      • 근위 속목동맥협착증에 의한 뇌경색의 치료에 전산화단층 혈관조영술을 이용한 증례

        The influence of the atheroma composition of the proximal internal carotid artery (ICA) on stroke is not well reported compared to its stenosis degree or surface ulceration. A 66-year-old man was admitted for right hemiparesis. Magnetic resonance (MR) angiography revealed mild stenosis with smooth surface in left proximal ICA. He was anticoagulated due to suspicious thrombi in the left atrial appendage. Two years later, he was re-admitted for left middle cerebral artery (MCA) territory infarction despite adequate anticoagulation. In spite of stronger anticoagulation, another infarction occurred in the left MCA territory during admission. CT angiography (CTA) revealed fatty atheroma at proximal ICA. No further stroke occurred on antiplatelet agents instead of anticoagulation. Another man was diagnosed MCA infarction without obvious stroke etiology. CTA suggested fatty atheroma beneath smooth-surfaced mild proximal ICA stenosis, which led us to prescribe antiplatelets. Evaluation of plaque composition using CTA seems to be useful in the management of stroke caused by proximal ICA stenosis. (Korean J Stroke 2008;10:41-44)

        객관 구조화 진료 시험에서 기기에 따른 제세동 술기 능력에 대한 가상 연구

        Purpose: Early defibrillation is essential for survival from ventricular fibrillation (VF). In Korea, assessment of clinical skills, including electrical defibrillation, has been part of the medical licensing examination since 2009. Although one defibrillator is used in the exam, various defibrillators are used in the real world. We wanted to know whether unfamiliar devices might affect defibrillation skill. Methods: Our research was performed during conduct of the objective structured clinical examination (OSCE) for sixth grade medical students. Three different defibrillators were used for the test; CodeMaster, LiFEGAIN, and HEARTSTART MRx. CodeMaster was the defibrillator used for education and training. In the test room, VF was simulated by use of a simulator (SimManⓇ), and one of the three defibrillators was placed randomly. A checklist, where eight items among a total of 13 items were for device operation, was used for assessment of defibrillation skill. The written exam and clinical practice score for emergency medicine and defibrillation skill score were investigated. In addition, each operation time of device (turn-on, charge, and shock) was calculated with review of video resources containing the entire exam process. Results: Among 65 students enrolled, 59 students were included for analysis. Students were divided according to CodeMaster (n=20), LiFEGAIN (n=15), and HEARTSTART MRx (n=24). No significant difference in the score on the written exam and clinical practice was observed among the groups. In addition, the scores for defibrillation skill and the time intervals did not differ among the groups. Conclusion: Unfamiliar devices may not affect defibrillation skill in medical students.

        무선 연속 당측정기에 의한 정상 개의 당 농도 평가

        Blood glucose curves in the management for diabetic patients have several limitations including intermittent assessment of blood glucose concentration, hospitalization, patient restraint, and repeated phlebotomy. The aim of this study was to apply and evaluate a wireless continuous glucose monitoring system (CGMS) in healthy dogs. Subcutaneous interstitial glucose concentrations in 7 dogs were continuously monitored and recorded by wireless CGMS. During induced hyperglycemia, the interstitial glucose concentrations were compared with whole blood glucose concentrations measured by glucometer and serum glucose concentrations measured by automated chemistry analyzer, respectively. There were no significant differences among interstitial, whole blood and serum glucose concentrations. The interstitial glucose concentrations had a good correlation to serum glucose concentrations. The real-time wireless CGMS is a valuable tool for monitoring system of glucose concentrations in dogs. Use of the CGMS for diabetic patients will provide accurate information over traditional blood glucose curves.

        KML 호환성 개선을 위한 명세서 검증 방안

        The main purpose of this study is to perform a specification validation, which had led to the strict designand business rules as defined in the specification, adding the previous methodologies such as well-formed andschema validation. It is intended for the enhanced user convenience, at the same time, proposes the validationsystem to provide the information compatibility with regard to application programs for non-expert. For this,the features of the specification of KML supported from VWorld 3D Desktop and Google Earth have analyzed. Based on this estimation, the system has been designed to inform potential problems and the applicability in3D Desktop and Google Earth for the user. KML information passed through the proposed validation is simplyexpressed on the map, and the information on the program selection and additional details instruct users as a textfile. When not being suitable for the proposed program, another program can be considered, and the problemswhich may be occurred are also announced to increase the compatibility of KML. 본 연구는 KML내부 구문에 대해 알지 못하는 일반사용자들이 KML파일을 사용하기 위한 최적의 지도서비스와 고도화된 검증도구를 사용할 수 있게 국내외 대표적인 저작도구를 분석하였다. 분석된 결과를 이용해 기존의검증도구를 고도화해 구글 어스나 브이월드 3D 데스크톱에서 사용가능 여부와 발생할 수 있는 문제점들을 사용자에게 알려주는 시스템을 설계했다. 사용자가 본 검증도구를 통해 검증을 완료하면 사용한 KML 정보를 지도기반으로 간단히 표현하고 구글 어스와 브이월드 3D데스크톱의 초기화면으로 보여주며 KML과 관련된 중요 사항을알려준다. 만약 적당한 프로그램이 없거나 다른 프로그램을 선택한다면 해당 프로그램 사용 시 발생할 수 있는 문제들을 사용자에게 알려주어 KML의 호환성을 높일 수 있었다.

        오개념이 포함된 불일치 상황에 대면한 학생들의 인지갈등 정도와 과학 개념변화의 관계

        본 연구는 오개념이 포함된 불일치 상황에 대면한 학생들의 인지갈등 정도와 과학 개념변화의 관계를 알아보고자 하였다. 이를 위해 공기의 무게와 용해의 개념을 묻는 문항을 직접 개발하였으며, 올바른 과학 개념을 가진 학생들에게 오개념이 포함된 불일치 상황을 제시하기 위해 논리제시문을 직접 개발하였다. 연구 결과, 첫째, 오개념이 포함된 불일치 상황에 대면한 학생들의 인지갈등은 불일치 상황에 대면하지 않은 학생들의 인지갈등보다 더 높았다. 불일치 상황에 대한 인식, 흥미, 불안, 인지적 재평가의 인지갈등의 네 가지 하위 요소 모두 불일치 상황에 대면한 학생들의 인지갈등이 불일치 상황에 대면하지 않은 학생들의 인지갈등보다 유의미하게 높았다. 둘째, 학생들의 응답 유형별 인지갈등 정도는 불일치 상황에 대면하여 개념변화가 일어났던 '정답 개념 $\rightarrow$ 제시문 개념' 유형이 가장 높았고, 그 다음으로 '정답 개념 $\rightarrow$ 정답 개념, '제시문 개념 $\rightarrow$ 제시문 개념' 유형의 순으로 나타났지만 불일치 상황에 대면하여 개념변화가 일어난 집단과 일어나지 않은 집단 간의 인지갈등 차이는 유의미하지 않았다. The purpose of this study is to investigate the relationship between cognitive conflict levels of elementary school students facing an anomalous situation, including misconception and conceptual change in science. Cognitive conflict in conceptual change plays a key role in science education. That the cognitive conflict levels vary given the same situation is worth of note. Two hundred thirty elementary school students participated in this study. The students were presented with logical arguments including misconceptions; then, the cognitive conflict levels and conceptual changes were examined. The results of this study were as follows. First, the students with a correct conception created more cognitive conflict when faced with an anomalous situation including a conception contrary to their correct conception than the students not facing an anomalous situation did. Second, the cognitive conflict was the highest in the group of students that changed their conception, but the difference between 'C $\rightarrow$ M' type and 'C $\rightarrow$ C' type was not statistically significant.

        오개념이 포함된 불일치 상황에서 초등학생의 학습자 특성에 따른 과학 개념변화

        This study analyzed the correlations among the characteristics of elementary school students and changes in science concepts when they were faced with misconceptions provided by the teacher with intellectual authority. We selected scientific attitude, scientific self-concept, and science process skill, which are expected to influence scientific conceptual changes, as the characteristics of learners and analyzed conceptual changes when using three concepts, the 'weight of air', 'dissolution', and the 'change of weight when water freezes'. The study participants consisted of 230 elementary school students (117 girls, 113 boys) in 5th and 6th grade. The results of this study were as follows. First, the scientific attitudes, curiosity and open-mindedness of the students who changed concepts were statistically higher than those of students who did not change concepts. However, in terms of science self-concepts and scientific process skill, no significant differences or no consistent results were obtained. Second, scientific self-concept showed a relatively high correlation with scientific attitude ($r=.55$) and competence correlated positively with curiosity ($r=.45$) and criticism ($r=.40$). 본 연구에서는 과학적 개념을 가진 초등학생들이 지적 권위를 가진 교사가 제공하는 오개념에 대면했을 때 학습자 특성별 과학 개념변화와 학습자 특성 간 상관관계를 분석하였다. 학습자 특성으로 과학적 태도, 과학 자아개념, 과학 탐구 능력을 선정하였으며, '공기의 무게', '용해', '물이 얼 때 무게의 변화'라는 세 개념을 이용하여 개념변화를 분석하였다. 연구 결과, 첫째, Changed 유형의 과학적 태도 및 호기심, 개방성이 Unchanged 유형보다 통계적으로 더 높게 나왔다. 하지만 과학 자아개념과 과학 탐구 능력의 경우, 두 유형 간 유의미한 차이가 없거나 차이가 있더라도 일관된 결과가 나타나지 않았다. 둘째, 과학 자아개념은 과학적 태도와 높은 상관($r = .55$)을 나타냈지만 과학 탐구 능력과 과학적 태도는 서로 관련이 없었다. 과학 자아개념의 하위 요소인 능력감은 호기심 ($r = .45$), 비판성($r = .40$)과 정적인 상관관계를 보였으며, 과학적 태도 하위 요소인 비판성과 호기심 ($r = .53$), 비판성과 개방성 ($r = .41$), 호기심과 개방성 ($r = .39$) 모두 정적인 상관관계를 나타냈다.
