        소아 설사 환자의 장 상피 세포 파괴에 의한 대변 내 Cytokeratin-19의 검출

        Purpose:Acute gastroenteritis in infancy is a very common disease with a frequency of a billion diarrhea per year, leading to 3 to 5 million deaths. Since the etiology is not always clear, the treatment plan can be quite difficult to make. The laboratory examinations of stool culture, along with complete blood counts may sometimes be helpful to find bacterial etiology, while occult blood in stool merely means the presence of gastrointestinal tract bleeding. We evaluated the presence of cytokeratin-19 in stools in differentiating bacterial and non-bacterial causes of childhood diarrhea to establish appropriate treatment plans. Methods:Among 201(male 124 patients, female 77 patients) patients with diarrhea, cytokeratin-19 examined by ELISA was positive in 40 patients. Laboratory findings including peripheral blood leukocytes count with differential count, ESR, CRP were compared between the cytokeratin-19 positive and negative groups. We also investigated the correlation between cytokeratin-19 and stool culture, leukocytes, rotavirus antegen test, and occult blood in stool. Results:The leukocytes count, neutrophil count in peripheral blood and ESR were higher in cytokeratin-19 positive group(P=0.030). There was significant correlation between serum CRP and cytokeratin-19(P=0.007). Sensitivity of cytokeratin-19 was 0.30 and specificity was 0.86 in serum CRP positive group and negative group. Conclusion:Cytokeratin-19 in stool was significantly increased in correlation with leukocytes and neutrophil count in peripheral blood, ESR, serum CRP and the specificity of cytokeratin-19 was 0.86 in the serum CRP positive group and the negative group. Therefore, the quantitification of cytokeratin-19 in stool was considered as indicator of differential diagnosis between clinically bacterial and non-bacterial infection groups. 목 적:영유아의 급성 위장관염은 매우 흔하여 전세계 소아에서 매년 10억회의 설사를 일으키며 그 중 3-5백만명이 사망한다. 그러나, 병인에 대한 이해가 부족한 실정이어서 원인에 따라 다른 치료 방침을 세우기 힘든 경우가 많다. 현재 소아 설사 환아의 원인 병원체 규명을 위해 임상에서 실시되는 검사로는 대변 배양 검사, 대변 내 rotavirus 항원 검사, 대변 내 백혈구 검사, 기타 혈액 검사 등이 있으나 장 점막 상피 세포의 손상 정도를 직접적으로 알 수 있는 방법은 없으며 단지 장 출혈 여부를 알아보기 위한 잠혈 반응 검사가 시행되고 있을 뿐이다. 저자들은 소아 설사의 세균성 원인과 비세균성 원인을 감별하는데 대변내의 cytokeratin-19의 양성과 음성의 결과 차이가 어느 정도로 유의한지 파악하여 소아 설사의 치료 방침을 결정하는데 도움이 되고자 한다. 방 법:설사 환아 201명(남아 124명, 여아 77명)에서 40명이 ELISA 방법으로 대변내 cytokeratin-19이 측정되었다. Cytokeratin-19 양성군과 음성군에서 말초 혈액 검사와 분획, C-반응성 단백과 적혈구 침강 속도를 비교하였다. 또한 cytokeratin- 19와 대변 배양 결과, 대변 내 백혈구 수, rotavirus 항원 검사 및 잠혈 반응 검사와의 관련성을 조사하였다. 결 과:Cytokeratin-19 양성군에서 말초 혈액의 백혈구 수, 중성구 및 적혈구 침강 속도가 증가되어 있었고(P=0.030), 혈중 C-반응성 단백도 대변 내 cytokeratin-19와 유의한 상관 관계에 있었다(P=0.007). C-반응성 단백을 대상으로 한 cytokeratin-19의 예민도는 0.30 정도이었으나 특이도가 0.86을 보였다. 결 론:대변 내의 cytokeratin-19는 혈중 백혈구 수치, 중성구 수치, 적혈구 침강 속도, C-반응성 단백의 증가시 같이 증가하였으며 C-반응성 단백과 특이도 0.86을 보여 대변 내의 cytokeratin-19의 측정이 소아 설사 환자에서 세균성과 비세균성 감염을 구분하는데 하나의 지표로 사용될 수 있을 것으로 사료된다.

        타가토스, 자일로스, 과당, 포도당으로 설탕을 대체하여 제조한 복분자 젤리의 이화학적 특성 연구

        . This study determined the physicochemical properties of jellies made with using sucrose and using combinations of these sugars: 1) sucrose 100% (SU), 2) sucrose:tagatose [1:1(w/w)](TA), 3) sucrose:xylose [1:1(w/w)](XY), 4) sucrose:fructose [1:1(w/w)](FR) and 5) sucrose:glucose [1:1(w/w)] (GL). When tagatose was added, both lightness and yellowness tended to decrease, and when xylose was added, the lightness decreased and the yellowness tended to increase. The texture of jelly prepared by replacing tagatose and xylose showed higher values in all items such as hardness, springiness, cohesiveness, chewiness, fracturability, and adhesiveness compared to that of jelly prepared only with sugar. Sweetness and sourness did not show any significant difference according to the type of sugar. Both elasticity and hardness were similar to those in the TPA analysis. The preference was higher in TA and XY than SU. In conclusion, it is thought that the replacement of sugar with a tagatose and xylose by 50% can be applicable for jellies, so we can provide low calorie jellies with reduced health risks.

        A Case of Young-Simpson Syndrome

        Young Simpson syndrome is a rare malformation syndrome characterized by congenital hypothyroidism, dysmorphic face, mental retardation, severe postnatal growth retardation, hypotonia and congenital heart abnormalities. In the present study, we report a case of 4-year-old girl with Young Simpson syndrome for the first case in Korea. Young Simpson 증후군은 선천성 갑상선 기능 저하, 특이한 얼굴 생김새, 정신 지체, 심한 성장 지연, 근력 저하, 선천성 심장 기형을 특징으로 하는 드문 질환으로 국내에서는 보고된 적이 한 번도 없는 질환이다. 저자들은 Young Simpson 증후군인 4살 여아를 경험하였기에 문헌 고찰과 함께 국내 최초로 보고하는 바이다.

        Glanzmann's thrombasthenia는 혈소판 표면의 fibrinogen과 von Willebrand factor(vWF)의 수용체인 당단백 ${\alpha}_{IIb}{\beta}_3$의 결함으로 인해 ristocetin을 제외한 모든 agonist들에 대해 응집 이상을 보여 혈소판 수와 형태는 정상이면서 심한 출혈 시간의 연장을 가져오는 상염색체 열성 유전 질환이다. 저자들은 II형 Glanzmann's thrombasthenia로 진단된 4세 여아에서 ${\beta}_3$ 유전자의 이상 중 보고되지 않은 부위의 이상을 최초로 밝혔기에 문헌 고찰과 함께 보고하는 바이다. Glanzmann's thrombasthenia is an autosomal recessively inherited hemorrhagic disorder that results from quantitative and qualitative abnormalities in platelet membrane glycoprotein IIb-IIIa, also known as ${\alpha}_{IIb}{\beta}_3$ integrin which is an adhesion receptor for fibrinogen and von Willebrand factor. We describe here a 4-year-old girl who had Glanzmann's thrombasthenia with the ${\beta}_3$ subunit missense mutation.

        알룰로스를 첨가하여 제조한 머핀의 특성 연구

        This study was intended to investigate the characteristics of muffins prepared with allulose which is low-calorie and physiologically functional rare sugar. Volume and specific volume of muffins increased significantly with allulose addition, but not with its contents. Muffin heights also increased as allulose was added more. Baking loss rate and moisture contents were not significantly different among samples. Allulose seemed to have nearly same water holding capacity with sucrose. Lightness of muffin crust and crumb both decreased, but redness and yellowness increased according to allulose, the reducing sugar addition by Maillard reaction. In the texture profile analysis, Hardness of AL100 was higher significantly (p<0.05) and springiness, cohesiveness and fracturability increased according to the allulose contents. In the sensory evaluation, AL0 was the sweetest and AL0 and AL25 got higher overall acceptance score. Physical characteristics were not different (p<0.05) among AL0, Al25 and AL50. As a result, 25% sucrose substitution with allulose can provide low-calorie, anti-hyperglycemic and tasty muffin.

        본 저자들은 전신성 홍반성 루푸스와 미만성 증식성 사구체신염으로 진단된 남아의 치료과정 중 발열과 혈구 감소증이 지속된 환아에서 이차성 혈구 탐식성 조직구 증식증을 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Hemophagocytic lymphohistiocytosis is a reactive disorder characterized by a generalized non-malignant histiocytic proliferation with prominent hemophagocytosis by stimulated histiocytes in the bone marrow and reticuloendothelial systems resulting in pancytopenia and liver dysfunction. Several diseases including infection, malignancy and autoimmune disease are known to be causative disorders. This case demonstrated histiocytic hemophagocytosis in the bone marrow, resulting in pancytopenia during treatment of systemic lupus erythematosus and did not show any underlying disease.

        밤 내피 및 외피의 이화학적 특성 및 박피율과의 상관분석

        Purpose: The purpose of this paper was to analyze the physicochemical properties related to peeling rate of Korean chestnut. Methods: Analyses were carried out for weight, thickness, polyphenol, amino acid and peeling ratio for Korean chestnut. The correlation between these physicochemical components and peeling ratio of chestnut was measured. Results: The average of thickness for inner shell and outer shell was 0.66 mm and 0.93 mm, respectively. The average peeling ratio was 77.51%. A significant negative correlation was found between peeling ratio and thickness of the inner shell ($r=-0.80^{***}$, p<0.001). Analysis for the polyphenol composition of inner shell by HPLC was carried out and peaks 2 ($r=-0.56^*$, p<0.05), 11 ($r=-0.68^{**}$, p<0.01), 14 ($r=-0.52^*$, p<0.05), 28 ($r=-0.66^{**}$, p<0.01) showed a significant negative correlation with the peeling ratio. Conclusion: Thickness of chestnut inner shell was determined as the most influential factors of peeling.

        박피 밤의 저장 조건에 따른 품질 변화 분석

        Peeled chestnuts were stored at different temperatures(-10℃ and -20℃) and after blanching at 90℃ for different lengths of time(0, 10, and 20 min) for 6 months. After the storage period, the texture, color, DSC and sensory characteristics were analyzed. Hardness, fracturability and chewiness increased with storage time, whereas springiness and cohesiveness decreased. Additionally, the L* decreased and a* and b* increased with storage time. Analysis of the DSC revealed that the peak of the retro gradation was approximately 46~60℃ and enthalpy for the peak increased with storage time. Additionally, the increment of enthalpy for samples stored at -20℃ and blanched was lower than that of samples stored at -10℃ and non-blanched samples. The sensory evaluation scores were also higher for samples stored at -20℃ and blanched samples than for those stored at -10℃ and non-blanched samples.
