Fever in cancer patients is often due to the following causes: evil qi and toxity stagnancy, disorders of qi and blood, deficiencies of zang and fu organs, and the disorder of yin and yang. The treatments given to cancer patients with a fever are according to five: (a) Excessive inner heat and toxicants: remove heat and the toxicant, induce purgation. We use Cheng-Qi-Tang plus Qing-Wen-Bai-Du-Yin. (b) Tangle of damp and heat, and qi stagnancy: remove damp and heat, smooth the qi channel. We use Gan-Lu-Xiao-Du-Dan or San-Ren-Tang. (c) Obvious blood and heat stagnancy: remove heat and blood stasis. We use Xue-Fu-Zhu-Yu-Tang. (d) Deficiency of spleen qi, inner heat caused by a yin deficiency: nourish spleen qi and yin to remove the inner heat. We use Bu-Zhong-Yi-Qi-Tang or Xiao-Jian-Zhong-Tang. (e) Prominent yin deficiency and hectic fever: replenish yin and remove inner heat. We use Qing-Hao-Bie-Jia-Tang or Chai-Qian-Mei-Lian-San. The pathogenesis of fever in cancer patients is complicated. We can see both deficiency and excess in one differentiation. Therefore, we must make sure of it, then we can get the most effective treatment.
The Influence of a Second Metal on the Ni/SiC Catalyst for the Methanation of Syngas

The catalytic performance of silicon carbide supported nickel catalysts modified with or without secondmetal (Co, Cu and Zn) for the methanation of CO has been investigated in a fixed-bed reactor using a feed consisting of25% CO and 75% H2 without any diluent gas. It has been found that the introduction of Co species can clearly improvethe catalytic activity of Ni/SiC catalyst, whereas the addition of Cu or Zn can result in a significant decrease in the catalyticactivity. The characterizations by means of XRD, TEM, XPS, CO-TPD and H2-TPR indicate that the addition ofCo could decrease the particle size of active metal, increase active sites on the surface of methanation catalyst, improvethe chemisorption of CO and enhance the reducibility of methanation catalysts. Additionally, the special interaction betweenCo species and Ni species is likely favorable for the dissociation of adsorbed CO on the surface of catalyst, and this mayalso contribute to the high activity of 5Co-Ni/SiC catalyst for CO methanation reaction. For 5Cu-Ni/SiC catalyst and 5Zn-Ni/SiC catalyst, Cu and Zn species could cover partial nickel particles and decrease the chemisorption amount of CO. These could be responsible for the low methanation activity. In addition, a 150h stability test under 2 MPa and 300 oCshowed that 5Co-Ni/SiC catalyst was very stable for CO methanation reaction.

Background: Many clinical trials have been conducted to evaluate sorafenib for the treatment of advanced NSCLC, but the results for efficacy have been inconsistent. The aim of this study was to evaluate the efficacy and safety of sorafenib in patients with advanced NSCLC in more detail by meta-analysis. Methods: This meta-analysis of randomized controlled trials (RCTs) was performed after searching PubMed, EMBASE, ASCO Abstracts, ESMO Abstracts, and the proceedings of major conferences for relevant clinical trials. Two reviewers independently assessed the quality of the trials. Outcomes analysis were disease control rate (DCR), progression- free survival (PFS), overall survival (OS) with 95% confidence intervals (CI) and major toxicity. Subgroup analysis was conducted according to sorafenib monotherapy, in combination with chemotherapy or EGFR-TKI to investigate the preferred therapy strategy. Results: Results reported from 6 RCTs involving 2, 748 patients were included in the analysis. Compared to sorafenib-free group, SBT was not associated with higher DCR (RR 1.31 (0.96- 1.79), p=0.09), PFS (HR 0.82 (0.66-1.02), p=0.07) and OS (HR 1.01 (0.92-1.12), p=0.77). In terms of subgroup results, sorafenib monotherapy was associated with significant superior DCR and longer PFS, but failed to show advantage with regard to OS. Grade 3 or greater sorafenib-related adverse events included fatigue, hypertension, diarrhea, oral mucositis, rash and HFSR. Conclusions: SBT was revealed to yield no improvement in DCR, PFS and OS. However, sorafenib as monotherapy showed some activity in NSCLC. Further evaluation may be considered in subsets of patients who may benefit from this treatment. Sorafenib combined inhibition therapy should be limited unless the choice of platinum-doublet regimen, administration sequence or identification of predictive biomarkers are considered to receive better anti-tumor activity and prevention of resistance mechanisms.

A 48-year-old woman presented with a 50-day history of irregular vaginal bleeding and lower abdominal pain. Ultrasound indicated an extremely large occupying lesion in the pelvic cavity that was highly suggestive of malignancy. Fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) was performed to further assess the nature of pelvic abnormality. PET/CT images demonstrated a diffusely lobulated mass ranging from cervix up to the inferior pole of kidneys with mild FDG uptake. Simultaneously, multiple nodules in bilateral lungs and a hypodense lesion in the right ventricle were shown without FDG-avidity. Based on the imaging results, the presumptive diagnosis was uterine intravenous leiomyomatosis with intracardiac extension and pulmonary benign metastases, which was subsequently confirmed by MRI and the lesion biopsy.

Post-earthquake fire is a common disaster which causes serious safety issues to infrastructures. This study aims to investigate the residual loading capacities of circular concrete-filled steel tube (CFST) columns under post-earthquake fire experimentally and numerically. The experimental programme contains two loading steps - pre-damage cyclic loading at room temperature and transient state tests with constant compression loads. Three finite element models are developed and validated against the test results. Upon validation, a total of 48 numerical results were generated in the parametric study to investigate the effects of thickness and strengths of steel tube, axial compression ratio and damage degree on the fire resistance of circular CFST columns. Based on the analysis on experimental and numerical results, the loading mechanism of circular CFST columns is discussed. A design method is proposed for the prediction of fire resistance time under different seismic pre-damage and compression loads. The predictions by the new method is compared with the newly generated experimental and numerical results and is found to be accurate and consistent with the mean value close to the unity and a coefficient of variation around 1%.

Background/Aims To determine the value of salivary pepsin in discriminating sub-types of gastroesophageal reflux disease (GERD) and GERD-related disorders. Methods Overall, 322 patients with different sub-types of GERD and 45 healthy controls (HC) were studied. All patients took Gastroesophageal Reflux Disease Questionnaire (GerdQ) and underwent endoscopy and 24-hour esophageal pH monitoring and manometry. Salivary pepsin concentration (SPC) was detected by using colloidal gold double-antibody immunological sandwich assay. Oral esomeprazole treatment was administrated in the patients with non-erosive reflux disease (NERD) and extra-esophageal symptoms (EES). Results Compared to HC, patients with erosive esophagitis, NERD, EES, EES plus typical GERD symptoms, or Barrett’s esophagus had a higher prevalence of saliva and SPC (all P < 0.001). There was no significant difference in the positive rate for pepsin in patients with functional heartburn or GERD with anxiety and depression, compared to HC. After esomeprazole treatment, the positive rate and SPC were significantly reduced in NERD (both P < 0.001) and in EES (P = 0.001 and P = 0.002, respectively). Of the 64 NERD patients, 71.9% (n = 46) were positive for salivary pepsin, which was significantly higher than the rate (43.8%, n = 28) of pathological acid reflux as detected by 24-hour esophageal pH monitoring (P = 0.002). Conclusions Salivary pepsin has an important significance for the diagnosis of GERD and GERD-related disorders. Salivary pepsin and 24-hour esophageal pH monitoring may complement with each other to improve the diagnostic efficiency.

The traditional balance strategy with a sort method has some disadvantages and can cause different types of deteriorations in an MMC system, including an increase in the switching frequency and distortions in the currents. However, quantitative analyses for these deteriorations are neglected during the design of the balance strategy, which decreases the effect of the strategy. This paper first develops accurate descriptions for an MMC system and the structure of its balance strategy. Based on these descriptions, the root cause and quantitative degree of the switching frequency increase are both determined for the first time. A quantitative analysis for the degree of the current distortion is obtained as well. Based on the above analysis, a state switcher and balancing-time calculator are proposed and introduced into the balance strategy, which effectively decrease the magnitudes of these deteriorations. The deteriorations caused by the traditional balance strategy and efficiency of the improved balance strategy are finally demonstrated via experimental results.

Background: Chronic inflammation could affect the occurrence and development of malignant tumors. To explore the levels of tumor necrosis factor ${\alpha}$ (TNF-${\alpha}$) and C-reactive protein (CRP) in patients accompanied by impaired glucose tolerance (IGT) and their clinical significance. Materials and Methods: A total of 210 patients hospitalized in Affiliated Hospital of Taishan Medical University from Jun., 2013 to Dec., 2014 were selected, in which 92 cases were accompanied by IGT. Meanwhile, 80 randomly-selected healthy people by physical examination were as the control. The levels of routine biochemical indexes, plasma TNF-${\alpha}$ and CRP in all subjects were measured. Results: Both systolic and diastolic pressures in hypertension group and hypertension plus IGT group were significantly higher than in control group (p<0.01), but there was no statistical significance between these two groups (p>0.05). The levels of fasting plasma glucose (FPG) and blood glucose 2 h after taking glucose in hypertension plus IGT group were markedly higher than other groups (p<0.01). Homeostasis model assessment-insulin resistance (HOMA-IR), TNF-${\alpha}$ and CRP contents were on the progressive increase in control, hypertension and hypertension plus IGT groups, but significant differences were presented among each group (P<0.01). Hypertension accompanied by IGT had a significantly-positive association with CRP, TNF-${\alpha}$, FPG and blood glucose 2h after taking glucose. Conclusions: The levels of plasma TNF-${\alpha}$ and CPR in patients with hypertension accompanied by IGT increase significantly, indicating that inflammatory reaction in these patient increases, thus suggesting that these patients should be focused regarding cancer prevention.

In our recent work (Ma et al., in Planta229(3):457–469, 2009a and 229(4):1077–1086, 2009b),two three-intron type III PKS genes, PcPKS1 and PcPKS2,were isolated from Polygonum cuspidatum Sieb. et Zucc. Phylogenetic and functional analyses revealed PcPKS1 is athree-intron chalcone synthase (CHS) gene, and PcPKS2 isfound to be a three-intron benzalacetone synthase (BAS)gene. The regular CHS encoded by a single intron genehave not been isolated and characterized from P. cuspidatum. In this work a further CHS with one intron (PcPKS3)and a stilbene synthase (STS) gene with three-intron(PcPKS5) were isolated and characterized by functionaland phylogenetic analyses. In comparison with PcPKS1, abifunctional enzyme with both CHS and BAS activity, theenzymatic product of recombinant PcPKS3 was naringenin,bis-noryangonin (BNY) and 4-coumaroyltriacetic acidlactone (CTAL) occurred as side products. The PcPKS5synthesized resveratrol and a trace amount of naringeninfrom p-coumaroyl-CoA. To our knowledge, PcPKS5 is thefirst reported three-intron STS gene in flowering plants. Inthis work, we speculated that this involved a possibleevolutionary route of plant-specific type III PKS superfamilyin P. cuspidatum.
