Most of fractal image coding methods have suffered from long encoding time. In this paper, we propose a new image coding method in order to reduce fractal-basedinage coding time. We can reduce the time taken to exploit the block by using 24 orientation a pattern-code to make it pissible that image with bigger domain can be efficiently exploited through block by smaller one. Finally, Compared the above with Jacquin's method one takes the improvement of compression rate on condition that decoded image quality should be maintainded to 27dB pp-SNR, And takes 42.53% reduced time to encode.

Objective: To investigate the effect of extracorporeal shock wave therapy (ESWT) on pressure ulcers which is a major, functionally-limiting medical problem impairing quality of life for many people each year. Method: Seven patients who had stage 3 pressure ulcers were enrolled for the study. Each patient was treated with 6 sessions of low-energy (0.10∼0.15 mJ/mm2, 1,000 impulses) ESWT. The length, width, depth and soft tissue biopsy of pressure ulcers were evaluated every 2 weeks for 6 weeks. Results: The length, width and depth decreased significantly after 2 weeks of ESWT application. Healthy granulation tissue was formed. Soft tissue biopsy revealed increased the number and size of capillaries and decreased inflammatory cells in treated case. Conclusion: ESWT promoted wound healing and revealed favorable histological changes in pressure ulcers. We suggest that ESWT can be used for the safety and effective management of pressure ulcer. Winged scapula is caused by the lesions of long thoracic nerve, spinal accessory nerve or dorsal scapular nerve. Majority of them are usually caused by injuries during sports activity, trauma, infection, intoxication and operative injuries. We report that, In this case, repeated shoulder flexion, abduction, external rotation & internal rotation movement induced winged scapula due to long thoracic nerve injury without strong direct trauma, infection or surgery.

Femoral nerve arises from the ventral rami of the second, third and fourth lumbar nerves and runs beneath the inguinal ligament, into the thigh. The nerve can be compressed anywhere along its course, but it is particularly susceptible at the iliopsoas groove and at the inguinal ligament. We experienced a 52 years old female who had weakness in knee extension and hypoesthesia in medial aspect of thigh and calf after swimming with breaststroke for several hours a day in a week. The diagnosis was established with clinical findings, nerve conduction study and needle electromyography. The mechanism of neuropathy are direct compressive irritation of nerve within inguinal ligament by sharp hip flexion and excessive stretch on nerve due to hip abduction and external rotation during breaststroke. We should consider possibility of femoral neuropathy in patient with weakness in knee extension or hypoesthesia in medial thigh and calf after swimming with breaststroke and distinguish between radicular lesion and peripheral lesion by physical exam and electrodiagnostic study.

Arthroscopic operation is useful method for the patients with intraartuciular abnormalities. Even though knee arthroscopy is a minimal invasive procedure, neurologic complication may occur rarely. We report the case of a 42-year-old woman who underwent knee arthroscopy. Postoperatively, she complainted weakness of left toe flexors and hypoesthesia on left sole. Nerve conduction studies and needle electromyography showed tibial nerve injury around popliteal fossa. Because of the closed nature of their procedure and proximity of neural structures to instruments, neurologc injury was occurred. More attetion about anatomy around popliteal fossa are needed for prevention of neurologic injury.

Background: The purpose of this study is to evaluate intercostal neuropathy after rib fracture and to determine the severity of intercostal neuropathy with using a numerical rating scale and according to the duration of pain and the body mass index. Material and Method: We measured the positive sharp wave and fibrillation on the intercostal and paraspinal muscles in the thoracic region by performing needle electromyography in 47 patients who had intercostal neuralgia after rib fracture and who had needed daily analgesic for more than three months. Result: We diagnosed 11 cases as intercostal neuropathy among the 47 cases. Of the total 11 cases, 8 were male and 3 were female and they were most often of an active generation in the community. The common location of intercostal neuropathy was the intercostal space below the rib fracture and from the 7th to the 12th intercostal rib area. The incidence of intercostal neuropathy was significantly related with multiple rib fracture rather than single rib fracture. The symptoms observed were chest pain (90.9%), sensory change (81.8%), paresthesia and numbness (63.6%), back pain (27.2%) and muscle atrophy (18.2%). The numerical rating scale, the duration of pain and the body mass index showed no significant correlation with the severity of intercostal neuropathy. Conclusion: We concluded that the electrodiagnostic approach with considering the affecting factors and the clinical findings will be helpful for diagnosing and treating persistent intercostal neuralgic pain (more than 3 months) after rib fracture. 배경: 이 연구는 늑골 수상 후 발생하는 늑간 신경병증에 대한 평가를 위하여 숫자통증등급, 통증 기간 및 체질량 지수에 따른 늑간 신경병증에 대한 심한 정도를 알고자 하였다. 대상 및 방법: 늑골 골절 수상 후 3개월 이상 동안 보존적인 치료에도 불구하고 심한 통증을 호소하는 환자 47명을 대상으로 근전도 검사를 시행하여 흉부의 늑간 및 척추 주위 근육에 대한 평가를 하였다. 결과: 47명 중에서 11명에 대하여 늑간 신경병증의 진단을 얻을 수 있었으며, 남자 8명과 여자 3명이었으며, 그 연령은 직업적으로 활동적인 연령대였다. 늑간 신경병증이 호발하는 부위는 7번부터 12번 부위 늑골 골절의 늑간이었다. 그 빈도는 단일 골절보다는 다발성 골절과 밀접한 상관 관계를 알 수 있었다. 그 증상으로는 흉통(90.9%), 감각이상(81.8%), 무감각(63.6%), 후부 흉통(27.2%), 그리고 근위축(18.2%) 순이었다. 근전도에 의한 숫자통증등급, 통증 기간 및 체질량 지수와 관계는 없었다. 결론: 늑골 골절 수상 후 3개월 이상 지속적인 통증을 호소하는 환자에서 지속적인 늑간 신경통에 대한 진단을 위한 평가로 근전도가 도움이 될 수 있다.
일측성 슬관절염에 대한 등속성 근기능 평가 및 운동치료의 효과

Knee osteoarthritis is one of the most prevalent arthritis that weakens the muscles. This study focused on evaluating muscular functionality of knee osteoarthritis subjects. Muscular strengths of muscles around knee and hip joints of middle aged female subjects aged over 40 suffering from knee osteoarthritis were evaluated by isokinetic dynanometer. Also, relation between the observed muscle imbalance in knee and hip joints and visual analogue scale was investigated. Subjects performed 8 weeks exercise on weakened muscles particularly on knee extensors and hip abductors and had their isokinetic muscular functionalities analyzed again. After the 8 weeks exercise, subjects` thigh circumferences were increased, accompanied with muscular strength improvements and decrease in visual analogue scale. Hence we emphasize the importance of exercise for muscular strength enhancement of knee extensors and hip abductors, in rehabilitation programs for knee osteoarthritis.
