        작업치료사를 대상으로 한 노인 인식 및 의사소통 현황

        목적 : 국내의 빠른 고령화로 인하여 노인 인구가 증가하고 있고, 작업치료의 주요 영역들에서 노인들과의 대면하는 비율이 높은 현 상황에서 작업치료사들을 대상으로 노인들에 대한 전반적 인식 및 노인들과의 의사소통 현황을 파 악하고자 하였다. 연구방법 : 2020년 4월부터 6월까지 전국의 병원 및 보건소 등에서 노인들을 대상으로 중재를 제공하는 작업치료사들 을 대상으로 온라인 설문을 활용하여 데이터를 수집하였다. 노인에 대한 인식, 노인과의 의사소통 저해요소, 노인과 의 의사소통 방식을 묻는 3종의 질문지를 배포하였으며 최종 82부를 자료 분석에 활용하였다. 결과 : 작업치료사의 노인에 대한 인식은 대체적으로 부정적인 항목인 높은 것으로 나타났다. 또한 노인들과의 의사소 통 저해요소로서 청각 관련, 언어이해 관련, 조음 관련 순으로 응답하였다. 노인들과의 의사소통 방식을 확인한 결과 작업치료사들은 대화반응 관리, 정서표현, 이해증진, 관계조절 순으로 노인들과의 원활한 의사소통을 유도하는 것으 로 나타났다. 결론 : 작업치료사들은 대상자들의 사회참여와 삶의 질 증진에 주요한 역할을 수행하는 보건의료전문가인 만큼 노인들 의 의사소통 방식 및 자신의 의사소통 기술에 대해 면밀히 파악하고, 개선된 의사소통 방법을 통하여 전문적인 치료 적 중재 및 관련 교육들을 제공해야 할 것이다. Objective : Due to the rapid aging of the country, the elderly population is increasing and the ratio of face-to-face meetings with the elderly in major areas of occupational therapy is high. The purpose of this study was to investigate the overall perception of the elderly and the status of communication with the elderly. Methods : This survey was conducted From April to June 2020. Occupational therapists who provide interventions for the elderly in hospitals and public health centers across the country were collected using online questionnaires. A total of 91 questionnaires were collected through a self-administered questionnaire, of which 82 were used for data analysis, excluding 9 with insufficient responses. Results : Occupational therapists generally had a lot of negative perceptions of the elderly. In addition, as factors that hindered communication with the elderly, they responded in order of hearing-related, language understanding related, and articulation related. Lastly, occupational therapists were found to induce smooth communication with the elderly in the order of conversation response management, emotional expression, understanding enhancement, and relationship control. Conclusions : Occupational therapists are health care professionals who play a major role in improving the subjects' social participation and quality of life. Therefore, it is necessary to provide professional therapeutic interventions and related education using improved communication skills along with an understanding of the elderly's communication skills and their level of communication.

        자원자 저속추돌실험을 통한 두부와 경부 충격량 연구

        Whiplash injury in low-speed traffic accidents are not objectively verified by medical equipment, thereby creating scope for misuse, which has resulted in huge social losses worldwide. The aim of this study was to examine the influence of low-speed vehicular rear-impact collisions on middle-aged men, and to analyze the head and neck injury criteria for the symptomatic human volunteers. Data was examined from the results of 50 dynamic sled tests, originally performed by Hong et al. (2012). In the previous tests, 50 men aged 3050 years were exposed to an impulse equivalent to a bumper-to-bumper rear collision under medical supervision, and no resulting whiplash injury was identified. In this study, for 6 subjects who experienced dull aches over their bodies, head injury criteria (HIC15) and neck injury criteria (Nkm) were calculated according to the accelerations, forces, and moments at the occipital condyle measured by motion capture system. Although there were no changes in magnetic resonance imaging findings in all subjects at the pre-/post-test orthopedic examination, 6subjects revealed mild aches around the shoulder, back, or lumbar area, and their symptoms disappeared within 2 days. The head and neck injury criteria, HIC15 (3.086± 2.942) and Nkm (0.077 ± 0.064) were obtained, and the maximum HIC15 and Nkm were found to be significantly lower than the critical injury assessment reference values (HIC15: 700, Nkm: 0.3). Moreover, even though 2 subjects were exposed to the same level of change of velocity (7.9 km/h), each Nkm was significantly different (0.179, 0.057). One can therefore conclude that Nkm can vary according to voluntary movements in the human subject.

        佛國士를 바라보는 세 가지 시선 : 『三國遺事』를 중심으로 especially on 『Samguk yusa』

        Utopia is said that it begins to deny the painful realities and to have the vision of future. However there are different utopia according to each position; namely, what is the class theybelong to defines their ideas. So the purpose of this paper is to find the meaning each utopia; the utopia of king, (Buddhist) monk, and slave. First, king didn't want to change the present state. They established the Hwang-Ryong temple(黃龍寺), and insisted that the temple had been the place where Kasyapa Buddha and Sakyamuni Buddha set on countless years ago. As you see, they intend to raised their dignity secretly, stressing on the successor of the sacred. There is no vision of the new world here. Second, A-do as a monk who propagated Buddhism to Shilla first, hoped that she would become the land of Buddha. Prudently he accepted the reality against him and had an effort to heat the situations. At last, his utopia was realized, although he died for three thousand months ago. Third, Wook-myon was a female slave in ShilIa. She never received the condition of slave. She desired the Pure-lantd(淨土), and became a Buddha in a blink through ascetic practice repeating the name of Amitabha. As I mention above, the king's utopia is different from the other's. King has a desire to protect the present conditions, while others deny it with a dream of change in their mind. Monk's utopia however, is unlike the slave's regarding the attitude toward the hard realities and the way they overcome. Monk takes cool, reasonable attitude to circumstances against him and perseveres in his efforts to establish his dream; on the other hand, the slave denies the painful realities thoroughly and hope to grip her utopia in a moment through ascetic practice relentlessly. In conclusion, how to achieve the utopia is different according to one's position; in other words, it is important to pay attention to aspiration for utopia in one's situation.

        PC-Crash를 이용한 교차로 보험사기 의심사례 분석

        According to statistics released by the Financial Supervisory Service, as of 2022, detected cases of insurance fraud in South Korea amount to approximately 1 trillion won, involving around 100,000 individuals. This reflects a 14.7% increase compared to the previous year in terms of the detected fraudulent amount. Such fraudulent activities are causing significant harm to honest policyholders. Notably, fraudulent claims related to automobile insurance account for approximately 44% of the total, indicating a prevalent issue with insurance fraud involving vehicles. Deliberate accidents involving automobiles can be assessed cautiously by observing the driving behavior. Drivers can manipulate three main controls: the steering wheel, accelerator pedal, and brake pedal. If these devices are noticeably manipulated in a way that deviates significantly from normal usage, it can be deemed as intentionally causing an accident. Evidence for driving behavior includes Event Data Recorder (EDR), Digital Tachograph (DTG), and video recording devices. Particularly, intentional accidents can be inferred from EDR data when there is evidence of intentional manipulation of the accelerator pedal. Additionally, situations where there is ample time and visibility to use the braking pedal but it is intentionally not engaged can also establish unintentional intent. This case study focuses on an incident where a vehicle intentionally collided with another violating the rules at an intersection. The intentional nature was determined using PC-Crash, reconstructing the black box video to reveal that the collision point within the intersection was outside the path of a left turn. The recreation of a sudden steering correction further supported the argument for intent.

        Comparison of Transthoracic Echocardiography With N-Terminal Pro-Brain Natriuretic Peptide as a Tool for Risk Stratification of Patients Undergoing Major Noncardiac Surgery

        Background and Objectives: The role of preoperative transthoracic echocardiography (TTE) for the risk stratification has not been well investigated yet. We compared the predictive power of TTE with N-terminal pro-brain natriuretic peptide (NT-proBNP), a representative biomarker that predicts perioperative cardiovascular risk, and investigated whether these tests have incremental value to the clinically determined risk. Subjects and Methods: We evaluated the Revised Cardiac Risk Index (RCRI), TTE, and NT-proBNP in 1,923 noncardiac surgery cases. The primary endpoint was a perioperative major cardiovascular event (PMCE), which was defined by any single or combined event of secondary endpoints including myocardial infarction, development of pulmonary edema, or primary cardiovascular death within 30 days after surgery. Results: All echo-cardiographic parameters including left ventricular ejection fraction, regional wall motion score index, and transmitral early diastolic velocity/tissue Doppler mitral annular early diastolic velocity (E/E’) were predictive of PMCE (c-statistics=0.579± 0.019 to 0.589±0.015), but none of these parameters were better than the clinically determined RCRI (c-statistics=0.594±0.019) and were inferior to NT-proBNP (c-statistics=0.748±0.019, p<0.001). The predictive power of RCRI {adjusted relative risk (RR)= 1.4} could be improved by addition of echocardiographic parameters (adjusted RR=1.8, p<0.001), but not to that extent as by addition of NT-proBNP to RCRI (adjusted RR=3.7, p<0.001). Conclusion: TTE was modestly predictive of perioperative cardiovascular events but was not superior to NT-proBNP. Moreover, it did not have incremental value to the clinically determined risk. The results of our study did not support the use of routine echocardiography before noncardiac surgery.

        불교적 感應의 담론 형성에 관하여 : 힘의 역학관계를 중심으로

        본 논문은 한국 고대-중세초기의 불교계 서사를 중심으로 ‘감응’의 언표가 어떻게 담론을 형성하는지를 살폈다. 불교교학 속에 나타난 감응은 위진 남북조 시기 神滅神不滅 논쟁을 거쳐 성인교화를 정당화하는 개념으로 정리되었다. 그것은 초월적 聖과 고통 받는 인간을 연결시켜주는 소통기제이며,기이한 사건을 수반한다. 그런데 이 소통기제는 종교 본래의 영역을 벗어나면서 힘과 욕망의 의미망을 이루면서 담론을 형성한다. 이때 聖은 위기상황을 타개해주고, 인간세상을 구원하며, 존재를 상승시켜주는 ‘힘’으로 현시된다. 초월적 힘을 담지한 자(성인)와 인간은 상하의 위계구조를 지닌다. 다시 말해 성인의 힘은 인간을 향해 밑으로 흐르는 것이다. 하지만 성인이 국가권력과 관계를 맺을 때, 힘은 한 방향으로만 흐르지 않는다. 국가는 이 힘을 포섭해서 부국강병의 이상을 이루고자 하며, 반대로 초월적 힘의 담지자는 인간권력을 끌어들여 현세에서 자신의 존재를 길이 각인시키고자 한다. 이 힘의 양방성에서 초월적 힘과 대등한 인간권력의 한 모습을 볼 수 있다.

        조선 전‧중기 조상신담론을 통해 본 사대부 주체 형성

        This article tries to illuminate the relationship of gentry and Shamanism which have been thought antipodal from the perspective of the accommodation of Shamanism by gentry culture focusing on discourses on ancestral gods in early and mid Chosun. Accordingly, the subject of gentry could be conceptualized as the results of discourse dynamics between the tow not as Neo Confucianism ideology. The order of discourses is as follows. First, the denial of death is the point to reflect Neo Confucianism which is considered as the ideology of gentry. From this point, discourse which is neither compatible nor conflicting with Neo Confucianism is made. Discourses on ancestral gods are located here. Second, ancestral gods and tombs are discourse devices to realize the denial of death. It strongly suggests the center of family community and the presence and the power of the dead. Third, interpretation works to give theoretical validity to these devices are developed. During the process, emotional universality across all the people regardless of status from gentry to slaves can be abstracted. Fourth, ancestral gods which can be said to be the core of the gentry culture are based on this universal emotion and are formed absorbing the symbols of shamanism such as ghosts. In other words, discourses on ancestral gods are created from the cultural capacity where Neo Confucianism and civil Shamanism tradition have been continuously communicated. Fifth, in the capacity of communication, new gentries who are different from the ideological subjects of Confucianism are born. They are the political people who acknowledge shamanism rituals to some extent, enforcing the government of the ruled. To summarize, the subjects of gentry were born not by purely reflecting the ideology of Confucianism but by penetrating and communicating the symbols and passions of the ruled. Those subjects are meaningful in the dimension of politics such as enforcement of governing the ruled. 본고는 조선 전‧중기 조상신담론을 사대부 문화의 무속수용이라는 차원에서 조명하고자 한다. 그 결과 사대부 주체를 성리학 이데올로기가 아니라 양자 간의 담론역학의 결과로 구체화 할 수 있었다. 논의의 순서는 다음과 같다. 첫째, 죽음에 대한 부인은 사대부의 이데올로기라고 할 수 있는 성리학조차 굴절시키는 지점이다. 이를 거점으로 성리학과 부합하지 않으면서도 충돌하지도 않는 담론 계열이 형성된다. 조상신담론은 바로 여기에 자리한다. 둘째, 조상신과 묘지는 죽음에 대한 부인을 실제화 시키는 담론적 장치다. 이는 가문공동체의 구심점이자 죽은 이의 현존과 권력을 강력하게 시사한다. 셋째, 이들 장치에 이론적 타당성을 입히기 위한 해석작업도 개진된다. 이 과정에서 사대부에서부터 노비까지 신분을 막론하고 전 民을 가로지르는 정서적 보편성을 추출할 수 있다. 넷째, 사대부 종법 문화의 핵심이라 할 수 있는 조상신은 이 보편적 정서를 기저로 하고 귀신이라는 무속의 표상을 흡수하여 형상화된 것이다. 바꾸어 말하면 조상신 담론은 성리학과 민간의 무속적 전통이 꾸준히 교섭해온 문화적 역량 안에서 산출된다. 다섯째, 이 교섭의 역량 속에서 성리학의 이념적 주체와는 다른 사대부 주체가 형성된다. 민간 음사를 일정 정도 묵인하는 가운데 이데올로기를 통해 피통치자들에 대한 통치력을 확립하는 정치적 주체다. 정리하자면 조상신담론을 통해 본 사대부 주체는 성리학 이념을 순수하게 반영하기보다는 피통치자들의 표상체계와 정념을 관통하고 교섭하면서 성립된다. 이 주체는 대민 통치력의 강화라는 정치적 차원에서 의의를 지닌다.
