99mTc - DISIDA를 이용한 간 혈류 중 동맥 분획의 측정

N/A Arteriai fraction of total hepatic blood flow was estimated by a new method, slope method, on radionuclid angiogram using 99mTc-DISIDA and was compared with that from 99mTc-Phytate radionu clide angiogram. This study included ll of normal subjects, 37 of intermediate group with various liver diseases, and 25 patients with liver cirrhosis. We analyzed the datas with slope method from radionuclide angiograms and the results were compared with hepatic arterial fractions from uptake method, introduced by Lee et al. at 1986. The hepatic arterial fractions from radionuclide angiograms using Tc-DISIDA and 99mTc- Phytate were 0.32 +- 0.09 and 0.31 +- O.11 respectively in normal subjects, and 0.75+0.18 and 0.77+0.21 respectively in patients with liver cirrhosis. The hepatic arterial fractions by the slope method was well correlated with those of the uptake method on 99mTc-DISIDA scan. There was high correlation between the hepatic arterial fractions from 99mTc-DISIDA and 99mTc-Phytate scans. Hepatic arterial fraction estimated by the slope method is a useful index for the diagnosis of liver cirrhosis and the evaiuation of status of portal hypertension.

Benign tumors of the esophagus occur rarely as compared with the incidence of carcinomas. Among the benign tumors of the esophagus such as leiomyoma, lipoma, hemangioma, papilioma. polyp, cystetc. leiomyoma is the most common, although less than 10 percents of leiomyomas of the gastrointestinal tract are found in the esophagus. Most of leiomyomas of the esophagus present as solitary lesions but multiple tumors occur in about 4 percents of cases. Leiomyomas of the esophagus are diagnosed by thorough clinical evaluation, esophagogram and upper gastrofiberscopic examination but histological confirmation requires surgical or necropsy specimens. Occasionally gastrofiberscopic polypectomized specimens are used. We report one case of multiple leiomyomas of the esophagus located in between 25 cm and 40 cm distance from the incisor in a 38-year-old man who complained of vague substernal discomfort for about 10 years. Gastrofiberscopic polypectomized specimen was used for preoperative histological confirmation. The pathologic examination of the specimen showed leiomyoma of the esophagus.
말기 신부전증을 동반한 전신성 홍반성 낭창 환자에서 동종 신장이식 증례

In systemic lupus erythematosus renal involvement is very common and it accounts for the leading cause of death of the disease. Since SLE is a systemic disease involving many organs and it is caused by altered immunological background many physicians tend to hesitate in employing hemodialysis and renal transplantation. We report the first renal transplantion in Korea which was performed in a patient with end-stage renal failure due to SLE. The patient had positive LE pre- parations, low serum complement, antibody to native DNA and renal biopsy showing diffuse proliferative lupus nephritis but she manifested decreasing clinical activity during hemodialysis. The patient received kidney allograft from living related donor after unilateral nephrectomy. There was no clinical or his- tological evidence of rejection or recurrence of lupus nephritis in the grafted kidney. The patient maintains normal renal function 18 months after renal trans- plantation.
