N/A Background: Authors modified the traditional continuous axillary brachial plexus block technique of Selander for purpose of increasing success rate and decreasing complications by use of commercial epidural anesthesia set. Method: Thirty-nine patients scheduled for upper extremity operations were injected with 40 ml of anesthetic solution by axillary perivascular technique, using 23-25G immobile needle at 2 cm from the pectoralis major. Tuohy needle was immediately introduced at 4 cm from the pectoralis major and pierced the expanded neurovascular sheath at an angle of 30 degree to the skin. The pop was well noted. Needle was advanced 0.5 to 3.0 cm and epidural catheter introduced through the needle. After removal of needle, occlusive dressing was done. Tip of catheter and spread of solution were demonstrated by fluoroscopy with contrast dye after completion of procedure. Result: Catheter insertion was successful at first attempt for all case. Total length of insertion was from 6 to 13(10.0+1.7) cm. Tip of catheter was placed in infraclavicular space(66.7%), about the humeral head(17.9%) and in upper arm in 3 cases as U-shape(7.9%). Catheters were maintained for 6.7+ 2.6(3-12) days. There were no complications such as: perforation of major vessels, needle trauma to nerve, infection, bleeding or hematoma. Conclusion: This study demonstrated continuous axillary brachial plexus block with epidural anesthesia set is safe, easy and convenient modification of technique of Selander.

Several methods for the management of intractable pain for various conditions including malignant disorders, herpes zoster, and low back pain and/or sciatica in 80 patients were described and analyzed. Neurolytic blocks of celiac plexus were given to the patients suffering from intra-abdominal pain from malignant origin or chronic pancreatitis. Herpes zoster was treated mainly by chemical sympathectomy via paravertebral route and occassionaly corticosteroid was administered epidurally with the local anesthetic, bupivacaine. All the patients with low back pain and/or sciatica were given epidural corticosteroid one to three times. Other peripheral verve blocks and regional corticosteroid therapy were given to the rest of the patients. Fair to excellent result was obtainde in 71% of the patients but the response was poor in the remaining 28% of the patients. It seems that earlier the patients were referred to the pain clinic, the better the results were.

<P>Transmembrane protein 165 (TMEM165), a Golgi protein, functions in ion homeostasis and vesicular trafficking in the Golgi apparatus. While mutations in <I>TMEM165</I> are known to cause human ‘congenital disorders of glycosylation’, a recessive autosomal metabolic disease, the potential association of this protein with human cancer development has not been explored to date. In the present study, we revealed that <I>TMEM165</I> is overexpressed in HCC and its depletion weakens the invasive activity of cancer cells through suppression of matrix metalloproteinase-2 (MMP-2) expression. Levels of <I>TMEM165</I> mRNA and protein were clearly increased in HCC patient tissues and cell cultures. Quantitative real-time RT-PCR analysis of fresh HCC tissues (n=88) revealed association of <I>TMEM165</I> overexpression with more frequent macroscopic vascular invasion, microscopic serosal invasion and higher α-fetoprotein levels. Notably, depletion of <I>TMEM165</I> led to a marked decrease in the invasive activity of two different HCC cell types, Huh7 and SNU475, accompanied by downregulation of MMP-2. Our collective findings clearly indicated that TMEM165 contributed to the progression of HCC by promoting invasive activity, supporting its utility as a novel biomarker and therapeutic target for cancer.</P>
Increasing Energy-Harvesting Ability of Piezoelectric Unimorph Cantilevers Using Spring Supports

We fabricated a spring-supported piezoelectric unimorph cantilever (SPUC) with enhanced energy-harvesting characteristics by using a 0.69Pb(Zr0.47Ti0.53)O3–0.31Pb(Ni0.6Zn0.4)1/3Nb2/3)O3 + CuO (0.5 mol%) thick film sintered at 950 C; a spring having a spring constant of 14,320 N/m was used as the cantilever support. The SPUC could generate an output power as high as 29 mW with a spring constant of 14,320 N/m across a resistance of 150 k ; this corresponded to a power density of 34 mW/cm3. We, therefore, that the thus-fabricated SPUCs when supported by a spring can harvest increased levels of energy.

This study was conducted to identify the habitat selection by the Eurasian otter (Lutra lutra) dependingon the type of waterfront in the Geumgang River region near the Daecheong Dam. As a result of thehabitat suitability analysis, the suitable region (very suitable and suitable) was found to be 48.8%, accountingfor 217.9km of the total of 446.3 km, and 17.2% or 76.5km of the total region was found to beunsuitable (unsuitable and very unsuitable). As a result of analyzing the types of waterfront area, it wasfound in the order of wetland> forest> grassland> bare land and the number of traces of otters perkilometer of each type of the waterfront areas was found to be rockface (36.0), bare land (0.92)> forest(0.8), grassland (0.76)>, and wetland (0.75). The result shows a relatively high number of otter traces forrockface compared with other waterfront types, though the composition ratio of rockface takes only asmall part of the overall waterfront areas. It is because rockfaces are safer from man-made disturbance,while providing homes and shelters, as well as locating nearby open water which has abundant amountof food resources, compared with other types of waterfront.

Objectives To analyze the prognosis of patients with low back pain according to the severity of abnormal lumbar lordotic angle and Ferguson's angle. Methods Data from electronic medical record of a total of 199 patients hospitalized in Korean medicine hospital were analyzed. With the sagittal view of lumbar spine X-ray, lumbar lordotic angle was defined as the angle between the superior endplate of L1 with the inferior endplate of L5. Ferguson’s angle was defined as the angle between the superior surfaces of the sacral with a horizontal line. ‘Measure Cobb’s Angle Tool’ of ‘INFINITT PACS’ was used to measure both angles. Lumbar lordotic angle and Ferguson's angle were subdivided into five sections. The number of days from onset and the duration of hospitalization were analyzed in each section. Results Severe hypolordotic lumbar lordotic angle group were found to have longer medical history and hospitalization duration than the control group. The ratio of long-term hospitalization (over 15 days) and chronic medical history (over 180 days) of patients with severe hypolordotic Ferguson’s angle were significantly higher than the control group. The ratio of patients with chronic medical history (over 180 days) was significantly higher in severe hyperlordotic Ferguson's angle group than the control group. Conclusions Severe hypolordotic lumbar lordotic angle was related to patient’s prognosis. Both severe hyperlordotic and severe hypolordotic Ferguson’s angle were found to be related to patient’s prognosis. However, a moderate loss of lumbar lordotic angle and Ferguson’s angle was not related to prognosis of low back pain patients. (J Korean Med Rehabil 2020;30(4):143-154)
