Risk factors associated with repeated epidural blood patches using autologous blood

Background: An epidural blood patch (EBP) is a procedure to treat intracranial hypotension that does not respond to conservative treatment. EBPs are commonly repeated when the symptoms persist. In this study, we used a large single-center retrospective cohort and evaluated the factors associated with repeated EBPs. Methods: From January 2010 to December 2020, a total of 596 patients were treated with EBPs for intracranial hypotension. We evaluated the factors associated with repeated EBPs in the entire population, in patients with spontaneous intracranial hypotension (SIH), and in those with available myelographies. Results: In a total of 596 patients, 125 (21.1%) patients required repeated EBPs, and 96/278 (34.5%) in SIH and 29/314 (9.2%) in iatrogenic population. In patients with SIH, international normalized ratio (INR) and cerebrospinal fluid (CSF) leakage on myelographies consistently exhibited significant associations (odds ratio [OR], 1.38; 95% confidence interval [CI], 1.02-1.87; P = 0.043 and OR, 2.18; 95% CI, 1.28-3.69; P = 0.004). In patients with iatrogenic injury, INR and CSF leakage on myelogram did not show difference in repeated EBPs. Conclusions: Repeated EBPs may be more frequently required in patients with SIH. Prolonged INR and CSF leakage were associated with repeated EBPs in patient with SIH. Further studies are needed to determine factors associated with repeated EBP requirements.

Association between inflammation-based prognostic markers and mortality of non-cardiac surgery

Background: To evaluate the association between inflammation and nutrition-based biomarkers and postoperative outcomes after non-cardiac surgery.Methods: Between January 2011 and June 2019, a total of 102,052 patients undergoing non-cardiac surgery were evaluated, with C-reactive protein (CRP), albumin, and complete blood count (CBC) measured within six months before surgery. We assessed their CRP-to-albumin ratio (CAR), neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and modified Glasgow Prognostic Score (mGPS). We determined the best cut-off values by using the receiver operating characteristic (ROC) curves. Patients were divided into high and low groups according to the estimated threshold, and we compared the one-year mortality.Results: The one-year mortality of the entire sample was 4.2%. ROC analysis revealed areas under the curve of 0.796, 0.743, 0.670, and 0.708 for CAR, NLR, PLR, and mGPS, respectively. According to the estimated threshold, high CAR, NLR, PLR, and mGPS were associated with increased one-year mortality (1.7% vs. 11.7%, hazard ratio [HR]: 2.38, 95% CI [2.05, 2.76], P < 0.001 for CAR; 2.2% vs. 10.3%, HR: 1.81, 95% CI [1.62, 2.03], P < 0.001 for NLR; 2.6% vs. 10.5%, HR: 1.86, 95% CI [1.73, 2.01], P < 0.001 for PLR; and 2.3% vs. 16.3%, HR: 2.37, 95% CI [2.07, 2.72], P < 0.001 for mGPS).Conclusions: Preoperative CAR, NRL, PLR, and mGPS were associated with postoperative mortality. Our findings may be helpful in predicting mortality after non-cardiac surgery.

Effects of long-term tubular HIF-2α overexpression on progressive renal fibrosis in a chronic kidney disease model

Renal fibrosis is the final manifestation of chronic kidney disease(CKD) regardless of etiology. Hypoxia-inducible factor-2 alpha(HIF-2α) is an important regulator of chronic hypoxia, and thelate-stage renal tubular HIF-2α activation exerts protective effectsagainst renal fibrosis. However, its specific role in progressiverenal fibrosis remains unclear. Here, we investigated the effectsof the long-term tubular activation of HIF-2α on renal functionand fibrosis, using in vivo and in vitro models of renal fibrosis. Progressive renal fibrosis was induced in renal tubular epithelialcells (TECs) of tetracycline-controlled HIF-2α transgenic (Tg)mice and wild-type (WT) controls through a 6-week adeninediet. Tg mice were maintained on doxycycline (DOX) for thediet period to induce Tg HIF-2α expression. Primary TECs isolatedfrom Tg mice were treated with DOX (5 μg/ml), transforminggrowth factor-β1 (TGF-β1) (10 ng/ml), and a combinationof both for 24, 48, and 72 hr. Blood was collected to analyzecreatinine (Cr) and blood urea nitrogen (BUN) levels. Pathologicalchanges in the kidney tissues were observed using hematoxylinand eosin, Masson’s trichrome, and Sirius Red staining. Meanwhile,the expression of fibronectin, E-cadherin and α-smoothmuscle actin (α-SMA) and the phosphorylation of p38 mitogenactivatedprotein kinase (MAPK) was observed using westernblotting. Our data showed that serum Cr and BUN levels weresignificantly lower in Tg mice than in WT mice following theadenine diet. Moreover, the protein levels of fibronectin andE-cadherin and the phosphorylation of p38 MAPK were markedlyreduced in the kidneys of adenine-fed Tg mice. These resultswere accompanied by attenuated fibrosis in Tg mice followingadenine administration. Consistent with these findings, HIF-2αoverexpression significantly decreased the expression of fibronectinin TECs, whereas an increase in α-SMA protein levels wasobserved after TGF-β1 stimulation for 72 hr. Taken together,these results indicate that long-term HIF-2α activation in CKDmay inhibit the progression of renal fibrosis and improve renalfunction, suggesting that long-term renal HIF-2α activation maybe used as a novel therapeutic strategy for the treatment of CKD.

의미연결망 분석을 활용한 교사-부모 의사소통 인식 탐색

본 연구는 의미연결망 분석을 통해 초등학교 교사와 학부모가 지각하는 의사소통 문제의 원인, 의사소통 문제의 개선방안에 대한 인식구조를 탐색함으로써 교사-학부모 간 교육 파트너쉽 형성을 위해 필요한의사소통 문제의 개선점을 모색하고자 하였다. 이를 위해 교사-학부모 간 의사소통에 관한 개방형 질문에 응답한 초등교사 72명, 학부모 94명의 응답내용을 의미연결망 분석 방법으로 분석하였다. 그 결과는다음과 같다. 교사-부모 의사소통의 주요 원인에 대하여 세부적인 내용의 차이는 있지만 부모와 교사는의사소통 원인에 대해서 상호 간 입장의 차이, 부모와 교사의 의사소통의 태도적 문제, 소통의 부족이라는 인식을 공유하고 있는 것으로 나타났다. 다음으로 교사와 부모가 제안하는 부모-교사 의사소통 문제의 개선방안으로 교사와 학부모 둘 다 원활한 의사소통을 위해 의사소통 방법을 개선하여 의사소통 기회를 더 늘리는 것을 제안하였다. 또한 교사와 학부모는 각 입장에서 태도의 변화를 제안하였는데, 구체적으로 교사는 부모가 방어적 태도를 버리고 교사의 전문성을 인정하고 존중하는 태도로 전환할 것을 촉구하였고, 부모는 교사에게 솔직하고 개방적인 태도를 요구하였다. 이에 더하여 교사들은 교권보호와 함께의사소통 과정 중 갈등 발생 시 교사를 보호하고 중재할 수 있는 상급기관의 개입과 부모교육 등 부모와교사 간 교육 파트너쉽 형성을 위해 필요한 일련의 시스템을 제도화할 것을 제안하였다. This study aimed to improve communication problems necessary for the formation of educational partnerships between teachers and parents by exploring the cause of communication problems that elementary school teachers and parents are aware of and how to improve communication problems. To achieve this, 72 elementary school teachers and 94 parents who answered open questions about communication between teachers and parents were analyzed using the semantic network analysis method. The results were as follows: Although there are differences in detail about the main causes of teacher-parent communication, parents and teachers share the perception that the causes of communication are differences in mutual positions, behavioral problems of parents and teachers, and lack of communication. Next, both teachers and parents proposed to improve their communication methods to increase communication opportunities for smooth communication. Teachers and parents also proposed a change in attitude from their respective positions, specifically urging parents to abandon their defensive attitudes and switch to an attitude of recognizing and respecting teachers expertise, while parents demanded a frank and open attitude from teachers. In addition, teachers suggested institutionalizing a set of systems necessary for the formation of educational partnerships between parents and teachers, including intervention by higher-level institutions that can protect teachers and mediating between teachers and parents in the case of conflicts during the communication process and parent education.

Objectives : This study reports a case of Soeumin patients with Herpes zostersMethods : We examined progress of Soeumin Herpes zoster patient's symptoms (skin blisters, pain, insomnia, etc.) and complications when during and after the treatments.Results : Soeumin Herpes zoster patient was diagnosed as Yang Depletion. Yongdamsagan-tang, Sweet BV, were effective for decrease the patient's skin blisters and pain. Soeumin Bojungikgi-tang prevented the patient's aftereffects.Conclusions : Soeumin Herpes zoster patient who had severe pain, blisters, and sleep disorder, was expected high risk of postherpetic neuralgia. But diagnosed Yang Depletion symptomatology and treated with Soeumin Bojungikgi-tang, patient recover completely without sequelae.

Genetic Variations of Drug Transporters Can Influence on Drug Response in Patients Treated with Docetaxel Chemotherapy

<P><B>Purpose</B></P><P>Dose-limiting toxicities of docetaxel are widely considered to be neutropenia, anemia, skin toxicity, and nausea. One of the factors that limit the use of docetaxel is its unpredictability of inter-individual variation in toxicity.</P><P><B>Materials and Methods</B></P><P>In order to identify the genetic factors that affect the risk of docetaxel-induced toxicities, we recruited patients who received docetaxel chemotherapy. We genotyped 92 patients with single-nucleotide polymorphisms (SNPs) in 5 genes: <I>CYP3A4</I> (<I>CYP3A4<SUP>*</SUP>1B</I>, <I>CYP3A4<SUP>*</SUP>18</I>, and <I>CYP3A4<SUP>*</SUP>3</I>), <I>CYP3A5</I> (<I>CYP3A5<SUP>*</SUP>2</I> and <I>CYP3A5<SUP>*</SUP>3</I>), <I>ABCB1</I> (C1236T, G2677G/T, and C3435T), <I>SLCO1B3</I> (rs11045585), and <I>ABCC2</I> (rs12762549).</P><P><B>Results</B></P><P>Out of 92 patients, 70 had grade 3 or 4 neutropenia; 4 had grade 1 or 2; and 18 had no toxicity (76.1%, 4.3%, and 19.6%, respectively). The findings of the SNP analysis showed that patients with TT genotype of <I>ABCB1</I> 3435C>T polymorphism showed significantly higher risk of neutropenia and anemia (p=0.029 and p=0.044, respectively). There were significant associations between docetaxel-induced leucopenia and 2677G/T of <I>ABCB1</I> and rs12762549 of <I>ABCC2</I> (p=0.025 and p=0.028, respectively). In a multivariate analysis, we observed that patients carrying 2677G>T in <I>ABCB1</I>might be associated with higher risk of chemo-resistance when treated with docetaxel (odds ratio [OR], 6.48; confidence interval, 1.92 to 21.94; p=0.003). In a subgroup analysis of non-small cell lung cancer patients, a significant association of tumor response with G2677T/A (OR, 4.54) in <I>ABCB1</I> and <I>SLCO1B3</I> (OR, 9.44) was observed.</P><P><B>Conclusion</B></P><P>Our data suggest that <I>ABCB1</I> (2677G/T) and <I>SLCO1B3</I> (rs11055585) might be major genetic predictors of docetaxel-related toxicities in patients receiving docetaxel chemotherapy.</P>
