Folie a Deux : FOLIE A DEUX IN FRATERNAL TWINS

The authors analyzed dynamically and discussed a case of folie a deux manifested as paranoid schizophrenic reaction in fraternal twins. The lives of the twins are just like that of a person. They have never seperated through their lives. They had lain in the mother's womb together, slept under a coverlet, sat side by side in the school and always went around everywhere in pairs. They have hardly had any intimate friends or neighbors, and even when they had a friend, he was not the friend of a twin but of both. It appears that they formed a microsociety, in which close association and free activity occurred sharing their thinkings, feelings, interests, apprehensions and hopes. Through such a life it is expected that they could not grow up independent personalities an their egos could not be differentiated to each individual. And it is probable that the two undifferentiated egos become a whole one and gain safety and satisfaction by their union. It will be very natural that the thought of one partner is transferred to the other through the symbiosis of personality. Most authors emphasize the dominant-submissive relationship between the primary patient and the secondary partner, but we think it is not inevitable in folie a deux. The long intimate association of partners isolated from the society will promote to form the morbid personality structure that is the soil in which delusions may be sown and flourish.

The turbulent eddy viscosity coefficient C<SUB>μ</SUB> that assumes constant value in the standard κ-? model is found to be a function of the time scale ratio Sκ/ε in boundary-free homogeneous shear flows. This functional form is derived by analyzing homogeneous turbulent shear flows. When such a functional coefficient C<SUB>μ</SUB> is applied to near-wall region of wall bounded flows, a universal wall damping function ?(y?) can be observed. And when it is applied to symmetric wake region of channel flows and free shear flows, it is found that there exists a universal wake function W(y/δ). Further when the functional coefficient C<SUB>μ</SUB> is applied to the outer part of free shear flows such as jets, wakes and mixing layers, it seems feasible to define a well-behaving function G(?) that takes account of the intermittency effect on the turbulent eddy viscosity.

Since its original designation by O'Brien and Stout in 1964 malignant fibrous histiocytoma has become well established as a tumor of probable histiocytic origin principally in the soft tissue of the extremities, The primary site is located in an extremity in approximately 70% of cases. Less common primary sites include the retroperitoneum, trunk, bone, head and neck but intraperitoneal maligmant fibrous histocytoma seldom occur in clinical practice. We report two cases of intraabdominal malignant fibrous histiocytoma in 54-year-old female and 44-year-old male whose chief complaints were growing nontender masses. The presenting sites were antimesenteric border of ilem in the former and ileum, mesentery and peritoneum in the latter.

Quantification of the regurgitation amount is important before and after valvular replacement sugery. Until now cardiac catheterization with cineventriculography, echocardiography have been used to measure the regurgitation amount, but also have many limitations EKG gated cardiac blood pool scan provides a simple, non-invasive method for quantify the regurgitation amount. By calculating the ratio of left ventricular to right ventricular stroke counts(stroke volume ratio) in gated bood pool scan, we measured the left ventricular regurgitation amount in 28 cases of valvular regurgitation and 25 cases of normal group. 1) Stroke volume ratio was higher in cases of valvular regurgitation(2.11±0.58) than in cases of normal control(l.15±0.31). (p〈0.01). 2) Stroke volume ratio was classified by regurgitation grade using X-ray cineventriculography. In grades of mild regurgitation(Grade I∼Ⅱ), stroke volume ratio was 2.02±0.29, and in grades of severe regurgitation(Grade Ⅲ∼Ⅳ), stroke volume ratio was 2.55±0.34e volume ratio was well correlated with the grade of X-ray cineventriculography. 3) Stroke volume ratio was classfied by functional class made in New York Heart Association. In classes of mild regurgitation (class I∼Ⅱ), stroke volume ratio was 2.08±0.26, and in classes of severe regurgitation (class Ⅲ∼Ⅳ), stroke volume ratio was 2.55±0.38, Stroke volume ratio well represented the functional class. 4) After aortic and mitral valve replacement in 28 patients, the stroke volume ratio decreased from 2.11±0.58 to 1.06±0.26. Gated blood pool scan provides a noninvasive method of qnantifying valvular regurgitation and assessing the result of surgical interventions.
