        피부 감염과 연관된 비월경성 독성 쇽 증후군 1례

        Toxic shock syndrome(TSS) is a multisystemic disease presenting with high fever, sunburn like rash that subsequently desquamates, and hypotension mainly caused by toxin producing strains of Staphylococcus aureus. It was first reported in 1978 by Todd et al, thereafter many patients have been reported. In children, TSS is rare and must be differentiated from other erythematous febrile diseases such as Kawasaki disease, scarlet fever, drug eruption etc. We experienced a case of TSS associated with staphylococcal cellulitis in 26-month old boy, who was presenting similar symptoms to Kawasaki disease at initial stage of illness. As time passed, the patient represented more typical symptoms of TSS and Staphylococcus aureus was isolated from cellulitis of the right elbow area. Therefore, we report this case with a brief review of related literatures.

      • 뉴로 퍼지기법을 이용한 엘리베이터 속도패턴의 정밀 제어

        기존의 엘리베이터 시스템은 모든 교통 상황에 대해서 고정된 속도 패턴을 사용함으로써 교통량 변화에 다양한 속도 패턴을 제공 할 수 없었다. 운송 속도와 승차감은 엘리베이터 속도 패턴을 결정하기 위한 두개의 중요한 요소이다. 기통과 정지 시에 변속 충격을 줄이기 위해서 가속과 감속 시간이 적절히 조정되어졌다. 운송능력을 향상시키기 위해서 교통량 변화에 맞추어 저크를 조정하엿고 이와 같은 방법으로 6개의 속도 패턴 곡선과 엘리베이터의 속도 제어를 위해서 뉴로 퍼지 시스템을 구현하였다. 구현된 뉴로 퍼지 시스템은 2개의 입력변수와 1개의 출력을 가진 시스템이다. 전반부는 교통량의 변화를 나타내며 후반부는 입력에 대응되는 속도 패턴을 적용시켰다.

        백일해에 대한 연령별 면역혈청학적 연구

        목적: 우리나라의 백일해에 대한 연령별 방어면역 상태를 평가하고자 면역혈청학적 연구를 시행하였다. 방법: 국내 4개 대학병원에 방문한 건강한 소아, 청소년 및 성인을 대상으로 하였다. 대상을 연령에 따라 7개 연령군으로 구분하였고 ELISA 방법을 이용하여 백일해의 항체가를 측정하였다. 연령군별 기하 평균항체가와 각 연령군별 양성 항체가를 보이는 피험자의 비율을 조사하였다. 제조사의 지침대로, 항체가가 24.0 EU/mL 이상인 경우를 양성으로 판정하였다. 결과: 전체 1,605명의 피험자가 연구에 참여하였고, 기하 평균항체가는 $56.16{\pm}50.54EU/mL$였다. 연령군별로는 11세 미만 소아 연령군($64.78{\pm}53.24EU/mL$)이 가장 높은 기하 평균항체가를 보였다(P<0.001). 양성 항체가를 보인 피험자의 비율은 전체 대상의 68.2%였고, 11세 미만 소아 연령군(76.5%)이 가장 높은 비율을 보였다(P<0.001). 결론: 소아 연령군과 비교하여, 청소년과 성인 연령군에서 백일해에 대한 낮은 기하 평균항체가를 나타냈으며 양성 항체가를 갖는 피험자의 비율이 낮다는 것을 확인하였다. Purpose: We conducted the immunoassay of pertussis according to ages, in order to evaluate protective immunity against pertussis in Korean populations. Methods: Healthy subjects were enrolled at four university hospitals in Korea. The subjects were grouped as seven age groups (every 10 years). Antibodies against pertussis toxin (PT) in sera were measured by enzyme linked immunosorbent assay (ELISA) kits. Geometric mean concentrations (GMC) of antibodies and the ratios of the subjects with seroprotective antibody levels were determined. The subjects with antibody titers ${\geq}24.0EU/mL$ were considered to seroprotective as the manufacturer's protocol. Results: Total 1,605 subjects (age: 2 months-65 years) participated in this study, and their GMC was $56.16{\pm}50.54EU/mL$. Among seven age groups, age group <11 year showed the highest GMC ($64.78{\pm}53.24EU/mL$) (P<0.001). In the analysis of the ratios of the subjects with seroprotective antibody titers, 68.2% of the subjects were proven to seroprotective, and age group <11 year also showed the highest ratio (76.5%) (P<0.001). Conclusions: We found that adolescences or adults (age group ${\geq}11$ year) showed lower levels of antibody against pertussis and lower ratio of the subjects with seroprotective antibody titers than children (age group <11 year).

        엘리베이터 속도 패턴의 퍼지 제어

        본 논문에서는 엘리베이터 속도 패턴을 결정하는데 중요한 2개의 요소인 편안한 승차감과 빠른 운송속도를 가지며 교통량 변화에 따른 다양한 속도 패턴을 제공 할 수 있는 새로운 속도 패턴 발생 방법을 제안한다. 기동과 정지 시에 변속 충격을 줄이기 위해서 가속과 감속 시간을 적절히 조정하고, 운송능력을 향상시키기 위해서 교통량 변화에 맞춰서 저크를 조정한다. 2입력 1출력의 퍼지 추론 시스템을 사용하여, 제안한 속도패턴을 가진 엘리베이터 시스템을 정밀 제어한다. In this paper, a new speed pattern generation method is proposed to offer various speed patterns for the traffic changes, with the comfortable driving and the rapid transportation speed that are two important factors to determine elevator speed pattern. To reduce the speed shift impulse, acceleration and deceleration times are appropriately adjusted to the elevator system when start and stop. In order to improve transportation capability, the jerk is also adjusted to the traffic change. Using fuzzy inference system with 2 input variables and 1 output, the elevator system controls precisely, with the proposed speed pattern.

        복합적인 기계적 자극이 중간엽 줄기세포의 골 분화에 미치는 영향

        In this study we investigated the combinational stimulation effects of uniaxial tension and intermittent hydrostatic pressure (IHP) on the differentiation of mesenchymal stem cells (MSCs) into osteoblast without any biochemical agents. For this, we designed a novel bioreactor which can generate not only tensile stimulation and IHP but also combinational stimulation of tension and IHP. We classified experimental groups based on the types of stimulation as follows: (1) NS: no stimulation as a control; (2) IHP: IHP stimulation only (magnitude: 0.1 MPa, intermittent pattern of hydrostatic pressure: 2 min/15 min (on/off), duration: 3 hr/day, for 2 days); (3) TS: tensile stimulation only (magnitude: 3%, frequency: 0.5 Hz, 48 hr in a row); (4) CS: combinational stimulation of (2) and (3). We conducted various biological analyses; DNA contents, ALP activity, Live & Dead staining, F-actin staining, von Kossa staining, and ALP staining. As results, the groups applied mechanical stimulation (IHP, TS, and CS group) showed higher proliferation rate than the group applied no stimulation (NS) at day 8. And amounts of ALP normalized by DNA contents in the groups of TS and IHP were higher than those in CS group at day 5 and day 8, respectively. The highest expressions of ALP and von Kossa staining were observed in IHP group. The expressions of ALP and von Kossa staining in CS were negligible compared with IHP. From this study, we concluded that the single type of mechanical stimulation (IHP or tension) has positive effects but the effect of combinational mechanical stimulation was imperceptible on osteogenesis of MSCs.

        설계영역 반복축소법에 의한 사출금형의 수지 유동균형을 위한 게이트 위치 최적화

        This study introduces an optimization methodology for the determination of gate location that ensures the melt flow balance within a part cavity of injection mold. A new sequential direct-search scheme based on the recursive reduction of the designer-specified gate design area is developed, and it is integrated with a commercial flow simulation tool for optimization. To quantify the level of melt flow balance, we employ the maximum difference among the fill times for the melt fronts to reach the boundary elements of part cavity as objective function. The proposed methodology is successfully applied in the case study of melt flow balancing in molding of a bar code scanner model. The result shows that the melt flow balance at the optimized gate positions is significantly improved from that for the initial gate position.

        휴대폰 커버 사출성형의 CAE 해석 및 최적화

        This paper deals with an CAE analysis and optimization of injection molding for a mobile phone cover. Two design goals are established in the optimization; one is to switch over the feed system from cold runner to hot runner for the purpose of reducing material costs, and the other is to minimize the warpage in order to improve product quality. By the full-factorial experiments for design parameters, we showed that the cold runner design could be changed to the hot runner design by replacing the current resin with a new resin of higher fluidity. In addition, we could significantly reduce the warpage of the cover product under the hot runner system by optimizing packing pressure and packing time.

        How to control the differentiation of mesenchymal stem cells (MSCs) into vascular lineage cells have still left much unclear even with numerous studies. Most in vitro studies on the differentiation of stem cells have utilized biochemical reagents. However, recent studies show the potentials of physical and co-culturing environments in control of differentiation. Therefore, we first factored out the in vivo conditions into biochemical reagents, co-culture, and physical stimulation. Then combinational effects of those factors on the differentiation of MSCs into vascular lineage cells were studied. For this EGM (R) -2, co-culture, and shear stress were adopted to meet biochemical, neighboring cells, and physical condition, respectively. A total 4 experimental groups were set depending on co-culture and shear stress. Cell density was 1×104cells/cm2 at all group. For the co-cultures, MSCs were mixed with human umbilical vein endothelial cells (HUVECs) in the ratio of 1:2. The 12 dyne/cm2 of shear stress was engaged: 4 hr/ day for 2 days followed by 48 hr after seeding. For the analyses, the immunofluorescent staining and PCR was performed to evaluate the differentiation of MSCs into vascular lineage cells. The co-culture group without shear stress showed the ECs-related markers even on the first day of the culture. And this was more observable on day 4. However, differentiation of MSCs into smooth muscle cells was found dominant when shear stress was engaged even MSCs were cultured with HUVECs and EGM (R) -2. From this study we confirmed that physical stimulation (12 dyne/ cm2 in our study) plays an important role in controlling differentiation of MSCs into vascular lineage cell types when MSCs were co-cultured with ECs.

        유방암 치료후에 발생한 속발성 급성 전골수성 백혈병

        N/A Acute leukemia after chemotherapy and/or radio- therapy for malignant disorder is called secondary acute leukemia(SAL) or therapy related leukemia. Typically, SAL is preceded by a prodrome phase or myelodysplastic syndrome (MDS) resulting in trilineage dysplasia manifested by pancytopenia, associated with cytogenetic abnormalities involving chromosome 5 and 7, and carries a poor prognosis. Although FAB M3 type(acute promyelocytic leukemia) of SAL had been rarely reported, recently there was a report on 16 cases of therapy-related acute promyelocytic leukemia. We hereby report a case of secondary acute promyelocytic leukemia after treatment of advanced breast cancer. A 62-year-old woman was admitted because of pancytopenia, purpura of legs and gingival bleeding. Since six years before admission when she was diagnosed as infiltrating ductal carcinoma of breast with metastases to lung, She had received CMF(cyclophosphamide, methotrexate, 5-fluorouracil) chemotherapy and radiotherapy, and underwent modified radical mastectomy after complete disappearance of pulmonary nodules, She had received further chemotherapy with cumulative doses of chemotherapeutic agents as following; 21g of cyclophosphamide, 410mg of doxorubicin, 21g of 5-fluorouracil, and 600 mg of methotrexate. Radiation doses to chest wall, axilla and supraclavicular area were 2880 to 3400cGy. The examination of bone marrow aspiration and biopsy showed acute promylocytic leukemia(FAR M3). Cytogenetic analysis of peripheral blood demonstrated 46XX[1]/46XX, t(7; 11) (p15; p15) [12]. The patient did not achieved a complete remission despite retinoic acid therapy and intensive chemotherapy with Ara-C and AMSA, and died 10 months after diagnosis of acute promyelocytic leukemia.
