      • 사회 경제적인 요인과 신경학적 질환 상태의 뇌졸중 환자의 스트레스에 미치는 영향에 대한 분석

        Background: The diseases are getting increasingly chronic as the fast development of medical and scientific technical advances prolong the human life span. Among many chronic diseases, the apoplexy leads to partial paralysis, speech disorder, perception disorder and paresthesia and motor disturbance. It also causes mental disorder to patients. This research was aimed to check the level of stress perceived by apoplexy patients according to the socioeconomic factors and neurological manifestations. Method: Applying the Neuman's stress measurement, we surveyed the stress level of 150 inpatients and outpatients who have been currently taken care of by the departments of neurology, neurosurgery and rehabilitation at a university hospital and two general hospitals in Busan. Results: The stress level related to the socioeconomic factors is higher in patients who have low education level, low income level, and financial dependence on offsprings. The stress level related to the neurological manifestations is higher in patients who have multiple strokes, bilateral paralysis, and other neurological sequelae (language, sensory, memory, particularly, gaiting disturbances and dependencies for active daily living). Conclusion: The results suggest that poststroke patients are highly likely to suffer from mental stress by various socioeconomic factors and neurological manifestations. Therefore, it can be concluded that poststroke patients need not only medical treatments and rehabilitation procedures but also more proactive mental remedies and social supports considering patient's backgrounds and characteristics.

      • Multiple Spinal Intradural Schwannomas Combined with Sequestered Lumbar Disc Herniation

        In this report, we describe a case of multiple intradural schwannomas in the spine combined with a upward sequestered disc herniation at L4-L5. It is often difficult to differentiate this tumor from lumbar disc herniation, especially a sequestered hernia. Gadolinium-enhanced MR images were a useful preoperative examination modality for differentiating these lesions from other diseases. Microscopically, the intradural tumors were successfully removed. The dura maters of the L2 and S1 were opened microsurgically, allowing the nerve fibers involved in the tumor to be identified. The involved fibers were cut around the tumor, and the lesion was resected while the intact nerve fibers were preserved. An upward sequestered lumbar disc herniation at L4-L5 was also successfully removed. Based on histological examination of the resected specimen, the tumors were diagnosed as schwannomas. Microsurgery allowed the tumors to be removed with minimal impairment from cutting of nerve fibers in the each nerve root.

      • 유한요소법에 의한 호안의 강제치환 형상에 관한 변형예측 및 현장계측

        This study used the finite element method analysis to estimate enforced replacement shapes and replacement depth. The data were expected to be used as the fundamental data of the design through the comparison with the results of the elastic wave exploration and investigation. The results of the research are summarized in the following. (1) After dividing the breakwater into 5models, on which elastic wave exploration was performed, the enforced replacement shape has been obtained by the finite element method. The result was satisfactory compared to the results of the elastic wave exploration. (2) The enforced replacement shapes of the breakwater were compared in different ways; in the design, in the elastic wave exploration in field and in this study. While the enforced replacement shape in the design is substantially different from that in elastic wave exploration in field, it turned out to be similar to the results of this research. (3)If an enforced replacement shape is standardized in configuration of breakwater, depth of soft soil and th value of soil property, it can be used as the standardized section in designing breakwaters.

      • 감광제/광촉매에 의한 공기오염물질 제거

        일반 주거환경 또는 재배시설 내의 공기오염물질인 농약 및 병원성 미생물을 인위적으로 제거하기 위해서 자연광 또는 인공광 조건하에서 몇몇 감광제/광촉매에 의한 광분해 촉진효과와 미생물의 살균효과 시험을 수행하였다. 광촉매로는 반도체 물질인 PC-1과 PC-2, 그리고 산화제로 사용되는 PC-3, PC-4, PC-5 및 PC-6과 감광제로 PS-7과 PS-8을 선발하였다. 그리고 시험화합물로는 유기인계 살충제인 dichlorvos와 dicarboximide계 살균제인 procymidone, 환경오염물질인 methyl tert-butyl ether (MTBE)를 선정하였고, 유해 미생물로는 Pseudomonas putida, Phytophthora capsici, Salmonella typhimurium, 및 Escherichia coli를 선정하였다. Dichlorvos의 경우에는 자연광과 인공광 조건하에서, 광촉매 무처리구에 비해 각각의 광촉매가 처리된 모든 처리구에서 4배 이상의 광분해 촉진효과를 보였으나, MTBE의 경우에는 단지 광촉매 PC-1 처리구만 이 광촉매 무처리구에 비해 약 17배 이상의 분해 촉진효과를 보였다. 또한 procymidone의 경우에도 PC-1, PC-6 및 PS-8에 의한 광분해가 약 3배 이상 촉진되었다. 공기 중의 미생물을 제거하기 위한 예비시험에서는 미생물 현탁액에 PC-1을 첨가하고 인공광을 조사함으로서 미생물의 생육이 저해됨을 확인할 수 있었다. 또한 평판배지에 도말된 Salmonella typhimurium은 광촉매 PC-1의 주변에서 미생물 생육이 저해됨을 보였다. 이상의 결과에서 볼 때, 광조건 하에서 이러한 감광제/광촉매를 이용한 기술은 농약을 비롯한 여러 환경 오염물질을 제거하여 재배시설 내의 작업자와 일반 대중의 환경오염원에 대한 노출을 예방할 수 있을 것으로 기대된다. For the artificial removal of the air pollutants such as pesticides, environmental toxicants, and pathogenic microorganisms in the greenhouse or the living environment, the accelerated photodegradation and the biocidal effects by some photosensitizers (PS)/photocatalysts (PC) were tested under the sunlight and/or artificial light. The selected photosensitizers/photocatalysts were the semiconductors (PC-1 and PC-2), the oxidizers (PC-3, PC-4, PC-5 and PC-6), the aromatic ketone (PS-7) and the aromatic amine(PS-8). The test toxic pollutants included the organophosphorus insecticide dichlorvos (DDVP), the dicarboximide fungicide procymidone, and the gasoline additive, methyl tert-butyl ether(MTBE) as the organic pollutants, and Pseudomonas putida, Phytophthora capsici, Escherichia coli and Salmonella typhimuriums as the pathogenic organisms. In the case of dichlorvos, all the photocatalysts selected showed more accelerated photodegradation than the control without photocatalyst under both the sunlight and artificial light. Whereas, only the photocatalyst PC-1 accelerated the degradation of MTBE about 17 times more than the control under both the sunlight and artificial light. Procymidone was much more degraded by the photosensitizer PS-8 and the two photocatalysts (PC-1, PC-6) than by PS-7. In the preliminary experiments to diminish the population of the microorganisms in the air, the photocatalyst-1 added to the microbial suspensions obviously inhibited the microbial growth under the artificial light. The photocatalyst-1 showed the bactericidal activity against Salmonella typhimurium spread on the nutrient broth agar medium. These results suggest that the photosensitizers/ photocatalysts under the light can remove some air pollutants and hence they can be used to reduce the exposure of the workers in the horticultural facilities and/or the public in the environment to the harmful pollutants.

        Background and Objectives : Epidermal growth factor receptor (EGFR), a 170-kDa transmembrane glycoprotein, appears to mediate epidermal growth factor (EGF) activity. Transforming growth factor-α and EGF produce their biological effects in numerous systems by stimulating the EGFR In this study, we examine the postnatal development of EGFR immunoreactivity in the different regions of the thalamus of the rat Materials and Methods : The present study is based on 28 postnatal cases of rat thalamus ranging from the day of birth, postnatal day 0 (P0) to 30 days (P3, P5, P10, P15, P20, P30), and these cases were compared with adult rat thalamus. Cryostat sections were processed free-floating with monoclonal antibody by immunohistochemistry Results : EGFR immunoreactivity in the thalamus of the rat showed very different patterns according to postnatal ages and thalamic areas. EGFR-immunoreactive cells appeared in the first two postnatal weeks, except the ventral posterior thalamic nuclei. In the early postnatal days, EGFR-immunoreactive cells appeared thalamic midline structures, increased progressively in the first two postnatal weeks, and followed mediolateral gradient. The mature patterns of EGFR-immunoreactive cells were achieved at P20 Conclusion : These data indicate that the maturation of EGFR-immunoreactive cells requires a relatively prolonged period of time to achieve an adult configuration. Many growth factors probably play protective or neurotrophic roles at EGFR-immunoreactive neurons of thalamus both young and adult rats In addition to difference in time of appearance in thalamic nuclei and developing pattern with mediolateral gradient suggest that EGFR-immunoreactivities are correlated with the appearance of the related functional.

        정상면역을 가진 환자에서 발생한 복부대동맥의 진균 감염성동맥류 1예

        감염성동맥류는 드물지만, 치명적인 질환이다. 진성 진균성 감염성동맥류는 더 드물게 발생한다. 진균감염은 사망률과 이환율이 매우 높다. 그러나 증상이 비특이적이고 일반적인 진단방법으로는 민감하게 진단이 되지 않기 때문에 종종 진단이 어렵다. 저자들은 면역이 정상이고 다른 위험인자가 없는 환자에서 복부대동맥에 발생한 진균성 감염성동맥류를 경험하여 이를 보고한다. 컴퓨터 단층촬영과 혈관 수술에 기초하여 진단하였으며, 수술 후 조직배양에서 균이 동정되지는 않았으나 절제한 혈관조직에서 칸디다 감염을 시사하는 발아가 있는 효모균이 관찰되었다. 한국에서 1988년에서 2007년까지 복부에 발생한 감염성동맥류 17증례를 고찰하였을 때 진성 진균성 감염성동맥류는 없었으며, 저자들이 경험한 것이 복부에서 발생한 진균감염성 동맥류로는 첫 증례이다. Infected aneurysms are uncommon, frequently fatal lesions. "True" fungus-infected aneurysms are even rarer. Fungal infections have high morbidity and mortality. However, diagnosis is frequently difficult, since the symptoms are non-specific and standard diagnostic procedures are often insensitive. We experienced a patient with persistent fever and negative blood cultures. The patient was immunocompetent and had no risk factors, and was diagnosed with a fungus-infected aneurysm based on computed tomography and vascular surgery. The vascular tissue revealed some narrow-based budding yeast within the thrombus, suggesting Candida infection. Seventeen cases of infected aneurysm of the abdomen were reported in Korea from 1988 to 2007, although none were "true" fungus-infected aneurysms, making this the first fungus-infected aneurysm of the abdomen in Korea. Prompt diagnostic procedures and aggressive treatment modalities are necessary for patients with occult infection and negative blood cultures, regardless of their immunocompetence, because of the high morbidity and mortality of this condition.
