The Effect of Extracorporeal Shock Wave Therapy on Lower Limb Spasticity in Subacute Stroke Patients

Objective To evaluate the effect of extracorporeal shock wave therapy (ESWT) on lower limb spasticity in subacute stroke patients.Methods We studied thirty hemiplegic subacute stroke patients with ankle plantar flexor spasticity. ESWT was applied for 1 session/week, with a total of 3 sessions at the musculotendinous junction of medial and lateral gastrocnemius muscles. Patients were evaluated both clinically and biomechanically at baseline, after sham stimulation, and at immediately 1 week and 4 weeks after ESWT. For clinical assessment, Modified Ashworth Scale (MAS), clonus score, passive range of motion of ankle, and Fugl-Myer Assessment for the lower extremity were used. A biomechanical assessment of spasticity was conducted by an isokinetic dynamometer. Two parameters, peak eccentric torque (PET) and torque threshold angle (TTA), were analyzed at the velocities of 60o/sec, 180o/sec, and 240o/sec.Results After sham stimulation, there were no significant changes between each assessment. MAS and PET (180o/sec and 240o/sec) were significantly improved immediately and 1 week after ESWT. However, these changes were not significant at 4 weeks after ESWT. PET (60o/sec) and TTA (60o/sec, 180o/sec, and 240o/sec) were significantly improved immediately after ESWT. Yet, these changes were not significant at 1 week and 4 weeks after ESWT as well.Conclusion Lower limb spasticity in subacute stroke patients was significantly improved immediately after ESWT. Although the therapeutic effect of ESWT reduced with time and therefore was not significant at 4 weeks after ESWT, the degree of spasticity was lower than that of the baseline. Future studies with a larger sample of patients are warranted in order to verify the protocols which can optimize the effect of ESWT on spasticity.

상장간막동맥증후군은 매우 드문 질환으로 식후 상복부 동통, 복부팽만, 구역, 구토, 체중감소 등의 상복부 폐쇄 증상을 일으키며 상부 위장관 조영술 등 방사선 검사로 진단할 수 있다. 평소 건강한 66세 남자 환자가 토혈로 내원 후상부위장관 조영술 및 복부전산화단층촬영 검사에서 상장간막동맥 증후군으로 인한 장폐쇄와 이차적인 식도 궤양 출혈로 진단되어 내과적 및 외과적 치료 후 좋은 결과를 얻은 증례를 보고하는 바이다. Superior mesenteric artery (SMA) syndrome is a rare disorder, characterized by compression of the third segment of the duodenum by the mesenteric artery at the level of the SMA, resulting in duodenal dilatation. The most characteristic symptoms are postprandial epigastric pain, fullness, voluminous vomiting, and eructation. The diagnosis may be difficult, but can be confirmed by upper gastrointestinal (UGI) contrast studies. We report a case of SMA syndrome in a 66-year-old patient with hematemesis. Endoscopy showed deep circular ulcerations with bleeding in the distal esophagus. Computed tomography (CT) and an UGI contrast series revealed distension of the stomach and duodenum, with a cut-off in the third portion of the duodenum. We treated the patient conservatively, but the patient`s symptoms did not improve. Ultimately, the patient underwent successful gastrojejunostomy with a favorable postoperative outcome. A Case of Coronary Air Embolism of the Left Coronary Arteries that Manifested as Cardiogenic Shock

Coronary air embolism remains a serious complication of coronary catheterization despite performing careful procedure to prevent this. We report here on a case of massive coronary air embolism that was complicated by cardiogenic shock in a 52-year-old male patient with angina pectoris. The patient had a stenosis in the middle left anterior descending artery (LAD) and percutaneous coronary intervention (PCI) was planned for the LAD lesion. During PCI, inadvertent manipulation of a pressure line induced massive air embolism in both proximal left coronary arteries, and this manifested as cardiogenic shock. The patient recovered after supportive measures and successive intracoronary injections of nitroglycerin and then he eventually underwent successful PCI. (Korean Circulation J 2007;37:334-336)

In this paper a method of detecting circle/rectangle images is considered for the alignment of a tape substrate. Hough transforms for circles and rectangles are used for the detection of the centers of circles and rectangles, respectively. The images of circles and rectangles can be detected robustly by adopting an averaging method for the Hough transform. The efficiency of the proposed method was proved by its application to sample images from a substrate testing machine.
