The occurrence of a trigeminal neuropathy associated with connective tissue disease is rare. Trigeminal neuropathy in connective tissue disease is predominantly sensory and it is characterized by numbness, hypesthesia, slurred speech, and touch pain. Although the pathogenesis of trigeminal neuropathy associated with connective tissue disease remains obscure, the main pathologic findings are vasculitis and neuritis. A case of trigeminal neuropathy associated with mixed connective tissue disease is described, and the incidence, symptoms, pathophysiology, treatment of the disease is discussed.

CC Chemokine CKβ8의 발현 조절 물질 탐색 및 저해활성 물질 분석

CKβ8은 CC 케모카인의 부류에 속하는 사이토카인의 일종으로, 단핵구, 호염구, 호산구, T 림프구, 자연살해세포, 수지상세포 등의 화학주성물질로 작용하는 것으로 알려져 있다. CKβ8은 염증반응 및 혈관신생 등과 같은 여러 가지 생체 반응에 역할을 하는 것으로 알려져 있다. 본 논문에서는 사람 CKβ8 유전자의 전사저해 물질을 선별하기 위하여 CKβ8/Luc 유전자를 발현하는 세포주를 확립하였다. CKβ8 유전자의 전사시작점에서 -299 부터 +10 프로모터 부위를 PCR에 의해 얻은 다음 포유세포 발현 플라스미드에 클로닝하였다. CKβ8/Lvc 유전자는 U937 세포내에서 PMA에 의해 10배 이상 발현이 증가한다. CKβ8/Luc 발현 U937 세포주는 식물 추출물에서 전사 저해제를 탐색하는데 이용되었다. 46 개의 식물 추출물 중에서 6개가 90% 이상 CKβ8/Luc의 전사를 저해하였다. 이들 물질에 대해 세포 독성 실험을 수행한 결과 그 중 한 개만이 세포 독성이 거의 없는 것으로 나타났다. 이 추출물은 붉나무에서 얻은 추출물이며 이 식물 추출물은 염증관련 질환에 대한 가능성이 있는 치료제로 이용될 수도 있을 것으로 추측된다. Ckβ8 belongs to a family of CC chemokines, which is known to act as a chemoattractant for monocytes, basophils, eosinophils, T lymphocytes, NK cells, and dendritic cells. CKβ8 is known to play roles in various biological processes such as inflammation and angiogenesis. In the present study, we developed U937 cell line expressing CKβ8/Luciferase fusion gene for screening of transcriptional inhibitors for human CKβ8 gene. Genomic sequences that contain CKβ8 promoter region from 299 to+10 nucleotide from the transcription start site was amplified by PCR and cloned into mammalian episomal plasmid. The CKβ8/Luc fusion plasmid was introduced into human promonocytic U937 cells and selected for stable U937 cells expressing the fused reporter gene. The expression of the fusion gene was induced by more than 10 folds upon treatment of phorbol myristate acetate. The stable U937 transfectant cells were used for searching transcriptional inhibitors for human CKβ8 gene from plant extracts. Among 46 plant extracts tested, six extracts exerted more than 90% inhibition of PMA-induced CKβ8 transcription. Cytotoxic tests demonstrated that one of extracts, which was extracted from Rhus chinensis Mill, had little cytotoxic effect. The extract may be used for developing a potential therapeutic agent for inflammatory diseases.

혈액 투석중인 환자에서 발생한 Yersinia Enterocolitica 패혈증에 의한 간과 비장 농양 1예

Yersinia enterocolitica, a gram-negative rod-like organism, has been recognized as an uncommon human pathogen producing a wide spectrum of clinical manifestations. It has been associated with gastroenteritis, acute ileitis, mesenteric lymphadenitis, and erythema nodosum. More recently polyarthritis, chronic arthritis, and Reiter syndrome have been related to previous infection with these organism. Liver and splenic abscesses have been found rarely in Yersinia enterocolitica bactermia, and these have not been reported yet in Korea. We report a case of multiple liver and splenic abscesses secondary to Yersinia enterocolitica septisemia in a chronic renal failure patient on hemodialysis.

위정맥류출혈에서 Histoacryl 주입으로유발된 폐색전증 1 예

N-Butyl-2-cyanoacrylate (Histoacryl) has been used successfully to control bleeding from esophagogastric varices since the first report in 1986 by Soehendra. Complications of Histoacryl injection include local sloughing with ulcer formation, rebleeding, sepsis, mediastinitis, esophageal strictures and fever. Systemic embolization, including pulmonary and cerebral embolization, has been reported. We describe here the presentation of an un-usual complication of Histoacryl injection. A 42-year-old woman presented with an attack of massive hematemesis. Endoscopy revealed bleeding evidence of gastric varices. Sclero-therapy was carried out using Histoacryl mixed with Lipiodol (ratio 1 :1), with complete solidification of the varix. Next morning she complained of dyspnea and tachycardia. The chest x-ray showed multiple metallic densities, consistent with cyanoacrylate mixed with lipoidol, located at central portion. She was diagnosed as having pulmonary embolism by convential precontrast chest CT. She was treated onservatively using oxygen and analge-sics. She recovered soon and was discharged without significant problems.
