재활치료 중단 후 보행 양상이 다시 악화된 뇌졸중 환자 1례에 관한 고찰

Objective: This study describes the case of a patient with a left basal ganglia (BG) infarct who had made almost a complete recovery upon discharge from the hospital in 2014 but who was re-admitted after 31 months with a worsened hemiplegic gait. Methods: The patient had undergone no rehabilitation treatment in the 31 months since his discharge. When the patient was re-admitted to our hospital in 2017, stance and gait data were collected on the patient using a treadmill gait analysis system. In addition, the patient underwent a manual muscle test (MMT) evaluation, and his Motricity index (MI) and modified Barthel index (MBI) scores were recorded. After rehabilitation for one month, the patient was reassessed, and the results were compared to those on the day of re-admission and those recorded in 2014. Results: Compared to the 2014 evaluation results, the patient's stance parameters and gait parameters had worsened at re-admission. However, there was no significant change in the patient's MMT grade or MI and MBI scores in comparison to the results of the 2014 evaluation. After one month of rehabilitation, the patient was re-evaluated again, but there was no significant change in comparison to the evaluation results at re-admission. Conclusion: Some of the stroke patients who have passed six months since the onset of their stroke may require ongoing rehabilitation although the functions of them is almost recovered. Because there is a possibility that recovered functions get worse again without any rehabilitation for a long time. And once the recovered functions get worse, re-recovery of them is not easy.

Objective: To confirm the immediate gait improvement in a post-stroke gait disturbance patient equipped with a weighted vest. Methods: We selected a patient who was able to walk without another’s help or with tools. The selected patient had an unstable gait because she had only started an independent gait within the past week, so we thought that a weighted vest could be very helpful for her. We first collected gait parameters using a treadmill gait analysis system while the patient walked on the treadmill without the weighted vest. After a 10-minute break, gait parameters were collected again while the patient walked on the treadmill while wearing the weighted vest. The gait parameters we collected included step length (cm), stance phase (%), swing phase (%), SW/ST, and gait line length (mm). For objective evaluation of gait improvement, we calculated the ratio of gait parameters of the right and left limbs. Results: The gait of the post-stroke patient was more symmetrical when wearing the weighted vest than without the weighted vest. Without the weighted vest, her step length ratio was 0.78, stance phase ratio was 0.88, swing phase ratio was 1.50, SW/ST ratio was 1.70, and gait line length ratio was 0.91. With the weighted vest, her step length ratio was 0.88, stance phase ratio was 0.90, swing phase ratio was 1.38, SW/ST ratio was 1.54, and gait line length ratio was 0.98. No side effects were observed due to the weighted vest.

Introduction: This case report assessed the effects of Korean medicine treatments on a chronic migraine patient who did not respond to triptan. Case Presentation: The patient received Korean medicine treatments, such as acupuncture, moxibustion, cupping, and herbal medication, during a hospitalization period. The effects of treatments were assessed with the Numerical Rating Scale (NRS) and migraine attack frequency. The intensity and attack frequency of migraines were significantly reduced after the complete treatment. Conclusions: For migraines, Korean medicine such as acupuncture and herbal medication could be an alternative to conventional therapy.

Objectives: The aim of this study was to evaluate the effectiveness of additional administration of Banha-sasim-tang formulation to patients with gastrointestinal symptoms that did not improve with Western medicines. Methods: The patients who met the inclusion and exclusion criteria were analyzed retrospectively. A paired t-test was performed on the Nepean Dyspepsia Index-Korean version (NDI-K) scores before and after administering the Banha-sasim-tang formulation. Results: A total of 27 subjects were included. The mean duration of taking the Banha-sasim-tang formulation was 15.6±3.1 days. The most commonly used Western medicine before taking the Banha-sasim-tang formulation was rebamipide. The means of the NDI-K score were 57.4±21.8 before administering Banha-sasim-tang formulation and 34.9±16.5 after administration, and the difference was statistically significant (p<0.0001). No serious side effects were encountered while taking the Banha-sasim-tang formulation. Conclusions: For patients whose gastrointestinal symptoms do not improve with Western medicines, additional administration of Banha-sasim-tang formulation may be an option. However, as this study is a retrospective chart review, prospective, large-scale, double-blind studies are needed in the future.

The importance to take into account second-order motion of semi-submersibles in waves is being emphasized because sometimes, the low frequency roll motions were enen greater than the wave frequency roll motions. The paper describes characteristics of second-order roll motion of semi-submersibles in regular and irregular waves with results obtained with model tests. From the model tests in regular head wave it was observed that a semi-submersible could take on a steady list angle in specific wave period Second-order motion characteristics of a semi-submersibles are investigated from the model tests at different drafts, wave and current speeds. Also, time domail analysis has been used to calculate the roll motion of the semi-submersible and the results gave a good agreement with those of the model test.

The goal of this study was to explore the role of inducible NOS (iNOS) and the delayed protection of exercise preconditioning from the relative myocardial ischemia reperfusion injury. Mice were randomly placed in one of five testing groups: wild-type mice group (n = 15), high-intensity wild-type mice acute exercised group (n = 6). low-intensity wild-type mice acute exercised group (n = 5), L-NAME + wild-type mice acute exercised group (11 = 5), and homozygous (-/-) iNOS gene knock-out mice acute exercised group (n = 5). All acute exercised groups ran on the treadmill (0% grade) at 65 m,`min as a high-intensity exercise and at 27-30 m/min as a low-intensity exercise. The results showed that all acute exercise significantly reduced magnitude of a myocardial infarction in biphasic manner. In accordance with these data, NFKB and HSP-70 protein expression was increased at 24 after exercise-induced preconditioning (PC) in both low-intensity wild-type mice acute exercised group and homozygous (-/-) iNOS gene knock-out mice acute exercised group compared with wild-type mice group. In contrast, neither endothelial nitric oxide synthase (eNOS) nor inducible nitric oxide synthase (iNOS) expression changed at both wild-type mice group and low-intensity wild-type mice acute exercised group. In conclusion, The strong evidence of this study indicates that HSP-70 is particulary cytoprotectiye against protein-damaging stress such as I-R. Also, this study demonstrate that the late phase of exercise-induced PC is not associated with up-regulation of eNOS and iNOS but infarct-sparing effect and mild up-regulation of NFKB and HSP-70 at the late phase of exercise-induced PC in wild-type mice and iNOS gene knock-out mice. Thus, this study identifies a specific protein that mediates late PC in vitro.
