E-2020 아프리카 사례를 중심으로 본 국경 말라리아 차단의 중요성

This study aims to analyze the reasons for the difficulties that African countries face with malaria elimination, despite the improvement of medical treatment and technology. Precedent studies on causes and resolutions of continuous malaria issues within African regions have been conducted with the focus of climatic environmental and unstable socioeconomic features of Africa. However, not many studies have paid attention to Africa’s high country density, which is a key variable in explaining the spread of infectious diseases. This article seeks for effective malaria eradication resolution and presents a useful policy alternative by focusing on the border malaria issues caused by the high country density. This study uses qualitative analysis method through a comparative analysis on E-2020 African countries, which had been selected by the WHO in 2016 for their potentials for malaria elimination. As border malaria issues have huge influence on African countries’ successful malaria elimination, how efficiently border malaria has been blocked will also be an important resolution. Strengthened border surveillance through public and private cooperation is expected to contribute to preventing inflow of malaria through borders. In addition, trans-border cooperation and international efforts are as critical as an individual effort of a country in successful malaria elimination. 본 연구는 말라리아 퇴치 관련 치료 및 기술 발전과 청정국의 증가에도 많은 아프리카 국가들이 여전히 말라리아 박멸에 어려움을 겪고 있는 원인을 분석하고자 한다. 아프리카 내 말라리아 박멸 실패 원인과 해결방안에 관한 기존 연구 대부분은 아프리카의 기후 환경적 특성과 불안정한 사회경제를 중심으로 분석하였다. 그러나 전염병의 확산을 설명하는 주요 변수인 높은 국가 간 밀집도를 고려한 연구는 많지 않다. 본 연구는 이러한 특성으로 인한 국경 유입 말라리아를 중심으로 아프리카 지역의 효과적인 말라리아 박멸 방안을 모색하고 유용한 정책적 대안을 제시한다. 이를 위해 세계보건기구가 2016년 말라리아 박멸 가능성이 높은 국가들로 언급한 E-2020 그룹 중에서 아프리카 지역 내 국가들을 주요 비교사례로 활용한 질적 연구 방법론을 사용하였다. E-2020 목표 달성 국가인 알제리, 미달성 국가인 카보베르데와 남아프리카 공화국은 모두 말라리아 박멸 과정 중 외부로부터 유 입되어 들어오는 국경 말라리아 문제로 인해 피해를 입었다는 공통점이 있다. 국경 말라리아를 효율적으로 차단할 수 있는지 여부는 아프리카 국가들의 성공적인 말라리아 박멸에 주요한 영향을 미친다. 본 연구는 국경 말라리아 대처에 대한 방안으로 인접 국가들의 협력을 통한 국경 감시 강화로 국내로 유입되는 말라리아를 효과적으로 예방할 수 있을 것으로 기대한다. 또한 장기적인 초국경 협력과 국제사회의 국제보건협력 노력이 필수적이다.
회음혈 봉약침 치료와 팔료혈 자침을 통한 만성전립선염/만성골반통증 증후군 치험 1례

Objectives: The purpose of this study was to report the effectiveness of Hoeeum Bee-venom Therapy and Balio Acupuncture Treatment in the management of chronic prostatitis/chronic pelvic pain syndrome. Methods: A patient with chronic prostatitis/chronic pelvic pain syndrome underwent Korean medical treatment, including Hoeeum Bee-venom Therapy and Balio Acupuncture Treatment. The severity of symptoms was assessed with a daily visual analog scale (VAS) for orchialgia and lower abdominal pain. In addition, we measured the National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) on the first and last days of visit. Results: The patient's symptoms were improved after treatment. The severity of orchialgia was reduced from VAS7 to VAS2, and lower abdominal pain was improved from VAS6 to VAS1. NIH-CPSI decreased from 21 to 5. Conclusion: Hoeeum Bee-venom Therapy and Balio Acupuncture Treatment are effective in treating chronic prostatitis/chronic pelvic pain syndrome.

Purpose: Melanoma-associated antigen C2 (MAGEC2) is an oncogene associated with various types of cancers. However, the biological function of MAGEC2 in circulating tumor cells remains unclear. In this study, we investigated the role of MAGEC2 using adapted suspension cells (ASCs), which were previously developed to study circulating tumor cells (CTCs). Methods: Differential gene expression in adherent cells (ADs) and ASCs was examined using RNA-seq analysis. MAGEC2 expression was assessed using reverse transcription quantitative polymerase chain reaction (RT-qPCR), immunoblotting, and ChIP-seq analysis. Depletion of MAGEC2 expression was performed using siRNA. MAGEC2-depleted ADs and ASCs were used to investigate changes in the proliferation rate and cell cycle. Then, the protein levels of signal transducer and activator of transcription 3 (STAT3), phosphorylated STAT3, and downstream of STAT3 were measured using control and MAGEC2-depleted ADs and ASCs. In ASCs, the direct effect of active STAT3 inhibition with Stattic, a STAT3 inhibitor, was assessed in terms of proliferation and apoptosis. Finally, an Annexin V/7-AAD assay was performed to determine the percentage of apoptotic cells in the Stattic-treated cells. Results: MAGEC2 was highly expressed in ASCs when compared with ADs. Depletion of MAGEC2 reduced the proliferation rate and viability of ASCs. To elucidate the underlying mechanism, the level of STAT3 was examined owing to its oncogenic properties. Tyrosinephosphorylated active STAT3 was highly expressed in ASCs and decreased in MAGEC2- depleted ASCs. Furthermore, on treating ASCs with Stattic, an active STAT3 inhibitor, the cells were markedly sensitive to intrinsic pathway-mediated apoptosis. Conclusions: High MAGEC2 expression may play an important role in the survival of ASCs by maintaining the expression of activated STAT3 to prevent apoptotic cell death.
위궤양으로 인한 위절제술 후 발생한 역류성 식도염 치험 1례

Objectives: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of reflux esophagitis. Methods: A 62-year-old male patient had reflux esophagitis that occurred one year after a proximal gastrectomy for gastric ulcer in 2020. The patient underwent drug treatment in 2021 and inpatient treatment in 2022 at Mokpo Hankook Hospital, but he did not improve. We treated him with Korean medical treatments, including herbal medicines (Pyungjingunbi-tang -gami), acupuncture, and moxibustion. His symptom severity was assessed with a daily visual analog scale (VAS) for heartburn, upper abdomen pain, and acid reflux. Results: After treatment, the patient's symptoms were improved. The severity of heartburn and acid reflux was reduced from VAS 7 to VAS 1, and his upper abdomen pain was improved from VAS 6 to VAS 1. The symptom frequency was also reduced. Conclusion: Korean medicine treatment could be an effective and quick treatment for reflux esophagitis.
