only were 1.71 cd/㎡ and 2.05 mA/㎠ at the same operating voltage. The transmittance of the device revealed 65 % in a range of visible light. Besides the reliability of the devices was confirmed that the device using the composite electrode revealed 2 times longer lifetime than that of the AgNWs only electrode device.

        정신분열병과 기분장애에서의 수면마비

        목 적 : 한국인들에게 가위눌림이라고 알려진 수면마비는 일반인들이 흔히 경험하는 현상이지만, 주요 정신질환에서의 발생율과 특성에 대한 연구는 드물다. 본 연구의 목적은 정신분열병 및 기분장애 환자와 일반인에서의 수면마비 발생율을 비교하고 그들이 경험하는 수면마비의 임상적 특징을 조사하는데 있다. 방 법 : 2002년 4월부터 9월까지 부산 소재 일 대학병원 정신과 입원 및 외래 환자 중 정신분열병, 양극성 정동장애, 또는 주요 우울증으로 진단 받은 환자들과 부산시내 거주하는 건강한 성인들을 대상으로 가위눌림 경험 설문지, 수면-각성 일정 설문지, 불면증 척도, 졸음 척도 등을 시행하였다. 결 과 : 단독성 수면마비 발생율은 환자군이 41.9%, 대조군이 38.7%이었다. 환자군은 대조군에 비해 중증과 중등도 수면마비의 비율이 높은 반면, 대조군은 환자군에 비해 경도 수면마비의 비율이 높았다(p<0.05). 수면마비의 첫 삽화를 경험한 나이는 두 군 모두 주로 $16{\sim}25$세에 분포하였다. 수면마비의 발생시간은 환자군과 대조군 모두 입면시와 각성시에 비해 수면 중간의 경험(환자군 52.3%, 대조군 63.6%)이 많았다(p<0.001). 환자군의 38.5%와 대조군의 38.2%는 수면마비를 경험하기 전 신체적, 심리적 스트레스가 선행했던 것으로 나타났다. 환각은 수면마비에서 흔히 동반되는 증상이었으며(환자군 90.8%, 대조군 85.5%), 두 군간에 유의한 차이는 없었다. 수면마비시 경험하는 감정으로는 공포(환자군 72.3%, 대조군 80.0%)가 가장 많았다. 결 론 : 정신분열병과 기분장애 환자들의 수면마비 발생율은 일반인들의 그것과 차이가 없었다. 그러나 환자들은 일반인보다 수면마비를 경험하는 횟수가 더 많았다. 수면마비에는 흔히 다양한 환각과 공포가 동반되었다. Objectives: Although sleep paralysis (SP) has been known as one of the symptoms of narcolepsy, recently it has become recognized as occurring frequently in the general population. However, the prevalence of SP and its associated factors in patients with major psychiatric disorders remain unknown. This study investigated the prevalence of SP and a variety of associated experiences in those patients. Methods: The subjects were 160 psychiatric patients and 143 age- and sex-matched controls. The Korean version SP questionnaire as well as the Sleep-Wake Schedule, Epworth Sleepiness Scale and Insomnia Severity Index were administered to all the subjects. The patients were referred from Dong-A University Hospital and consisted of 74 diagnosed as schizophrenia (47.7%), 26 as bipolar disorder (16.8%) and 55 as major depression (35.5%). Results: Nearly 42% of the patient group and 39% of the control group had experienced at least one episode of SP in their lifetime, with no significant difference between the groups. However, the patient group had experienced SP more frequently than the control group. Among all subjects, no gender difference in SP incidence was found. The peak age of onset was in the range of 16-25 years for both groups. Over eight tenths of both groups reported hallucinations and over seven tenths of both groups experienced fear accompanying SP. Conclusion: This study shows that there is no difference in the lifetime prevalence of SP between psychiatric patients and the general population, whereas frequency of SP experience is higher in psychiatric patients. Terrifying hallucinations and fearful feelings frequently accompany SP in both groups.

      • 사행도를 가진 복합단면 하도에서의 홍수파특성

        사행특성을 가진 복합하도에서의 홍수파해석을 위하여 사행도 모형을 개발하였다. 사행도 모형을 지배 방정식, 질량보존 오차 및 홍수파 해석 등을 통하여 기존의 세가지 홍수터 모형과 비교하였다. 사행도 모형에 의한 오차는 1.5-1.8% 정도로 나타났으나 분리하도 모형의 경우에는 1.5, 2.0의 사행도에서 각각 9.1, 27.4%로 큰 오차가 발생하였다. 사행도 모형은 홍수터를 가진 사행하도에서의 홍수전달 특성해석을 위하여 적용되었다. 사행도 모형에 의한 수치실험 결과, 홍수터의 조도와 홍수터의 하폭이 증가할수록, 사행도가 감소할수록 감쇠비와 유하시간비는 증가하였다. 본 모형은 보합하도 단면을 가지며 사행하는 국내 하천에서의 부정류 해석에 적용성이 클 것으로 판단되었다. The sinuosity model has been developed to simulate to developed to simulate the floodwave in meandering channels by solving the extended Saint-Venant equation with the Preissmann scheme. The suggested model is compared with three conventional floodplain routing methods in terms of governing equations, mass conservation error and floodwave analysis. The sinuosity model produces the mass conservation error of 1.5-1.8%, however the separate channel model produces 9.1% and 27.4% for sinuosity of 1.5 and 2.0, respectively. The model has been used to simulate flow in an idealized meandering river with a floodplain. The attenuation ratio and the travel time ratio are found to increase as the floodplain roughness and width increase and as the sinuosity factor decreases. The model is expected to contribute the floodwave analysis in sinuous channel with compound corss sections.
