        복합영화관에서 화재시 과밀공간 파악을 통한 관객의 피난 행태연구

        The purpose of this study is to investigate specific crowding areas resulted from diverse physical factors of multi-plex theaters In a fire evacuation, Identify psychological factors of human behaviors affecting those crowding, and more accurately predict the evacuation route of people To achieve these purposes, the architectural characteristics of three multi-plex theaters have been chosen, and the evacuation experiments through the computer simulation called Simulex and surveys for occupants were also earned out for each on the these theaters Finally, interviewing with fire fighters, the results between the Simulex and the surveys were compared and analyzed The conclusions from this study are as follows (l)Among several human behaviors in the Familiarity, Space using pattern, Turning back, Accessbility, and landmark perception are different in each theater (2)Crowding usually happens cross areas between theater inside and corridors, and Crowding rate depends on the number of cross areas (3)Specialists pointed out the possibility of wayfindind difficulties due to visual accessbilities

        Combination Therapy with Systemic Steroids, an Antiviral Agent, Anticoagulants, and Stellate Ganglion Block for Treatment of Sudden Sensorineural Hearing Loss

        Background and Objectives: Sudden sensorineural hearing loss (SSNHL) is commonly defined as a loss of at least 30 dB in three contiguous frequencies occurring within 3 days. Systemic steroid administration has become the most widely accepted treatment option for SSNHL. Since viral infection and vascular compromise are considered specific causes of SSNHL, antiviral agents, anticoagulants, and stellate ganglion block have been used for its treatment, although the evidence of their effectiveness is weak. The present study evaluated the hearing recovery rate in the combination therapy group (systemic steroids, antiviral agent, anticoagulants, and stellate ganglion block) in comparison with patients treated with systemic steroids alone. Subjects and Methods: A total of 85 patients diagnosed with SSNHL were treated with combination therapy (group A, 46 patients) or systemic steroids only (group B, 39 patients). Hearing improvement was defined as a hearing gain of more than slight improvement using Siegel's criteria. All patients were treated with a 10-day course of systemic steroids (10-mg dexamethasone for 5 days, followed by tapering for 5 days). Acyclovir, heparin, and stellate ganglion block were included in the group A treatment regimen. Results: The overall rate of hearing improvement was 60.9% (28/46 patients) in group A, which was significantly higher than that (38.5%, 15/39 patients) in group B. The distribution of prognostic factors was not significantly different between the two groups with the exception of the degree of initial hearing loss, which was more severe in group A. Upon analysis according to prognostic factors, group A showed a better hearing improvement recovery rate than group B in patients with hearing loss >70 dB, age >41 years, dizziness, and early treatment (<1 week). Conclusions: Thus SSNHL patients treated with combination therapy have a higher likelihood of hearing improvement than those treated with systemic steroids alone.

        Evaluation of the Interaural Attenuation for Pure Tone Audiometry in Korean

        Background and Objectives: The masking should be used in pure tone audiometry when the difference between the air-conduction threshold of the tested ear and the bone-conduction threshold of the non-tested ear is more than interaural attenuation (IA). But the standardized average value of IA in Korean has not been evaluated until now. The purpose of this study is to evaluate the interaural attenuation in Korean population. Subjects and Methods: Of the patients who visited our department from March, 2007 to August, 2009, 44 patients were selected who had unilateral severe sensorineural hearing loss or deafness and contralateral normal hearing. When the difference between air-conduction threshold with and without masking was more than 10 dB, the interaural attenuation at each frequency was obtained by subtracting the contralateral bone conduction from the air-conduction threshold without masking. Results: The interaural attenuations at each frequency were 45-75 dB (250 Hz), 50-75 dB (500 Hz), 40-75 dB (1 kHz), 45-85 dB (2 kHz), 50-90 dB (4 kHz). 77.2% of the subjects showed interaural attenuation of 55-65 dB at 250 Hz, and 80.9% in 50-60 dB at 500 Hz, 79.5% in 55-65 dB at 1 kHz, 83.4% in 45-60 dB at 2 kHz, and 76.4% in 50-65 dB at 4 kHz. Most of the patients were distributed 15-25 dB higher than the minimal interaural attenuation at 1 kHz. At 2 kHz and 4 kHz, most of the patients were distributed around 15 dB of the minimal interaural attenuation. Conclusions: These results can be used as reference values for masking of pure tone audiometry. But, further studies with large population should be done to get more precise results and contributing factors to IA values.

        Disordering of Clay Layers in the Nylon 6/Clay Nanocomposites Prepared by Anionic Polymerization

        As a preliminary work for the preparation of nylon 6/c1ay nanocomposites by reactive extrusion, nylon 6/c1ay nanocomposites were prepared by anionic polymerization in a flask. In order to investigate the effect of the intercalation of clay layers, the clay feeding times, such as in pre-mixing where the clay was fed before initiation of polymerization and in after-mixing method where the clay was fed after initiation of polymerization, were changed. The appearance of the WAXD peak of nanocomposites prepared by the pre-mixing method was obvious and the tensile strength was decreased compared with that of pure nylon 6, which indicates that the clay layers were not dispersed and distributed. During the preparation of the nanocomposites by the after-mixing method, disordering of the clay layers was observed with increasing clay addition time and was suspected to result from the rapid polymerization of nylon 6 within the clay layers.

      • A Critical Appraisal of the Definition of Sarcopenia in Patients with Non-Alcoholic Fatty Liver Disease: Pitfall of Adjusted Muscle Mass by Body Weight

        Aims: Traditionally, sarcopenia has defined as amount of absolute muscle mass adjusted by height in the elderly people. However, relative muscle mass adjusted by weight has used extensively in most non-alcoholic fatty liver disease (NAFLD) studies. Here, we attempted to investigate the pitfall of sarcopenia in NAFLD according to adjustment methods. Methods: Adult subjects (n=1,343) who underwent a health check-up at the healthcare center were finally included for analysis, except for 682 subjects. Total skeletal muscle (TSM), appendicular skeletal muscle (ASM) and fat measured using the bioelectrical Impedance Analysis (BIA). The weight-adjusted skeletal muscle mass index (wSMI) and height-adjusted SMI (hSMI) calculated by dividing the total ASM by weight or the square of height, respectively. Fatty liver diagnosed by using abdominal sonography. Results: Prevalence of sarcopenia defined by wSMI in the NAFLD group was significantly higher than in the control group (1.3% vs. 8.8%, P<0.001). But there was no difference in the prevalence of sarcopenia defined by hSMI between the control and NAFLD groups (2.0% vs. 0.8%, P=0.055). The concordance rate of the two methods was 0.29% (4/1343) NAFLD. Because body weight was most potent independent risk factor for NAFLD in multivariable logistic regression analysis, when we adjusted parameters adjusted by body weight, prevalence of abnormality of almost all parameters increased in NAFLD population. When the bilirubin (or platelet) adjusted by weight, prevalence of abnormal bilirubin (or platelet) increased in NAFLD subjects. However, prevalence of abnormality of non-metabolic parameter (bilirubin or platelet) did not increase in NAFLD, after adjusting by height. Only metabolic parameters showed relationship with NAFLD, after adjusting by height Conclusions: The concordance rate of sarcopenia defined by wSMI and hSMI was very low. As NAFLD is highly associated with body weight, attention should be given in the case of studying the relationship of NAFLD with sarcopenia adjusted by body weight.

        The Relationship between Dog Ownership and Physical Activity in Korean Adults

        Background: Many studies have reported that dog ownership is effective in encouraging physical activity. However, the association between living with a dog and enhanced physical activity has not been clearly verified. Thus, this study aimed to investigate the effects of dog ownership on the amount of physical activity in a representative sample of Korean adults. Methods: This cross-sectional study administered the International Physical Activity Questionnaire (IPAQ) to 1,299 individuals who visited the international healthcare center of one hospital in Seoul, Korea, between August and December in 2018. Based on responses to the IPAQ survey, the amount and duration of physical activity was calculated to analyze relationships with dog ownership. Results: The total amount (P=0.02) and duration of physical activity (P=0.02) and the duration of vigorous-intensity physical activity (P=0.04) were significantly higher among dog owners than non-dog owners. The duration of daily physical activity increased with dog ownership by 18.6 minutes (P=0.01). A comparison of dog owners according to whether they walked with their dogs revealed that dog walking had no significant effect on the amount and duration of physical activity, inactivity, or health-enhancing physical activity status. There was a sex-specific association between dog ownership and total duration of physical activity in females. Conclusion: The duration of physical activity increased by dog ownership. However, the higher levels of physical activity among dog owners may not necessarily be explained by dog ownership or walking with dogs. This outcome suggests that raising a dog can help promote physical activity.
