        Analyzing Nursing Leadership at an Academic Historical Event: A Descriptive Study Based On Social Networks

        Purpose: To analyze the leadership network structure among nursing leaders in Spain identified through the Grupo40Enfermeras y Universidad event. Methods: A descriptive cross-sectional study using social network analysis was used. Study sample consisted of 210 individuals, of whom 119 received nominations as referents. Structural analysis of the network was conducted using centrality and cohesion. Results: A network structure was generated in which different leadership strategies were identified through InDegree, Eigenvector, and Betweenness Centrality. Five leaders were identified as bridges to other individuals using Betweenness. The whole network presented little cohesion although two highly cohesive cores were detected by K-core measurements. Conclusion: A strategy is needed to support nursing leaders with high degree of Betweenness to serve as bridges to connect other nursing leaders

        Clinical, Psychopathological, Physical, and Sleep Evolution in Adolescents with Restrictive Anorexia Nervosa Participating in a Day Hospital Program

        Objective To analyze the clinical, psychopathological, physical, and sleep-related evolution of adolescents with restricting-type of anorexia nervosa (AN-R) after 10 weeks of a daytime hospital program. Methods Body composition, physical activity and sleep were measured objectively before and after 10 weeks of treatment. In addition, psychopathology and body image disturbances were measured with a self-report questionnaire. Results Fourteen female adolescents with AN-R (14.3±1.6 years old) participated in the study. A significant increase was found in eight of the ten variables for body composition (p<0.05). There were no significant changes in psychopathology, body image disturbances or physical activity. Concerning sleep, a significant, moderately standardized and substantial increase in night latency was found (p=0.002), and there was a significant, small standardized and substantial decrease in night efficiency (p=0.035). Conclusion After 10 weeks of follow-up with adolescent patients with AN-R who attended a day hospital program, there was a positive evolution of body composition. However, with regard to sleep patterns, there was a worsening of latency and night efficiency. Therefore, sleep care should be addressed in acute treatment programs for adolescents with AN-R.

        Effect of Gymnema sylvestre Administration on Glycemic Control, Insulin Secretion, and Insulin Sensitivity in Patients with Impaired Glucose Tolerance

        Gymnema sylvestre, a plant typical of India, has long been known for its hypoglycemic effects. The objective of this study was to evaluate the effect of G. sylvestre administration on glycemic control, insulin secretion, and insulin sensitivity in patients with impaired glucose tolerance (IGT). A randomized, double-blind, placebo-controlled clinical trial was conducted in 30 patients with IGT. Fifteen patients randomly received G. sylvestre in doses of 300 mg b.i.d. and the other 15 received placebo in the same way. Before and after the intervention, anthropometric and metabolic measurements were taken, including 2-h oral glucose tolerance test (2-h OGTT), fasting plasma glucose, glycated hemoglobin A1c (A1C), and the lipid profile panel. Areas under the curve of glucose and insulin were calculated, as well as the insulinogenic, Stumvoll, and Matsuda indices. Wilcoxon, Mann–Whitney U, and chi-square or Fisher's exact tests were performed, and a P-value ≤.05 was considered statistically significant. There was a significant reduction in 2-h OGTT (9.1 ± 1.2 vs. 7.8 ± 1.7 mmol/L, P = .003), A1C (5.8 ± 0.3% vs. 5.4 ± 0.4%, P = .025), body weight, body mass index, and low-density lipoprotein cholesterol levels in the G. sylvestre group, with an increment in the Matsuda index (1.8 ± 0.8 vs. 2.4 ± 1.2, P = .008). At the end of the intervention, 46.7% of the patients obtained normal values in A1C. In conclusion, G. sylvestre administration in patients with IGT decreased 2-h OGTT and A1C, increasing insulin sensitivity. There were also improvements in anthropometric measures and the lipid profile.

        Transanal Evisceration Caused by Rectal Laceration

        Transrectal evisceration caused by colorectal injury is an unusual entity. This pathology is more frequent in elderly patients and it is usually produced spontaneously. Rectal prolapse is the principal predisposing factor. An 81-year-old woman was taken to the hospital presenting exit of intestinal loops through the anus. After first reanimation measures, an urgent surgery was indicated. We observed the absence of almost every small intestine loop in the abdominal cavity; these had been moved to the pelvis. After doing the reduction, a 3 to 4 cm linear craniocaudal perforation in upper rectum was objectified, and Hartmann’s procedure was performed. We investigated and knew that she frequently manipulate herself to extract her faeces. The fast preoperative management avoided a fatal conclusion or an extensive intestinal resection. Reasons that make us consider rectal self-injury as the etiologic factor are explained.

        Dear Editor, The rec(8)dup(8q)inv(8)(p23.1q22.1) chromosome associated with San Luis Valley Syndrome (SLVS OMIM 179613) is usually diagnosed in Hispanic patients from the USA Southwest where a founder carrier Spaniard lived around 1800 [1, 2]. This rec(8) has an 8q duplication of 47.90 Mb and an 8p deletion of 11.65 Mb [3, 4]. Excluding two de novo rec(8)dup q chromosomes characterized only by G-bands and included in a recent compilation [5], cytogenomic analyses identified nine comparable de novo der(8)dup q/del p chromosomes with or without a simultaneous 8p gain. We describe a Mexican mestizo girl with a de novo SLVS-like der(8) but with a concomitant 8p22p23.1 duplication.

        Mechanosynthesis of graphene nanoribbons from waste zinc–carbon batteries

        The mechanosynthesis route is a physical top–down strategy to produce different nanomaterials. Here, we report the formation of graphene nanoribbons (GNRs) through this route using carbon bars recovered from discarded alkaline batteries as raw material. The mechanosynthesis time (milling time) is shown to have an influence on different features of the GNRs such as their width and edges features. TEM revealed the presence of GNRs with widths of 15.26, 8.8, and 23.55 nm for the milling times of 6, 12, and 18 h, respectively. Additionally, the carbon bars evolved from poorly shaped GNRs for the shortest milling time (6 h) to well-shaped GNRs of oriented sheets forming for the longest milling time. Besides GNRs, graphene sheets (GNS) of different sizes were also observed. The Raman analysis of the 2D bands identified the GNS signal and confirmed the GNRs nature. ID/IG values of 0.21, 0.32, and 0.40 revealed the degree of disorder for each sample. The in-plane sp2 crystallite sizes (La) of graphite decreased to 91, 60, and 48 nm with increasing peeling time. The RBLM band at 288 cm−1 confirmed the formation of the GNRs. Mechanosynthesis is a complex process and the formation of the GNRs is discussed in terms of a mechanical exfoliation, formation of graphene sheets and its fragmentation to reach GNR-like shapes. It is shown that the synthesis of GNRs through the mechanosynthesis route, besides the use of recycled materials, is an alternative for obtaining self-sustaining materials.

        Use of supercritical methanol/carbon dioxide mixtures for biodiesel production

        The use of supercritical conditions for the production of biodiesel from both vegetables oils and waste-oils may be of great industrial interest because it can be carried out without those catalysts necessary in the conventional transesterification process, therefore avoiding a complex separation between the product and the catalyst. However, the use of supercritical alcohol requires higher operating temperatures and pressures. In this work, CO2 was added to the reaction mixture in order to reduce the operating conditions (temperature, pressure and molar ratio of alcohol to vegetable oil). The novelty of using CO2 may have two advantages: a possible combination of supercritical CO2 extraction of the oil and its subsequent transesterification reaction without CO2 depressurization, and a reduction of the supercritical temperature and pressure of the mixture. The effects of temperature (280-350 oC), pressure (140-280 bar), methanol- to-oil molar ratio (20-30), CO2-to-methanol molar ratio (0.05-0.2) and residence time (0-45minutes) on the yield of methyl esters (biodiesel) were studied in a batch reactor, obtaining in all cases a relatively low increase in the yield when CO2 was present in the medium. The yields of biodiesel were tested with three vegetable oils used as model compounds (palm, sunflower and borage), obtaining similar results.

        A novel approach for the rigid body guidance synthesis of planar RRPR linkages

        This paper demonstrates that the maximum number of poses that can be exactly reached by a planar RRPR linkage is four, while the design for five poses requires the relaxation of displacement constraints at one prescribed pose. To this end, a novel and explicit condition for the synthesis of RP dyads with five prescribed poses is correspondingly introduced. It leads to a new and efficient approach to synthesize planar RRPR linkages, which is equipped with a branching identification procedure. Two successful case studies are provided to validate the applicability of the proposed synthesis approach.

        Exercise training modulates the gut microbiota profile and impairs inflammatory signaling pathways in obese children

        Childhood obesity has reached epidemic levels and is a serious health concern associated with metabolic syndrome, nonalcoholic fatty liver disease, and gut microbiota alterations. Physical exercise is known to counteract obesity progression and modulate the gut microbiota composition. This study aims to determine the effect of a 12-week strength and endurance combined training program on gut microbiota and inflammation in obese pediatric patients. Thirty-nine obese children were assigned randomly to the control or training group. Anthropometric and biochemical parameters, muscular strength, and inflammatory signaling pathways in mononuclear cells were evaluated. Bacterial composition and functionality were determined by massive sequencing and metabolomic analysis. Exercise reduced plasma glucose levels and increased dynamic strength in the upper and lower extremities compared with the obese control group. Metagenomic analysis revealed a bacterial composition associated with obesity, showing changes at the phylum, class, and genus levels. Exercise counteracted this profile, significantly reducing the Proteobacteria phylum and Gammaproteobacteria class. Moreover, physical activity tended to increase some genera, such as Blautia, Dialister, and Roseburia, leading to a microbiota profile similar to that of healthy children. Metabolomic analysis revealed changes in short-chain fatty acids, branched-chain amino acids, and several sugars in response to exercise, in correlation with a specific microbiota profile. Finally, the training protocol significantly inhibited the activation of the obesity-associated NLRP3 signaling pathway. Our data suggest the existence of an obesity-related deleterious microbiota profile that is positively modified by physical activity intervention. Exercise training could be considered an efficient nonpharmacological therapy, reducing inflammatory signaling pathways induced by obesity in children via microbiota modulation.

        Specific contact resistance of IGZO thin film transistors with metallic and transparent conductive oxides electrodes and XPS study of the contact/semiconductor interfaces

        In this work, the specific contact resistance (ρc) between amorphous indium-gallium-zinc-oxide (IGZO) semiconductor and different contact electrodes was obtained from thin film transistors (TFTs). Ti/Au (10/100 nm), aluminum doped zinc oxide (AZO, 100 nm) and indium tin oxide (ITO, 100 nm) were used as source/drain electrodes to fabricate IGZO TFTs. Chemical states of the contacts/semiconductor interfaces were examined by depth profile X-ray photoelectron spectroscopy (XPS) analysis to explain the origin of the differences on specific contact resistance. The lowest ρc achieved using Ti/Au was related to the formation of a TiOx interlayer due to oxygen atoms diffusing out from the semiconductor under layer, increasing the carrier concentration of IGZO at the interface and lowering the ρc. On the contrary, no interfacial reactions were observed between IGZO and AZO or ITO source/drain. However, IGZO resistivity increased with ITO contacts likely due to oxygen vacancies filling during ITO deposition. This fact seems to be the origin of the high contact resistance between IGZO and ITO, compared to IGZO-AZO and IGZO-Ti/Au interfaces.
