        Effects of Disease Detection on Changes in Smoking Behavior

        Purpose: This study was conducted to investigate the effect that detection of chronicdisease via health screening programs has on health behaviors, particularly smoking. Materials and Methods: We analyzed national health insurance data from 2007 and 2009. Subjects who were 40 years of age in 2007 and eligible for the life cycle-based national health screening program were included. The total study populationcomprised 153518 individuals who participated in the screening program in 2007 and follow-up screening in 2009. Multiple logistic regression analyses were conducted by sex, with adjustment for health insurance type, socioeconomic status, body mass index, diabetes, hypertension, hyperlipidemia, and family history of cardiovascularand/or neurovascular disease. Results: Among men with smoking behaviorchanges, those newly diagnosed with hyperlipidemia were more likely to show a positive health behavior change, such as smoking cessation, and were less likely to have a negative behavior change (e.g., smoking initiation). Additionally, men newly diagnosed with diabetes showed lower rates of negative health behavior changes compared to those without disease. Body mass index (BMI)≥25, compared to BMI<23, showed higher rates of positive health behavior changes and lower rates of negative health behavior changes. Newly diagnosed chronic disease did not influence smoking behavior in women. Conclusion: Smoking behavior changes were only detected in men who participated in health screening programs. In particular,those newly diagnosed with hyperlipidemia were more likely to stop smoking and less likely to start smoking.

        A 49-year-old male presented with horizontal binocular diplopia without facial pain or skin lesion. Limitation of medial gaze in the left eye was revealed on neurological examination, which is accompanied by peripheral facial nerve palsy ipsilaterally. The diagnosis had been made based on the diffusion restriction lesion of left pontine tegmentum. We may denominate a “seven-and-a-half syndrome” and clinician should maintain a high level of awareness of the various syndromes associated with pontine lesions

        The purpose of this study was planning of the Mini-School for reducing the class size of elementary school in Seoul. Ultimately, the paper proposes the mini-school type, branch mini-school(6, 8, 10 class) and independence mini-school(12, 18 class). I classify into the four-type's elementary school which was analyzed statistical data of the excessive school and the high-density class in Seoul. I calculate moderate the type and the class scale applied with Chungnang-Gu, which has the excessive school and the high-density class, and suggest virtually Mini-School Positioning Map Model.

        The hTERT‑VNTR2‑2nd alleles are involved in genomic stability in gastrointestinal cancer

        Background hTERT contains a high density of minisatellites, of which rare alleles of hTERT-VNTR2-2nd have been reported to be associated with prostate cancer. This shows an association between VNTR and cancer, but this repeat sequence is likely to be associated with genomic instability. Therefore, we investigated the effects of hTERT-VNTR2-2nd on gastrointestinal cancer and the relationship between repeated sequence and chromosome instability. Methods A case–control study was performed using DNA from 818 cancer-free controls, 539 cases with gastric cancer, 275 cases with colon cancer and 274 cases with rectal cancer. To determine whether minisatellites affect gene expression, expression levels were examined using TERT-reporter vectors in cell lines. In addition, the length of the hTERT-VNTR2-2nd alleles were determined in blood and cancer tissues from 107 gastric cancers, 112 colon cancers and 76 rectal cancers patients to determine whether the repeat sequence was associated with genomic instability during cancer development. Results No statistically significant association between hTERT-VNTR2-2nd and risk of gastrointestinal cancer was detected. However, it has been shown that VNTRs inserted into the enhancer region can regulate the expression of TERT in gastrointestinal cancer cells. Moreover, hTERT-VNTR2-2nd was analyzed in matched blood and cancer tissue from patients with gastrointestinal cancer and in seven among 294 subjects, and hTERT-VNTR2-2nd was found to be rearranged. Conclusions We suggest that minisatellites are associated with genomic instability in cancer and that the hTERT-VNTRs region may increase hTERT expression in gastrointestinal cancer cells.

        플립러닝 체육수업 운영에 대한 초등 교사의 인식 분석

        Objectives The purpose of this study was to explore information and implications for applying flipped learning to physical education classes by analyzing elementary school teachers' perceptions of flipped learning physical education classes. Methods A questionnaire survey of 223 elementary school teachers across the country, focusing on teachers teaching in the G Province, and 210 data were collected and frequency analysis was conducted using SPSS 23.0 for Windows. Results Elementary school teachers recognized that flipped learning physical education classes were necessary, and a high percentage of respondents responded to the purpose and purpose of flipped learning physical education classes as ‘reflecting the needs of the times for future education’ and ‘freeing up time for the development of learner-centered activities.’ There was a strong perception that the operation of flipped learning physical education classes required teacher competencies such as ‘class design ability’ and ‘communication skills’. In addition, 17.6% of primary school teachers experienced flipped learning physical education classes, which were mainly taught in competitive and challenging areas, and the duration of operation was mainly around 6 months, and the operation was recognized as having a higher effect than traditional physical education and classes when managing flipped learning physical education classes. The educational implications of the research are as follows: First, flipped learning physical education classes have a high educational effect and sufficient meaning to be an alternative to future physical education. Second, in order to effectively practice flipped learning physical education classes, it is urgent for teachers to develop their ability to design lessons, and the possibility of practice can be enhanced through teacher learning community, faculty training, etc. Third, it was suggested that in order to effectively operate flipped learning physical education classes, systematic preparation is necessary by providing a comprehensive environment such as digital ethics education and the construction of IT infrastructure. Conclusions Elementary teachers' experience and perception of flipped learning physical education classes are expanding, but they must be supported by the establishment of an environment that increases their viability. 목적 이 연구의 목적은 플립러닝 체육수업에 대한 초등 교사의 인식을 확인하여 그 교육적 시사점을 찾는 것이다. 방법 G도에 위치한 초등학교에서 근무하는 교사들을 중심으로 전국 시도교육청 소속의 초등 교사 223명을 대상으로 설문 조사를진행하였고, 210부의 자료를 수집하여 SPSS 23.0 for Windows를 이용해 빈도분석을 실시하였다. 결과 초등 교사들은 플립러닝 체육수업은 필요하다고 인식했고, 플립러닝 체육수업의 취지와 목적에 대해 ‘미래 교육을 위한 시대적요구 반영’과 ‘학습자 중심 활동 전개를 위한 시간 확보’라고 응답한 비율이 높았다. 플립러닝 체육수업 운영에 있어 ‘수업 설계 능력’ 및 ‘의사소통 능력’과 같은 교사 역량이 요구된다는 인식이 컸다. 또한 초등 교사의 17.6%가 플립러닝 체육수업을 경험하였는데, 주로 경쟁영역과 도전영역을 중심으로 지도하였고 운영 기간은 주로 6개월 내외였으며 플립러닝 체육수업을 운영했을 때 기존 체육과 수업보다 높은 효과를 봤다고 인식했다. 연구 결과에 따른 교육적 시사점을 제시하면, 플립러닝 체육수업은 교육적 효과가 크기에미래 체육교육을 위한 하나의 대안이 될 수 있다. 또한, 플립러닝 체육수업을 효과적으로 운영하기 위해서는 교사의 수업 설계 역량함양이 시급한데 교과 연구회, 연수 등을 통해 실천 가능성을 높일 수 있으며, 원활한 운영을 위해 디지털 윤리교육, IT인프라 구축과같은 제반 환경을 마련하여 체계적인 준비가 필요하다. 결론 플립러닝 체육수업에 대한 초등 교사의 경험과 인식은 확장되고 있으나 실천 가능성을 높일 수 있는 환경 구축이 뒷받침되어야한다.

        플립러닝 초등체육수업 설계모델

        The purpose of this study was to develop a flipped learning physical education class design model that can be used in the field of elementary physical education. For the development of the design model, an initial design model was developed through literature research and validated. Internal validation was conducted through expert validity review and usability evaluation using the method of multiple choice questions, open-ended questionnaires, and in-depth interviews. After conducting external validation through field evaluation, it was completed as a final flipped learning physical education class design model. The final design model is schematized in a bottom-up structure, which reflects the meaning of the word flipped learning and the characteristics of teaching and learning. In addition, it is implied that the flipped learning physical education class should be designed by reflecting the learner's situation and needs through cooperative instructional design. The design model consists of analysis, design, development, implementation and evaluation stages. In the analysis stage, learner analysis, curriculum analysis, and educational environment are analyzed. In the design stage, cooperative class design, flipped learning-based physical activity design, and physical activity support strategy are designed. In the development stage, learning materials and growth-oriented evaluations are developed. In the implementation stage, there are design elements to be taught in common regardless of domains and design elements suitable for the characteristics of each domain. Common design elements include team-based physical activity implementation, instruction by learner's individual level, online/offline physical activity connection, promotion of learner-learner and learner-teacher interaction, and encouragement of learner's voluntary participation. Finally, the evaluation stage consists of reflection, growth-oriented evaluation, and class evaluation. 이 연구의 목적은 초등체육교육 현장에서 활용할 수 있는 플립러닝 체육수업 설계모델을 개발하는 것이다. 이를 위해 전문가 타당성 검토와 사용성 평가를 거쳐 내적 타당화를 했고 현장평가로 외적 타당화를 진행한 후최종 플립러닝 체육수업 설계모델로 완성했다. 최종 설계모델은 상향식 구조로 도식화되어있는데 이는 플립러닝이라는 낱말이 가진 의미와 교수학습 특색이 반영된 것이며 협력적 교수설계로 학습자 상황과 요구를 반영해 플립러닝 체육수업을 설계해야 한다는 의도가 함축되어있다. 설계모델은 분석, 설계, 개발, 실행, 평가 단계로 구성되어있다. 분석단계에서는 학습자 분석, 교육과정 분석, 교육환경을 분석하고 설계단계에서는 협력적 교수설계, 플립러닝 기반 신체활동 설계, 신체활동 지원 전략을 설계해야 하며, 개발 단계에서는 교수학습자료 개발 및 성장중심평가를 구안해야 한다. 실행단계에서는 영역 무관하게 공통으로 지도해야 할 설계 요소와 영역별 특색에 맞는 설계 요소가 기술되어 있는데 공통 설계 요소로는 팀 기반의 신체활동 실행, 학습자개인 수준별 지도, 온·오프라인 신체활동 연계, 학습자-학습자와 학습자-교수자 간 상호작용 촉진, 학습자 자발적 참여 유도가 있다. 마지막으로 평가 단계에서는 성찰, 성장중심평가, 수업 평가를 하도록 구성되어있다.

        작업치료사의 직무 만족도 관련요인

        목적 : 본 연구는 일부 병원의 작업치료사를 대상으로 직무특성, 스트레스, 만족도를 파악하고, 직무만족도에 영향을 미치는 요인들을 분석하기 위한 것이다. 연구방법 : 2005년 11월 5일까지 대한작업치료사 협회에 등록된 임상작업치료사로서 일반적 특성, 직무특성, 직무스트레스, 직무만족도를 포함한 총 63문항의 구조화된 설문지를 가지고 자료 수집을 하였으며, SPSS 프로그램을 통해 일반적 특성과 업무에 따른 직무만족도 및 하부요인 만족도의 차이를 알아보기 위해 t-검정, 직무요인은 ANOVA검정, 직무만족도에 영향을 미치는 독립변수들을 평가하기 위하여 위계적 다중회귀분석(hierarchical multiple regression)을 실시하였다. 결과 : 업무만족도는 1일 치료 환자수가 적을수록, 환자치료에 더 많은 시간을 사용할수록, 직무자율성 요인의 스트레스가 낮을수록 높으며, 소아․청소년을 담당하는 치료사의 업무만족도가 높았다. 직업만족도는 1일 치료하는 환자의 수가 적을수록, 동료수가 2~4명인 경우는 높았지만, 연령이 낮고, 조직체계 요인의 스트레스가 높을수록 낮았다. 보상 및 기회만족도는 남자작업치료사, 연령대가 30~34세 이상, 기혼자, 1일근무시간이 8시간이하, 근무기간이 5년 이상, 급여가 월 200만원 이상의 경우 높았다. 전체만족도는 연령이 30~34세 이상인 경우, 1일 치료 환자수가 적을수록 높았고, 연령이 낮을수록 낮았다. 직무스트레스 요인과 직무만족도의 상관관계를 보면, 모든 직무스트레스 요인은 직무만족도와 역상관관계를 보이며, 특히 직무자율성 결여, 관계갈등, 조직체계 및 보상부적절 항목은 모든 만족도와 통계적으로 의미 있는 역상관관계를 보여주었다. 결론 : 직무만족도를 높이기 위하여 관계갈등과 조직체계의 스트레스 요인을 줄이고, 직무 자율성을 높이는 것이 중요하며, 이를 위하여 작업치료사들의 업무권한 및 자율성을 개선시키고, 조직 내 구성원들 간의 관계개선 및 의사소통을 원활하게 하고 복지후생 및 인사체계의 합리성과 관련된 조직문화 개발, 그리고 성과급이나 교육의 기회제공과 같은 보상과 관련된 제도적 장치가 필요하다고 하겠다. Objective : This study was performed to identify the relationship between occupational stress and job satisfaction among occupational therapists. Methods : A questionnaire was distributed and collected in November 2005. The Questionnaire consisted of 63 items including demographics, job characteristics, occupational stress, and job satisfaction. Occupational stress was categorized into eight sub-scales: physical environment, job demand, insufficient job control, interpersonal conflict, job insecurity, organizational system, lack of reward, and occupational atmosphere. By using a factor analysis, job satisfaction was classified into three categories: ‘task,’ ‘occupation,’ and ‘reward and opportunity.’ Results : Task satisfaction was higher in occupational therapists who treat fewer patients a day, have more treating time per patient, and treat children and adolescents. Occupational satisfaction was higher in occupational therapists who treat fewer patients a day and work with 2 to 4 colleagues. However, it was negatively associated with younger age and a lot of stress was related to the organizational system. Reward and opportunity satisfaction was higher in occupational therapists who are male, 30 to 34 years old, married, work less than 8 hours a day, have worked more than 5 years, and who make more than 2 million Korean won per month. Occupational therapists who were 30 to 34 years old and treat fewer patients a day felt a higher level of total job satisfaction. However, this was negatively related to higher occupational stress due to a lack of reward and the organizational system. Conclusion : The results suggest that occupational stress might play a significant role in decreasing job satisfaction of occupational therapists. In addition, improvement in job authorities and communication channels within an organization, and setting up a reward system such as salary increases and career development, are needed.
