      • 골수염의 진단에 있어서의 Tc-99m Hexamethylpropylene amine oxime 백혈구 스캔의 임상적 유용성

        배경 : 골수염의 진단에 있어 Technetium-99m을 이용한 삼상 골스캔(Tc-99m MDP스캔)은 단순방사선 검사상 정상소견을 보이는 경우에 쓰이는 1차적 검사로 매우 높은 민감도를 보인다. 그러나 Tc-99m MDP 스캔은 골절이나, 정형외과적 삽입물, 신경병적 관절병변과 같이 뼈의 재형성이 증가되는 상황에서도 양성소견을 보여 특이도가 낮은 단점이 있다. 이러한 단점을 보완하고자 골수염의 진단에 있어 Tc-99m HMPAO 백혈구 스캔의 유용성을 알아보고자 하였다. 대상 및 방법 : 골수염이 의심되어 Tc-99m HMPAO 백혈구 스캔을 시행했던 15명의 의무기록을 후향적으로 분석하였다. 골수염의 확진은 임상관찰 소견과 배양검사 결과와 병리소견을 기준으로 하였다. 결과 : 골수염의 진단에 있어 Tc-99m HMPAO 백혈구 스캔의 민감도와 특이도는 각각 100%와 78%였고 Tc-99m HMPAO 스캔의 경우는 100%와 18%였다. 가장 흔한 원인균은 methicillin-resistant S. aureus였다. Tc-99m MDP 스캔에서 양성소견을 보였던 환자중 3명의 환자가 Tc-99m HMPAO 백혈구 스캔에서 음성 소견을 보였으며 이 환자들은 추적관찰상 골수염이 없는 것으로 확인되었다. 결론 : Tc-99m HMPAO 백혈구 스캔은 골수염의 존재를 진단하는데 있어 민감도와 특이도가 높으며, 특히 뼈의 재형성이 증가되어 Tc-99m MDP 스캔의 특이도가 낮게 나타나는 상황에서 유용하리라 사료된다. Background Three phase bone scintigraphy, performed with technetium-labeled diphosphonates (Tc-99m MDP scan), is the very sensitive radionuclide procedure of choice for diagnosing osteomyelitis. But, Tc-99m MDP scan is less specific when bone remodeling is increased. To overcome this limitation, we evaluated the diagnostic value of technetium-99m hexamethylpropyleneamine oxime (HMPAO) labeled leukocyte scan in osteomyelitis. Methods : We retrospectively reviewed 15 patients with suspected osteomyelitis who performed Tc-99m HMPAO labeled leukocyte scan. The diagnosis of osteomyelitis was based on clinical course and bacterial culture, and pathologic findings. Results The sensitivity and specificity of Tc-99m HMPAO leukocyte scan was 100% and 78%, and those of Tc-99m MDP scan was 100% and 18%, respectively. The most common organism was Methicilein-Resistant S. aureus. Among the patients that showed persistent accumulation on Tc-99m MDP scan, three patients showed no uptake on Tc-99m HMPAO leukocyte scan and these patients were confirmed that had no osteomyelitis. Conclusion : Tc-99m HMPAO leukocyte scan is a useful test for determining the presence or absence of osteomyelitis, especially under condition that bone remodelin is increased.

      • A Case of Papillary Thyroid Cancer Recurring as an Esophageal Submucosal Tumor

        <P>A 75-year-old woman who underwent a total thyroidectomy for papillary thyroid cancer 7 years previously presented with a palpable neck mass. Computed tomography (CT) showed two metastatic masses on the thyroid bed and another mass that looked benign originating from the esophageal wall. Endoscopic ultrasonography (EUS) showed a hypoechoic mass in the esophageal wall that looked similar to a gastrointestinal stromal tumor. The mass on the esophagus had intense fluorodeoxyglucose (FDG) uptake in positron emission tomography-computed tomography (PET-CT), which suggested the possibility of malignancy. Subsequently, after surgery, the mass in the esophagus was confirmed as a metastasis from the thyroid papillary carcinoma. Here we report this unusual case of papillary thyroid cancer that recurred as an esophageal submucosal tumor.</P>

        A multicenter, prospective, randomized clinical trial of marine mussel-inspired adhesive hemostatic materials, InnoSEAL Plus

        Purpose: InnoSEAL Plus is an adhesive, coagulant-free hemostatic material that mimics the adhesion mechanism of marine mussels. This study reports on the safety and efficacy of InnoSEAL Plus for patients with hemorrhage after hepatectomy despite first-line hemostasis treatments. Methods: This is a multicenter, prospective, single-blinded, randomized clinical trial involving 96 hepatectomy patients. TachoSil was used as a comparator group. Three-minute and 10-minute hemostatic success rates were monitored. Rebleeding rates were also observed. Safety was assessed by recording all novel undesirable symptoms. Results: InnoSEAL Plus showed a 3-minute hemostasis rate of 100%, while TachoSil had a rate of 98.0% (48 of 49 patients), demonstrating that the 2 had similar hemostatic efficacies. The difference in efficacy between the test and comparator group was 2.04%, and the lower limit of the one-sided 97.5% confidence interval was –1.92%; as this is greater than the noninferiority limit of –23.9%, the 2 treatments were equivalent. Meanwhile, the 10-minute hemostatic success rate was the same in both groups (100%). No rebleeding occurred in either group. In the safety evaluation, 89 patients experienced adverse events (45 in the test group and 44 in the comparator group). The difference between the 2 groups was not significant. No death occurred after application of the test or comparator group product. Conclusion: Given that InnoSEAL Plus is a coagulation factor-free product, the hemostasis results are encouraging, especially considering that TachoSil contains a coagulation factor. InnoSEAL Plus was found to be a safe and effective hemostatic material for control of bleeding in hepatectomy patients.

        만성 바이러스성 간염에서 수상세포, HLA-DR 및 CD8+림프구의 역할 : 면역조직화학적 연구 An Immunohistochemical Study

        Backgrounds/Aims: This study focuses on the pathogenesis of inflammatory reaction and cell necrosis in patients with chronic viral hepatitis and examines the possible effects of follicular dendritic cells, HLA-DR and CD8+ presenting lymphocytes by analyzing their expression and the histological activity index (HAI) in liver tissues. Methods: Liver biopsy specimens were obtained from 59 patients with chronic hepatitis B and from 26 patients with chronic hepatitis C. The expressions of dendritic cells, HLA-DR and CD8+ presenting lymphocytes were determined by immunohistochemical stain. Results: The incidence of lymphoid follicle and/or lymphoid aggregates in portal tracts of the liver was higher in chronic hepatitis C than it was in chronic hepatitis B (84.6% vs. 15.3%, p=0.000). Follicular dendritic cells were exclusively expressed within lymphoid follicles and/or lymphocyte aggregates in portal areas. HLA-DR restricted cells were mainly observed in portal and periportal areas as well as in the area of piecemeal necrosis. CD8+ lymphocytes were diffusely expressed in portal and periportal areas and within intralobular parenchymal sinusoids. The expression of dendritic cell and HLA-DR was more frequently observed in moderate chronic hepatitis than in mild chronic hepatitis. While that of CD8+ lymphocyte expression was more frequent in severe chronic hepatitis with a high HAI score. Conclusions: The follicular dendritic cells may trap viral antigens in intraportal lymphoid follicle and present them to HLA-DR and CD8+ presenting lymphocytes. It is suggested that the associated expression of dendritic cells, HLA-DR and CD8+ presenting lymphocytes in liver tissues may play one of the biological role in immune injury in chronic viral hepatitis.(Korean J hepatol 2000;6;448-455)

        Cloud computing is a new technology which targets to reduce cost and improve work efficiency as the hottest trend in information and technology. Therefore, businesses need to be more interested in adopting this strategic technology in setting up a future business plan. From the corporate aspect, the future value of a company and the development direction are depending on how the brand-new technology trend is introduced and taken advantage of. In this point of view, this study has suggested a unique and distinctive cloud computing service method for SMEs through the development of Cloudcast technology. In fact, many SMEs have been hesitating in introducing the cloud computing even though they were very aware of its usefulness. However, this study has helped them to adopt the new technology. In addition, this technology has been developed under a concept of storage expansion based on Infrastructure as a Service (IaaS). Cloudcast could be a very useful tool to SMEs which usually require fast work processing and quick data sharing. In addition, data can be provided in a constant manner upon multiple client accesses, which is the biggest difference, compared to the conventional Internet storage concepts such as Amazon S3 service. This study would offer SMEs a specific way to introduce the latest technology in a faster and more convenient manner and help them enhance their global competitiveness through cost reduction and improvement of work efficiency.

      • 레이저 패터닝이 원형 22MnB5관의 동적 충돌성능에 미치는 영향

        자동차 충돌과정에서 운전자의 안전을 확보함과 동시에 충돌에너지를 흡수하기 위해서 동일한 판넬에 강도구배를 주는 기술은 매우 효과적이다. 레이저 패터닝은 동일한 소재에 강도 구배를 줄 수 있는 가장 정밀한 방법이기 때문에 유용하게 사용된다. 본 논문에서는 동적 충돌성능을 평가하기 위해 그림 1과 같은 시험환경을 준비했으며, 레이저 패터닝을 이용하여 22MnB5소재의 원형관에 0˚, 45˚, 90˚, 어그제틱, 허니컴 형상으로 국소강화한 시편을 제작한다. 레이저 패터닝을 하지 않은 원형 관은 충돌 시 단순히 바깥쪽으로 팽창하면서 일정한 형상으로 변형한다. 하지만, 레이저 패터닝으로 동일한 원형관에 강도 구배를 의도적으로 부여할 경우, 강도가 약한영역이 먼저 변형하면서 변형모드가 달라지게 된다. 이러한 현상을 이용하여, 충돌성능에 유리하도록 의도적으로 변형모드를 유도하는 것이 가능하다는 것을 실제 실험으로 확인했다. 그림 2와 같이 패터닝 형상에 따른 충돌성능을 비교하기 위해 변위에 따른 충돌 하중과 변위에 따른 흡수 에너지의 변화 등을 비교했다. 이때 동등한 비교를 위해 동일한 변위에서의 하중과 에너지 흡수율, 초기 에너지 대비 충돌 후 흡수 에너지의 비율 등을 비교했다. 그 결과 헬리컬 형상으로 패터닝한 튜브의 에너지 흡수율이 가장 높다는 것을 확인했으며, 이는 이전 연구에서 준정적으로 압궤시험을 한 결과와 동일하게 나타났다. 우리는 왜 헬리컬 형상으로 패터닝을 했을 때 에너지 흡수율이 가장 높게 나오는 지 확인하기 위해 그림 3과 같이 충돌 후 원형관의 형상과, 해당 형상으로 변형되는 과정을 초고속카메라로 촬영하여 분석했다.

      • A New Approach to Overcoming Biliary Complications after Laparoscopic Living Donor Liver Transplantation

        Aims: Biliary strictures and bile leaks are one of the major causes of morbidity after liver transplantation. Biliary reconstruction of multiple ducts in grafts is challenging in living donor liver transplantation (LDLT). We introduce our new approach of dunking duct reconstruction of the bile duct for reducing bile duct complications in LDLT. Methods: Twenty consecutive LDLT with laparoscopic living donor graft from February to September 2017 were analyzed through a comparison of 10 conventional duct-to-duct reconstructions (DDR) and 10 dunking hepaticocholedocostomy (DHC) in bile duct anastomoses. Results: At a mean follow-up of 5.4 months, median graft duct number for DDR is one and for DHC is two. Overall, 3 patients developed leakage (15%) and 4 patients suffered stricture (20%). For biliary anastomosis with DDR, the incidence of stricture and leakage was significantly higher (40%, 20%) than DHC (0%, 10%). Conclusions: In conclusion, DHC in graft with multiple ducts as well as in graft with single bile ducts has excellent results with regard to minimizing biliary complications. Therefore, DHC can be recommended as the preferred method for the presence of multiple ducts in the graft in LDLT.
