Successful Pregnancy and Childbirth in a Patient with Diabetic Kidney Disease Receiving Hemodialysis

Pregnancy outcomes for women uncontrolled diabetes mellitus are poor. Women with chronic kidney disease and end-stage renal disease, if pregnant, will develop pregnancy-related complications. In the case of severe renal failure and diabetes mellitus, the incidence of prematurity, low birth weight, and neonatal death increases. Despite these risks, there have been few reported cases of successful pregnancy outcomes for these women. Adherence to a schedule of strict glucose monitoring, blood pressure and volume control, correction of anemia, and intensive hemodialysis may play important roles in the management of these women. Successful pregnancy and childbirth in a patient with diabetic kidney disease receiving hemodialysis have not been previously reported in Korea, to our knowledge. Herein, we report a case of a successful pregnancy outcome in a patient with diabetic end-stage renal disease undergoing hemodialysis.

Although activated charcoal is an effective treatment for most toxic ingestions, aspiration of activated charcoal can be fatal. Here, we report that in 5 charcoal aspiration cases, bronchoscopy with suction and lavage was an effective way to remove charcoal from the lungs. Patients showed high APACHE II scores (range: 10-29), and either low PO2 levels, or low CO2 retention. After bronchoscopic removal of the aspirated charcoal, symptoms of hypoxia, CO2 retention, localized wheezing, and pneumonic infiltration as determined by chest radiography, improved in most patients. We report 5 cases of successful treatment of charcoal aspiration with bronchoscopic toilet.
Numerical Analysis of an Air - cooled Ammonia Condenser with Plate Fins

Ammonia has been used as refrigerant for more than 100 years in absorption as well as in compression systems. Due to its poisonous and inflammable properties, however, its use has been mainly on heavy industrial plants in which regular maintenance is available. For these systems, condensers are generally water-cooled. This is suitable for large systems over 20RT but is not suitable for small systems. In order to apply ammonia for a small system, it is important to adopt an air-cooled condenser. In this study, simple numerical analysis of an air-cooled condenser for an ammonia refrigeration system has been carried out. The condenser is designed as horizontal tubes with plate fins attached at the outer surface to enhance the air-side heat transfer rate. Effect of fin shape and arrangement are studied in detail. Since the local heat transfer coefficient is highest at the leading edge, heat flux is highest at the edge and decreases along the distance. Conditions of inlet air are also varied in the study and condenser length that is required for full condensation is calculated. The results show that it is important to enhance both the air-side and internal heat transfer coefficients.
Repeated Clustering to Improve the Discrimination of Typical Daily Load Profile

The customer load profile clustering method is used to make the TDLP (Typical Daily Load Profile) to estimate the quarter hourly load profile of non-AMR (Automatic Meter Reading) customers. This study examines how the repeated clustering method improves the ability to discriminate among the TDLPs of each cluster. The k-means algorithm is a well-known clustering technology in data mining. Repeated clustering groups the cluster into sub-clusters with the k-means algorithm and chooses the sub-cluster that has the maximum average error and repeats clustering until the final cluster count is satisfied.

Background: Prostaglandin J2 (PGJ2) and its metabolites ツ12-PGJ2 and 15-deoxy-ツ12,14-PGJ2 (15d-PGJ2) are naturally occurring derivatives of prostaglandin D2 that have been suggested to exert anti-inflammatory effects in vivo. 15d-PGJ2 is a high-affinity ligand for the peroxisome proliferator-activated receptor ャ (PPARャ) and has been demonstrated to inhibit the induction of inflammatory response genes, including inducible NO synthase and tumor necrosis factor メ, in a PPARャ -dependent manner. Objectives: To analyze the effectiveness of 15d-PGJ2 on the expression of TLR 1-10, IL-1モ, 6, 8, TNF-メ, and iNOS mRNA in human melanocytes stimulated with LPS. Methods: Melanocytes were treated with LPS and 15d-PGJ2 in a dose-dependent manner and cell proliferation was determined using the MTT assay. Todetermine the effect of 15d-PGJ2 on the expression of TLR 1-10, IL-1モ, 6, 8, TNF-メ, and iNOS mRNA in human melanocytes in the presence or absence of LPS, melanocytes were treated with LPS and 15d-PGJ2 for 24 h. We performed RT-PCR for mRNA expression. Results: In this study, TLR 1-10, IL-1モ, 6, 8, TNF-メ, and iNOS mRNA expression was examined, and the expression increased after LPS stimulation. The increased expression was inhibited with 15d-PGJ2. These results revealed a similar pattern to normal human keratinocyte. Conclusion: 15d-PGJ2 may play a role in the pigmentary disorder via anti-inflammatory action.
