저자들은 아급성 갑상선염을 진단 받고 보존적 치료를 시행 후 호전된 환자에서 2년 후 그레이브스병이 발병한 1예를 경험하여 아급성 갑상선염 환자에서 자가면역항체가 양성을 보일 경우 추후 그레이브스병의 발생에 대한 지속적인 추적 관찰이 필요할 수 있음을 강조하면서 보고하는 바이다. Subacute thyroiditis is a spontaneously resolving inflammatory disease of the tyroid gland, which is usually associated with a viral infection and genetic factors. In some cases of this disorder, thyroid autoantibodies can appear, probably due to the inflammatory release of thyroid antigens, althoughtheir pathophysiological role in the course of this disease is not yet fully understood. The occurrence of Graves` disease after subacute thyroiditis is extremely rare, and only a few cases have been reported. Here, we report a case of a middle-aged woman who developed Graves` disease after subacute thyroiditis.

본 연구는 그동안 소외되었던 수도권 지역의 거주민들의 지역성을 바탕으로 실제 수도권 지역주민들을 대상으로 그들의 지역성과 지역이미지, 지역참여도가 지역언론 이용에 어떤 영향을 미치는지 구조적 관계를 알아보기 위해 구조방정식(Structural Equation Model: SEM)을 통해 살펴보았다. 분석 결과, 지역성과 지역언론매체 이용의 상관관계는 거주기간이 지역정체성에 영향을 주며 지역정체성은 지역이미지에 영향을 주는 것으로 나타났다. 또한, 거주기간과 지역참여도 정도 역시 각각 지역언론매체 이용도와 유의미한 관계가 있다는 것으로 나타났다. 궁극적으로 본 연구는 지역언론, 특히 지역일간지의 위기를 되짚어 보며 지역일간지와 지역 민들의 관계를 알아보고 지역정체성과 지역참여도가 지역일간지 구독에 어떠한 영향을 주는지를 통해 지역일간지가 나아가야 할 구체적 방향을 제시했다. The goal of this study was to examine the effects of locality, local images, local community participation on local press usage among Gyung-gi and Incheon metropolitan residents. A 'Structural Equation Model (SEM)' method was used to their structural relationship among variables. The results indicated that the relationship between locality and local press usage were as follows: first, residence period affects locality, which then affects local images. And, residence period and local community participation levels are significantly related to local press usage. Finally, assuming the crisis of local media in Gyung-gi and Incheon metropolitan areas, the conclusion of the study attempted to show a couple of proposals of local media.

저자들은 20년 전 정상지질혈증성 편평 황색종을 진단받고 특별한 치료 없이 지내온 환자에서 20년 후 다발성 골수종이 진단된 1예를 경험하여 정상지질혈증성 편평 황색종 환자의 지속적인 추적관찰이 필요함을 강조하면서 보고하는 바이다. Diffuse plane xanthoma is a group of plane xanthomas that appear as yellow to yellowish-brown flat patches or slightly elevated plaques with a widespread distribution. It is often associated with, or preceded by several years of, hematologic problems. Here, we describe a 63-year-old woman diagnosed with diffuse normolipemic plane xanthoma who developed multiple myeloma 20 years later. This case suggests that patients with diffuse normolipemic plane xanthoma should be followed for their entire life to detecthealth problems. (Korean J Med 2011;81:107-110)

뇌 병변 및 신경외과적 수술 후 발생하는 중추성 요붕증은 드물며, 갈증 중추인 삼투수용체에 병변이 동반될 경우 전형적인 요붕증의 증상이 구갈, 다음 증상의 발현이 없어 진단이 부적절해지거나 늦어질 수 있다. 저자들은 전교통 동맥류 결찰술을 시행받은 환자에서 무갈증이 동반된 중추성 요붕증 환자를 경험하여 두 가지 질환이 동시에 발생할 수 있음을 염두에 두고 빠른 진단 및 데스모프레신과 적절한 수분공급 등의 치료에 특별한 주의가 필요함을 강조하고자 문헌고찰과 함께 보고하는 바이다. Central diabetes insipidus and adipsic disorders are often accompanied by abnormalities such as hypothalamic lesion. The development of hypothalamic diabetes insipidus with adipsia after clipping of an anterior communicating artery aneurysm has rarely been reported. Recently, we experienced a 52-year-old woman who presented with adipsia, polyuria, and hypernatremia after clipping of a ruptured aneurysm in the anterior communicating artery. She was diagnosed with hypothalamic diabetes insipidus and adipsic disorder and treated with intranasal desmopressin and regular water intake. This is the 11th case of adipsic diabetes insipidus associated with an anterior communicating artery aneurysm in the world, and the first reported case in Korea. (Korean J Med 77:S1217-S1221, 2009)

저자들은 급성 훼장염 환자에서 심한 고칼슘혈증을 동반한 부갑상선 선종에 의한 원발성 부갑상선 기능항진증을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Primary hyperparathyroidism is characterized by hypercalcemia and hypophosphatemia induced by an increased level of parathyroid hormone(PTH). Patients with primary hyperparathyroidism are usually asymptomatic, but can present with symptoms of renal stones, peptic ulcer disease, muscle weakness and depression. In addition, primary hyperparathyroidism-related with hypercalcemia is considered to represent a risk factor for the development of pancreatitis. We report a case of a 66-year-old woman with primary hyperparathyroidism who was accompanied with acute pancreatitis. The chief complain was abdominal pain and general weakness. The serum levels of amylase, lipase, calcium and intact PTH were 17391U/L, 4160IU/L, 21.2mg/dL and 1836pg/mL. A parathyroid mass was detected by neck computed tomography, ultrasonography and 99m-Tc sestamibi scintigraphy. After the surgical procedure, the symptoms were improved and the calcium level was normalized.
