Vertebrobasilar insuffciency(VBI) is also refered to as vertebrobasilar transient ischemic attack(V-B TIA). Vertebrobasilar insufficiency is often undiagnosed or misdiagnosed due to complexity of the symptoms. Transcranial doppler(TCD) can be used to possible to record the blood flow velocities in the cerebral arteries through the intact cranium. Transcranial doppler and magnetic resonance angiography(MRA) are both noninvasive techniques that can be used in vertebrobasilar insufficiency, but transcranial doppler is more sensitive with respect to evaluating stenotic lesion of the arteries than magnetic resonance angiographv. Diagnostic values of transcranial doppler were assessed in patients with vertebrobasilar insufficiency. A case of vertebrobasilar insufficiency is here reported. A 62-year-old man with dizzness, nausea and tremor because of vertebrobasilar insufficiency was admitted at Wonkwang Oriental Medicine Hospital. He was treated with the herbal medicine Chunmagudung-um and with acupunture. Results showed improvement of symptoms and favorable transcranial doppler readings. The process used in this successful treatment is here described.

현재 클라이언트/서버 구조는 가장 활발히 연구되고 응용되고 있는 소프트웨어 개발 구조로서, 이러한 구조를 가지는 시스템을 구축하는데 있어 최근에는 분산이질 환경이 자리를 잡아가고 있다. 또한 최근 기업들간에 제품 경쟁력을 유지하기위한 방안으로서 정보의 관리는 중요한 자산으로 인식되고 있으며, 이러한 정보를 분산시키고 공유하는 기술이 관심의 초점이 되고 있다. 본 고에서는 OMT를 기반으로한 객체지향 모델링 기법 그리고 CORBA를 기반으로 한 분산객체기술을 바탕으로 분산이질 설계변경관리 시스템을 위한 모델링 방법을 제시하며, 이를 클라이언트/서버 구조를 가진 실제 응용 프로그램을 분석, 설계하는 방안을 제시하고 있다.

Azathioprine is frequently used for the treatment of inflammatory bowel diseases (IBD) such as Crohn`s disease and ulcerative colitis. Lymphomas, squamous cell carcinomas, and undifferentiated pleomorphic sarcomas have been reported among patients receiving azathioprine therapy. Herein, we report a case of pleomorphic liposarcoma of chest wall which occurred in a 44-year-old man with Crohn`s disease taking azathioprine. He was diagnosed with Crohn`s disease 3 years ago after suffering from abdominal pain and hematochezia for 12 years. He had been taking 50 mg of azathioprine per day for 23 months when he visited the thoracic and cardiovascular surgery clinic due to right chest palpable mass that had rapidly grown during the past 2 months. Excisional biopsy was performed and the mass was diagnosed as pleomorphic liposarcoma. Therefore, he underwent radical excision of the right chest wall mass, which measured 11.0×6.5 cm in size. He is scheduled to receive radiation therapy and chemotherapy. (Korean J Gastroenterol 2013;62:248-252)
해상국립공원 도서지역 관리를 위한 드론의 활용에 관한 연구 - 드론 유형과 입도객 파악을 중심으로 -

본 연구의 목적은 해상국립 내 도서의 관리에 적합한 드론의 유형 및 입도객에 관한 정보를 취득하는 것이다. 연구대상지는 한려해상국립공원 내 25개 도서이며, 이를 3개의 구역으로 구분하여 조사하였다. 2019년 10월부터 11월까지 구역별로 각기 다른 유형의 드론을 운행하여 정보를 취득하였다. 운행 결과, 고정익은 타 유형에 비하여 중·장거리의 해상공원 관리에 유리한 것으로 파악되었으나, 계도 방송을 위한 정지비행은 적합하지 않은 것으로 나타났다. 회전익의 경우는 근접비행을 통한 영상취득 및 계도방송에 적합하였으며, 단·중거리 비행에 유리한 것으로 판단되었다. 헬기형 회전익의 경우에는 영상취득 및 계도방송이 가능하였으나, 프로펠러로 인한 소음 및 진동이 단점으로 나타났다. 입도객은 총 410명이었으며, 주요 행위는 낚시였다. 소혈도, 대혈도, 새여 등의 도서가 위치한 A구역의 도서 입도객 비율이 타 구역에 비해 높게 나타나, 이 지역에 대한 관리가 더욱 필요한 것으로 나타났다. 드론에 의한 계도 방송이 낚시객의 관리에 긍정적 영향을 미친 것으로 나타났다. The purpose of this study was to obtain information about the type of drones suitable for the management of entrants and entrants of islands in the marine national park. The research sites were 25 islands in the Hallyeohaesang National Park. The target islands were divided into three zones, and were investigated with different types of drones. The survey period was from October to November, 2019. As a result of the operation of drone airframe, drone with fixed wings was found to be favorable for the management of marine parks in medium and long distances compared to other types, but stopping flights for broadcasting was found to be unsuitable. Drone with rotational wings was found to be suitable for image acquisition and broadcasting through close flight. However, it was deemed suitable for short and medium distance flights because of the fast battery consumption. In the case of helicopter rotorcraft drone, image acquisition and broadcasting were possible, but noise and vibration caused by propellers were disadvantageous. The number of entrants to the islands totaled 410 and the main act was fishing. The proportion of entrants to the islands in Area A was higher than that of other areas, and thus it was deemed more necessary to manage the area. Broadcasting was found to have had a positive effect on the management of fishers.

Pressure sore(decubitus ulcer) is the an ulceration and necrosis of the skin and underlying tissue usually occuring on a bony prominence of the body after prolonged or repeated pressure. This study was designed to evaluate the effects of an oriental medicine(Sipjeondaebo-tang) on pressure sores complicated by cerebrovascular diseases. Methods: As For treatment, we used Sipjeondaebo-tang. Results: Both patients were treated with Sipjeondaebo-tang. After treatment, the pressure sores of two patients were improved. Conclusions: This study suggests that Sipjeondaebo-tang is effective in treatment of pressure sores(decubitus ulcer) complicated by cerebrovascular diseases.

업무흐름 관리 시스템은 기업, 연구소 등에서 BPR이나 IT 투자에 대한 효과를 가시화할 수 있는 시스템으로서 그 중요성이 점차 증대되고 있다. 업무흐름 관리 시스템은 점점 복잡해지는 기업의 업무흐름을 체계적으로 관리하고 자동화하기 위한 소프트웨어이다. 이 시스템에서 정의한 업무흐름을 실제로 수행하기 위한 소프트웨어 서비스를 업무흐름이 시스템에서 정의한 업무흐름을 실제로 수행하기 위한 소프트웨어 서비스를 업무흐름 수행 서비스(Workflow Enactment Service)라고 부른다. 본 논문에서는 실제 개발한 업무흐름 관리 시스템의 구조와 업무흐름 수행 서비스의 설계 모습과 구현에 대하여 기술한다.

디지털 환경이 급속히 변화됨에 따라 다양한 식별체계가 도입되면서, 식별자 간의 상호운용성 문제가 심각한 문제로 대두되었다. 특히 동일 분야의 식별체계 간에 상호운용이 되지 않음으로써 효용성 저하 및 상호운용성 시스템 구축의 비용 부담 등이 문제로 대두되었다. 본 논문에서는 확장식별자 기반의 ID 매시업 서비스를 제안하여 이 문제를 해결하고자 한다. 음원 분야의 대표적인 식별체계인 UCI와 ICN은 개별적으로 매우 우수한 식별자이지만, 상호운용을 위해서는 식별자의 수정이나 복잡한 시스템 도입이 필수적이었다. 본 논문에서 제안한 ID 매시업 서비스는 두 식별체계의 기본 구조와 변환 서비스를 전혀 수정하지 않고도 외부에 상호운용 기능을 제공한다. As digital environment rapidly changes, various identification infrastructures have been introduced. It inevitably caused the Interoperability issue between the identifiers. Especially, Interoperability issue between the same kinds of identifiers raised the problems such as decreasing usefulness and cost overhead for making bridge system. In this paper, we resolve this issue by suggesting ID mashup service based on XRI. Although both UCI and ICN are the dominant identification infrastructures in the soundsource domain, the modification of identifiers or the requirement of complex system are essential for Interoperability. The ID mashup service suggested in the paper is able to provide interoperable functions to outer world without modifying the structures and resolution service of both identification infrastructures.

Background: Endoscopic surgical procedure has recently been applied to not only intraabdominal or intrathoracic surgery but also saphenous vein harvesting, because it is less invasive and more cosmetically advantageous. Carbon dioxide insufflation during an endoscopic saphenous vein harvesting may cause an adverse effects on arterial blood gas and hemodynamic variables. This study was conducted to evaluate the safety of carbon dioxide insufflation during endoscopic saphenous vein harvesting. Methods: Patients in ASA physical status III or IV, scheduled for an endoscopic saphenous vein harvesting (n=30) were gathered for the evaluation. Until the end of the procedure, controlled mechanical ventilation (tidal volume: 10 ml/kg, respiratory rate: 10 rates/min) and ventilator mode was fixed in this tidal volume and respiratory rate. Arterial blood gas analysis, end-tidal carbon dioxide, blood pressure and heart rate were measured before and at 10, 20, 30, 40 minutes after carbon dioxide insufflation. Results: Preinsufflation values of PaCO2 (partial pressure of arterial carbon dioxide) and PETCO2 (partial pressure of end-tidal carbon dioxide) were 33.4 ± 3.6 mmHg and 24.1 ± 4.1 mmHg, respectively. PaCO2 was significantly increased at 30 and 40 minutes after carbon dioxide insufflation (40.1 ± 7.4 mmHg and 41.4 ± 8.6 mmHg). PETCO2 was significantly increased at 20, 30 and 40 minutes after carbon dioxide insufflation(27.6 ± 5.5 mmHg, 28.9 ± 7.0 mmHg and 29.6 ± 7.8 mmHg). But, the magnitude of difference between PaCO2 and PETCO2 was not significantly different. Conclusions: During endoscopic saphenous vein harvesting, PaCO2 was significantly increased compared with preinsufflation values. Careful monitoring of PaCO2 is mandatory during the procedure.
