      • 불응성 자가면역질환에서의 자가조혈모세포이식

        배경: 기존의 치료에 불응하고 예후가 불량한 자가면역질환 환자들에게 최근 고용량 면역억제 및 조혈모 세포이식이 새로운 치료방법으로 대두되고 있다. 저자들은 다발성 경화증(multiple sclerosis, MS) 및 류마티스 관절염(rheumatoid arthritis, RA) 등 2명의 자가면역질환 환자들에서 자가조혈모세포 이식을 시행하였다. 방법: 말초혈액 조혈모세포 가동화를 위하여 cyclophosphamide (4 g/㎡) 및 granulocyte colony stimulating factor (10 g/kg/day)를 투여하였고, CD34+ 세포를 분리·채집 하였다, 이식 전처치로 MS 환자에서 BEAM 및 antihymocyte globulin (ATG) (3.75 mg/kg), RA 환자에서 fludarabine (180 mg/㎡), ATG (10 mg/kg)와 busulfan (8 mg/kg)을 투여하였다. 결과: 호중구 수가 500/㎕ 이상으로 회복되는 기간은 MS 환자에서 9일, RA 환자에서 15일이었다. 혈소판이 20.000/㎕ 이상으로 회복되는 가간은 RA 환자에서 9일 이었고, MS 환자에서는 혈소판 감소증이 발생하지 않았다. 비혈액학적 독성으로 MS 환자에서 WHO 1도의 오심 및 점막염이 관찰되었다. MS 환자는 이식 6개월 후까지 시력감소가 남아있었으나, 이식전에 관찰되던 감각이상 및 운동장애 등의 신경학적 이상 소견은 더 이상 관찰되지 않았다. RA 환자는 이식 1개월 후 관절 증상 및 검사소견의 호전을 보였다. 결론: 불응성 자가면역질환 환자에서 고용량 면역억제 및 조혈모세포이식은 적은 독성으로 높은 치료효과를 기대할수 있으며, 향후 이 시술의 임상적 의의를 규명하기 위하여 전향적이고 장기적인 연구가 필요할 것으로 사료된다. Background: High-dose immunosuppressive therapy followed by autologous hemathpoietic stem cell transplantation (HSCT) has been proposed as a new approach to treat severe, refractory autoimmune diseases. We describe two patients with refractory autoimmune diseases (one multiple sclerosis 〔MS〕and one rheumatoid arthritis〔RA〕) who underwent T-cell-depleted autologous peripheral bleed stem cell transplantation for the first time in Korea. Methods: We mobilized autologous stem cells with cyclophisphamide (4 g/㎡) and granulocyte colony-stimulating factor (10 ㎍/kg/day). Stem cells were enriched ex vivo using CD34-positive immunoselection and reinfused after high-dose chemotherapy with BEAM and antithymocyte globulin (ATG) (3.75 mg/kg) in MS, or fludarabine (180 mg/㎡), ATG (10 mg/kg) and busulfan (8 mg/kg) in RA. Results: The engraftment with an absolute nerutrophil count greater than 500㎕ occurred on day 9 in MS and 15 in RA, respectively. The time to nontransfused platelet count greater than 2.000/㎕ was 9 day in RA. MS patient did not show ant episode of thrombocytopenia. Regimen-related non-hematopoietic toxicity was minimal. For 6 months since HSCT, them patient with MS had been free from previously existed sensory and motor abnormalities except decreased visual acuity. Then patient with RA and only one tender joint and two mildly swollen joints with improvement in laboratory parameters at one month after HSCT. Conclusion: These results underscore the feasibility and potential efficacy of intensive immunosuppression followed by autologous HSCT for treatment of intractable autoimmune diseases. The durability of remission, however, remains to be clarified.

      • 한국산 호도핵과의 항산화 성분 분석및 생리활성

        Biological active compounds were extracted from Juglans sinensis Dode and purification as silica gel column chromatography and HPLC. These compounds were identified as 1,4-benzenediol, 4-hydroxy-benzoic acid and 1,2-bis-benzyl butanoate by GC-MS,IR and NMR-spectrum respectively. The hydrogen donating ability of 1,4-benzenediol isolated from Jugans sinensis Dode had stronger than commercial antioxidants such as BHA and BHT and the results of autooxidation inhibition of isolated 1,4-benzenediol were similar to BHA and BHT. The nitrate scavenging activity was increased as much as 90% in 100㎍/㎖ of 1,4-benzenediol isolated from Juglans sinensis Dode. Activity of xanthine oxidase inhibition indicated that 1,4-benzenediol had high inhibition effect on the xanthine oxidase.

      • 준비운동과 정리운동이 수영에서의 혈액내 젖산농도 및 심박수에 미치는 영향

        The purpose of this study is to examine the effect of warming up in swimming on Lactate Density which makes man be exhausted. And this study is to analyze the effect of cooling down in removing Lactate and the relation with heartrate. The test was practiced in the swimming pool of H University and 14 university students, who has two years experience in swimming and are healthy, were selected for this study. Seven students were warming up and cooling down group and seven were other group. Warming up and cooling down was done 15minutes. This exercise was done 5minutes swimming and 1minutes rest during twenty minutes. The measurement of Lactate and heartrate was conducted stable period, right after warming period and exercise period of body per 5minutes (3times). Just after exercise, cooling down and restoration period per 5minutes (3times), the measurement was conducted. And after cooling down(after 35minutes), the measurement was practiced. The average and standard deviation were conducted on each analysis part. The average difference between groups was analyzed by Two-way ANOVA. The difference in group was examined by Bonferroni test at the p<.05 level of significant. The results as follows 1. Warming up 1). Just after warming up, each group's lactate amount in blood has significant change (P<.05). 2). Just after warming up, lOminutes exercise, 15minutes exercise and 20minutes, each group's heartrate has significant change.(p<.001, p<.01, p<.05, p<.01) 3). All groups have significant change on the lactate and heartrate through exercise (p<.05, p<.01, p<.001). 2. Cooling down 1). After cooling down, each group's lactate amount in blood has no significant change. 2). Each group's heartrate has significant change just after cooling down after 5minutes and 10 minutes(p<.01, p<.05). 3). All groups have significant change on the lactate and heartrate through restoration period(p<.05, p<.01, p<.001).

        An integrated analysis of elbasvir/grazoprevir in Korean patients with hepatitis C virus genotype 1b infection

        Background/Aims: In the Republic of Korea, an estimated 231,000 individuals have chronic hepatitis C virus (HCV) infection. The aim of the present analysis was to evaluate the safety and efficacy of elbasvir/grazoprevir (EBR/GZR) administered for 12 weeks in Korean patients who were enrolled in international clinical trial phase 3 studies. Methods: This was a retrospective, integrated analysis of data from patients with HCV genotype (GT) 1b infection enrolled at Korean study sites in four EBR/GZR phase 3 clinical trials. Patients were treatment-naive or had previously failed interferon-based HCV therapy, and included those with human immunodeficiency virus coinfection or Child- Pugh class A cirrhosis. All patients received EBR 50 mg/GZR 100 mg once daily for 12 weeks. The primary endpoint was sustained virologic response at 12 weeks after completion of therapy (SVR12, HCV RNA <15 IU/mL). Results: SVR12 was achieved by 73 of 74 (98.6%) patients. No patients had virologic failure and one discontinued from the study after withdrawing consent. SVR12 rates were uniformly high across all patient subgroups. A total of 16 patients had nonstructural protein 5A resistance-associated substitutions at baseline (16/73, 22%), all of whom achieved SVR12. Adverse events (AEs) reported in >5% of patients were fatigue (6.8%), upper respiratory tract infection (5.4%), headache (5.4%), and nausea (5.4%). Thirteen patients (17.6%) reported drug-related AEs, two serious AEs occurred, and two patients discontinued treatment owing to an AEs. Conclusions: In this retrospective analysis, EBR/GZR administered for 12 weeks was well-tolerated and highly effective in Korean patients with HCV GT1b infection.

      • 다양한 운동형태의 일회성 운동에 따른 혈액성분 및 혈장량 변화에 대한 연구

        Acute Blood Compositions change were determined in 18 Healthy college aged-men completing three counterbalanced running trial at different exercise intensities: trial 1 at 50%LT, trial 2 at 100% LT intensity and 150%LT intensity. For each trial, blood samples were collected at pre-exercise(baseline), immediately post-exercise(PE), 24 hours(24h) and 48 hours post-exercise. Serum samples were analyzed for Hematocrit(Hct), Red Blood Cell(RBC), White Blood cell(WBC), Hemoglobin(Hb), and plasma Volume Change(PVC). In addition, capillary blood samples were collected for analysis of blood lactate concentrations during incremental test to determine LT. All samples were compared to pre-exercise(baseline). In assessing the blood compositions change variables, significant difference was determined in chages of Hct, RBC, Hb at various intensities over time. However, there was no significant difference in WBC, PVC at any intensity. Consequently, there was significant difference between the 150%LT and the 50%LT for the blood compositions change. I thought that the 50%LT and the 100%LT which was used the exercise protocol with 400 calories consumption didn't support the stimulation enough to response during the exercise even though 150%LT stimulated subjects to get less response than previous studies.

        자매에서 발생한 중증 마이코플라즈마 폐렴에서 스테로이드 치료

        Numerous evidences support the pathogenesis that M. pneumonias pneumonia is associated with cell-mediat-ed immune reaction. We report 2 cases of severe M. pneumoniae pneumonia in previously healthy sisters, who were both admitted during an epidemic of M. pneumoniae pneumonia. The elder sister, who was 16 years old, was admitted with M. pneumoniae pneumonia. She showed no response to treatment with clarithroycin and levo-floxacin, and eventually progressed to severe ARDS, requiring mechanical ventilation. After treatment with hydrocortisone (200 mg/day), there were rapid improvements in clinical manifestations and chest radiographic findings. The younger sister, who was 14 years old, was admitted 10 days later, Presenting with fever but no pneumonic lesions on chest radiograph. Just like her sister, the infection showed no response to clarithromycin. Fever persisted and pneumonic consolidation with mild pleural effusion was noticed in the left lower lobe on the 5th hospital-day. After treatment with oral prednisolone (1 mg/kg), she showed rapid defervescence and on the 8th hospital day, no pneumonic lesions were detectable on chest radiograph. Given the fact that the pathogenesis of pneumonia and extrapulmonary manifestations in M. pneumoniae infection is immune-mediated, an immuno-suppressive therapy would be validated for selected patients with M. pneumoniae infections.

        甲狀腺炎과 Lithium 治療에 의한 甲狀腺機能低下症

        A case of a female Manic Depressive Illness patient with goitrous hypothyroidism is presented. After ten and half months of lithium maintenance therapy starting during her first psychiatric admission, she was admitted again due to acute recurrent manic episode. On admission, she complained of easy fatigability and cold intolerance, and thyroid gland was palpable and enlarged in moderate degree. Thyroid function test revealed markedly increased TSH and moderately decreased thyroxine levels. TRH stimulation test showed an exaggerated response. Needle biopsy specimen revealed chronic lymphocytic and immune complex thyroiditis. However, antithyroglobulin and antimicrosomal antibodies were negative. In pretreatment thyroid evaluation on her first admission, neither goiter nor thyroid function abnormality was detectable and no signs of underlying thyroid disease or hereditary predisposition to hypothyroidism were present. Within three weeks of discontinuation of lithium, goiter was no more palpable and within five weeks, thyroid function test returned to normal range. The possibility that patients with underlying thyroiditis may be particularly susceptible to a rapid onset of lithium-induced hypothyroidism is discussed.

      • 월출산국립공원 수질특성에 관한 연구

        Because mountain streams have narrow widths and steep slopes compared to streams and lakes, hours of immobility are considerably short, and the quality of mountain streams has been greatly influenced by rainfall, land-use, components of the streambed and types of forests. During the summer when heavy rainfall occurs, eroded earth and sand flow into mountain streams and cause temporarily high turbidity. It is also the cause of eutrophication, which is frequently found in downstream. As evaluation standards for the quality of mountain streams were not established in Korea, this quality was evaluated by examining some items based on quality standards for drinkable water and research on the influences of pollution in mountain streams, which showed that these were seldom in downstream water systems. Mountain streams within Wolchulsan National Park flow into a reservoir and when characteristics of mountain streams from forests and rainfall that flow into mountain streams through forest ecosystems were studied, results of measuring its changes are as follows. Monthly changes of pH were low between April and August and higher starting October, and the EC was measured higher in the winter and the spring and tended to be low starting April. The EC on the 19th of May and the 23rd of October and the concentration of floating materials and COD_(Mn) were observed to be higher due to the influence of rainfall on the 19th of May and the 22nd of October, and it was judged that the washing out of dried deposits attached to the water pipe and the leaching of woods were influenced by rainfall. It is considered that real-time monitoring of water quality for a long time and water quality conditions during and after rainfall should be examined for ensuring basic data of water quality and evaluating characteristics and seasonableness of mountain streams, which may be greatly changed by artificial influence in the future.
