Atopic dermatitis (AD) is an inflammatory skin disorder that is both uncomfortable and distressing to patients, and its prevalence has been steadily increasing. It is obvious that the identification of efficient markers of AD in plasma would offer the possibility of effective diagnosis, prevention, and treatment strategies. In this study, a proteomic approach was used to analyze plasma glycoproteins from both children with AD and healthy child donors. Several protein spots showing significant quantitative changes in the AD patients were identified. Through sequential studies, it was confirmed that CD5L and ApoE were significantly up-regulated or down-regulated, respectively, in the plasma from AD patients compared with that from healthy donors. In addition, we suggest that the up-regulated CD5L in AD patients causes eosinophilia by inhibiting apoptosis or promoting the proliferation of eosinophils either in combination with or without IL-5. The glycoproteomic data in this study provides clues to understanding the mechanism of atopic alterations in plasma and suggests AD-related proteins can be used as candidate markers for AD.

<P><B>Abstract</B></P> <P>Recently, we selected three tea (<I>Camellia sinensis</I>) cultivars that are rich in taste, epigallocatechin-3-<I>O</I>-gallate (EGCG) and epigallocatechin-3-<I>O</I>-(3-<I>O</I>-methyl)-gallate (EGCG3″Me) and then cultivated them through asexual propagation by cutting in the same region. In the present study, proton nuclear magnetic resonance (<SUP>1</SUP>H NMR)-based metabolomics was applied to characterize the metabotype and to understand the metabolic mechanism of these tea cultivars including wild type tea. Of the tea leaf metabolite variations, reverse associations of amino acid metabolism with catechin compound metabolism were found in the rich-taste, and EGCG- and EGCG3″Me-rich tea cultivars. Indeed, the metabolism of individual catechin compounds in the EGCG3″Me-rich cultivar differed from those of other tea cultivars. The current study highlights the distinct metabolism of various tea cultivars newly selected for cultivation and the important role of metabolomics in understanding the metabolic mechanism. 증례 : 바터 팽대부 종양에 동반된 부적절 항이뇨호르몬 분비 증후군 (SIADH) 1예

부적절 항이뇨호르몬 분비 증후군(SIADH)은 저나트륨혈증을 보이는 환자의 흔한 원인이며, 악성 종양이 그 중요한 원인 중 하나이다. 저자들은 복통과 딸국질을 주소로 내원한 73세 남자 환자에서 SIADH를 진단하였으며, 그 원인으로 바터 팽대부 종양을 진단하여 치료하였기에 문헌고찰과 함께 보고하는 바이다. The syndrome of inappropriate antidiuretic hormone secretion (SIADH) is one of the most common causes of hyponatremia and results from inappropriate release of antidiuretic hormone (ADH). SIADH may be caused by a variety of malignant tumors, including central nervous system disorders, intrathoracic disorders and various pharmacologic agents. We experienced a case of SIADH associated with carcinoma of the ampulla of Vater. A 73-year-old man was admitted to hospital with abdominal pain and hiccup. Laboratory findings showed that the serum sodium concentration and serum osmolarity were extremely low, while the urine sodium concentration and urine osmolarity were inappropriately high, and the patient was diagnosed with SIADH. Endoscopy showed an ill-defined nodular ampulla of Vater and a biopsy revealed a carcinoma. Resection of the ampullary carcinoma corrected the abnormal sodium levels and restored the laboratory findings to normal. The normal conditions have been continuous for 6 months. To our knowledge, this is the first case of SIADH caused by an ampullary carcinoma. (Korean J Med 72:415-419, 2007)

<P>With the increase of tea (<I>Camellia sinensis</I>) consumption, its chemical or metabolite compositions play a crucial role in the determination of tea quality. In general, metabolite compositions of fresh tea leaves including shoots depend on plucking seasons and tea cultivators. Therefore, choosing a specific plucking time of tea leaves can provide use-specified tea products. Artificial control of tea growing, typically shade treatments, can lead to significant changes of the tea metabolite compositions. However, metabolic characteristics of tea grown under various shade treatment conditions remain unclear. Therefore, the objective of the current study was to explore effects of various shade conditions on metabolite compositions of tea through a <SUP>1</SUP>H NMR-based metabolomics approach. It was noteworthy that the levels of catechins and their derivatives were only influenced at the initial time of shade treatments while most amino acids were upregulated as amounts of shade and periods were increased: that is, the levels of alanine, asparagine, aspartate, isoleucine, threonine, leucine, and valine in fresh tea leaves were conspicuously elevated when shade levels were raised from 90% to 100% and when period of shade treatments was increased by 20 days. Such increased synthesis of amino acids along with large reductions of glucose level reflected carbon starvation under the dark conditions, indicating remarkable proteolysis in the chloroplast of tea leaves. <P>Adult stem cells have been studied as a promising therapeutic modality for the functional restoration of the damaged heart. In the present study, a strategy for enhancing the angiogenic efficacy of human mesenchymal stem cells (hMSCs) using micro-RNA was examined. We investigated whether micro-RNA-146a (miR-146a) influences the secretion of vascular endothelial growth factor (VEGF) and angiogenesis of MSCs. Our data indicated that miR-146a-transfected hMSCs (hMSC<SUP>miR-146a</SUP>) decreased the expression of neurofibromin 2, an inhibitor of p21-activated kinase-1 (PAK1). miR-146a also increased the expression of Ras-related C3 botulinum toxin substrate 1 and PAK1, which are known to induce VEGF expression, and the formation of vascular branches was increased in hMSC<SUP>miR-146a</SUP> compared to hMSCs treated with VEGF. VEGF and p-Akt were increased in hMSC<SUP>miR-146a</SUP>. Furthermore, injection of hMSC<SUP>miR-146a</SUP> after ischemia/reperfusion (I/R) injury led to a reduction of fibrosis area and increased VEGF expression, confirming the regenerative capacity such as reparative angiogenesis in the infarcted area. Cardiac functions in I/R injury were improved following injection of hMSC<SUP>miR-146a</SUP> compared to the I/R group. Taken together, these data suggest that miR-146 is a novel microRNA that regulates VEGF expression, and its use may be an effective strategy for enhancing the therapeutic efficacy of hMSC transplantation into the I/R-injured heart.</P>
노인의 사회적 배제가 자살생각에 미치는 영향과 적응유연성의 조절효과

본 연구의 목적은 노인의 사회적 배제가 자살에 미치는 영향에 대한 적응유연성의 조절효과를 검증하는 것이며, 연구결과는 다음과 같다. 첫째, 인구사회학적 특성에 따른 자살 생각에 영향을 미치는 요인으로 연령, 교육수준, 혼인상태, 자녀와의 동거유무가 차이가 있는 것으로 나타났다. 연령은 75세 이상 80세 미만이 자살 생각을 가장 많이 갖는 것으로 나타났으며, 교육수준은 학력이 낮아질수록 자살 생각이 증가하는 것으로 나타났다. 그리고, 이혼, 사별, 별거 등으로 혼자된 노인과 자녀와 비 동거 상태인 노인일수록 자살생각을 많이 갖는 것으로 나타났다. 둘째, 노인의 사회적 배제가 높을수록 자살생각이 높은 것으로 나타났다. 셋째, 노인의 사회적 배제가 자살생각에 미치는 영향에 대한 적응유연성의 조절효과가 있는 것으로 나타났다. 마지막으로 기울기 검정 결과는 사회적 배제가 높은 집단의 적응유연성이 낮은 것으로 나타났다. 본 연구결과 노인의 자살생각이 자살행위로 이전되기 전 예방을 위한 사회적 차원의 정책적 방향성을 제시하고자 함이며, 보호요인인 적응유연성에 대한 실천적 함의를 후속연구로 제언하고자 한다. The purpose of this research is to verify that adjustment resilience has a moderating effect on the influence of social exclusion regarding suicide in senior citizens, and the research results are as follows: First, the demographic sociological factors that affect suicidal impulse have been shown to be age, education level, marriage status, and child co-residence. Senior citizens between the ages of 75 and 80 have been shown to have the most suicidal impulses, whereas the tendency to have such suicidal impulses increases as the level of education decreases. Furthermore, those seniors who are unmarried or single due to divorce, death, or separation and those who do not live with their children tended to have stronger, more frequent suicidal impulses. Second, seniors have been shown to suffer stronger suicidal impulse when subjected to more serious social exclusion. Third, it has been verified that adjustment resilience has a strong moderating effect on the influence of social exclusion regarding the suicidal impulses of senior citizens. Last, the results of the inclination test (Hypothesis Test for Slope of Inclination) have shown that adjustment resilience tends to be lower in the group that is more socially excluded. The purpose of this research is to suggest the direction of social policy creation in order to prevent suicide by senior citizens and set forth the practical implication regarding adjustment resilience as a protection factor for further studies.
