        Recently, railway has been pointed to the efficiency of transportation, rapid transit, and comfortable train ride. the construction of railway near the downtown area and station building are increasing for maximization of utilization and convenience. but the heavy of transportation and rapid transit lead to increase noise and vibration. the noise and vibration of railway may cause the civil appeal, decline in the serviceability and insufficiency of environmental standard. Therefore, the solution to avoid environmental matters becomes one of the most important factor in constructing railway. In this study, analysis fatigue stress joint of low vibration floating slab-anti vibration equipment is carried.

        Recently the construction of railway sections passing the central area of cities and stations under railway lines are increasing, and then it is urgently required to take the countermeasures against the railway vibration and the second-phase noise radiated from it. The most efficient countermeasure, out of technologies developed up to now, is the floating slab track which is the track system isolated from the sub-structure by springs. In other countries, the source technologies for anti-vibration design and vibration isolator - one of key components - have been developed and many installation experiences have been accumulated. However, in Korea, since the system design technology and technologies for key components are not yet developed, the foreign system are being introduced without any adjustment, and the key component, vibration isolator, depends on imports. Thus, the rate of technology self-support is very low. Therefore, to achieve the competitiveness in domestic and overseas markets, the core technologies of floating slab track and its components should be developed immediately.

        체외수정 환자에서의 난포액 및 혈중 혈관 내피 성장인자

        목적: 난포생성에서 황체기능의 작용까지 여성생식과정의 여러 단계가 혈관생성에 관련되어 있다. 본 연구는 체외수정을 시행하는 환자에서 혈액 및 난포액내 VEGF (Vascular endothelial growth factor) 농도를 측정하고자 하였다. 연구 방법: 2003년 8월부터 2005년 7월까지 본원 불임클리닉에서 체외수정시술을 받은 29명의 환자를 대상으로 하였으며 과배란 유도는 GnRH agonist를 이용한 단기투여법 (short protocol)을 사용하였다. 난자 채취시에 혈액과 난포액을 채취하여 -60℃에서 보관하였다가 효소면역 정량법 (ELISA, enzyme-linked immunosorbent assay)으로 혈청과 난포액의 VEGF 농도를 측정하였다. 결과: 총 29명의 환자중 10명의 환자에서 임신이 되었다 (34.5%). 난포액내 VEGF 농도와 혈청내 VEGF 농도 및 연령 사이에는 상관계수 0.428로 유의한 양의 상관관계를 보였고, 난포액내 VEGF 농도와 난포수 및 채취된 난자수 사이에는 상관계수 -0.493로 유의한 음의 상관관계가 나타냈다. 하지만 난포액내 VEGF 농도와 혈청내 VEGF 농도는 상관관계를 보이지 않았다. 대상 여성 중 임신군과 비임신군을 비교하였을 때 난포액내 VEGF 농도는 비임신군에서 1468.38±727.33으로 임신군의 676.48±542.07에 비해 유의하게 높게 나타났다. 결론: 본 연구의 결과를 바탕으로 황체기에서의 호르몬 생성부전이나 난포액내 부적절한 환경으로 인하여 상승된 VEGF의 농도가 체외수정 (IVF-ET)의 결과와 연관되어 있는 것으로 사료된다. Objective: Several aspects of female reproduction, from folliculogenesis to corpus luteum function, are related to angiogenesis. The purpose of this study is to measure the concentrations of vascular endothelial growth factor (VEGF) in follicular fluid and serum in patients during In Vitro Fertilization - Embryo Transfer (IVF-ET) cycles. Methods: In our prospective study, twenty-nine patients who underwent in vitro fertilization by GnRH agonist short protocol were assessed at the our infertility clinic from Aug. 2003 to July 2005. Serum VEGF and follicular fluid VEGF levels were measured in all patients at the time of oocytes retrieval. The assay technique used in this study was ELISA for serum and follicular fluid VEGF. Results: Of 29 cycles, 10 cycles were pregnant (34.5%). A positive correlation existed for follicular fluid VEGF and chronologic age (r=0.428, p-value=0.021). Follicular fluid VEGF concentration showed an inverse relationship with the total number of oocytes retrieved and follicles (r=-0.493, p-value=0.007; r=-0.474, p-value=0.009). But there was no statistically significant relationship between follicular fluid VEGF concentration and serum VEGF concentration (ρ=0.347). Follicular fluid VEGF concentration was significantly higher in the non-pregnant group (1468.38±727.33 pg/mL) compared to the pregnant group (676.48±542.07 pg/mL) (p-value=0.003). Conclusion: Our data provide some of the evidences that elevated VEGF concentrations in the follicular fluid are associated with poor conception rates in the IVF-ET cycles.

        Rupture of teratoma is rare. Ovary cystic teratoma was occurred 95% in benign ovary tumor, but chemical peritonitis arising from ruptured cystic teratoma is a scarce case. Peritoneal tuberculosis is also rare disease nowadays. It causes easily fatigue, abdominal distension, intermittent abdominal pain and ascities insidiously. We have recently experienced a case of chemical peritonits from ruptured both ovary cystic teratoma of ovary with superimposed peritoneal tuberculosis in 44 year-old woman and report our scarce case with a brief review of the literature.

        자궁경부암에서의 COX-2 와 VEGF 발현에 관한 연구

        목적 : 자궁 경부암을 포함하여 여러 악성 종양에서 cyclooxygenase-2 (COX-2)의 과발현이 종양형성의 단계에 관련 있다고 보고되고 있다. COX는 prostaglandin형성에 관여하는 효소인데 두 도가지 이성체가 알려져 있다. 평소에는 정상조직에서는 거의 발현되지 않는 혈관신생과 관련하여 종양형성에 관여하는 COX-2와 과발현의 유발은 조직의 저산소증에 의해 조절되는 것으로 알려져 있으며, 이런 저산소증은 vascular endothelial growth factor (VEGF)의 발현을 유발한다고 한다. 이에 저자들은 자궁경부암중 침윤성암 25예, 미세침윤성암 7예, 그리고 상피내암종 9예의 조직에서 COX-2 및 VEGF의 과발현과 미세혈관밀도의 상관관계를 알아보고자 하였다. 연구 방법 : 1999년 1월부터 2001년 12월까지 경희대학교 산부인과에서 자궁경부암으로 진단받은 41예와 정상자궁경부 조직 4예를 대상으로 COX-2 단클론성 항체와 Rabbit anti-human VEGF 항체를 이용하여 면역조직 화학 염색을 시행하였다. 또한 factor Ⅷ 항체를 이용하여 미세혈관밀도를 측정하였다. COX-2와 VEGF의 면역조직화학적 염색결과는 면역반응의 염색 강도와 염색된 세포의 수에 따라 음성 (0), 약양성 (light staining, 1), 중간양성 (moderate staining, 2), 강양성 (heavy staining, 3)으로 반정량적 판독을 하였다. 결과 : COX-2 면역반응은 상피세포 기저막과 암 침윤부위가 특히 강하게 발현되는 것을 볼 수 있었다. 그리고 상피내암종, 미세침윤성 자궁경부암, 침윤성 자궁경부암으로 갈수록 강양성의 반응도가 증가하는 것을 관찰할 수 있었고 (p=0.006), VEGF 면역반응 역시 암이 진행할수록 양성의 빈도가 증가하는 경향을 확인하였다 (p=0.079). 이들 COX-2 면역반응과 VEGF 면역반응은 상관관계가 있는 것으로 확인되었다 (r_(s)=0.686, p<0.01). 또한 이들의 면역반응은 미세혈관 밀도와 상관관계를 보이는 것으로 관찰되었다. 결론 : COX-2와 VEGF의 발현이 자궁경부 종양의 발생초기 단계에 중요한 상호역할을 하는 것으로 사료된다. Objective : The purpose of this study was to know the potential of growth, invasion and metastasis of uterine cervical cancer associated with neovascularization and to investigate whether the enhanced COX-2 (Cyclooxygenase-2) and VEGF (Vascular Endothelial Growth Factor) expression is showed through CIS to invasive cervical cancer and to examine that COX-2 and VEGF expressions are associated with prognostically worse pathological variables and had a direct correlation with tumor MVD (microvessel density). Methods : From January 1999 to December 2001, 45 paraffin-embedded cervical specimens were obtained by surgical resection in the Kyung Hee University Medical Center. The study group included 25 cases in invasive squamous carcinomas, 7 cases in microinvasive carcinoma, 9 cases in carcinoma in situ, 4 cases in the benign uterine diseases. All tissues were subjected to immunohistochemical staining for COX-2, VEGF and microvessel density (MVD), and immunostaining was scored using a four-point scale with (0) indicating no expression, (1) light staining, (2) moderate staining, (3) heavy staining. Results : As the cervical tumorigenesis progressed, there was a significant increase of MVC (microvessel count) and COX-2 expression. There is a significant correlation between COX-2 and VEGF expression (r_(s)=0.686, p<0.01) in cervical neoplasms. There was no correlation between COX-2, VEGF expression and clinicopathologic factors in cervical cancer. Conclusion : These findings provide evidence that the expression of COX-2 and VEGF is involved in the promotion of angiogenesis in cervical neoplasms and plays an important role in early invasion.

        골밀도 평가방법과 임플란트 안정성 변화의 관찰

        Purpose: The aim of this study was to examine the correlation of the subjective and the objective evaluation of edentulous ridge bone quality, and to evaluate the change of the dental implant stability in each bone density group for early healing period after implant installation. Methods: Sixty-seven implants(Osstem implant$^{(R)}$, Seoul, Korea) were included in this study. We evaluated the bone density by 2 methods. The one was the subjective method which was determined by practitioner s tactile sense, the other was the objective bone type was based on Hounsfield units. The implant stability in each bone type group was assessed by resonance frequency analyzer(Osstell mentor$^{(R)}$). Data were analyzed for the change of the implant stability, and they were compared to verify the difference of groups at the time of installation, 2, 6, 10, 14 weeks postoperatively. Spearman's correlation was used to demonstrate the correlation between the subjective and the objective evaluation of the bone density, and analysis of variance(ANOVA) was used to analyze the differences of implant stability at each time point. Results: There was no close relation between the subjective and the objective evaluation of the bone density(r=0.57). In the subjective groups, there was statistically significant difference between the type 1 and 3 at 10 weeks and between the type 2 and 3 at 14 weeks. In the objective groups, there was no statistically significant difference between the D 1, 2, 3, 4, and 5 group with regard to RFA from baseline to 14 weeks(P>0.1). Conclusions: The implant stability increased over time during the study, and it was improved with bone density proportionally after 2weeks postoperatively. It is recommended that the decision of bone density is base on Hounsfield unit for implant loading time.
