Endoscopic Retrograde Cholangiopancreatography in Patients with Previous Acute Coronary Syndrome

Background/Aims: Endoscopic retrograde cholangiopancreatography (ERCP) is considered a high-risk procedure in patients with previous acute coronary syndrome (ACS); however, clinical studies are rare in the literature. The aim of this study was to investigate the safety and efficacy of ERCP in patients with previous ACS. Methods: We retrospectively reviewed the medical records of patients with previous ACS who underwent ERCP between January 2007 and August 2012. The clinical characteristics, ERCP diagnoses, treatment results, and complications were analyzed. Results: Fifty patients underwent ERCP an average of 41.6 months after ACS. The most common indication for ERCP was calculous cholangitis. After deep biliary cannulation, endoscopic sphincterotomy, biliary stone removal and endoscopic biliary drainage were successfully performed. Immediate postsphincterotomy bleeding occurred in seven patients, which was successfully controlled using endoscopic therapy. Elevation of cardiac troponin I levels were observed in three patients (6%) before ERCP, and all of these patients were diagnosed with pancreatobiliary disease combined with recurrent ACS, which was treated with coronary artery stent insertion (n=2) and balloon angioplasty (n=1). Conclusions: Therapeutic ERCP is effective and safe in patients with previous ACS. Cardiac troponin I elevation should be considered a warning sign for recurrent ACS in patients who undergo ERCP. 목적: Present an unusual case 방법: A 73-year-old Korean woman, 3-0-3-3, was transferred to our department because vaginal bleeding for three months.. In 25 years ago, she underwent subtotal hysterectomy with BSO due to myoma, in other hospital. The MRI showed that tumor size was more than 4 point 5 centimeter and there was no sign of lymphadenopathy. The level of SCC and CEA were increased.She underwent laparoscopic radical cervicectomy wih pelvic lymphadenectmy. I performed parametrial dissection and ureterolysis after systemic pelvic lymphadenectomy. I started dissecting the ureter up to the crossing with the uterine artery. Then the anterior parametrium was entered by blunt dissection working on the ureter. It was easier to unroof the ureter following its course into the anterior parametrium, woking directly on its adventitial sheath along its lateral and superior aspect. 결과: There were no intra-and postoperative complications. The final histopathologic report showed squamous cell carcinoma. She is receiving adjuvant chemotherapy and radiation after surgery. 결론: When we perform laparoscopic radical cervicectomy, unroofing the ureter following its course into the anterior parametrium is easier and feasible, woking directly on its adventitial sheath along its lateral and superior aspect.
Identification of a norovirus from diarrheic dog in Gwangju, Republic of Korea

Noroviruses are a major cause of gastroenteritis in humans and animals worldwide. In 2021, canine norovirus (CNoV) infection was detected at an animal clinic in Gwangju area, South Korea. A seminested polymerase chain reaction was developed to amplify a 478 bp fragment of the RdRp gene of CNoV. The phylogenetic analysis of this fragment confirmed the strain to be genogroup IV.2 (Dog/ GIV.2/gw/s377/2021/KOR), which exhibited the highest similarity to the feline NoV strain GIV.2/ CU081210E/USA/2010 (accession no. NC_045762) with 95.1% nucleotide (nt) identity and 98.7% amino acid (aa) identity. These research findings indicate that the detected norovirus in dogs is genetically similar to a feline-origin norovirus, suggesting easy cross-species transmission among animals.

Bovine tuberculosis is an important zoonosis worldwide. Mycobacterium bovis, the causative agent of this disease in cattle, is also a pathogen for humans and several economically important animals. The cases of tuberculosis are reported in two cow found at slaughter house located in Gwangju city. Histopathologically, in the lymph nodes, granulomas consisted of large areas of necrosis surrounded by variable thick bands of cellular infiltrate containing macrophages, Langhans-type multinucleated giant cells and lymphocytes. Lesions in the lung followed the same developmental pattern as did lesions in the lymph nodes with some exceptions. With the acid-fast staining, numerous mycobacteria were revealed in the lung and lymph nodes. M bovis was confirmed as a causative agent in these cattle using bacterial isolation and PCR and restriction fragment length polymorphism method based on a unique 12.7kb fragment insertion sequence from the Mycobacterium tuberculosis genome and the pncA polymorphism. The insertion element IS6110 and IS1081 were present M bovis isolated from lungs and lymph nodes of cattle using PCR assay. These cases are interesting and important in public health aspect that M bovis-infected cattle were found during a routine post-mortem inspection at slaughter house.

Species-specific TaqMan(R) probe-based real-time PCR assays were developed for detection of beef, pork, chicken, duck, goose and turkey. The primer and probe sets used in this study were designed to be complementary to fibroblast growth factor (FGF) for cattle and pig, mitochondrial NADH dehydrogenase (ND) subunit 3 and ND2 for chicken and duck, 12S rRNA for goose and turkey, respectively. As internal positive control we used conserved region in the ribosomal 18S RNA gene to ensure the accuracy of the detection of target DNA by real-time PCR. We confirmed that real-time PCR assays with the primer and probe sets were positive for cattle, pig and chicken intended target animal species with no cross-reactivity with other non-target animal species. Only >50 ng DNA of beef show cross-reactivity in the determination of duck. Using species-specific primer and probe sets, it was possible to detect amounts of 0.1 ng DNA of cattle and pig, 1.0 pg DNA of chicken, duck and turkey, and 0.1 pg DNA of goose for raw samples, respectively. The detection limits were 0.1 ng DNA of cattle, 1.0 ng DNA of pig and 1.0 pg DNA of chicken for DNA mixtures (beef, pork and chicken) extracted from heat-treated (121oC/5 min) meat samples. In conclusion, it can be suggested that the TaqMan(R) probe-based assay developed in this study might be a rapid and specific method for the identification of meat species in raw or cooked meat products.
식용란의 플루오로퀴놀론계 합성항균제의 잔류에 관한 조사

This surveies were carried out to investigate the residual levels of fluoroquinolones in chicken and quail eggs by bioassay and HPLC method. The eggs of 240 samples collected from market and farm in Gwangju Metropolitan city were examined from May to December in 2003. Residual antibiotic materials were detected from 47 samples of the 240 eggs by bioassay. Of the 240 eggs assayed, ciprofloxacin, danofloxacin, norfloxacin, ofloxacin, orbifloxacin and perfloxacin were not detected but enrofloxacin was detected from 5 samples in 228 chicken eggs and 1 sample in 12 quail eggs using HPLC with fluorescence detector by multi-residue method. 2 sample eggs in 6 sample which were detected by HPLC were not positive with bioassay. The average residual concentration of enrofloxacin was 0.494 ㎎/㎏ in 6 positive samples. The highest residual concentration of enrofloxacin was 1.83 ㎎/㎏.
광주지역 동물보호소내 유기견의 개심장사상충과 개 브루셀라병 감염 실태조사

This study was conducted to investigate the prevalence of canine heartworm infections, canine brucellosis and hematologic values from 153 free roaming dogs in the area of Gwangju city from March to November 2006. Nineteen (12.4%) of 153 samples tested with modified Knott`s technique showed positive reaction for microfilariae. Polymerase chain reaction using specific primers for D immitis amplified the expected product from all samples of 19 microfilaremic canine blood samples as determined by the modified Knott`s test for microfilariae. The seasonal infection rates of microfilariae were higher in the spring season (10/19, 52.6%) than in the other seasons. The major hematological findings in microfilaremic dogs were mild leukocytosis and mild monocytosis. A total of 100 dogs randomly selected from 153 free roaming dogs were negative for canine brucellosis by serological test using immunochromatographic antibody test kit.
