N/A To evaluate antirheumatic efficacies and possibilities on treatment of rheumatoid arthritis with recombinant interferon-gamma, 44 patients with rheumatoid arthritis were treated with IFN-γ for 20 days and were analysed from the standpoint of Ritchie index, patient's assessment, laboratory findings and sideffect. The observed results are as follows: 1) 44 patients were enrolled, 40 patients could be evaluated. Of these, 22 were responders (55%) according to the criteria for treatment success. 2) In 35 patients, stabilized therapy was continued concomittant with the IFN-γ and were no significant differences with respect to the results of treatment. 3) In responders, Ritchie index decreased from 28 on the day of recruitment into the study to 15 on day 20. 4) The patients evaluation of morning pain and change in mobility showed more improvements in responders than non-responders. 5) Monitering of laboratory parameters revealed no therapeutically relevant changes. Side-effects occurred in 14 out of 40 patients. In 10 of these patients, transient fever was recorded.

Xanthogranulomatous cholecystitis is an uncommon focal or diffuse destructive inflammatory lesion of the gallbladder. It is characterized macroscopically by yellowish tumor-like mass in the wall of the gallbladder, and microscopically characterized in the early stages by a large number of foamy histiocytes and acute inflammatory cells, and in the )ater stages by increasing fibrosis. It has been usually mistaken for gallbladder cancer and often seen with stones. A case is presented in which a 59-year-old woman with clinical gallbladder cholesterol polyp was found at surgery to have a edematous and thickened wall of gallbladder, multiple black pigmented stones and 3 x 0.5cm sized yellowish mass on gallbladder bed which was confirmed later as xanthogranulomatous cholecystitis, pathologically. The acute intermittent porphyria (AIP) is characterized biochemically by increased excretion of porphyrins and porphyrin precursors, S-aminolevulinic acid (ALA) and porphobilinogen (PBG). AIP presents with abdominal pain, mental disturbances and neurologic dysfunction. A 24-year-old woman with a medical history of AIP was admitted to our hospital because of abdominal pain and proximal muscle weakness. She was treated by carbohydrate loading with intravenous glucose. Twenty five days later, she developed tachycardia, fluctuation of blood pressure and syndrome of inappropriate secretion of antidiuretic hormore (SIADH). On 90th day, intubation was required to control her breathing. Complete flaccid paralysis developed, and the patient became comatose. The neurologic effects of acute porphyria are generally reversible, but the clinical course of severe case is complicated with respiratory failure, fluid electrolyte imbalance. AIP should be considered in differential diagnosis of patient with unexplained respiratory failure. (Kor J Gastroenterol 2000;36:413 - 418)

Systemic lupus erythematosus (SLE) is a multisystem inflammatory disease that manifests various symptoms. Mesenteric vasculitis (MV) is one of the serious complications of SLE and carries a high mortality rate. Although MV is the main cause for acute abdominal pain in patients with SLE, it is very rare for the pain to be the first clinical manifestation of the disease. A 34-year old female presented with sudden onset abdominal pain accompanied by small intestinal bowel edema observed on abdominal computed tomography. We performed a diagnostic laparoscopy, as vital signs were becoming unstable and the diffuse abdominal tenderness was worsening rapidly. The examination showed a severe jejunal infarction; thus, the patient underwent a small bowel segmental resection. A histological examination revealed multiple, hemorrhagic, small-vessel vasculitis, and later serologic autoimmune markers were consistent with SLE. We suggest that SLE be considered in the differential diagnosis of young females presenting with an acute abdomen and unexplained enteropathy. A surgical approach such as exploratory laparoscopy could be an option in search for the cause. (Intest Res 2012;10:305-308)

A 40-year-old man with systemic lupus erythematosus (SLE) and diabetes was found to have sepsis with multiple small hepatic abscesses secondary to Yersinia pseudotuberculosis which were detected by computed tomography (CT) scan and blood cultures. Sepsis with Y. pseudotuberculosis is uncommon but usually seen in patients with underlying liver diseases or diabetes. A few of those patients are accompanied by liver abscesses. Those patients with liver abscesses invariably have multiple small abscesses. CT scan of the liver was important in demonstrating the multiple small liver abscesses. Identification of the pathogen on blaod culture and elevated serum antibody titer to Y. pseudotu-berculosis are useful for diagnosis. Although rare, Y. pseudotuberculosis should be also considered as a possible cause in febrile patient with immunocompromised state such as SLE, diabetes or hemochromatosis.

Giardia lamblia, a gastrointestinal protozoan, is one of the most common disease-causing parasites in the world. Giardiasis is primarily encountered in areas with poor sanitation, but it is also seen in more developed countries. A possible sequela of Giardia infections of the bowel is reactive arthritis or synovitis. Few reports of synovitis secondary to giardiasis exist in the literature. Arthropathy secondary to giardiasis is uncommon, but may be underdiagnosed. We present a 23 year-old woman who had polyarthritis after Giardia lamblia infestation. The synovitis subsided with treatment of the giardiasis with metronidazole. The diagnosis of Giardia synovitis should be suspected by the presence of Giardia cysts in the stool. Although uncommon, giardiasis can cause severe synovitis that may be confused with a septic joint. (Korean J Med 61:82-85, 2001)

Background/Aims: Gallstone disease is one of the major cause of morbidity in adults. Hemodialysis (HD) has been found to increase risk of gallstone formation following increase in bile cholesterol and biliary saturation index. There are very few data about the prevalence of gallstone in hemodialysis patients and so, we performed this study. Methods: We compared the prevalence of gallstone disease in patients with end-stage renal disease (ESRD) treated with HD with that in 208 age- and sex-matched controls (non-uremic control group). Patients who had chronic liver disease, renal disease, and diabetes were excluded from the control group. Results: In our study, we found a prevalence of gallstone of 18.3% in patients with ESRD treated with HD, which was significantly higher than 5.3% in the non-uremic control group. In most of our HD patients, cholecystolithiasis was asymptomatic. HD patients with cholecystolithiasis were statistically significantly older than patients without gallstones. We did not find a difference in sex, duration of HD treatment between patients with and without gallstones. Conclusions: Our results suggest that the prevalence of gallstone disease is higher in patient with ESRD treated with HD than in non-uremic population in Korea.
