흉막액 Adenosine Deaminase 활성도의 진단적 가치에 관한 연구

Adenosine deaminase(ADA) is an enzyme which irreversibly hydrolyzes adenosine into inosine and ammonia. Recently, as preventive medicine and chemotherapy have made remarkable progress. cases of pulmonary tuberculosis and tuberculous pleuritis have greatly decreased, but their incidence remain still high in Korea. At the same time, we have seen an increasing number of patients with malignant pleuritis. Therefore, we often have a problem on the differential diagnosis of pleuritis. We have investigated 67 cases of pleuroperitoneal effusions to see the specificity of ADA determination in their diagnosis. The pleural effusions were due to tuberculosis (41 cases), malignancy(17 cases), pneumonia(3 cases), and systemic lupus erythematosus(1 case). The peritoneal effusions were due to liver cirrhosis(5 cases). The results obtained are as follows: 1) In tuberculous effusions, the ADA activity in pleural fluid was clearly higher tban that in all other effusions. 2) In almostly a)1 patients with pleuroperitoneal effusions, serum ADA activity fell within the reference limits for ADA activity in normal humal serum. 3) In tuberculous effusions, total lymphocytes count in pleural fluid was significantly higher than that malignant effusions, but there was no statistical correlation between the ADA activity and the total lymphocytes count in both groups. 4) There was no statistical correlation between the ADA activity and the total protein in tuberculous and malignant pleural effusions respectively. It is suggested that measurement of ADA activity in pleural effusions is of value in the differential diagnosis of patients with tuberculous pleuritis from those with malignant pleuritis and others, paticularly when the results of other laboratory and clinical tests are negative, if the value is over 50 unit/L.

We investigated the changes in a contaminated bay sediment when some oxygen releasing compounds(ORC) were put in it: is was found that the ORCs such as CaO2 were good at suppression of H2S and removal of phosphorus by changing the neutral pH into weak alkaline pH. In particular, performance of CaO2 was the best of all probably because of the faster establishment in O2 generation and weak alkaline condition. Consequently, it was concluded that ORCs instead of sea cultivation or red clay spreading would promote the conditions better in which O2 should kept being generated and active microbial population for bioremediation should be maximized.
한국인에서 다발성 내분비선종 증후군의 초기 증상 및 임상 양상

Purpose: Multiple endocrine neoplasia (MEN) syndrome is an inherited, autosomal dominant disease that presents as a combination of several endocrine tumors. Early diagnosis of this syndrome is difficult, because of the nonspecific symptoms and signs. This study analyzed early manifestations and clinical characteristics in patients with MEN syndrome. Methods: Medical records were retrospectively reviewed and telephone interviews were conducted with 35 patients diagnosed as MEN syndrome at Samsung Medical Center from December 1994 to December 2009. Results: The 35 patients had been diagnosed as MEN1 (n=14), MEN2A (n=19) and MEN2B (n=2). The early manifestations of the 14 MEN1 patients were related with hyperparathyroidism (n=5), pituitary tumor (n=3), and pancreatic endocrine tumor (n=2). There were tumors of the parathyroid gland in all 14 patients, anterior pituitary in eight patients, and pancreatic islet cells in seven patients. Four cases were incidentally detected during the screening examination. Six cases harbored a MEN1 gene mutation. The twenty-one patients diagnosed with MEN2 comprised medullary thyroid cancer (n=20), adrenal pheochromocytoma (n=15), and hyperparathyroidism (n=4). The MTC-related symptoms in the 21 MEN2 patients included neck mass or discomfort in 12 patients and pheochromocytoma-related symptoms in seven patients. Two cases were detected through familial genetic screening test. The RET gene mutationwas detected in 19 cases. Conclusion: Early manifestations of MEN syndrome were very different between the types of MEN and the types of its presenting tumor. The early diagnosis and proper management of MEN requires awareness of the clinical characteristics of each expressed tumor and is influenced by genetic screening methods. The purpose of this study was to evaluate effects of chitin on open wound healing in rabbits. Using 15 rabbits, a round, full thickness defect of skin with 2cm in diameter was made on the ventral surface of the each ear. The experimental animals were divided into 3 groups:control group untreated group), base ointment group (group treated with base ointment), and chitin ointment group(group treated with water soluble chitin ointment). A total of 30 wounds were made and each subgroup was allocated 10. On the 7th day after initial wounding, each wound site including surrounding tissue was taken for macroscopic and histological observations. The amount of epithelialization and granulation tissue were measured by Image Analysis System.In macroscopic examination, larger amount of exudate and granulation tissue were observed in water soluble chitin ointment group compared to control and base ointment groups.In histological examination, water soluble chitin ointment group showed thick epithelium and larger amount of granulation tissue and fibroblast deposition. The areas of epithelialization and granulation tissue were significantly increased in water soluble chitin ointment group compared to control and base ointment groups.In conclusion, water soluble chitin accelerated open wound healing by accelerating reepithelization and granulation tissue formation.
