Desflurane이 흰쥐 Isolated Heart의 수축기능에 미치는 영향

Background : Desflurane, a fluorinated methyl-ethyl ether, has some advantageous properties including low blood solubility, stability in soda lime, and resistance to biodegradation. Desflurane in vivo has demonstrated myocardial depressant property. The purpose of this study was to test the direct effects of desflurane on myocardial contractile function and coronary flow in the isolated heart. Methods : Twelve isolated rat hearts were continuously perfused with modified Krebs solution containing 6, 9 and 12 vol% of desflurane for 10 min at each concentration. Systolic left ventricular pressure and rate of change of ventricular pressure (dp/dt) were measured. Heart rate and coronary flow were also measured. To differentiate direct vasodilatory effect of desflurane from an indirect metabolic effect due to autoregulation of coronary flow, oxygen delivery, myocardial oxygen consumption and percent oxygen extraction were calculated. Results : Heart rate (control 266 22 beats/min) decreased to 250 23 beats/min at 6 vol%, 236 26 beats/min at 9 vol% and 223 22 beats/min at 12 vol% of desflurane. Systolic left ventricular pressure and dp/dt decreased in a concentration-dependent manner. In spite of decrement of myocardial oxygen consumption, coronary flow (control 12.0 1.2 ml/min) increased to 12.8 1.6 ml/min at 6 vol%, 12.9 1.6 ml/min at 9 vol% and 13.7 1.4 ml/min at 12 vol% of desflurane. Oxygen delivery increased proportionally with coronary flow. Percent oxygen extraction decreased in a concentration-dependent manner. Conclusion : These results suggest that desflurane has a direct myocardial depressing and coronary vasodilating effect in a concentration-dependent manner. (Korean J Anesthesiol 1998; 35: 825∼830)
Laryngeal Mask 를 이용한 전신마취시 수술후 인후통에 대한 관찰

The laryngeal mask as an new form of airway has recently been described. This mask is introduced blindly into the hypopharynx to form a seal around the larynx, so permitting spontaneous or positive pressure ventilation without passing the larynx, therefore associated with much reduced incidence of laryngeal damage. But endotracheal intubation is associated with a significant rate of layngeal damage and subsequent vocal cord pathology. This study was designed to investigate the effects of postoperative sore throat related to insertion of laryngeal mask airway and to compare with these effects after endotracheal intubation. One hundred randomly selected female patients who had gynecological procedures under general anesthesia were surveyed to ascertain the incidence of sore throats. The results were as follows; 1) The incidence of sore throat after endotracheal intubation with the Portex tube was 48% but 12% after insertion of laryngeal mask. 2) The average volume of air required to inflate the cuff of laryngeal mask sufficiently to form air-tight seal around larynx was 11.4±2.8ml and cuff pressure was 25.6±13.2cmH2O when the laryngeal mask 3 was used for Korean adult female.

This study was conducted to find pork quality to meet the needs of consumers. Thus, the meat quality and fatty acid composition of longissimus muscles from purebred pigs (Landrace, Yorkshire, and Duroc) and three-way crossbred LYD pigs were compared and evaluated. Chemical compositions of longissimus muscles were significant (p<0.05) different among pigs. Duroc contained significant (p<0.05) higher fat contents than other pigs, whereas significant (p<0.05) higher moisture contents were observed in Landrace, York-shire, and LYD pigs compared to those of Duroc pigs. The values of pH24 h and pH14 d were the highest in Landrace pigs. Myoglobin contents of LYD pigs were higher (p<0.05) than those of purebred pigs. Regarding meat color, Duroc and Yorkshire pigs had higher redness values than Landrace and LYD pigs, while Landrace pigs had the lowest (p<0.05) color values among all pigs. There was no significant difference in shear force or water holding capacity (WHC). Duroc pigs maintained the lowest drip loss during 14 d of cold storage. In sensory evaluation, the marbling scores of Duroc pigs were higher (p<0.05) than other pigs. Regarding fatty acid compositions, total USFA, poly-, n-3, and n-6 contents were the highest (p<0.05) in LYD pigs, while total SFA contents were the highest (p<0.05) in Duroc pigs. Based on these results, purebred pigs had superior overall meat quality to crossbred pigs.

In this study, the effect of flow settling means on the performance of fan tester were numerically investigated by using a computational fluid dynamics(CFD). The airflow rate was calculated from the pressure differential across a flow nozzle in the measuring plane and the flow settling means were generally installed in the chamber of the fan tester to provide proper airflow patterns ahead of the measuring plane. The predicted nozzle differential pressures with uniform inlet velocities were compared with the values of the ANSI/AMCA 210-99 to verify the performance of the commercial CFD code CFX 5.6. The influence of flow settling means on the measurement of airflow rate in a fan tester were discussed with various porosities and inlet jet velocities. The results obtained show that the proper band of porosities exist to meet the AMCA standard in a specified inlet jet velocity.
