        각종 불소처치 이후 시간변화에 따른 타액내 불소농도 변화에 관한 연구

        구강내의 불소농도를 증가시키는 여러 방법에는 불화된 상수도를 섭취하거나 불소보조제를 복용하는 전신적 투여방법과 불소치약, 불소양치액, 불소젤의 도포와 같은 국소적 투여방법이 있다. 그동한 다양한 delivery system을 통한 국소도포용 불소제제가 임상적으로 우식증을 예방하는데 도움이 된다는 사실이 확인되었다. 최근의 연구에서 불소의 항우식효과는 치아주의의 oral fluid environment속에서 불소의 중요성을 지적하고 있다. 비자극성 전타액내 불소농도는 주어진 시간동안 치면과 상호작용이 가능한 aqueous phase의 불소를 나타낸다고 한다. 따라서 타액내의 불소 농도를 조사함으로써 간접적으로 우식예방효과에 대한 정보를 얻을 수 있다. 우식증예방을 위해 환자에게 여러 가지 국소도포방법 중 한 가지를 추천하기에 앞서 이들 각 방법이 타액내 불소농도를 증가시키는데 얼마나 효과적인지 알 필요가 있다. 이에 저자는 국소적 불소도포후 구강내 불소의 잔류량과 시간별 농도를 비교하고자 현재 국내에서 많이 쓰이고 있는 서로 다른 네가지 불소제제를 사용하고 난 뒤 시간변화에 따른 타액내 불소농도를 HMDS를 이용한 확산법과 불소이온전극을 사용하여 측정하고 다음과 같은 결과를 얻었다. 1. 비자극성 혼합타액내 평균 불소농도는 0.0152ppm±0.0091ppm.이었으며, 비자극성 타액분비율은 0.34-0.36ml/min으로 각군간에 통계적 유의차는 없었다. 2. 불소제제 사용직후를 제외하고는 모든 구간에서 타액내 불소 농도는 APF gel군> neutral gel군>불소양치액군>불소치약군의 순이었고, 불소처치 120분 경과후 불소치약군의 타액내 불소농도가, 180분 경과후 불소양치액군의 불소농도가 대조군과 통계적 유의차를 보이지 않은 반면, 6시간 경과후까지 APF gel군> neutral gel군의 타액내 불소농도가 대조군에 비해 유의성있게 높게 유지되었다(p<0.05) 3. 불소제제 사용직후부터 120분 경과후까지 타액내 잔류불소량(AUC0-120min)은 neutral gel군>APF gel군> 불소양치액군>불소치약군의 순이었고, neutral gel군과 APF gel군이 대조군과 나머지 두 군에 비해 유의하게 높은 값을 보였다.(p<0.05) Several alternatives for increasing the fluoride concentration in the mouth, such as water fluoridation, ingestion of fluoride supplements, fluoride paste, fluoride mouthrinse, application of fluoride gel are available. There is an impressive body of evidence that the topically deliverd fluorides are clinically effective in inhibiting the progression of dental caries. Recent studies on the cariostatic action of fluoride have indicated the importance of fluoride in the fluid environment of the teeth. The fluoride levels in unstimulated whole saliva can be considered indicative of F in the aqueous phase available for interaction with the tooth surface at a given time. The retention of F in the mouth after topical fluoride treatment is considered to be an important factor in the clinical efficacy of F. The aim of this study was to determine the elevation and clearance of fluoride in whole saliv after the following topical flouride treatments using HMDS-diffusion technique and fluoride ion electrode. The obtained results were as follow: 1. Average salivary fluoride concentration in the unstimulated whole saliva was 0.0152ppm±0.0091ppm. Unstimulated salivary flow rate was between 0.34-0.36ml/min and there was no statistically significant difference among the groups(p.0.05) 2. Except for the immediate time after treatment, fluoride levels followed as APF gel>neutral gel>F-rinse>F-paste. There was no statistical difference between the salivary F concentration of F-paste group and that of control group after 2 hours. In case of F-rinse group, after 3 hours the concentration had dropped to baseline value. But there was statistically significant difference among the F concentraion of F gel groups and that of control group(p<0.05) 3. The mean ?? values were followed as neutral gel>APF gel>F-rinse>F-paste, and the values of the two former groups were significantly higher than those of the two latter groups(p<0.05).

        The Agl systems containg various mol % of AgBr and Cul were synthesized. X-ray analyses revealed that all synthesized systems had face centered cubic structures. Using Impendence analyzer electrical conductivity measurements of these systems were carried out at the temperature region of 20 to 220℃. From this measurement, it was found that the activation energy of the system containg 5 mol % AgBr showed the highest conductivity and the smallest activation energy.

      • 탈지작업 근로자의 트리클로로에틸렌 폭로에 관한 조사

        Trichloroethylene(TCE) is a widely used organic solvent, especially in degreasing process of metal manufacturing, however few data concerning its exposure and poisoning were obt-ainable. This survey was performed for more information of TCE exposure in this country. Urine sanmples were collected from 144 TCE handling workers and 46 non-TCE-exposed workers. TCE concentrations of four workshops in working environment were analyzed. The results were as follows; 1. TCE exposed workers were 48 male and 96 female employees. Average total duration of employment of these workers was 39.0 months, average duration of works per day was 10.7 hours, and average duation of TCE handling was 469.7 minutes. 2. The average concentration of urine total trichloro-compounds was 156.32㎎/ g crea-tinine, and that of trichloroacetic acid was 69.2㎎/g creatinine. Of those workers, 78 workers had the concentrations of urine trichloroacetic acid over 75㎎/g, which is the biological exposure index (BEI) of urine trichloroaxetic acid in Korea. Duration of work per day and duration of TCE- related work were significantly longer in the group of workers whose urine trichloroacetic acid concentration was over the criteria level of BEI. 3. TCE concentrations of three workshops were over the level of ACGIH TLV(threshold limit value), 50ppm in TWA (time -weighted average). In remaining one workshop, duration of TCE related works was only one hour per day. There was statistically significant correl-ation between the concentration of TCE in working environment and the proportion of workers in a workshop whose urinary trichloroacetic acid concentration was over the criteria level of BEI. 4. Responses to self-administered questionnaire were not meaningful for differentiation of psychoneurologica symptoms due to chronic TCE exposure from othres.

        정중구개봉합부의 해부학적 구조에 관한 전산화 단층촬영을 이용한 연구

        교정용 임플란트 사용에 있어서 정중구개봉합부위의 골조직의 높이에 대한 구체적인 해부학적 자료가 없어서 임상에 적용시 이론적 근거가 부족한 것이 사실이었다. 이에 본 연구는 전산화단층촬영과 재구성 프로그램인 V works 4.0 (Cybermed Inc, Seoul. Korea)을 이용하여 임플란트 식립을 위한 해부학적 근거를 얻고자 하였다. 성인 남자 14명. 성인 여자 14명의 보철용 임플란트를 위한 CT 자료를 통해 정중 시상면에서 상악골의 길이를 계측하고 정중구개봉합 부위에서 골조직 두께를 측정하며 교정용 임플란트를 안전하게 식립할 수 있는 구체적 부위를 설정하고자 하여 다음과 같은 결과를 얻었다. ANS에서 PNS까지의 상악골의 길이를 계측한 결과 남자에서는 평균 51.08 mm, 여자에서는 평균 47.34 mm 였다. 남녀 사이에는 통계적으로 유의한 차이가 있었다 (p<0.05). 정중구개봉합부위의 골조직 두께를 측정한 결과 남자의 ANS-PNS길이의 중점에서 구개평면을 따라 PNS쪽으로 15 mm지점을 제외하고는 남녀 모두에서 6 mm이상의 골조직이 존재하였다. 정중구개봉합부위에 교정용 임플란트를 적용할 때 안정적인 식립부위는 구개평면을 따라서 ANS로부터 남자는 19.43 mm 후방부위. 여자는 17.62mm 후방부위에 해당하였다. 이상의 결과로 정중구개봉합부위의 안정적인 식립부위에서는 교정용 임플란트 식립이 적합하다고 판단되며 이를 바탕으로 정중구개봉합부위의 교정용 임플란트를 활용하여 교정치료 영역을 넓힐 수 있을 것이다. There is lack of data for the amount of vertical bone in the midpalatal region for miniscrew implantation. The purpose of this study was to measure the structure of the midpalatal suture area using CT image and V-works 4.0 program (Cybermed Inc, Seoul, Korea). CT images of 14 male and 14 female adults were reconstructed. In detail, it was 1) to measure the length of maxilla on the midsagittal plane 2) to measure vertical bone height in the midpalatal area 3) to establish the zone of safety for miniscrew implantation. The following results were obtained, The mean length of ANS-PNS was 51.08 mm in males and 47.34 mm in females. There was a statistically significant difference between males and females (p < 0.05). The vertical bone height of the midpalatal suture area was above 6mm except for 15 mm posterior from the central point of ANS-PNS in males. The zone of safety was located 19.43 mm posterior from the ANS in males while it was 17.62 mm in females along the palatal plane. These results support that the safety zone of the midpalatal area is suitable for screw implantation. Midpalatal miniscrew implantation is a powerful tool in modern orthodontics. Through many applications, it can expand the modern orthodontic field.

        법랑라세포종의 면역조직화학적 및 전자현미경적 연구

        Clinicopathologic analysis of ameloblastoma in 24 cases and their ultrastructural and immunohistochemical studies, especially to cytokeratin expressions according to morphologic classification were performed to clarify the tumor cell differentiation and origin. Reaction with broad spectrum and high molecular weight cytokeratin were generally positive in the stellate cell of the follicular and acanthomatous ameloblastoma, suggesting more squamous differentiation than peripheral basal cell type. Although ultrastructually common origin from oral epithelium was evidenced by presence of tonofilaments and desmosome, variations were prominent among the different histopathologic types of tumor. In granular cell ameloblastoma there were pleomorphic osmiophilic lysosome-like granules.

      • 진행성 위암에 동반된 위 칸디다증 1예

        Gastric candidiasis is an uncommon disorder. It is an opportunistic pathogen in immune compromised individuals, alcoholics, persons treated with broad-spectrum antibiotics, immunosuppressive and chemotherapeutic agents. Besides, the use of drugs that reduce the gastric acid production may be a cause of gastric candidiasis. We experienced a case of gastric candidiasis following advanced gastric adenocarcinoma. A 85-year-old woman was admitted to our hospital because of epigastric soreness. Endoscopic findings revealed a large ulcero-infiltrating mass with yellow exudate at cardia of the stomach, which suggested an advanced gastric cancer. Endoscopic biopsy revealed spores and pseudohyphae infiltrating through the ulcerated stomach wall, diffuse infiltration of tumor cells and abortive tumor glands. After treatment with fluconazole 100 mg daily for 21 days, no evidence of the gastric candidiasis was seen on endoscopic biopsy and pathologic findings. The case is herein reported of gastric candidiasis following advanced gastric adenocarcinoma along with a review of corresponding literature.
