        Intraoperative abortion of adult living donor liver transplantation

        Purpose: This study aimed to report intraoperative abortion of adult living donor liver transplantation (LDLT). Methods: From June 1997 to December 2016, 1,179 adult LDLT cases were performed. 15 cases (1.3%) of intraoperative abortions in LDLT were described. Results: Among 15 cases, 5 intraoperative abortions were donor-related, and remaining 10 cases were recipient-related. All donor-related abortions were due to unexpected steatohepatitis. Among remaining 10 recipient-related intraoperative abortions, unexpected extension of hepatocellular carcinoma was related in 5 cases. Two cases of intraoperative abortions were related to bowel inflammation, and 2 cases were associated with severe adhesion related to previous treatment. One recipient with severe pulmonary hypertension was also aborted. Conclusion: Complete prevention of aborted LDLT is still not feasible. In this regard, further efforts to minimize intraoperative abortion are required.

        Mildly Elevated Cardiac Troponin below the 99th-Percentile Upper Reference Limit after Noncardiac Surgery

        Background and Objectives: In patients with perioperative cardiac troponin (cTn) I below the 99th-percentile upper range of limit (URL), mortality according to cTn I level has not been fully evaluated. This study evaluated the association between postoperative cTn I level above the lowest limit of detection but within the 99th-percentile URL and 30-day mortality after noncardiac surgery. Methods: Patients with cTn I values below the 99th-percentile URL during the perioperative period were divided into a no-elevation group with cTn I at the lowest limit of detection (6 ng/L) and a minor elevation group with cTn I elevation below the 99th percentile URL (6 ng/L < cTn I < 40 ng/L). The primary outcome was 30-day mortality. Results: Of the 5,312 study participants, 2,582 (48.6%) were included in the no-elevation group and 2,730 (51.4%) were included in the minor elevation group. After propensity score-matching, the minor elevation group showed significantly increased 30-day mortality (0.5% vs. 2.3%; hazard ratio, 4.30; 95% confidence interval, 2.23–8.29; p<0.001). The estimated cutoff value of cTn I to predict 30-day mortality was 6 ng/L with the area under the receiver operating characteristic curve 0.657. Conclusions: A mild elevation of cTn I within the 99th-percentile URL after noncardiac surgery was significantly associated with increased 30-day mortality as compared with the lowest limit of detection.         DFN 모델과 스미어드 균열 모델을 이용한 불연속 암반의 3차원 수리해석

        본 연구에서는 3차원 불연속균열망을 이용하여 불연속 암반을 모사하여 스미어드 균열 모델을 적용한 불연속 암반의 등가연속체 해석기법에 대해 연구를 실시했다. 연속체 해석 코드인 FLAC3D를 이용하여 모델링을 수행하였고, 해당 적용 모델의 신뢰성을 검증하기 위해 불연속체 해석에 널리 사용되는 3DEC코드를 통해 모델의 메쉬 사이즈, 불연속면의 밀도 및 불연속면의 간극에 대한 민감도 분석을 함께 실시하여 제안 모델의 신뢰성을 검증할 수 있었다. 본 논문에서 제안된 등가연속체 해석기법을 이용한 불연속 암반의 수리해석에 대한 연구결과는 교통 터널, 석유비축기지, 방사성폐기물 처분장 등의 지하 구조물 설계 및 건설 분야에 기초자료로 활용될 것이라고 판단된다. A three-dimensional(3D) equivalent continuum modeling was performed to analyze hydraulic behavior of rock mass considering discontinuities by using DFN model and smeared fracture model. DFN model was generated by FLAC3D and smeared fracture model was applied by using FISH functions, which is built-in functions in FLAC3D, for equivalent continuum model of fractured rock mass. Comparative analysis with 3DEC, which is for discontinuum analysis, was conducted to verify reliability of equivalent continuum analysis by using FLAC3D. Similar results of hydraulic analysis under the same conditions could be achieved. Equivalent continuum analysis of fractured rock mass by using DFN model was implemented to compare with existing analytical methods for inflow into the tunnel.

        <P><B>Abstract</B></P> <P>Poly-γ-<SMALL>D</SMALL>-glutamic acid (PGA) of anthrax is an important pathogenic factor due to its anti-phagocytic activity. Additionally, PGA has the ability to activate mouse macrophages for the secretion of cytokines through Toll-like receptor (TLR) 2. Peptidoglycan (PGN), a major bacterial cell-wall component, induces inflammatory responses in the host. We assessed whether PGA can induce maturation and cytokine expression in immature mouse dendritic cells (DCs) in the existence of muramyl dipeptide (MDP), the minimum motif of PGN with immunostimulatory activity. Stimulation of immature DCs with PGA or MDP alone augmented expression of costimulatory molecules and MHC class II proteins, which are all cell surface markers indicative of maturation. The observed effects were further enhanced by costimulation of PGA and MDP. PGA alone was sufficient to induce expression of TNF-α, IL-6, MCP-1, and MIP1-α, whereas MDP alone did not under the same conditions. Treatment with MDP enhanced PGA-induced expression of the tested inflammatory mediators; however, the synergistic effect found for PGA and MDP was not observed in TLR2- or nucleotide-binding oligomerization domain (NOD) 2–knockout DCs. Additionally, MDP augmented PGA-induced MAP kinases and NF-κB activation, which is crucial for expression of cytokines. Furthermore, MAP kinase and NF-κB inhibitors attenuated MDP enhancement of PGA-induced cytokine production. In addition, co-culture of splenocytes and PGA/MDP-matured DCs induced higher expression of IL-2 and IFN-γ compared to that of splenocytes and PGA-matured DCs. Collectively, our results suggest that PGA and MDP cooperatively induce inflammatory responses in mouse DCs through TLR2 and NOD2 via MAP kinase and NF-κB pathways, subsequently leading to lymphocyte activation.</P> <P><B>Highlights</B></P> <P> <UL> <LI> MDP enhances PGA-induced maturation of mouse DCs. </LI> <LI> MDP enhances production of inflammatory mediators by PGA. </LI> <LI> TLR2, NOD2, MAP kinase, and NF-κB signaling pathways are involved in PGA/MDP-induced cytokine production. </LI> <LI> MDP increases lymphocyte activation induced by PGA-matured DCs. </LI> </UL> </P>

      • 전산유도체 NFATc.β에 대한 항체생산

        전사유도체 NFAT는 면역반응의 유도와 활성 조절에 중요한 역할을 하는 cytokines의 발현에 필수적 역할을 한다. 현재 서로 다른 유전자로부터 생산되는 4종류의 단백질과 각 단백질들의 RNA splicing variants들이 발견되었으며, 단백질 1차 구조와 기능의 유사성으로 이 단백질들을 NFAT family로 분류하고 있다. 본 연구에서는 NFAT family member중의 하나인 NFATc.β의 재조합 단백질을 생쥐에 주사하여 이 단백질에 대한 항체를 생성유도하였으며, immunoblot assay를 통하여 혈장내에 항체 생성을 확인하였다. Transcription factor NFAT plays a pivotal role in the inducible production of several cytokines that are important for coordinating the immune effector response. According to the similarity in their primary structure and function, 4 different proteins that are encoded by distinct genes and their RNA splicing variants define a new family of transcription factors, an NFAT family. Here we have generated a polyclonal antibody against human NFATc.β. and NFAT family member, by immunizing mice with a bacterial recombinant protein of human NFATc.β(214-462). Antibody production was confirmed with immunoblot analysis.

      • Error influence of radar rainfall estimate on rainfall runoff

        Radars have been widely employed to detect precipitation and to predict rainfall. However, the radar-based estimate of rainfall is affected by uncertainties or errors such as mis-calibration, beam blockage, anomalous propagation, and ground cutter. Even though these uncertainties of radar rainfall estimate (RRE) have been studied, their effect on a runoff simulation especially to the peak discharge and peak time have not been much focused. Therefore, the objective of current study is to analyze the effect of the RRE uncertainties or errors based on synthetic simulation of RRE and its effect on peak discharge. First of all, mean of modeled radar rainfall is fixed (e.g., 100mm) and its error variance was set as ±10mm, ±20mm, ±40mm, and ±50mm independent to each grid cell. This independent simulation is based on white-noise process. The second simulation included a spatial-correlation between grid cells in simulating the error variance. The relationship between the distances of rain gauges and the corresponding correlations was modeled with the power law function. The parameters of the function were estimated through meta-heuristic method (specifically harmony search). Moreover, in order to find the correlation of observed data, the whole data from 27 rain gauges in the basin and the corresponding RRE from the dual polarization radar on Mt. Bisl in Korea were employed. The results of the former simulation (independent errors to each grid cell) show that the bias of the peak discharge is increased along with the variance increased, which is caused by influence of zero values. In the latter simulation (spatially correlated errors between grid cells), the results show that the peak discharge variance from the latter presents much larger than that of the former. Furthermore, the spatial distribution pattern of the modeled radar rainfall exhibited very similar to that of the real rainfall. Finally, we concluded that the error variance of RRE on runoff simulation leading bias and high uncertainty.

        Implications of Exonuclease Activity of Bacteriophage P2 Old Protein for Lambda Exclusion

        Temperate bacteriophage P2 has a nonessential gene called old (overcoming lysoginization defection). In the presence of old, the growth of the host (Escherichia coli) with recBC^- genotype is inhibited, and another bacteriophage, lambda, cannot superinfect. The Old protein has been shown to possess an exonuclease activity. Three mutant P2s (old 1. old 17, old 49) which did not exhibit lambda exclusion were obtained and each old gene was cloned into expression vectors to produce hexahistidine-tagged proteins. The proteins were affinity-purified and shown to lose its exonuclease activity on both double-stranded and single-stranded DNA substrates. Thus, it was concluded that the lambda exclusion was related to Old's exonuclease activity.
