      • 투시조영검사 시 환자 피폭선량 저감화 대책에 관한 연구

        Purpose : To make an offer to the direction for minimizing the dose, we considered some measures and put the measure into practice. Materials and Methods : We measured the fluoroscopy dose with an SHMAIZU 3, HIDACHI 1, PHILIPS 2, and DAP meter (PD-4100L, Torex, Japan) from April 1 to September 18, 2009. We attached a DAP meter in front tube of the fluoroscopy equipment and calibrated the fluoroscopy equipment using a dose meter. We recorded the fluoroscopy time, a number of spot exposures and DAP before and after the activity of reducing the exposure dose. The results were compared with each other to suggest some effective ways of reducing the exposure dose. Results : 1. The average time of each examination was growing down except in colon study. It was distinguished to be reduced in the upper gastrointestinal series of 71.3% and small bowel series of 63.5%. 2. A number of spot exposure time was decreased in all examinations especially endoscopic retrograde cholangiopancreatography of 22.6% and voiding cystourethrography of 14.4% excluded in the upper gastrointestinal series. 3. This study indicated innovative results at reducing DAP which was the aim of this study. Conclusion : To reduce patient dose, we made a standard ourselves although there aren’t any standard DRL in Korea except in some examinations. It is a task of great significance to provide the data that would be a preliminary data for other hospitals that would need the data when they manage exposure doses. Exposure dose management is required for durability and repetitive doses so that it is the most important for a systematic management. For this we will concentrate on the maintenance and management exposure dose regular.

      • 투시조영 검사에서의 면적선량의 측정 및 비교 : 식도조영, 상복부위장관조영, 연하조영 중심으로

        Purpose : This thesis is, esophagography, UGI series, swallowing study using DAP meter measure in checking and fluoroscopy time, body weight, to learn about the correlation with image number, compared with earlier studies and seek the ways of reducing the radiation dose. Materials and Methods : October- December 2008 was an experiment of 212 patients. Philips has used its equipment, DAP meter adherent in front of X-ray tube calibration, and using the ion-chamber. By checking the patients weight, fluoroscopy time, image number and DAP values were measured. Results : According to the increase in esophagography fluoroscopy time as an increase in the DAP had paid attention to statistics (P<0.05). In UGI series, according to the increase in weight, fluoroscopy time, image number as an increase in the DAP had paid attention to statistics (P<0.05). In swallowing study, fluoroscopy time and DAP had paid attention to statistics (P<0.05). In all test, depending on the increase in fluoroscopy time also DAP had increased gradually. In case of our hospital, the average DAP and fluoroscopy time with advanced studies to compare at least 2.4 times, respectively, 6.5 times as high was investigating. In UGI series, fluoroscopy time at least 1.7 times, DAP 2.6 times, image number also was 2.4 time higher. Conclusion : To reduce patient dose for fluoroscopy examination, in here and now, they should evaluate patient dose and accurately analyze related factors of exposure dose. It comes to the conclusion that patient dose is reduced in good image quality by perfonning the examinations for reference of advanced studies or recommend dose recommended dose in international organizations.

      • 소아의 식도 상복부위장관조영검사에서 생식선 차폐 유무에 따른 생식선량에 관한 연구

        목적: 본 연구는 소아의 하복부 전체를 감싸는 랩스커트 형식의 차폐도구를 제작하여 소아의 식도조영검사와 상복부위장관조영검사에서 실험을 통해 생식선 차폐 유무에 따른 생식선의 흡수선량의 변화를 알아보고, 이를 줄일 수 있는 방안을 알아보고자 하였다. 방법: 2011년 1월부터 3월까지 본원에서 식도조영검사와 상복부위장관조영검사를 시행한 소아 50명을 대상으로 검사 중 평균 투시시간과 저격촬영의 수를 알아보았다. 파악된 평균 투시시간은 331.5초와 촬영된 영상은 18장이었으며 이를 적용하여 소아팬텀과 유리선량계를 이용하여 생식선이 포함되지 않도록 조사영역을 설정한 후 생식선 차폐 유무에 따른 생식선의 흡수선량을 비교하였다(협조군). 또한 협조가 어려운 소아를 고려하여 위의 파악된 투시시간과 저격촬영의 수의 1/10의 조건으로 생식선이 포함되도록 조사영역을 설정한 후생식선 차폐 유무에 따른 생식선의 흡수선량을 비교하였다(비협조군). 결과: 1. 협조군의 생식선 흡수선량 결과 협조군의 경우 생식선 차폐 유무에 따른 흡수선량 결과, 차폐를 시행하였을 경우 난소 99.5 µGy, 자궁 84 µGy, 전립선 33 µGy, 정낭 15 µGy로 나타났다. 차폐를 하지 않았을 경우 112.5 µGy, 자궁 86.5 µGy, 전립선 34 µGy, 정낭 28 µGy로 생식선 차폐 시행으로 난소, 자궁, 전립선에서의 선량 변화는 미미했으나, 정낭의 경우 약 46.4%의 선량감소를 보였다. 2. 비협조군의 생식선 흡수선량 결과 비협조군의 경우 생식선 차폐 유무에 따른 흡수선량 결과, 차폐를 시행하였을 경우 난소 262 µGy, 자궁 201.5 µGy, 전립선 67 µGy, 정낭 40.5 µGy로 나타났다. 차폐를 하지 않았을 경우 난소 767 µGy, 자궁은 512 µGy, 전립선은 787 µGy, 정낭 1429 µGy로 나타나차폐를 시행하였을 때보다 난소는 약 3배, 자궁은 약 2.5배, 전립선은 약 12배, 정낭은 약 35배나 높게 나타났으며 생식선 차폐 시행으로 무려 약 61~97%의 선량감소를 보였다. 결론: 소아의 식도 위장조영검사 시 생식선 피폭을 줄이기 위해서는 관심부위만 조사영역을 최소화하여 검사를 진행함과 동시에 생식선 차폐라는 보다 적극적이면서도 각별한 노력이 수반될 때 소아의 생식선 피폭을 줄일 수 있을 것으로 생각된다. Purpose: We studied pediatric absorbed dose while performing the pediatric esophagography and upper gastrointestinal series using gonadal shielding. Materials and Methods: We recorded pediatric absorbed dose of gonad during the esophagography and upper gastrointestinal series from January 2011 to March 2011. And we recorded average fluoroscopic time and spot images of 50 pediatric patients. It wes measured such as 331.5s and 18 images. We used pediatric phantom and radiophotoluminescent Glass Rod Detector (GRD). First area of field was to include the gonads (cooperation). Second area of field was not to include the gonads considering active pediatrics (noncooperation). By applying the measured values, we compared absorbed does of gonad according to existence of gonadal shielding. Results: 1. Cooperation : Absorbed dose of gonads was measured on 99.5 μGy (ovary), 84 μ Gy (uterus), 33 μGy (prostate) and 15 μGy (seminal vesicle) include gonadal shielding. On the contrary without gonadal shielding, absorbed dose was measured on 112.5 μGy (ovary), 86.5 μGy (uterus), 34 μGy (prostate) and 28 μGy (seminal vesicle). Result of comparison of the absorbed dose seminal vesicle with gonadal shielding was reduced more than without that (46.4%). 2. Noncooperation : Absorbed dose of gonads was measured on 262 μGy (ovary), 201.5 μGy (uterus), 67 μGy (prostate) and 40.5 μGy (seminal vesicle) include gonadal shielding. On the contrary without gonadal shielding, the absorbed dose was measured on 767 μGy (ovary), 512 μGy (uterus), 787 μGy (prostate) and 1429 μGy (seminal vesicle). Result of comparison of absorbed dose with gonadal shielding was reduced more than without that (61~97%). The absorbed dose of ovary, uterus, prostate with shielding was reduce more than 4.5~12 times.Conclusion: Operators should examine pediatric fluoroscopy examinations to reduce patient’s dose by minimizing the area of field and using gonadal shielding for gonadal dose reduction.

        현대 교육과정의 관점에서 본 "정보"교과서 "정보기기" 영역의 탐구적 경향 분석

        ``정보기기의 구성과 동작`` 영역은 5차 교육과정에서 개정 교육과정에 이르기까지 지속적으로 정보교과의 교육과정에 포함되어 왔다. 그러나 이 영역에 대한 내용선정이나 교수방법 등과 같이 교육적 본질에 대한 논의는 전혀 이루어지지 않았다. 따라서 본 연구는 현대 교육과정의 관점에서 ``정보기기의 구성과 동작`` 영역의 탐구적 경향을 분석하였다. 4개의 교과서를 대상으로 분석한 결과, 1개의 교과서만이 탐구적 교과서로 나타났다. 연구 결과를 토대로 본 연구는 교과서의 제작에서는 내용의 중요성과 학습활동의 관점을 함께 고려할 것, 융복합의 시대에 기본이 되는 교과로서 단순 암기의 지식보다는 성찰을 제공하는 교과임을 고려해야 함을 제안하였다. All the informatics textbooks of the fifth, sixth, seventh and revised curriculums have dealt with the structure and operation of information equipment. But there haven`t been any discussions at all on the educational nature of this section involving the selection of what and how to teach. The purpose of this study was to examine ``the structure and operation of information equipment`` sections of informatics textbooks from a perspective of modern curriculum to see whether there was any inquiry tendency in the textbooks. As a result of analyzing four textbooks, only one textbook turned out to be inquisitive. The findings of the study suggest that the importance of textbook content and the perspective of learning activities should be taken seriously when textbooks are produced, and the fact that in today`s age marked by a global tend of fusion, informatics is one of the fundamental subjects to encourage students to examine themselves instead of just sticking to rote learning should be taken into consideration as well.

        이미지 처리에 관한 인공지능 모델의 발전에 따라 개인정보 유출 문제가 가속화되고 있다. 인공지능은 다방면으로 삶에 편리함을 제공하지만, 딥러닝 기술은 적대적 예제에 취약성을 보이기 때문에, 개인은 보안에 취약한 대상이 된다. 본 연구는 ResNet18 신경망 모델에 얼굴이미지를 학습시킨 후, Shadow Attack을 사용하여 입력 이미지에 대한 AI 분류 정확도를 의도적으로 저하시켜, 허가받지 않은 이미지의 인식율을 낮출 수 있도록 구현하였으며 그 성능을 실험을 통해 입증하였다.

      • 소아 투시조영검사 시 Pulse rate별 화질분석을 통한 저선량 모드 선택에 관한 연구

        Purpose : To purpose of this thesis is a research about reduction plan of radiation dose without reduction of image quality through understanding of the relationship between Dose Area Product(DAP) of Digital Flat Panel Detector(DAPD) equipment and pulse rate on pediatric fluoroscopy studies. Materials and Methods : From 2009 December to 2010 February, 40 patients under the age of 5 children who visited Asan Medical Center to check on pediatric fluoroscopy studies have been enforced(Voiding Cysto urethrography(VCUG: 20 people and Upper gastrointenstinal CUGI: 20 people). The measuring equipment used DAP meter, DAP was measured from identical patient follows per 1 minutes in pulse rate and compared a reduction. Also doctor enforced a question investigation with the object and analyzed the relationship with image quality. Results : The result were as follows; 1) From DFPD equipment, based on 30f/s of UGI examination when compared with 15f/s, at 61.8% of DAP were measured. Also 7.5f/s at 36.3%, 3.7f/s at 17.7% DAP were measured. In case of VCUG, 15f/s at 62.6%, 7.5f/s at 41.9%, 3.7f/s at 25.4% of DAP were measured. 2) Questionnaire results of radiologist about image quality, 75% of the respondents were allowed to 7.5f/s, 25% were allowed to 3.7f/s. Conclusion :In conclusion, when considering about image quality and DAP reduction followed in changes pulse rate, the use of 7.5f/s is most desirable. Therefore, we should try to reduce pediabic radiation dose through methods such as using 7.5f/s.
