        동적 모의실험 및 분석을 통한 보행훈련장치 연구

        A gait training system for patients with gait disorder was developed. These systems leads gait joint movements given the patient who cannot carry out an independent gait. It provided a normal stance-swing ratio of 60 : 40 because of the eccentricity revolution of a gear. In this study, motion trajectory of a knee & ankle joint was evaluated and compared a measured trajectory with three-dimensional motion analyses system with the results of multi-body dynamics simulation during the gait training in a prototype of a developed gait training system. In general, predictions from the simulation study were in good agreement with experimental results from the motion analysis. The clinical application by a continuous dynamics simulation studies and gait analysis was expected to be developed by gait training system which would be helpful to recover gait ability of patients with gait disorder.

        非小細胞性肺癌의 放射線治療 成績

        From 1979 to 1982, 80 patients with unresectable non-small-cell lung cancer without metastasis were treated with high-dose radiotherapy to the primary and to regional lymph nodes with or without supraclavicular lymphatics in the Department of Therapeutic Radiology, Seoul National University Hospital. Of these, 56 patients(70%) were completely evaluable and 59 patients(74%) had squamous cell carcinoma, 13% large cell undifferentiated carcinoma and 8% adenocarcinoma, 21 patients(26%) had StageⅡ and 59 patients(74%) had Stage Ⅲ. The complete and parial response rate in the high-dose(≈6,000 rad) radiotherapy was 70% with 19% complete response, 69 patients(86%) failed in the treatment, by the failure pattern, 64% had local failure alone, 35% had local failure and distant metastasis and 1% had distant metastasis alone. The failure rate in the thorax were 76% in squamous cell carcinoma, 40% in adenocarcinoma and 20% in large cell undifferentiated carcinoma. Preliminary result shows that actuarial survival at 1, 2 and 3 years were 56%, 26% and 20% in overall patients and 64%, 37% and 21% in StageⅡ and 54%, 21% and 18% in StageⅢ, respectively. Overall median survival was 14 months; 17 months in StageⅡ and 13 months in StageⅢ. 8 patients(10%) have lived a minimum of 2 years with no evidence of disease. There was no fatal complication confirmed to be induced by radiotherapy, so definitive high-dose radiotherapy was tolerated well without major problems and resulted in good local control and survival.

        주조직적합항원이 불일치하는 마우스 동종 조혈모세포이식에서 IL-2로 유도된 CD4+CD25+ T세포를 이용한 이식편대숙주병의 억제

        Background: In kidney transplantation, donor specific transfusion may induce tolerance as a result of some immune regulatory cells against the graft. In organ transplantation, the immune state arises from a relationship between the immunocompromised graft and the immunocompetent host. However, a reverse immunological situation exists between the graft and the host in hematopoietic stem cell transplantation (HSCT). In addition, early IL-2 injections after an allogeneic murine HSCT have been shown to prevent lethal graft versus host disease (GVHD) due to CD4+ cells. We investigated the induction of the regulatory CD4+CD25+ cells after a transfusion of irradiated recipient cells with IL-2 into a donor. Methods: The splenocytes (SP) were obtained from 6 week-old BALB/c mice ($H-2^d$) and irradiated as a single cell suspension. The donor mice (C3H/He, $H-2^k$) received $5{\times}10^6$ irradiated SP, and 5,000 IU IL-2 injected intraperitoneally on the day prior to HSCT. The CD4+CD25+ cell populations in SP treated C3H/He were analyzed. In order to determine the in vivo effect of CD4+CD25+ cells, the lethally irradiated BALB/c were transplanted with $1{\times}10^7$ donor BM and $5{\times}10^6$ CD4+CD25+ cells. The other recipient mice received either $1{\times}10^7$ donor BM with $5{\times}10^6$ CD4+ CD25- cells or the untreated SP. The survival and GVHD was assessed daily by a clinical scoring system. Results: In the MLR assay, BALB/c SP was used as a stimulator with C3H/He SP, as a responder, with or without treatment. The inhibition of proliferation was $30.0{\pm}13%$ compared to the control. In addition, the MLR with either the CD4+CD25+ or CD4+CD25- cells, which were isolated by MidiMacs, from the C3H/He SP treated with the recipient SP and IL-2 was evaluated. The donor SP treated with the recipient cells and IL-2 contained more CD4+CD25+ cells ($5.4{\pm}1.5%$) than the untreated mice SP ($1.4{\pm}0.3%$)(P<0.01). There was a profound inhibition in the CD4+CD25+ cells ($61.1{\pm}6.1%$), but a marked proliferation in the CD4+CD25- cells ($129.8{\pm}65.2%$). Mice in the CD4+CD25+ group showed low GVHD scores and a slow progression from the post-HSCT day 4 to day 9, but those in the control and CD4+CD25- groups had a high score and rapid progression (P<0.001). The probability of survival was 83.3% in the CD4+CD25+ group until post-HSC day 35 and all mice in the control and CD4+CD25- groups died on post-HSCT day 8 or 9 (P=0.0105). Conclusion: Donor graft engineering with irradiated recipient SP and IL-2 (recipient specific transfusion) can induce abundant regulatory CD4+CD25+ cells to prevent GVHD.

        의료보험 진료비 심사 간소화에 대한 방법론적 연구

        After the introduction of National Medical Insurance in 1989, the medical demand has rapidly increased. The impact of increased medical demand was followed by an increase in the number of claims in need of review. We studied a new, fair method for reducing the number of claims reviewed. We analysed 90,583 outpatient claims submitted between September and October; claims were made for services given August of 1994. We finally suggested a screening system for claims review using a statistical method of discriminant analysis of the medical costs. The results were as follows. 1. In the cut-off group, age, days of medication, number of hospital or clinic visits, and total charge were significantly high. The cut-off rates according to the hospital-type and existence of accompanied disease were significantly different 2. According to ICD, the cut-off rate was highest in peripheral enthesopathies and allied syndromes(20.76%), lowest in acute sinusitis(0.93%). The mean charges were significantly different according to ICD and existence of cut-off. 3. We build discriminant functions by ICD with such discriminant variables as patient age, sex, existence of accompanied disease, number of hospital or clinic visits, and 9 detailed hospital or clinic charges included in claim. 4. We applied the discriminant function for screening those claims that were expected to be cut-off. The sensitivities comprised from 40% to 70%, and specificities from 70% to 95% by ICD. Acute rhinitis had highest sensitivity(100.00%) and other local infections of skin and subcutaneous tissue had highest specificity(98.45%). The expected number of cut-off was 17,762(19.61%). The total sensitivity was 49.62%, the total specificity was 82.57% and the error rate was 19.66%. We lacked economic analysis such as cost-benefit analysis. But, if the new method of screening claims using discriminant analysis were applied, the number of claims in need of review will reduce considerably.

        직장장애인의 건강체력 관리 어려움

        본 연구는 직장장애인의 건강체력 관리의 어려움을 이해하고자 현상학적 연구방법을 적용하여 연구를 수행하였다. 이를 위해 한국장애인고용공단 산하 J직업능력개발원에 재학 중인 훈련생중 직장 근무경험이 있는 장애인 2명을 대상으로 심층면담을 실시하여 자료를 수집ㆍ분석하였으며, 연구목적 달성을 위해 Colaizzi(2002)의 현상학적 분석방법을 적용하였다. 이는 일반적인 내용을 묶어 추상화 시키고 의미 있는 주제를 도출해 낸 후 개인적인 관심과 공통적인 속성을 도출하였으며, 연구의 진실성 확보를 위해 심층면담과 결과의 분석과정 그리고 글쓰기 등 연구전반의 과정에 대해 삼각검증법을 실시하였다. 직장장애인의 건강체력 관리의 어려움에 대한 연구결과로서 `자기관리 한계`, `직무수행 곤란`, `환경제약`, `체육인프라 제약` 라는 4개의 주제와 `자기관리 소홀`, `자기관리의 어려움`, `시간 관리의 불규칙성`, `업무스트레스 및 인간관계의 갈등`, `편의시설 제약`, `이동권 제약`, `장애인체육지도자 부재`, `장애인체육시설부족`, `장애인체육 프로그램 부재` 라는 9개의 하위주제를 도출하였다. 이러한 연구결과를 바탕으로 직장장애인의 건강체력 증진을 위해 정책적 개입에 대한 필요성을 시사하였다. This study applied the phenomenological research method to understand difficulties in health/fitness care of workers with disabilities. For this, data was collected/analyzed by performing in-depth interview with two people with disabilities who had experience in work among trainees of J Research Institute for Vocational Education & Training under Korea Employment Agency for the Disabled. To achieve the research objective, Colaizzi(2002)`s phenomenological analysis was applied. After drawing meaningful themes that tied general contents for the abstraction, personal interests and common attributes were drawn. To secure the veracity of the study, the triangulation was conducted for the overall processes of the study such as in-depth interview, analytical process of results, and writing. In the results of the study on health/fitness care of office workers with disabilities, total four themes such as `limitation in self-management`, `difficulty in job performance`, `environmental constraint`, and `restriction in sports infrastructure`, and total nine sub-themes such as `negligence in self-management`, `difficulty in self-management`, `irregularity of time management`, `work stress & interpersonal conflicts`, `lacking in amenities`, `limitation to mobility right`, `absence of sports instructors for the disabled`, `lacking in sport facilities for the disabled`, and `absence of sport programs for the disabled` were drawn. The results of this study were helpful to understand difficulties in health/fitness care of office workers with disabilities, and also suggested the necessity of institutional interventions to establish the health service support system for the disabled between works and medical institutes, to develop healthcare support programs for workers with disabilities, to promote the employment stability by establishing the sport/work environment for workers with disabilities, and to expand the mobility right and amenities for workers with disabilities.

        It is well known that a method of liquid flow-rate measurement in closed conduits by measuring the mass of liquid delivered into a weighing tank is the most accurate way to calibrate flowmeter for liquid. Most of developed countries already had a standard calibration system using this method and this could cover enough range of flow-rate to satisfy the calibration demands of industry. KWATER(Korea Water Resources Corporation) constructed the biggest flow-meter calibration system in Korea and evaluated the uncertainty of the system.

        멀티미디어 데이타를 지원하는 화일 시스템의 설계 및 구현

        멀티미디어 데이타는 여러 모노미디어 데이타들과 이들의 시간, 공간 정보를 포함하고 있다. 기존의 화일 시스템은 화일을 단순한 바이트열로 간주하기 때문에 멀티미디어 데이타를 표현하기 위해서는 여러 개의 화일을 생성해야 하며, 관리 정보도 별도의 화일로 구성해야 한다. 본 논문에서는 이와 같은 기존 화일 시스템에서의 번거러움을 없애줄 수 있는 멀티미디어 화일 시스템(MFS)을 설계 및 구현한 결과를 보인다. 특히 멀티미디어 화일의 내부 구조를 정의하고, 여러 모노미디어 데이타들을 통합 관리하는 새로운 자료 구조를 제시한다. 이 시스템은 멀티미디어 데이타의 논리적 구성을 고려한 내부 구조를 가지므로 멀티미디어 화일 구성을 쉽게 하고, 화일 시스템에서 사용되는 mode와 같은 시스템 자원의 낭비와 프로그래머의 부담을 줄이는 장점을 제공한다. The multimedia data is composed of several monomedia data and their temporal and spatial relations. In traditional file systems, a file is simply considered as a byte-stream. Therefore, in order to construct a multimedia file, we should create several independent files with an additional file for storing administrative information. This paper presents design and implementation of a multimedia file system which can avoid the inconvenience of maintaining several files to represent a multimedia file. Specifically, we define a new internal file structure and data structures to integrate and manage the multimedia data. This system provides a file structure, taking into account the logical configuration of a multimedia data, that enables us to construct a multimedia file easily and requires less system resources. Furthermore, it reduces the burden of users.
