Primary pulmonary amyloidosis is a rare disorder. It can be classified into tracheo-bronchial, diffuse alveolar septal and nodular parenchymal form. Tracheo-bronchial amyloidosis can be further subdivided into diffuse and focal varieties. We report a case of diffuse tracheo-bronchial amyloidosis in a 49-year old man comfirmed by broncos-copic biopsy. He has suffered from cough and exertional dyspnea over 20 years and recently atelectasis of fright middle and lower lobe was found on chest X-ray with aggravation of symptom. The chest CT showed irregular thickening of tracheo-bronchial wall with calcification in addition to collapse of right middle and lower lobe. The bronchoscopy showed multiple variable sized submucosal nodules in lower trachea and both sides of bronchi, and biopsy revealed amyloid deposition at the subepithelial layer. We discussed this case with review of literature.

Background and Objectives: Identifying patients with high bleeding risk (HBR) is important when making decisions for antiplatelet therapy strategy. This study evaluated the impact of ticagrelor monotherapy after 3-month dual antiplatelet therapy (DAPT) according to HBR in acute coronary syndrome (ACS) patients treated with drug eluting stents (DESs). Methods: In this post-hoc analysis of the TICO trial, HBR was defined by 2 approaches: meeting Academic Research Consortium for HBR (ARC-HBR) criteria or Predicting Bleeding Complications in Patients Undergoing Stent Implantation and Subsequent DAPT (PRECISE-DAPT) score ≥25. The primary outcome was a 3–12 months net adverse clinical event (composite of major bleeding and adverse cardiac and cerebrovascular events). Results: Of the 2,980 patients without adverse events during the first 3 months after DES implantation, 453 (15.2%) were HBR by ARC-HBR criteria and 504 (16.9%) were HBR by PRECISE-DAPT score. The primary outcome rate was higher in HBR versus non-HBR patients (by ARC-HBR criteria: hazard ratio [HR], 2.87; 95% confidence interval [CI], 1.76–4.69; p<0.001; by PRECISE-DAPT score: HR, 3.09; 95% CI, 1.92–4.98; p<0.001). Ticagrelor monotherapy after 3-month DAPT was associated with lower primary outcome rate than ticagrelor-based 12-month DAPT regardless of HBR by ARC-HBR criteria, with similar magnitudes of therapy effect for HBR and non-HBR patients (p-interaction=0.400). Results were consistent by PRECISE-DAPT score (p-interaction=0.178). Conclusions: In ACS patients treated with DESs, ticagrelor monotherapy after 3-month DAPT was associated with lower rate of adverse clinical outcomes regardless of HBR, with similar magnitudes of therapy effect between HBR and non-HBR.

<▼1><P>Donor–acceptor (D–A) copolymers have been proved to be excellent candidates for efficient polymer solar cells (PSCs).</P></▼1><▼2><P>Donor–acceptor (D–A) copolymers have been proved to be excellent candidates for efficient polymer solar cells (PSCs). An easy and powerful strategy of D–A copolymer design to enhance the performance of PSCs would advance their industrialization. Here we demonstrate an effective molecular design method using the simple molecular mechanics function of MM2 (Molecular Mechanic program 2) & MMFF94 (Merck Molecular Force Field 94) calculation and noncovalent conformational locking effects in the D–A polymer backbone. It is shown to play an important role in D–A copolymers with a highly ordered structure through intra- and/or intermolecular interactions. We report a newly designed D–A copolymer donor, poly(benzodithiophene-dibenzophenazine), P(BDP-DTPz), using our strategy which exhibits good solubility, high molecular ordering and excellent charge carrier mobility balance. A maximum power conversion efficiency (PCE) of 6.2% is achieved with a P(BDP-DTPz):PC71BM blend under 1.5 G solar irradiation. This work will provide a new perspective for molecular design of D–A copolymers in PSCs.</P></▼2>
Cellular-Aided Device-to-Device Communication: The Benefit of Physical Layer Network Coding

<P>We introduce a cellular-aided device-to-device (D2D) communication model in which two D2D transmitters (Txs) send independent messages W-1 and W-2 to their intended D2D receivers (Rxs) with the help of a base station (BS). Specifically, at the first phase, two Txs send their signals to both the BS and two Rxs and then at the second phase the BS sends its signal to the Rxs. We consider a feasible channel assumption that D2D channels are not always available at the BS and each Tx, thus resulting in an outage. For the proposed cellular-aided D2D scheme, the Txs send their messages via physical layer network coding, and thus, the BS is able to decode W-1 circle plus W-2 and broadcasts it to the Rxs. In consequence, from the fact that an outage does not occur if each Rx can decode W-1 or W-2 from its D2D links, the outage probability is significantly reduced compared with the D2D only communication where each Rx needs to decode its own message</P>

<P>The proximal ulna, particularly the course of the posterior border, has a complex three-dimensional (3D) morphology which has been highlighted recently due to its clinical relevance in relation to surgical treatments. 3D computed tomography (CT) reconstruction and computer-aided design (CAD) based software can help to visualize the complex anatomy and thus aid the investigation of the more detailed morphology of the proximal ulna.</P>
