        홍삼류의 섭취가 비만과 혈중 지질의 상호관계에 미치는 영향

        비만은 동맥경화의 위험인자로서 혈중에 triglyceride의 농도를 증가시키고, 상대적으로 HDL-Cholesterol의 농도를 감소시킴과 동시에 수축기 혈압을 상승시킨다. 본 연구에서 홍삼제품류를 4년 또는 5년 동안 복용해 온 건강한 사람 (ginseng군)과 홍삼제품을 복용하지 않은 건강한 사람 (control군)을 대상으로 하여 신체 계측치로부터 비만지수를 구하고 이것과 혈중의 triglyceride (TG) 농도, TG/HDL-Cholesterol ratio 및 수축기 혈압과의 상호관계를 연구한 바, ginseng군에서는 대조군에 비해 혈중의 TG 농도가 일정하게 유지되었고, TG/HDL-Cholesterol ratio 및 수축기 혈압도 일정하게 유지되었다. 이러한 현상은 ginseng군에서 기호품으로 alcohol을 섭취하거나, 흡연을 한 경우에도 상관없이 일정하게 유지되었다. 이 결과는 홍삼제품류를 장기 복용하면 비만의 저하 및 고혈압 또는 동맥경화의 위험인자가 억제될 수 있고, 결론적으로 비만, 고혈압 및 동맥경화를 예방할 수 있을 것으로 추정한다. Obesity is the risk factor of atherosclerosis and not only increases triglyceride concentration in blood but also decreases relatively the ratio of TG to HDL-Cholesterol in blood. In case of obesity, systolic blood pressure is also increased in responding the increase of TG in blood. Index of obesity in red ginseng-taking group (ginseng group) was lower as compared with non-red ginseng-taking group (control group). The TG concentration, the ratio of triglyceride to HDL-cholesterol in blood and systolic blood pressure were decreased in the subjects of ginseng group compared with that in control group. It is inferred that long-term intake of ginseng products may help to prevent the risk of atherosclerosis and obesity.

        Objectives: The assessment of global left ventricular function and volumes is important in patients with coronary artery disease (CAD) and serves as a valuable diagnostic and prognostic marker. The purpose of this study was to compare left ventricular ejection fraction (LVEF) and volume parameters as measured by 640-slice multidetector CT (MDCT) and two-dimensional transthoracic echocardiography (2D-TTE). Methods: We retrospectively analyzed 125 consecutive patients (Men=56%, mean age, 62.0±12.9 years) who underwent 640-slice MDCT to evaluate known or suspected CAD between October 2011 and March 2012. Agreement for the LVEF and volume parameters by 640-slice MDCT and 2D-TTE was assessed by Pearson’s correlation coefficient for linear regression and Bland-Altman analysis. Results: On 640-slice MDCT, average LVEF was 61.6 ± 12.6 %. On 2D-TTE, average LVEF was 62.8 ± 14.9%. Correlations between 640-slice MDCT and 2D-TTE were that LVEDV (r: 0.801, p<0.001); LVESV (r: 0.929, p<0.001) and LVEF (r: 0.862, p<0.001). Conclusions: There was a good correlation between 640-slice MDCT and 2D-TTE in the evaluation of left ventricular function and volume parameters in practical and clinical situation.

        체형, 성격, 소증 기반 단축형 사상체질 진단설문지(KS-15) 개발 및 타당도 연구

        Objectives The aim of this study is to develop a Korea Sasang Constitutional Diagnostic Questionnaire (KS-15) with practicality and validity. Methods The data of body shape, temperament and symptoms were collected from 11 nationwide multi-centers in Korean Medical Clinics between Nov. 2009 to Aug. 2012. KS-15 algorithm was developed through Multinomial Logistic Regression and was applied for test group to verify it's classificatory function. Results The weight value of variables of KS-15 were different between male and female. The concordance of KS-15 with the expert in training group was 58.34% (72.8% in Tae-eumin, 54.8% in Soeumin, and 44.1% in Soyangin). The concordance of KS-15 with the expert in test group was 63.1% (63% in Tae-eumin, 83.3% in Soeumin, and 51.9% in Soyangin). Conclusions KS-15 seems to have suitable implement with practicality and validity. Further study is necessary to verify it accuracy and cut-off.

      • Anti-Platelet Function of the Non-Saponin Fraction from Panax ginseng C.A. Meyer (Korean red ginseng)

        고려홍삼의 비사포닌분획(NSF)은 높은 농도의 트롬빈(0.2~$2{\mu}/ml$)에 의해 유도된 사람혈소판 응집반응을 강하게 억제했다. 그 응집정도는 $Ca^{2+}$ 길항제인 verapamil의 응집억제 정도보다 강했다. Verapamil은 혈소판 응집반응을 저해시키는 혈소판내 cAMP의 생성을 증가시켰지만, NSF는 cAMP의 생성증가에는 현저한 영향이 없었다. NSF는 $TXA_2$의 생성과 serotonin의 방출을 억제시키고 있었다. NSF는 혈소판막 인지질로부터 arachidonic acid의 방출을 억제하지 않고, arachidonic acid를 $TXA_2$로 전환시키는 어떤 경로를 차단하므로 혈전중의 치료 및 예방에 중요한 역할을 할 것으로 추측된다. The non-saponin fraction(NSF: lipophilic fraction) from Korean red ginseng showed an inhibitory activity in the aggregation of human platelets induced by a high dose of thrombin (0.2 to 2 u/ml), the extent of which was stronger than that of verapamil, a $Ca^{2+}$ antagonist. NSF did not affect the elevation of the intracellular cAMP concentration which inhibits platelet aggregation, as shown in the case of verapamil, but inhibited the production of $TXA_2$ and the release of serotonin. NSF does not inhibit the release of arachidonic acid from membrane phospholipids, but inhibits a certain pathway transforming arachidonic acid to $TXA_2$. NSF may play a critical role in preventing and curing thrombotic diseases.

        The non-saponin fraction(NSF: lipophilic fraction) from Korean red ginseng showed an inhibitory activity in the aggregation of human platelets induced by a high dose of thrombin (0.2 to 2 u/㎖), the extent of which was stronger than that of verapamil, a Ca^(2+) antagonist. NSF did not affect the elevation of the intracellular cAMP concentration which inhibits platelet aggregation, as shown in the case of verapamil, but inhibited the production of TXA₂ and the release of serotonin. NSF does not inhibit the release of arachidonic acid from membrane phospholipids, but inhibits a certain pathway transforming arachidonic acid to TXA₂. NSF may play a critical role in preventing and curing thrombotic diseases.

        자궁근종과 성호르몬 대사물과의 연관성

        Objective: To elucidate 1) whether there are any differences in the urine concentrations of steroid hormone metabolites between patients with leiomyoma and normal controls 2) the correlation between urinary profiles of steroid hormones and leiomyomas of the uterus according to their type, location, volume, and weight. Materials of Methods : The study population consisted of 37 premenopausal patients with uterine leiomyoma and the control group consisted of 25 premenopausal normal volunteer women without uterine leiomyoma. Confirmation of the existence of uterine leiomyoma was done by ultrasonography and histopathological examination after surgery. The volume of the leiomyoma was estimated by trans-abdominal and/or trans-vaginal ultrasonography. The Leiomyomas were divided into 3 types (subserosal, intramural and submucosal). Seventeen patients had subserosal type of leiomyoma, 10 with the intramural type and 10 with the submucosal type. The locations of the leiomyoma were also divided into 3 groups (fundus, body and isthmus). Seventeen patients showed a fundus location, 10 in body, and 10 in isthmus. We compared urinary profiles of the endogenous steroids between patients with leiomyomas and normal controls, and also investigated the relationship between urinary profiles of the endogenous steroids and leiomyomas according to their type, location, volume and weight by using highly sensitive Gas Chromatography-Mass Spectrometry (GC-MS) system. Results: The mean ages of the patients with leiomyomas and the control group were $43.1{\pm}5.6$ and $40.6{\pm}7.2$ years, the weights were $63.4{\pm}7.3$ and $59.4{\pm}8.1\;kg$, and their heights were $155.4{\pm}4.8$ and $159.3{\pm}4.8\;cm$ respectively. Seventeen patients had subserosal, 10 had intramural, and 10 had submucosal leiomyomas. There were 17 patients with leiomyoma located in fundus, 10 in body and 10 in isthmus. $17{\beta}$-estradiol, 5-AT, 11-keto ET, $11{\beta}$-hydroxy An, $11{\beta}$-hydroxy Et, THS, THA, THE, a-cortolone, a-cortol, $\beta$-cortol, $11{\beta}$-OH Et/$11{\beta}$-OH An and E2/E1 were significantly increased in patients with leiomyoma than in the control group. $17{\beta}$-estradiol was significantly increased in the intramural and the submucosal types than in the subserosal type. There was no significant difference in the concentrations of urinary steroids according to the locations of leiomyomas. There was no significant relationship between the concentration of urinary steroids and the volume of the leiomyomas. $17{\beta}$-estradiol significantly decreased as the weight of uterus increased (r=-0.322, p=0.04). Conclusion: The concentrations of steroid hormone metabolites were generally increased in patients with leiomyoma but were not significantly related to the volume and weight of the leiomyomas. Our study suggests that steroid hormones may be involved in the initiation of leiomyomas but may not be involved in their progression. In addition, the concentrations of steroid hormone metabolites are not related to the leiomyoma type and location.
