Background : About 30% of patients in the hospital are under nourishment. A large portion of people are undernourished when they are admitted to the hospital and in the majority of these, undernutrition develops further while in hospital. Patient-Generated subjective global assessment(PG-SGA), a method of nutritional assessment based on clinical judgment, has been widely used to assess the nutritional status of adults for both clinical and research purposes. Modified PG-SGA can be used as a nutrition screening tool, and a comprehensive nutrition assessment is desirable for those who are malnourished. The purpose of this study is to develop a simple, reliable and valid nutrition screening program(NSP). Methods : We applied the nutrition screening program(NSP) to adult inpatients who are at risk of malnutrition using the Modified Patient-Generated subjective global assessment(PG-SGA) and objective assessment(electrical medical record data). Results : Therefore simple and beneficial NSP is developed for patients with severe malnutrition. Using NSP. nutritional information of the severe malnutrition patient should be shared with the physicians and they should be taken care of by clinical dietitians to improve their nutritional status. Conclusion : The Modified PG-SGA and nutrition screening tool are simple, reliable and valid nutrition screening program(NSP).
Inactivation of Mind bomb-1 causes the microadenoma coupled with goblet cell hyperplasia

The Notch pathway plays a central role in many developmental processes by controlling binary cell-fate decisions, and aberrant Notch signaling is of ten associated with tumorigenesis. The activation of Notch signaling promotes tumorigenesis in pre-T cells, but suppresses tumor development in keratinocytes. In the intestine, loss-of-function and gain-of-function studies reveal that Notch signaling regulates cell fate decisions and the inhibition of Notch signaling by a γ-secretase inhibitor turns proliferating cells in adenomas into differentiated goblet cells, suggesting a potential therapeutic benefit in colorectal cancer. Here we show that conditional inactivation of mind bomb-1(mib1), an essential component for generating functional Notch ligands^(10), in the intestinal epithelium causes aberrant crypt formation and goblet cell hyperplasia, coupled with the loss of p21 expression, Wnt overexpression, and β-catenin activation. Interestingly, these mice display microadenomas, similar to the adenomatous foci within human mixed adenomatous-hyperplastic polyps. Our study demonstrates that Mib1 plays a critical role in the early phase of colorectal tumorigenesis and the microadenomatous progression coupled with goblet cell hyperplasia is a novel mechanism that differs from the well-known APC-β-catenin pathway.
Osteoprotegerin-Ligand in osteoporosis, arthritis, and mammary gland development

The morphogenesis and remodelling of bone requires the synthesis of bone matrix by osteoblasts and its coordinated resorption by osteoclasts. The TNF family molecule Osteoprotegerin-ligand(OPGL; also known as ODF, TRANCE, and RANKL) is a key factor stimulating the differentiation and activation of osteoclasts, and is therefore essential for bone remodelling and calcium mobilization from the bones. Inactivation of the opgl gene in mice results in a complete block in osteoclast development that leads to severe osteopetrosis and a failure in tooth eruption. OPGL exerts its effects on osteoclasts by binding to the TNFR family receptor RANK(receptor activator of NFkB). Osteoprotegerin(OPG) acts as a soluble decoy receptor and competes with RANK for binding to OPGL. The balance between OPGL and OPG levels determines osteoclast activation, skeletal calcium release, and bone remodelling. Abnormalities in the OPGL/RANK/OPG system lead to the increased bone resorption that underlies the bone damage of postmenopausal osteoporosis, Paget's disease, bone loss in metastatic cancers and crippling rheumatoid arthritis. In mammals, calcium transport from the mother to the fetus and neonates is a vital process to preserve the species. Mothers meet the increased requirements for calcium during pregnancy and lactation by doubling their intestinal calcium absorption and demineralizing their skeletons via activation of bone-resorbing osteoclasts. Recently we demonstrated that OPGL and RANK, the master regulators of skeletal calcium release, are essential for the formation of the lactating mammary gland, the organ required for transmission of maternal calcium to neonates in mammalian species. Moreover, we found that pregnancy hormones induce OPGL expression in mammary epithelial cells and milk protein genes, β-casein and whey acidic protein, are induced by OPGL through C/EBPβ transcriptional factor, thus a mechanism independent of STAT5.

Phellinus linteus was examined for its anticancer activity using an animal model. Water extract of Phellinus Zinteus was prepared from artificially cultivated mycelia. Neither toxicity nor abnormal changes of hematological ` parameters were observed in the rat given orally with high doses of drug extract for 15 days. ICR mice were transplanted with Sarcoma-180 tumor cells intraperitoneally and drug extract was daily given to the mice from 1 day after tumer transplantation for 3 weeks. Administration of drug extract significantly prolonged the survival duration of Sarcoma 180-transplanted mice. For the better understanding of the anticancer activity, we have examined the effect of the drug extract administration on various killer cell functions, such as natural killer(NK) cells, cytotoxic T-lymphocytes (CTL) and macrophages which have been known to be main effector cells in immune responses against tumors. The results from the 4 hr <sup>51</sup>Cr-release assay have shown that the drug extract augments mouse NK cell activity but neither CTL nor macrophages. It is possible, then, that the anticancer activity of the Phellinus linteus may be associated with augmentation of NK cell function in the cancerated hosts.

We isolated a metalloprotease (MP) homologue from an abalone muscle cDNA library. A 3284-kb full-length cDNA encoding a predicted polypeptide of 667 amino acids was sequenced. The abalone MP Haliotis discus hannai (HdMP) exhibited a domain structure typical of the peptidase M4 family, a 21-amino acid N-terminal hydrophobic signal sequence followed by a long propeptide sequence of 347 amino acids and the mature protease domain comprising 299 amino acids. The mature region contains features characteristic of a zinc protease, including a zinc-binding motif (HEXXH) and an active site. The protein showed 32-38% amino acid sequence identity with other known MP sequences and with a hypothetical protein from owl limpet. The mRNA transcript is expressed in almost all tissues, with high expression in the mantle and adductor muscle of healthy abalones, and is expressed constitutively during the early developmental stages after fertilization. Lipopolysaccharide or poly I:C stimulation induced the expression of the HdMP transcript in the digestive track and gills of abalones. Collectively, these results suggest that HdMP could play multiple roles in defense, the immune response, growth, and regulation of reproduction.

Purpose: The aim of this study was to evaluate the efficacy and safety of adjunctive lacosamide therapy in pediatric patients aged ≥4 years with drug-resistant epilepsy (DRE). Methods: Medical records of children aged 4 to 19 years treated with lacosamide as adjunctive therapy for DRE were retrospectively reviewed. The patients were divided into two groups according to their age at the start of lacosamide treatment: group A (aged 4–15 years) and group B (aged 16–19 years). Changes in seizure frequency from baseline, adverse events, and the retention rate were evaluated at each follow-up visit. Results: Sixty-two patients (33 males and 29 females) with a mean age of 11.4 years (range, 4 to 19) were included. The mean duration of follow-up was 20.1±12.9 months. The mean maintenance dose of lacosamide was 6.7±4.8 mg/kg/day. Forty-two patients (67.7%) were responders (≥50% reduction in seizures) with 19.4% (12/62) achieving freedom from seizures. The response rate did not differ significantly between groups A and B (67.6% vs. 68.0%, P=0.795) and was not affected by the concomitant use of sodium channel blockers. Significant independent factors associated with a good response to lacosamide treatment were a shorter duration of epilepsy (P=0.035) and fewer concomitant anti-seizure medications (P=0.002). Mild transient adverse events were observed in 20 patients (32.3%). Conclusion: Lacosamide adjunctive therapy was efficacious and tolerated in children aged ≥4 years with DRE. Early use of lacosamide may be helpful for a good response to drug-resistant seizures.

Purpose: This study aimed to analyze the effect of screening by using endoscopy on the diagnosis and treatment of gastric cancer. Materials and Methods: The clinicopathologic characteristics of gastric cancer were compared in individuals who underwent an endoscopy because of symptoms (non-screening group) or for screening purposes (screening group). The distributions of gastric cancer stages and treatment modalities in 2006 and 2011 were compared. Results: The proportion of patients in the screening group increased from 45.1% in 2006 to 65.4% in 2011 (P<0.001). The proportion of stage I cancers in the entire patient sample also increased (from 60.5% in 2006 to 70.6% in 2011; P=0.029). In 2011, the percentages of patients with cancer stages I, II, III, and IV were 79.9%, 8.2%, 10.9%, and 1.1%, respectively, in the screening group, and 47.9%, 10.8%, 29.8%, and 11.5%, respectively, in the non-screening group. The proportion of laparoscopic and robotic surgeries increased from 9.6% in 2006 to 48.3% in 2011 (P<0.001), and endoscopic submucosal dissection increased from 9.8% in 2006 to 19.1% 2011 (P<0.001). Conclusions: The proportion of patients diagnosed with gastric cancer by using the screening program increased between 2006 and 2011. This increase was associated with a high proportion of early-stage cancer diagnoses and increased use of minimally invasive treatments.
