        안와내이물의 임상 양상

        목적: 저자들이 경험한 안와내이물의 임상 양상에 대해 알아보고자 하였다. 대상과 방법: 2012년 7월부터 2015년 11월까지 서울아산병원 안과에 내원하여 안와내이물을 진단 받고 수술적 치료를 시행한 환자 14명의 의무기록을 후향적으로 분석하였다. 안와내이물의 종류와 위치, 초진 당시 시력과 안구 운동 상태, 이물 발생 후 수술까지의 시간, 수술 결과 및 합병증 등을 알아보았다. 결과: 환자의 평균 나이는 45.1세였고 대부분이 남자(13명, 92.9%)였다. 이물 종류는 금속 9명(64.3%), 비금속 무기물 3명(21.4%), 나무 2명(14.3%)이었다. 2명(14.3%)은 안와내이물이 안구를 관통한 경우였고, 8명(57.1%)은 이물이 안와원뿔 내에 위치하였다. 안와내이물과 동반된 안와 합병증으로는 안와골절(5명, 35.7%)이 가장 흔하였고 나무이물로 인한 안와연조직염(1명, 7%)이 있었다. 안와내이물 발생 4개월 이후 제거수술을 받은 경우가 6명(42.9%) 있었고, 이 중 4명은 신경과적 문제로 뇌 자기공명영상 검사를 위해 안와 내금속이물 제거가 필요한 경우였다. 최종 경과 시 교정시력이 Snellen 시력 0.1 이하인 경우는 6명(42.9%)으로 이들은 모두 이물 발생시부터 동반된 손상으로 인해 교정시력이 0.1 이하인 경우였으며, 초기 시력이 양호한 경우는 최종 시력도 모두 양호하였다. 4명 (28.6%)에서 이물 발생 당시 안구운동제한이 있었으나, 최종 경과 시 1명에서만 경미한 안구운동제한이 관찰되었다. 안와내이물 제거술과 관련하여 새로운 합병증이 발생한 예는 없었다. 결론: 안와내이물은 대부분 남성 환자들에서 사고로 인해 발생하였고, 금속이물이 많았다. 수상 당시 안구파열이나 시신경손상이 동반 된 경우를 제외하고는 시력 예후는 양호하였다. 안와 및 안구 합병증이 동반된 이물이나 나무이물의 경우 발생 직후 수술적 제거를 시행하여야 한다. 다만 최근 뇌 자기공명영상 검사를 위해 무증상 금속이물을 지연 제거해야 하는 경우가 많아지므로, 환자 특성에 따라 수상 초기에 적극적인 제거를 고려해야 할 것으로 생각된다. Purpose: To evaluate the clinical characteristics of intraorbital foreign bodies as well as the treatment outcomes. Methods: This was a noncomparative interventional case series. Clinical data and radiographic images were gathered via retrospective chart reviews of 14 patients who underwent surgical removal of intraorbital foreign bodies by an oculoplastic surgeon at the Asan Medical Center, Seoul, Korea between July 2012 and November 2015. Results: The mean age of patients was 45.1 years and 13 patients (92.9%) were male. There were 9 metallic; 3 nonmetallic, inorganic; and 2 organic intraorbital foreign bodies in this series. The most common orbital complication was orbital wall fracture (8, 57.1%), and one patient had orbital cellulitis associated with a wooden foreign body. Six patients (42.9%) underwent surgical removal of foreign bodies in a delayed setting, and 4 of them needed surgery to allow for the brain magnetic resonance image tests to evaluate neurologic problems. There were 6 patients (42.9%) who had a postoperative corrected visual acuity worse than 20/200, and all of them had poor visual acuity at the time of injury due to associated eyeball or optic nerve injuries. Four patients (28.6%) had eyeball movement limitations from the initial trauma, but only 1 patient had persistent limitations postoperatively. There were no other complications associated with surgical removal. Conclusions: The majority of patients with intraorbital foreign bodies were male who had periorbital traumas. The most common foreign body was metal, and orbital wall fractures were common. The poor visual prognosis was related to the eyeball or optic nerve injuries from the initial trauma. The urgent surgical removal should be performed for organic foreign bodies or associated orbital/ocular injuries. Metallic foreign bodies may also be considered for removal to allow for possible brain magnetic resonance image evaluations in the future.

        금속 함유 이온성 액체 촉매상에서의 프로필렌 카보네이트의 합성

        In this study, three different metal-containing ionic liquid catalysts were prepared by metal insertion and characterized by various physicochemical analytic methods. The catalytic performance of the metal containing ionic liquids in the cycloaddition of CO2 with propylene oxide (PO) to produce propylene carbonate (PC) was investigated under the solvent free condition. The order of approximate rate constants (Kapp) for the metal containing ionic liquid catalysts was (MeIm)2ZnCl2, > (MeIm)2FeCl2 > (MeIm)2CuCl2. These results are in accord with the experimentally obtained activity order of the different metal containing ionic liquid catalysts.

        홍화의 플라보노이드 성분 분리 및 항산화 활성

        In this study, isolation of antioxidative compounds was performed for development of anti-oxidizing agent. $CHCl_{3}$, $H_{2}O$, 30%, 60% MeOH, MeOH fractions were examined antioxidative activity by DPPH method, TBARS assay, and SOD like activity. It was revealed that 30%, 60% MeOH fractions had significant antioxidative activity. From 30%, 60% MeOH fraction, nine compounds were isolated and elucidated kaempferol $3-O-{\alpha}-L-rhamnopyranosyl$ $(1{\rightarrow}6)-{\beta}-D-glucopyranoside$ (1), quercetin $7-O-{\beta}-D-glucopyranoside$ (II), quercetin $3-O-{\alpha}-L-rhamnopyranosyl$ $(1{\rightarrow}6)$ ${\beta}-D-glucopyranoside(rutin)$ (III), 6-hydroxykaempferol $3-O-{\beta}-D-glucopyranoside$ (lV), kaempferol $3-O-{\beta}-D-glucopyranosyl$ $(1{\rightarrow}2)$ ${\beta}-D-glucopyranoside$ (V), kaempferol $3-O-{\beta}-D-glucopyranoside$ (VI), luteolin (VII), quercetin $3-O-{\beta}-D-glucopyranoside$ (VIII), apigenin $7-O-{\beta}-D-glucuronopyranoside$ (IX) through physicochemical data and spectroscopic methods (Negative FAB-MS, $^1H-NMR$, $^{13}C-NMR$). Entirely, all compounds had similar antioxidative activity, but more OH group had more antioxidative activity.

        In present study, classification and quality control of Genus Polygonatum were developed using the isolated from Polygonati Odorati Rhizoma and Polygonati Rhizoma. 3 components were isolated from Butanol fractions of Polygonati Rhizoma, and 2 components were isolated from Hexane and Butanol fractions of Polygonati Odorati Rhizoma. All the components were obtained using silica gel and ODS column chromatography. The compounds were identified as adenosine, 14-hydroxylfurost-5-ene-3-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}2$)-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}4$)-O-${\beta}$-D-galactopyranosyl-26-O-${\beta}$-D-glucopyranoside, 22-O-methyl-14-hydrocxyfurost-5-ene-3-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}2$)-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}4$)-O-${\beta}$-Dgalactopyranosyl-26-O-${\beta}$-D-glucopyranoside, ${\beta}$-Sitosteryl-3-O-${\beta}$-D-D-glucopyranoside, 14-hydoxylfurost-5-ene-3-O-${\beta}$-Dglucopyranosyl-($1{\rightarrow}2$)-O-[${\beta}$-D-xylopyranosyl-($1{\rightarrow}3$)]-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}4$)-O-${\beta}$-D-galactopyranoside through physicochemical data, spectroscopic methods ($^1H$-NMR, $^{13}C$-NMR, Mass) according references. The quality control of genus Polygonatum were conducted using HPLC quantitative analysis of 14-hydroxylfurost-5-ene-3-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}2$)-O-${\beta}$-D-glucopyranosyl-($1{\beta}4$)-O-${\beta}$-D-galactopyranosyl-26-O-${\beta}$-D-glucopyranoside, 14-hydoxylfurost-5-ene-3-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}2$)-O-[${\beta}$-D-xylopyranosyl-($1{\rightarrow}3$)]-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}4$)-O-${\beta}$-D-galactopyranoside in 30 samples collected throughout Korea and China. This method provided a tool for standardization of mix or misusing the commercial Odorati Rhizoma and Polygonati Rhizoma. As a result, contained quantity of 14-hydroxylfurost-5-ene-3-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}2$)-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}4$)-O-${\beta}$-D-galactopyranosyl-26-O-${\beta}$-D-glucopyranoside was measured $0.008{\pm}0.006%$ and 14-hydoxylfurost-5-ene-3-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}2$)-O-[${\beta}$-D-xylopyranosyl-(13)]-O-${\beta}$-D-glucopyranosyl-($1{\rightarrow}4$)-O-${\beta}$-Dgalactopyranoside was measured $0.026{\pm}0.012%$.

      • 사례분석을 통한 안전문화 연구

        This research paper carries out the qualitative case study about nuclear events and accidents in the point of view of human factors. In order to perform the task, the structure of human factors about nuclear accidents is taken effect. For the detailed structure, domestic accidents and foreign accidents should be classified and characteristics, organizational culture, manpower structure and policy environment of Korean nuclear reactor should be compared and analyzed. Also, the importance and frequency of occurred events are comprehended through the horizontal classification of nuclear accidents and the structure of human factors in the steps of workers, managers and general controller is attempted through the vertical classification. Furthermore, an organized analysis framework about the occurrence of nuclear events and accidents is provided after separating the stages into individuals and the team to distinguish the characteristics of individuals and the organization. The provided analysis framework can contribute to the establishment of the theory for nuclear safety culture’s inherent model by applying to Kori 1 SBO case on 2011 and Tokaimura nuclear accident in Japan.
