Purpose: To report a single case of surgical repair of the canalicular defects and congenital eyelid colobomas associated with Tessier No. 3 craniofacial cleft. Case summary: A one-month-old girl presented with eyelid colobomas and discharges from the eyes. The patient was diagnosed with a Tessier No. 3 craniofacial cleft with bilateral lower eyelid colobomas medial to the puncta. At the age of 55 months, examination under general anesthesia revealed mid-canalicular obstructions in both lower canaliculi. After pentagonal excision of eyelid colobomas in the left upper and both lower eyelids, both ends of the canaliculi were found at the cut edge of the lower eyelids. After the repair of canalicular defects and bilateral nasolacrimal duct silicone tube intubation, the primary closure of the eyelid defect was performed layer by layer. Although there was no subjective improvement of epiphora in the left eye, a subjective improvement of epiphora in the right eye was achieved, and tear meniscus height in the right eye was halved. Additionally, the eyelid colobomas were cosmetically well repaired at postoperative 6 weeks. The patient still had mild tearing symptoms, but did not complain any longer of discharge at postoperative 4 months. Conclusions: Tessier No. 3 craniofacial cleft with eyelid colobomas can be associated with canalicular defects and nasolacrimal duct obstructions. Surgical repair of the canalicular defects associated with eyelid colobomas should be considered to achieve a functional recovery of the lacrimal drainage system. 신경병증성 통증 환자에서 지속적 진통을 위한 5% 리도카인의 사용

Background: The use of neurolytic agents to control neuropathic pain has been described from the last century Phenol and ethyl alcohol have been widely used as neurolytic agents, however, their neurolytic effect is variable in efficacy and duration of action, and infrequently accompanied with grave complications. It has been found that 5% lidocaine causes irreversible conduction blockade in animal studies. The goal of this study was to evaluate the neurolytic effect of 5%o lidocaine on various neuropathic pain syndromes for prolonged analgesia. Methods: Twenty-five patients with a diagnosis of neuropathic pain including trigeminal neuralgia (n = 7), postherpetic neuralgia (n = 10), and postsurgical neuralgia (n = 8) were selected after failure of routine therapeutic regimens. After performing a diagnostic nerve block with 1% lidocaine and 5% lidocaine was injected. The patients were followed for 6 months. Visual analog scale (VAS) scores and side effects were recorded for each patients. Results: A significant decrease in pain scores after neurolytic blockade with 5% lidocaine was seen in all of three pain groups. All the patients reported immediate and prolonged pain relief lasting from 4 weeks to 6 months. None of patients exhibited any appreciable side effects or complications. Conclusions: We suggest that 5% lidocaine may be used safely and effectively for the purpose of prolonged analgesia in selected patients with intractable neuropathic pain syndromes.

A 43-year-old male was admitted for reconstruction surgery to repair posterolateral rotatory instability of the knee. At the request of the surgeon, ranitidine was slowly administered intravenously immediately before the patient was transferred to the operating room to reduce the incidence of acid reflux. A few minutes later, during the transfer of the patient, anaphylaxis developed with hypotension, nausea, dyspnea, change in consciousness and urticaria. The patient had no previous history of any allergic tendency. After appropriate management, he recovered without complication. Following surgery, an allergic skin prick test was performed to determine if the patient was allergic to any of the drugs used during the surgery. According to the results, only ranitidine showed a positive reaction. (Korean J Anesthesiol 2009;56:79~82)

터널 시공 중 막장 매핑은 터널의 위험도 판단과 향후 지보패턴 결정에 중요한 역할을 한다. 본 연구에서는 막장관찰 기록지를 이용한 기존의 수기 매핑 방식을 탈피하여 모바일을 이용한 디지털 매핑 시스템을 제시하였다. 현장에서 디바이스를 기반으로 작성된 매핑 자료는 메인서버와 연동되어 자동으로 데이터베이스화가 되며 담당자 및 관련전문가들 사이에서 실시간 피드백이 가능하도록 구현하였다. 매핑의 정밀한 표현 방식 및 다량의 데이터 누적에 의한 속도 저하 문제는 Douglas-Peucker 알고리즘 등을 이용하여 해결하고자 하였다. 본 시스템은 향후 다양한 현장 검증 및 추가적인 기능 개선 등을 통해 완성도가 높아질 것으로 기대된다. Tunnel face mapping plays an important role in risk analysis and infrastructure support decisions during tunnel construction. In this study, a digital mapping system using a mobile device is employed instead of existing facemapping methods that rely upon face mapping sheets. The mobile device is then connected to the main server in the field, where a tunnel-specific database is compiled automatically. This information provides real-time feedback on the tunnel face to construction personnel and engineers, thus allowing for rapid assessment of tunnel face stability and infrastructure needs. The Douglas-Peucker algorithm, among others, is employed to resolve problems arising from the detailed mapping and speed problem by data accumulation. This system is expected to raise program optimization through field verification and additional functional improvements.

오늘날 인공지능(Artificial Intelligence, AI) 분야에서 가장 구현하기 어려운 분야 중 하나는 추론이다. 근래 추론 분야에서 영상과 언어가 결합한 다중 모드(Multi-modal) 환경에서 영상 기반의 질의 응답(Visual Question Answering, VQA) 과업에 대한 AI 모델이 발표됐다. 얼마 지나지 않아 VQA 모델의 성능을 개선한 GVQA(Grounded Visual Question Answering) 모델도 발표됐다. 하지만 아직 GVQA 모델도 완벽한 성능을 내진 못한다. 본 논문에서는 GVQA 모델의 성능 개선을 위해 VCC(Visual Concept Classifier) 모델을 ViT-G(Vision Transformer-Giant)/14로 변경하고, ACP(Answer Cluster Predictor) 모델을 GPT(Generative Pretrained Transformer)-3으로 변경한다. 이와 같은 방법들은 성능을 개선하는 데 큰 도움이 될 수 있다고 사료된다.
GAN을 이용한 달의 영구 그림자 영역 관찰에 관한 연구

일본 우주항공연구개발기구(Japan Aerospace Exploration Agency, JAXA)는 2007년부터 2017년까지 달 탐사선 셀레네(Selenological and Engineering Explorer, SelEnE)가 관측한 데이터를 수집하고, 연구했다. JAXA는 지구 상층 대기에 존재하는 산소가 자기장의 꼬리 부분에 실려 달로 이동한다는 사실을 발견했다. 하지만 이 연구는 아직 진행 중이며 달의 산화 과정 규명에 추가 연구가 필요하다. 본 논문에서는 생성적 적대 신경망(Generative Adversarial Networks, GAN)으로 달 분화구의 영구 그림자 영역을 제거하고, 물과 얼음을 발견하여 선행 연구의 완성도를 향상하고자 한다. 실험에 사용할 모델은 CIPS(Conditionally Independent Pixel Synthesis)다. CIPS는 실제 같은 영상을 고해상도로 합성한다. 합성할 데이터의 최적인 가중치 초기화 및 파라미터 갱신 방법, 활성 함수 조합은 실험을 통해 확인한다. 필요에 따라 앙상블 학습을 할 수도 있다. 성능평가는 FID(Frechet Inception Distance), 정밀도, 재현율을 사용한다. 제안한 방법은 진행 중인 연구의 시간과 비용을 절약하고, 인과관계를 더욱 명확히 밝히는 데 도움 될 수 있다고 사료된다.

In this study, an effective method for the non-linear system modeling using the synergy effect between neuro-fuzzy and genetic algorithms(GAs) is proposed. The GA is used for selecting significant input variables in building neuro-fuzzy System, ANFIS (Adaptive Neuro-Fuzzy Inference System), in order to effectively build nonlinear system models. For verification of validity, the proposed method is applied to Box-Jenkins furnace problem and the Mackey-Glass chaotic problem. The both experiment results are presented to demonstrate the effectiveness of the proposed method and the comparison with other methods.

N/A There have been many conflicts concerning the mechanism of exercise limitation in chronic obstructive pulmonary disease (COPD) and mitral stenosis(MS). We performed exercise test with incremental cycle ergometer and measured ventilatory response and heart rate response in 10 normal controls, 10 COPD patients, and 10 MS patients. The results were as follows: (1) COPD and MS patients showed decreased maximal exercise capacity (67±16%, 54±4%, of maximal predicted oxygen uptake) compared with the control group (88±11%). (2) COPD patients showed much higher maximal minute ventilation, lower breathing reserve (100±17% of maximum predicted value, 7±6%), and normal anaerobic threshold (45±7% of maximal predicted oxygen uptake) compared with the MS patients (51±10%, 50±8% and 33±4%) and controls (59±15%, 46±125, and 56±9%), which indicated an exercise limitation due to an abnormality in the respiratory system. (3) The MS patients showed loawer anaerobic threshold, lower maximal minute ventilation, and normal breathing reserve compared with the COPD patients and normal controls, which indicate an exercise limitation due to an abnormality in the cardiovascular system. With the above results, it is thought that an incremental cycle ergometer exercise test is useful in diagnosing the limitation factor of maximal exercise in respiratory and cardiovascular system disease.
